1.Diagnostic superiority of endoscopic ultrasonography in 38 cases of primary gastric lymphoma and its value in follow-up

Objective To evaluate the diagnostic superiority of endoscopic ultrasonography (EUS) in primary gastric lymphoma (PGL) and its value in follow-up.Methods From January 2012 to June 2015,38 patients with suspected PGL under regular gastroendoscopy,biopsy and surgery operation received EUS combined with targeted deep biopsy.T test and chi-square test were performed for statistical analysis.Results Among 50 patients suspected for PGL under regular gastroendoscopy,38 patients were confirmed pathologically.The sensitivity of EUS examination with targeted deep biopsy in PGL was 86.8% (33/38),the specificity was 83.3% (10/12) and accuracy was 86.0% (43/50).The accuracy of EUS examination with deep biopsy was higher than that of regular gastroscopy,and the difference was statistically significant (86.0% (43/50) vs 57.9% (22/38),x2 =19.4,P<0.05).The main endosonographic characteristics of PGL were partial or diffuse thickening of stomach wall presented with,even or uneven hypoechoic lesions and extramural enlarged lymph nodes.Compared with pathological stage after surgery,the accuracy of T and N stage of EUS was 8/8 and 7/8.The times to achieve complete remission of Helicobacter pylori (H.pylori) eradication group and H.pylori eradication group were (3.3±0.8) months and (4.6± 0.9) months,respectively,and the difference between the two groups was statistically significant (t =4.3,P<0.05).Conclusions The detection rate of EUS combined with targeted deep biopsy is high,which could clearly indicate the depth and extent of lesion invasion and guide clinical stage and selection of therapy.EUS could effectively evaluate efficacy of PGL treatment.
2.Research on UKF control of epileptic-form spikes in neural mass models.

Neural mass models are able to produce epileptic electroencephalogram (EEG) signals in different stages of seizures. The models play important roles in studying the mechanism analysis and control of epileptic seizures. In this study, the closed-loop feedback control was used to suppress the epileptic-form spikes in the neural mass models. It was expected to provide certain theory basis for the choice of stimulus position and parameter in the clinical treatment. With the influence of measurement noise taken into account, an unscented Kalman filter (UKF) was added to the feedback loop to estimate the system state and an UKF controller was constructed via the estimated state. The control action was imposed on the hyper-excitable population and all populations respectively in simulations. It was shown that both UKF control schemes suppressed the epileptic-form spikes in the model. However, the control energy needed in the latter scheme was less than that needed in the former one.

3.The study on ultrastructure of arteriole in patients with hypertensive intracerebral hemorrhage

Objective To examine ultrastructural features of the arteriole responsible for intracerebral hemorrhage or the perforating branches artery around hematoma in patients with hypertensive intracerebral hemorrhage and explore the mechanism of hypertensive intracerebral hemorrhage. Methods Twelve hypertensive patients with CT proved intracerebral hemorrhage underwent operation. The small artery specimens were obtained through cortex fistula and their ultrastructures were observed under the electron microscope. Results Twelve specimens including 4 cases of duty arteriolae and 8 cases of perforating branch arteriolae were collected, Different degrees of degeneration were observed in three layers of the arteriola in all 12 specimens. Changes in endothelial cells included endothelial cell necrosis, collapse, or fallen of from endomembrane, accompanied by degeneration of internal elastic membrane, such as uneven thickness, absence of intermittent and medial smooth muscle cell necrosis. Myofilaments in the cytoplasm were condensed to form a high electron-dense cytoplasm. No micro-aneurysm was observed. Conclusions The pathological changes of cerebral small artery walls load-bearing layer in hypertensive patients include internal elastic layer rupture, smooth muscle layer of degeneration, decreased elasticity and increased fragility. Small artery walls may rupture, resulting in bleeding under the condition of rapid dynamic changes of blood flow.
4.Anxiety state and serum angiontensin Ⅱ levels in patients with vitiligo

Objective To evaluate the possible correlation between anxiety state and serum angiontensin Ⅱ levels in patients with vitiligo.Methods Sixty patients with progressive vitiligo and 30 normal human controls were included in this study.Self-rating anxiety scale (SAS) was used to evaluate the anxiety state,and enzyme linked immunosorbent assay was performed to determine the serum level of angiontensin Ⅱ,in these subjects.Results SAS score was significantly increased in patients compared with the controls (46.13 ±11.72 vs.36.73 ± 12.59,t =3.50,P < 0.01),and 48.3% of the patients were affected by anxiety.The SAS score and prevalence of anxiety were statistically higher in female than in male patients (t =2.47,x2 =4.58,both P < 0.05),in unmarried than in married patients (t =2.59,P < 0.01; x2 =6.17,P < 0.05),in patients with lesions on exposed areas than in those with lesions on unexposed areas (t =3.60,P < 0.01; x2 =5.84,P <0.05).In patients affected by anxiety,the level of serum angiontensin Ⅱ was significantly elevated compared with patients without anxiety (63.83 ± 10.92 vs.40.74 ± 8.70 ng/L,t =9.09,P < 0.01 ).Conclusions Patients with vitiligo are usually affected by anxiety.The serum level of angiontensin Ⅱ is closely correlated with anxiety state in patients with vitiligo as well as with the development of vitiligo,and may have an impact on the initiation and progression of vitiligo.
5.The Influence of Insulin on Antagonistic Effects of Phentolamin and Tolasuline on Norepinephrine-induced Vasoconstriction

The antagonistic effects of phentolamin(PL) and tolasuline(TS) on norepinephrine(NE)-induced vasoconstriction were investigated in vitro on aortic rings of normotensive and spontaneously hypertensive rats in Kreb-Henseilet solution containing 40?IU/ml insulin.The results showed that in the presence of high level insulin,the antagonistic actions of ?-receptor blocking agents,PL and TS,were markedly decreased.The pharmacodynamic parameters indicated that the affinity index and action intensity of PL and TS were significantly reduced in solution containing high level insulin.The experimental results showed that high concentration insulin could decrease the antagonistic effects of ?-receptor blocking agents on NE-induced vasoconstriction.

