1.Anxiety disorders in preschool children

Anxiety disorders are the commonest emotional disorders in children, who may be at increased risk of the later develop-ment of other types of mental disorder. Early diagnosis and intervention should be applied. In this article, epidemiology, major factors rele-vant and effective therapy of anxiety disorders in preschool children are summarized.
2.Study of bone marrow srt omal cells on a three-dimensional nano-zirconia porous scaffold for bone tissue engineering

Objective Nowadays, in the field of bone tissue engineering , searching for a kind of scaffold , which is both strong enough and biocompatible , is still a hot topic .Objective of this study is to evaluate the three-dimensional nano-zirconia porous scaffold′s effect on bone marrow stromal cells early attachment and proliferation . Methods Preparing nano-zirconia porous scaffold and testing its porosity and its mechanical strength .Hemolysis experiment was taken to evaluate its biosafety .Canine bone marrow stromal cells ( BM-SCs) were extracted from an adult beagle and cultured in vitro.The third generation cells were seeded onto the scaffold and induced to os-teoblasts under condition medium .Cells early attachment on scaffold was observed by SEM .MTT and ALP tests were taken to evaluate cells proliferation and osteogenesis within two weeks . Results Nano-zirconia porous scaffold in our study presented a porous structure with a well connected inner .Its porosity and compressive strength were (92.667 ±0.324)%and 4.38 MPa, respectively.Hemolysis rate was 2.19%, which was demonstrated to be safe to human body .Canine BMSCs could colonize the surface of the materials , bridge macro-poros-ity and proliferate on the scaffold .A values taken by the method of MTT showed that cells could proliferate quickly on the scaffold after 3, 6, 9, 12 d (A values were 0.604 ±0.059, 1.215 ±0.019, 1.385 ± 0.254, and 1.506 ±0.050), comparing with that on the first day (A value was 0.261 ±0.034).Statistical difference was found (P <0.01) .The ALP level raised up significantly on day 7 and 14 ( A val-ues were 0.032 ±0.002 and 0.062 ±0.007), comparing with that on the first day (A value was 0.016 ±0.003).Statistical difference was found (P<0.01). Conclusion The three-dimensional nano-ZrO2 porous scaffold in our study presented satisfactory biocompati-bility.It could offer a microenvironment for osteoblasts growth and secreted osteogenesis marker in vitro .
3.Degradation kinetics of chlorogenic acid, cryptochlorogenic acid, and neochlorogenic acid at neutral and alkaline pH values.

The degradation kinetics of chlorogenic acid (5-CQA), cryptochlorogenic acid (4-CQA), and neochlorogenic acid (3-CQA) in aqueous solution at 37 degrees C and different pH values (7.05, 7.96, 9.25) were investigated in the present work. The results indicated that 3-, 4- and 5-CQA tended to remain stable in acidic pH circumstance, and unstable in neutral and alkaline pH circumstance. With the increase of the alkalinity, the degradation of 3-, 4- and 5-CQA was increased leading to a less amount of total CQA and was satisfactorily described by the Weibull equation. Meanwhile, caffeic acid was not detected after the degradation of CQA. Moreover, the degradation of 3-CQA and 5-CQA tended to be converted to 4-CQA, and the degradation of 4-CQA tended to be converted to 3-CQA rather than 5-CQA. The comparison of the degradation kinetics parameters of 3-, 4- and 5-CQA at neutral and alkaline pH values showed that the orders of the rate constant (k) values were 4-CQA > 3-CQA > 5-CQA, while the orders of the degradation half life (t½) values were 4-CQA < 3-CQA < 5-CQA, indicating the orders of the stabilities of 3-, 4- and 5-CQA at 37 degrees C and neutral and alkaline pH values were 4-CQA < 3-CQA < 5-CQA.
4.Investigation of geriatric syndromes in very elderly people in Beijing community

