2.Analysis of vascular risk factors for cerebrovascular reserve in the aged patients

Objective To explore the relationship between the common vascular risk factors and cerebrovascular reserve in the aged patients.Methods 700 patients with sex unlimited were selected from Zhejiang Hospital from July 2007 to July 2010.The clinical data about gender,age,smoking history,drinking history,hypertension,diabetes,dyslipidemia,bilateral carotid artery B ultrasonic examination were collected.The patients were divided into vascular risk factors group(n=400)aged(73.4± 6.8)years and control group(n=300)aged(72.4± 6.9)years.Intracranial vascular reaction was dertermined using transcranial doppler ultrasonography combined with breathholding test to calculate the breath-holding index(BHI)and evaluate cerebrovascular reserve.Results BHI in vascular risk factors group and control group were(0.57±0.18)and(0.97±0.31),respectively,and cerebrovascular reserve in the patients with vascular risk factors was decreased than without risk factors(t =10.87,P < 0.01).Multifactor retrospective analysis showed hypertension,hyperlipidemia,diabetes and smoking were related with cerebrovascular reserve(F=6.51,P =0.000),and the degree to influnce from small to large was followed by diabetes,hypertention,smoking and hyperlipidemia.Conclusions Cerebrovascular reserve is damaged in the aged patients with vascular risk factors.Hypertension,hyperlipidemia,diabetes,smoking may affect cerebrovascular reserves among which the greatest influence is in diabetes.
3.Epidemiological characteristics and risk factors of hospital patients with clo-norchiasis

Objective To investigate the epidemiological characteristics and risk factors of clonorchiasis of hospital patients in Heilongjiang Province,so as to provide the evidence for formulating the control and prevention strategies. Methods A total of 2 359 suspected patients from human parasitic disease research institute of Harbin medical university were investigated. Total?ly 6 718 stool samples and 2 359 serum samples were tested by Kato?Katz technique and the enzyme linked immune method re?spectively. Meanwhile,the information of the patients was collected by questionnaires. Results Totally 513 suspected patients were infected with Clonorchis sinensis. The infection rate of the suspected patients was higher in the ≥29 group(P<0.05), and the infection rate and positive rate of blood tests of the rural suspected patients were both higher than those of the urban sus?pected patients(both P<0.05). The habit of eating raw fish and shrimp was the risk factor of clonorchiasis. Conclusion Clo?norchiasis is one of the main food?borne parasitic diseases in Heilongjiang Province. The habit of eating raw fish and shrimp is the risk factor of clonorchiasis.
4.Study of bandage contact lens on cornea epithelial healing after Epi-LASIK

Objective To observe the effect of bandage contact lens on cornea epithelial healing after Epi-LASIK,and to evaluate the efficacy and safety.Methods Prospective case-control study.The 92 cases(181 eyes) performing Epi-LASIK were devided into observation group and control group.The 46 cases(91 eyes) of the control group were covered with routine nursing and the other 46 cases (90 eyes) of the observation group were served with bandage contact lens after Epi-LASIK.The symptoms and signs,time of corneal epithelial recovery,visual acuity and pain of postoperation of the two groups were examined within one week.Results After Epi-LASIK treatment,there was no significant difference in visual acuity between the two groups(P > 0.05).At the 1 st,3rd,and 7th day after Epi-LASIK treatment,the ache grading results of the observation group were (2.53 ± 0.62),(1.39 ± 0.71),(0.07-± 0.25);and the ache grading results of control group were (3.22 ± 0.79),(1.74 ± 0.49),(0.26 ± 0.49).Pain scores of observation group were significantly lighter than that of the control group (P < 0.05).After Epi-LASIK treatment,the average time of corneal wound healing in the observation group and control group were (2.80 ±0.86) days and (4.07 ±0.68) days respectively.The average time of postoperative corneal wound healing in the observation group was shorter than control group(P < 0.05).Conclusion The application of bandage contact lens on cornea epithelial healing after Epi-LASIK treatment can reduce the pain of patients and promote the recovery of cornea in myopia patients.
5.Early subclinical dysfunction of right cardiac system induced by anthracycline manifested as morphological changes