8.Correlation between cardiovascular risk factors and arterial elasticity in elderly hypertensive patients

Objective To investigate the relationship between cardiovascular risk factors and arterial elasticity in elderly hypertensive patients.Methods 253 subjects were divided into two groups:hypertension group (n=141) and non-hypertension group (n =112).Carotid-femoral pulse wave velocity (C-FPWV),carotid-radial PWV (C-RPWV),height and weight were determined.Fasting plasma levels of blood glucose (BG),blood lipids,insulin,endothelins (ET),nitric oxide (NO) and high sensitivity C-reactive protein (hsCRP) were assessed.Results C-FPWV and CRPWV were higher in hypertension group than in non-hypertension group [(11.7±1.9) m/s vs.(9.7±1.1)m/s,(11.7±1.7) m/s vs.(9.4±1.1)m/s,t=8.43 and-6.30,both P<0.01].Body mass index (BMI),systolic blood pressure (SBP),diastolic blood pressure (DBP),levels of triglyceride (TG),BG,ET,NO and hsCRP,HOMA-IR had significant differences between the two groups (t=-5.27,-4.18,-6.00,6.29,-4.18,-4.86,-3.41,respectively,all P<0.05).Pearson correlation analysis showed that CFPWV was correlated with SBP,age,DBP,fasting BG,NO,ET,TG,HOMA IR,cholesterol (CHO),hsCRP (r=0.534,0.374,0.340,0.338,-0.306,0.242,0.228,0.225,-0.218,0.178,respectively,all P<0.05); C-RPWV was correlated with age,DBP,SBP,NO,TG (r=0.312,0.319,0.241,-0.197,0.151,respectively,all P<0.05).Multiple regression analysis demonstrated that C-FPWV=4.640 + 0.081× HOMA IR+ 0.047× age+0.035×SBP+0.29×TG-0.017×NO+0.014×ET+0.132×hsCRP,r2=0.514; CRPWV=3.161+0.034×age+0.033×DBP+0.313×fastingBG+0.013×ET-0.013×NO,r2=0.390.Conclusions The decreases of arterial elasticity are closely related with endothelial function,insulin resistance,levels of blood lipids and hsCRP in elderly patients with hypertension.
9.Regulation of bone morphologenetic protein 4/Smad signaling pathway on the apoptosis of mouse primordial follicle oocytes

Objective To explore the effect and mechanism of the bone morphologenetic protein 4 (BMP4)/Smad signaling pathway on the apoptosis of mouse primordial follicle oocytes .Methods Three-day-old Kunming mouse ovarine tissues were digested by the two-step enzymatic method to extract and purify oocytes .The cultured oocytes were divided into three groups: the normal culture medium (Con group), the medium with BMP4 (BMP4 group), and the medium with BMP4 and BMP4 inhibitor ( BMP4+inhibitor group ) .TUNEL was used to examine the effects of BMP 4 on the survival of the primordial follicle oocyte;Immunohistochemical staining and Real-time quantitative PCR were performed to investigate the expressions of p-Smad1/5/8, sohlh2, c-kit and foxo3a;siRNA interference, sohlh2 plasmid transfection and LY294002 treatments were performed to explore the mechanism of the BMP 4/Smad signaling pathway on the apoptosis of oocytes . Results TUNEL results demonstrated that the ratio of apoptotic oocytes in BMP 4 group was significantly lower than that in the Con group ( P <0.05 ) and the BMP4 +inhibitor group ( P <0.05 ); BMP4 significantly promoted the nuclear translocation of Smad and inhibited the nuclear translocation of foxo 3a, the mRNA and protein levels of sohlh2 and c-kit remarkably increased in BMP4 group.The effect of BMP4 on the oocyte survival was significantly repressed after sohlh 2 siRNA transfection.Sohlh2 overexpression up-regulated the expression of p-foxo3a, and this activity was abolished by LY 294002.Conclusion BMP4/Smad signaling pathway may inhibit primordial follicle oocyte apoptosis , via up-regulation of the expression of sohlh2 and c-kit, and then down-regulation of the nuclear translocation of foxo 3a.
10.Evaluation of Validity of Lipoprotien(a) as a Diagnostic Test for Coronary Heart Disease and the Stegnotic Degree of Coronary Artheroscle-

Objective To evaluate the validity of lipoprotien(a) [Lp(a)] concentration as diagnostic test for coronary heart disease (CHD) and stegnotic degree of coronary artherosclerosis. Methods 416 cases of patients who were assessed with coronary angiography were divided into four groups according to the stegnotic degree of coronary artherosclerosis. There were no stenosis and lipid plaque group, single branch group, double branch group, three branch and suffusive stenosis group. TC, HDL - C, LDL - C, ApoAl, ApoB, Lp(a) were examined in early morning fasting for 12 hours after the next admission day. Other risk factors were also recorded, such as hypertension. Results When Lp(a) concentration was used alone to diagnose CHD, the best cutoff value was 15 mg/dl. The crude agreement was 58.2% , the sensitivity was 57.5% , and the specificity was 59.3% . Along with the stegnotic degree of coronary artherosclerosis heavier, the levels of the crude agreement and the sensitivity had a indreasing trend,and levels of the false negative rate had a deoreasing trend. Conclusion The validity of Lp(a) concentration as diagnostic test for CHD is not satisfactory.