Objective To investigate the geriatric syndromes in the very elderly people in Beijing community,in order to provide the basis for management of geriatric syndromes and rational use of medical resources.Methods All community-dwelling residents aged 95 years and over living in Haidian district and some residents aged 85 years and over living in Haidian,Chaoyang and Xicheng districts were investigated in household in Beijing.The survey included the general condition,functional status and geriatric syndromes such as falls,senile urinary incontinence,chronic pain syndrome,chronic constipation and malnutrition occurrence.Results 336 seniors with the mean age of (96.0± 3.0) years were investigated.The percentage of people who had falls within 1 year,chronic pain,and chronic constipation was 27.2% (88/324),37.3% (122/327) and 47.6% (160/336) respectively.The incidence of urinary incontinence was 37.1% (118/318),with 33.3 % (46/138) in males and 40.0% (72/180) in females,and there was no significant gender difference in the incidence of urinary incontinence (P=0.220).The incidences of malnutrition and malnutrition risks was 10.4% (31/299) and 50.5% (151/299).37.2% of subjects could walk outdoor with a better physical function.The self-rated health satisfaction was lower in elderly people with malnutrition and urinary incontinence than in subjects without those diseases (40.0% vs.70.4%,34.0% vs.64.6%,x2 =25.20,25.30,both P<0.001).26% of them had never heard of geriatrics.Conclusions Geriatric syndromes are common in community-dwelling older seniors.It is urgent to spread the geriatric knowledge and set a reasonable goal for diagnosis and treatment in the seniors.

A statistical analysis was conducted to evaluate the publication status in medical research of Shandong province in 2013, based on papers covered in SCI-Expanded database, aimed to provide an objective assessment of the level of medical research in Shandong Province.Factors involved in the analysis including medical papers authored by researchers in Shandong province, numbers and types of publication, distribution of author's location, institution, type of periodical, and impact factor.
6.Clinical analysis of treatment of nonunion of humeral shaft fracture with bidentate intramedullary interlocking nails

[Objective]To observe the effect of the nonunion of humeral shaft fracture treated by bidentate intramedullary locking nails.[Method]Twenty-five cases of fracture nonunion of humeral shaft were cured by bidentate intramedullary locking nails from January 2000 to November 2004.and they received operations to remove their previous instruments.After the bidentate intramedullary nails was inserted via antegrade approach,and autogenous cancellous bone was grafted.[Result]Twenty of the 25 cases were available for follow up.The follow up lasted 10 to 26 months.All cases achieved solid union within 4 to 10 months.There were no complication such as incision infection or occurring of osteomyelitis.[Conclusion]Bidentate intramedullary locking nails can provide the effective control of harmful shearstress,it can provide firm internal fixation,And this methods is an ideal method for treatment of humeral shaft fracture nonunion.
7.Reverse anterior tibial artery flap for the repair of foot skin defect

Objective To investigate the feasibility of clinical use of reverse anterior tibial artery flap for the repair of foot skin defect.Methods With the perforating branch of the anterior tibial artery as turning point,a reverse flap was utilized to repair the foot skin defect.The donor site was directly sutured or covered by full-thickness skin grafting.Results The flap and skin graft survived in all the 7 patients.Follow-up observations for 2~24 months(mean,16 months) in 6 patients showed normal color,texture,and appearance of flaps.Conclusions The reverse anterior tibial artery flap can be used for the construction of foot skin defect,with advantages of simple performance and good safety.
8.Repair of dorsal skin defects of the foot with nerved anterolateral thigh flap

Objective To discuss the use of anterolateral thigh flap for the repair of large defects of soft tissues in dorsum of the foot.Methods A total of 38 cases of dorsal skin defects of the foot was repaired with the anterolateral thigh flap containing the lateral femoral cutaneous nerves.Corresponding vessels and sensory nerves were anastomosed. Results The flaps survived in all the cases.The sensation in dorsum of the foot recovered completely within 2~4 months after operation and the touch,pain,and temperature sensation of the donor area recovered within 3~6 months after operation.Follow-up observations in the 38 cases for 4~18 months(mean,9 months) showed flaps with normal color,texture,and appearance.Conclusions It is recommended to repair large-scale defects in dorsum of the foot with nerved anterolateral thigh flap.
9.Mailuoning protects against ischemic brain injury by inhibiting oxidative stress