Objective Both right and left ventricular function should be taken into account in the assessment of anthracycline (ATC)-induced cardiotoxicity.The aim of this study was to assess the subclinical dysfunction of right cardiac system in patients with newly diagnosed lymphoma who received ATC treatment by echocardiography.Methods A total of 74 patients with lymphoma who received ATC treatment were enrolled.Each patient underwent transthoracic echocardiographic examination before chemotherapy as well as after two,four and six cycles of ATC remedy.Right atrial (RA) and right ventricular (RV) end-diastolic area (EDA) and end-systolic area (ESA) were calculated.RV end-diastolic volume (EDV) and end-systolic volume (ESV),as well as RV ejection fraction (EF) were measured simultaneously.Tissue Doppler imaging (TDI) measurements of systolic and early or late diastolic myocardial velocities of RV free wall at tricuspid annuals were also analyzed.Two-dimensional speckle tracking echocardiography (2DSTE) was conducted to evaluate RV free wall strain along with strain rate.Results None of the echocardiographic parameters showed significant alteration after two and four cycles of chemotherapy compared with those at baseline (P>0.05).At the end of the therapy (i.e.after six cycles of ATC treatment),there was still no statistical difference on TDI data aswell as 2DSTE measurements (P>0.05).An unexpected finding was that the RAEDA((6.6±1.9) cm2 vs (7.7±2.4) cm2) and RAESA ((8.8±2.5) cm2 vs (10.8±2.8) cm2) revealed obvious dilatation after six cures of the regimen compared with those at baseline (P<0.01).Similar morphologic characteristics displayed on the RVEDA ((14.1 ±3.4) cm2 vs (16.2±3.7) cm2) and RVESA ((7.9±1.9) cm2 vs (9.0±2.2) cm2) (P<0.01)simultaneously.Furthermore,RVEDV ((29.8±10.5) ml vs (37.0±12.7) ml) and RVESV ((12.7±4.4) ml vs (15.0±5.2) ml),as well as RVEF ((59.4±5.8)% vs (56.4±5.8)%),in patients with lymphoma presented statistically significant difference between basic state and the level after six cycles of chemotherapy (P<0.01).Meanwhile,no marked change was detected on left ventricular ejection fraction(LVEF) throughout the follow-up period (P>0.05).Conclusions Echocardiography can be used easily and noninvasively to assess right cardiac system subclinical dysfunction.ATC-induced cardiotoxicity of right cardiac system is firstly manifested as morphological changes than the measurements with novel echocardiographic techniques.In addition,RVEF expresses as a valuable parameter for assessing subtle RV impaired performance in patients with lymphoma received ATC therapy.
6.Expression and clinical significance of ELOVL6 gene in high-grade serous ovarian carcinoma

Objective To investigate the expression of elongation of very long-chain fatty acids family member 6 (ELOVL6) in high-grade serous ovarian carcinoma (HSOC), and explore the correlation between its expression and clinical prognosis in these patients. Methods The expression of ELOVL6 at mRNA and protein levels were respectively detected by reverse transcription (RT)-PCR and immune histochemistry method in 12 cases with normal ovarian tissues and 172 cases with HSOC from primary tumor site, forty of which had paired peritoneal metastatic tissues. Results (1) The results tested by RT-PCR showed that ELOVL6 expression in normal ovarian tissue was 4.8±1.1, while 1.2±0.7 in primary tumors and 1.8 ± 0.9 peritoneal metastatic sites in HSOC. Compared with normal ovarian tissue, the level of ELOVL6 mRNA was significantly lower in HSOC (P<0.05). There was no significant difference between primary and peritoneal metastatic sites in HSOC (P=0.610). It was shown that ELOVL6 protein localized in cytoplasm of ovarian cancer cell by immunostaining assay. (2) ELOVL6 expression was observed in all normal ovarian tissue, 70.2%of G1-G2 and 48.8%of G3 HSOC (P<0.05). ELOVL6 expression in drug-resistant group were significantly lower than that in non-resistant group (39.1% vs 65.0%, P<0.01). The median disease-free survival was 41 months in the ELOVL6-positive group and 39 months in ELOVL6-negative group (P>0.05). The total median survival was 52 months in ELOVL6-positive group and 44 months in ELOVL6-negative group (P>0.05). Conclusion Low expression of ELOVL6 may correlate with the poor differentiation and drug resistance in HSOC.
7.Clinical and OTC gene mutation analysis of one child with ornithine transcarbamylase deficiency

Objective To analyze the clinical and OTC gene mutation characteristics of 1 case with ornithine transcarbamylase deficiency (OTCD) and to deepen the understanding of OTCD.Methods One case of 14-month female OTCD patient was analyzed.Clinical data of the child patient was collected and venous blood 2 mL from the patient and her parents was extracted respectively.Polymerase chain reaction was used to amplify the fragment where various exon of OTC and its neighboring intron were distributed, followed by direct sequencing to detect mutation.Results It was showed that late-onset OTCD child patient had contracted the disease for 3 months,with intermittent drowsiness, vomiting and psychomotor development regression.Cerebellar ataxia was the main symptom of the child patient when she was taken to Tianjin Children's Hospital.According to brain MRI, the lesion was severe.Blood chemistry showed mild hepatic lesion and increased blood ammonia.According to urine gas chromatography mass spectrometry analysis,there was a rise in uracil and orotic acid.OTC genetic testing showed the child patient and her mother were in the 8th exon,c.852C > G (p.Y284X).Missense mutation occurred in this locus.The mother had normal phenotype.Conclusions Clinically OTCD has the symptoms of hyperammonemia and the resulting in varying degrees of damage to the nervous system and the liver.Without clinical specificity, this disease is easy to be misdiagnosed.Methods like blood ammonia and urine metabolic disease screening, blood amino acid analysis and genetic testing help confirm the disease earlier.As for treatment, early intervention and chronic control of blood ammonia level to guard against hyperammonemia will lead to better curative effect.
8.Effects and Safety of Metformin Combined with Vildagliptin on the Glycemic Control for Patients with Newly Diagnosed Type 2 Diabetes Mellitus

