ObjectiveTo investigate the incidence, clinical features and preventive measurement of secondary hepato-biliary neoplasm (HBN) in patients with hepatolithiasis. MethodA clinical retrospective study was made on 27 hepatobiliary malignancy patients found among 556 consecutively admitted cases of hepatolithiasis from Jan. 1996 to Sep. 2000.Results The average incidence of hepatobiliary malignancy in patients with hepatolithiasis is 4.8%. In this series, preoperative definite diagnosis was made in 4 cases (4/27, 14.8%). Atypical liver abscess, refractory biliary system infection, progressive obstructive jaundice, CT or BUS scan abnormal findings, and epigastric masses are all clues to concurrent malignancy in patients of hepatolithiasis. ConclusionThere is high incidence of hepatobiliary malignancy in hepatolithiasis patients. Prophylactic partial hepatectomy for segments with stone and chronic infection may be an important measure to prevent ensuing malignancy.

Objective:To observe the clinical efficacy of buccal acupuncture plus exercise therapy in treating scapulohumeral periarthritis (SP). Methods:Sixty-three SP patients were intervened by buccal acupuncture plus exercise therapy. The treatment was given once every day, 5 sessions as a course, with a 2-3 d interval between two courses. The therapeutic efficacy was observed after 3 courses of treatment. Results:Thirty-five patients were recovered, 23 cases were improved, while 5 cases showed ineffective, and the total effective rate was 92.1%. Conclusion:Buccal acupuncture plus exercise therapy can produce a significant efficacy in treating SP, and thus is worth promotion in clinic.

Objective To establish a specific, sensitive and applied method for the detection and differentiation of dengue virus types Ⅰ-Ⅳ,Japanese encephalitis virus and yellow fever virus. Methods Based on the genomes sequence analysis,6 pairs of primers were designed. The special capture probes of dengue virus types Ⅰ-Ⅳ,Japanese encephalitis virus and yellow fever virus were amplified, cloned and sequenced. Then the microwell plates were precoated using these capture probes, and the forward primers were labeled using biotin. The samples were then amplified using the biotin labeled forward primers and reward primers. The microwell plate hybridization was processed for detecting and differentiating the virus. The precoated DNA concentration, precoated time, hybridization temperature and hybridization time were optimized carefully. Results The A value of positive samples were over 0.5,while the average A value of the negative samples was less than 0.1.The S/N value exceeded 10.0.Sensitivity experiment suggested the method of PCR-ELISA could detect the virus RNA in 107 times dilution, while RT-PCR could detect the virus RNA in only 106 times dilution. The stability experiment of PCR-ELISA using DVⅠ suggested that the within-batch coefficient of variation was 6.21%,the between-batch coefficient of variation was 9.92%;the within-batch coefficient of variation in negative control was 1.92%,and the between-batch coefficient of variation in negative control was 3.68%.No visible changes were found on the performance of the coated microwell plates when stored in 4℃for 6 monthes. Conclusion PCR-ELISA is a more sensitive and specific method than RT-PCR is in the early detection and type identification of dengue Ⅰ-Ⅳ types virus,Japanese encephalitis virus and yellow fever virus.

Objective To investigate changes in glucocorticoid receptor (GR) expression and activity in lung tissue of acute lung injury induced by lipopolysaccharide (LPS) within 24h in rat. Methods A total of 70 Wistar rats were divided randomly into LPS treatment group and LPS+ Dexamethasone (Dex) treatment group. The GR mRNA and GR protein expressions in the lung tissue of LPS challenged rats were assessed by RT-PCR and Western blot at different time points. Electrophoretic mobility shift assays (EMSA) were used to determine the GR activity in the lung tissue. Results The expression level of GR mRNA was depressed, but it returned to normal level at 24h after LPS challenge. The expression level of GR was also lowered, reaching the lowest level at 8h. GR activity was decreased, reaching the lowest level at 1h, and remaining lower than that of normal control at 24h. Dex treatment showed no obvious effect on GR activity during the late period of treatment. Conclusion The GR protein expression decreases in lung tissue of acute lung injury in rats, and it maybe associated with the decreased expression of mRNA and accelerated degradation of GR protein. The activity of GR is inhibited sharply, resulting in glucocorticoid resistance.

Objective To find a solution to the long-term storage of enormous amount of imaging data in hospital picture archiving communication system (PACS). Methods Medical imaging data collected by devices of Digital Imaging and Communications in Medicine-3 (DICOM3) and non-DICOM3 were transformed into the storage format, metadata instance, and expression pattern of digital images standardized by DICOM3. Based on the requirements of data utilization and storage system and the optimal principle of function and value, the reasonable flow of imaging data in PACS network were scientifically allocated by application of on-line, near-line, and off-line storage patterns and by reasonable allocation of the volume and clusters of storage devices. Results The PACS storage system established by our method, characterized by super capacity of error tolerance, permanent unrestricted storage, quick in retrieval, and quick, convenient and reliable in storage and reading, could meet the requirements of long-term storage and management of enormous amount of medical imaging data. Conclusion Grading storage is the effective method for long-term storage of enormous amount of medical imaging data in PACS.

Objective To investigate the clinical efficacy of warming and unblocking acupuncture in treating scapulocostal syndrome.Method Eighty patients with scapulocostal syndrome meeting the inclusion criteria were allocated by simple randomization to warming and unblocking acupuncture and conventional acupuncturegroups, 40 cases each. The warming and unblocking acupuncture group received warming and unblocking acupuncture at points Tianzong(SI11), Jianjing(GB21)and Dazhui(GV14)on the affected side and affected chest Huatuo jiaji(Ex-B2)points and uniform reinforcing-reducing acupuncture at points Quchi(LI11), Hegu(LI4)and Waiguan(TE5).The conventional acupuncture group received uniform reinforcing-reducing acupuncture at the same points as those in the warming and unblocking acupuncture group after arrival of qi.Both groupswere treated once every other day, 10 days as a course, for a total of one course.Pain severity was assessed using the Pain Visual Analogue Scale(VAS) in the two groups of patients before and after treatment.Theclinical therapeutic effects were evaluated by follow-up at one month after the completion of treatment.Result The total efficacy rate was 95.0% (38/40) in the warming andunblocking acupuncture group and 80.0% (32/40) in the conventional acupuncture group; there was a statistically significant difference (P<0.05).The PainVisual Analogue Scale(VAS) score was significantly lower in the two groups of patients after treatment than before treatment; there was a statistically significant difference (P<0.05).The VAS score was decreased more in the warming and unblocking acupuncture group after treatment and at the follow-up one month after the completion of treatment (P<0.05).Conclusion Warming and unblocking acupuncture has clinically a better therapeutic effect onscapulocostal syndrome.

Objective: To investigate the difference between the uptake of different hydrophobe drugs into the O/W microemulsion systems and micellar solutions. Methods:Based on the phase diagram, the incorporation of 3 poorly water soluble drugs into Cremophor RH-40-alchohol-isopropyl myristate-water microemulsions and Cremophor RH-40 surfactant systems were examined. Results: There was no significant difference between miconazole nitrate and indometacin into the O/W microemulsion systems and micellar preparation. There was, however,a significant increase in the micellar solutions of ibuprofen amount than in the microemulsion systems. Conclusion:Improvement of solubility by O/W microemulsion and micellar system depends on the solubility of the drug in the dispersed oil phase and is significant only for high lipophilic drugs.