Objective To investigate the effects of Mailuoning on oxidative stress and ischemic brain injury. Methods A total of 126 healthy male Kunming mice were divided into sham operation (n = 18), normal saline control (n = 54) and Mailuoning (n = 54) groups. A middle cerebral artery occlusion (MCAO) model was induced Two hours after MCAO,Mailuoning injection and equivalent saline were injected via the tail vein in the Mailuoning and normal saline control groups, respectively, and then they were injected every other 24 h.Neurological score was performed, and brain water content, infarct volume, membrane potential,as well as protein oxidative stress metabolites such as 3-nitrotyrosine (3-NT), lipid oxidative stress metabolite 4-hydroxy-2-nonenal (HNE) and nucleic acid oxidative stress metabolite 8-hydroxy-2'-deoxyguanosine (8-OHdG) were detected at 12, 24 and 72 h after MCAO. Results Mailuoning injection could significantly improve the neurological function of cerebral ischemia in mice, decrease brain edema, and reduce infarct volume at different time points after cerebral ischemia Of those, it was most significant at 72 h. Mailuoning injection could reverse the decreased mitochondrial membrane potential in cerebral cortex and internal capsule, and significantly downregulate the increased 3-NT, HNE and 8-OHdG in cerebral cortex, internal capsule and serum after ischemia, of those, the effect of reducing HNE was most significant.Conclusions Mailuoning injection may effectively protect against ischemic brain injury in mice,and its mechanism is associated with inhibiting oxidative stress, particularly anti-lipid oxidation.
10.The safety and efficiency of non-invasive pressure support ventilation through a non-invasive ventilation-helmet in children after surgical repair of congenital heart disease

Objective To analyze the safety and effect of non-invasive pressure support ventilation in 32 patients by using a helmet and to give the appropriate way of patients who need non-invasive ventilation ( NIV) support after congenital heart disease surgery. Methods Patients over one year old after congential heart disease surgery were admitted in our Department of Cardiovascular Thoracic Surgery from July 2015 to December 2015. Patients who get clinically improved within one hour were divided into the early improved group( Group-E) ,otherwise they were classified to non-early improved group( Group-NE) . The general infor-mation,diagnosis, indication of NIV, ICU and hospital stay, complications, and mortality were collected. Results Thirty-two patients were engaged in this study,including 18 patients(56. 25%) in Group-E and 14 patients(43. 75%) in Group-NE. Patients who got improved in the first hour might have a higher incidence of avoiding reintubation[83. 33%(15/18) vs. 42. 86%(6/14),P=0. 02]. The heart rate,respiratory rate, pH,PaO2/FiO2 and lactate were improved in Group-E compared with Group-NE after the first hour by using helmet. At the end of NIV,the oxygenation showed no difference but the PaCO2 was lower in Group-E. In Group-E,the values showed a trend of improvement,while the values in Group-NE showed not only no statis-tical significance in different time points but also seemed to have a tendency of hypercapnia and reduced com-fort behavior scale in the end of NIV. There were 6 cases in Group-E and 10 cases in Group-NE developed ventilation associated pneumonia with the incidence of 33. 33%(6/18) and 71. 43%(10/14),respectively, which was significant difference (χ2 =4. 571,P =0. 03). The total duration of mechanical ventilation of Group-E was shorter than that of Group-NE [ ( 136. 72 ± 151. 49 ) h vs. ( 252. 79 ± 155. 33 ) h, P <0. 05 ] . Conclusion NIV through a helmet in children could be well tolerated and avoid re-intubation. Patients who get improved earlier may have more clinical advantages,such as less time of mechanical ventilation and lower incidence of postoperative complications. Early improvement can be considered as a valuable indicator wheth-er the patient needs to use NIV continuously.