Objective:To study the effects and safety of metformin combined with vildagliptin on the glycemic control for patients with newly diagnosed type 2 diabetes mellitus.Methods:60 patients with type 2 diabetes mellitus who were treated from February 2015 to April 2016 were selected and divided into the control group and the observation group according to different treatment methods.The control group was treated with routine treatment.The observation group was treated with vildagliptin based on the control group.The blood glucose,glycosylated hemoglobin,two-hour postprandial blood glucose and serum as well as urinal amylase were measured before and after treatment,and the clinical curative effect of the two groups and the levels of interleukin-6,tumor necrosis factor and C-reactive protein were compared.Results:After treatment,the total effective rate of observation group was 90%,which was significantly higher than that of the control group(66.7%,P<0.05).After treatment,the serum interleukin-6,tumor necrosis factor,C-reactive protein and fasting blood glucose,glycosylated hemoglobin and postprandial blood glucose levels were significantly lower than those of the control group[(7.63± 1.12)dvs(8.68± 1.30)d;(7.23± 0.95)d vs(7.89± 1.20)d;(11.14± 1.56)d vs(12.12± 1.89)d];[(12.12± 1.89)d vs(ll.20± 1.34)d;(6.89± 0.96)d vs(8.23± 1.10)d;(1.65± 0.23)d vs(3.65± 0.48)d] (P<0.05).After treatment,the INS level of observation group was significantly lower than that of the control group (P<0.05) and the GLP-1 level was significantly higher than that of the control group (P<0.05).Conclusion:Metformin combined with vildagliptin could effectively control the blood glucose of patients with newly diagnosed type 2 diabetes and enhance the safety.
9.Evaluation of subclinical dysfunction of left ventricle induced by anthracycline in patients with lymphoma by ;two-dimensional speckle tracking echocardiography and real-time three-dimensional echocardiography

Objective To evaluate the subclinical dysfunction of left ventricle (LV) induced by anthracycline(ATC) in patients with newly diagnosed diffuse large B-cell lymphoma (DLBCL) by two-dimensional speckle tracking echocardiography (2DSTE) as well as real-time three-dimensional echocardiography (RT3DE).Methods Traditional echocardiography images and RT3DE images were acquired from 59 patients with DLBCL before,after the completion of two cures(100 mg∕m 2)and four cures of the regimen(200 mg∕m 2).LV global longitudinal strain(GLS),global circumferential strain(GCS),LV apical rotation and basal rotation,LV end-diastolic volume (EDV),end-systolic volume (ESV),stroke volume(SV) and ejection fraction(EF)were calculated simultaneously.Results Compared with baseline, LV apical rotation and basal rotation reduced significantly after two cures and four cures of therapy [LV apical rotation:(5.34±1 .80)°vs (3.80±1 .45)°vs (2.96±1 .1 8)°;LV basal rotation:(-3.32±1 .14)°vs (-2.65±1 .12)°vs (-2.56±1 .19)°;both P <0.01].LV GLS and GCS decreased significantly till four cures of treatment compared with those at baseline[GLS:(-21 .62±2.5 1)% vs(-20.1 7±2.74)%;GCS:(-26.34±4.76)% vs (-23.27 ±4.73)%;both P <0.01].The alternation on EDV,ESV,SV and EF manifested no visible degradation during the whole procedure (P > 0.05 for all). Conclusions Cardiotoxicity during the early phase of anthracycline treatment can be detected via 2DSTE prior to the traditional echocardiographic expression of ventricular systolic function.The left ventricular rotation index seems to be more sensitive than strain parameters for the estimation of early cardiac injury in patients with ATC chemotherapy.There is no safe dose for anthracycline in all patients with DLBCL treated with anthracycline even at lower doses.
10.Immunogenicity of DNA and Recombinant Sendai Virus Vaccines Expressing the HIV-1 gag Gene

Combinations of DNA and recombinant-viral-vector based vaccines are promising AIDS vaccine methods because of their potential for inducing cellular immune responses. It was found that Gag-specific cytotoxic lymphocyte (CTL) responses were associated with lowering viremia in an untreated HIV-1 infected cohort. The main objectives of our studies were the construction of DNA and recombinant Sendal virus vector (rSeV) vaccines containing a gag gene from the prevalent Thailand subtype B strain in China and trying to use these vaccines for therapeutic and prophylactic vaccines. The candidate plasmid DNA vaccine pcDNA3.1(+)-gag and recombinant Sendai virus vaccine (rSeV-gag) were constructed separately. It was verified by Western blotting analysis that both DNA and rSeV-gag vaccines expressed the HIV-1 Gag protein correctly and efficiently. Balb/c mice were immunized with these two vaccines in different administration schemes. HIV-1 Gag-specific CTL responses and antibody levels were detected by intracellular cytokine staining assay and enzyme-linked immunosorbant assay (ELISA) respectively. Combined vaccines in a DNA prime/rSeV-gag boost vaccination regimen induced the strongest and most long-lasting Gag-specific CTL and antibody responses. It maintained relatively high levels even 9 weeks post immunization. This data indicated that the prime-boost regimen with DNA and rSeV-gag vaccines may offer promising HIV vaccine regimens.
