1.Analysis of hospital culture's impact on employee satisfaction of a tertiary hospital

Hospital culture plays a more and more important role,among various factors that influence employee satisfaction.Study on hospital culture is gradually growing out of a pure conceptual research and emerging into an area of correlation analysis. Our studies on the hospital culture developments and employee satisfaction in a tertiary hospital,analyzed the correlations between hospital culture and employee satisfaction and its impact,with recommendations for reform strategies.

OBJECTIVE:To explore hospital pharmacists service mode and the subject construction of hospital pharmacy. ME-THODS:Through reviewing the transformation process of hospital pharmacy and pharmacist's duty,the present structure,con-tent,working model and achievements of pharmacy in our hospital were introduced based onlarge pharmacyof taking clinical pharmacy as core,paying attention to personnel training,technical innovation,physican,pharmacist and nurse cooperation,multi-ple discipline integration,etc.;the direction of hospital pharmacy was explored. RESULTS:Many adjustments were made to facili-tate the smooth and orderly development of hospital pharmacy under the new situation,as that establish the rational drug use and quality safety system,drug supply and operation management system working mode;that the development strategy of pharmacy was identified asfocusing on the rational drug use,combining the management functionandproviding professional pharmaceuti-cal care for the clinic and patients,providing professional technical support for the hospital decision and medical management;that the construction of clinical pharmacy specialty/key discipline and drug clinical trial technology platform in accordance with the international standard and so on were lined in the project of pharmacy;that quality control circle was cited,pharmaceutical supervi-sion and pharmaceutical care were expanded to the public and the fine;that a fine,professional pharmacist team was cultured. CONCLUSIONS:The construction of future hospital pharmacy needs the concept oflarge pharmacywith multi-disciplinary inte-gration to develop towards standardization,refinement,specialization and modernization.
3.Phosphodiesterase 5 inhibitors treatment of congenital heart disease with pulmonary arterial hypertension

Pulmonary arterial hypertension is common in patients with congenital heart disease.An effective reduction in pulmonary vascular resistance could improve the survival rate in the future.Research in the pathophysiology of pulmonary arterial hypertension shows that phosphodiesterase 5 inhibitors result in pulmonary vasodilation,decrease vascular resistance.Sildenafil,a inhibitor of phosphodiesterase 5 has been permitted to treat pulmonary arterial hypertension oversea now.Phosphodiesterase 5 inhibitors are proved to reduce pulmonary vascular resistance significantly,improve the survival rate in the future and be well tolerated.This review contains the changes of hemodynamics,exercise tolerance,side effect and pharmacokinetics of phosphodiesterase 5 inhibitors.
4.Studies on the relationship of vitamin D receptor gene distribution and bone mineral density in patients with maintenance peritoneal dialysis

Objective To explore frequency distribution of vitamin D receptor (VDR) genotype and the correlations between polymorphism of VDR gene Bsm Ⅰ sites and bone mineral density in the patients undergoing maintenance peritoneal dialysis.Methods A case-control study was conducted based on 82 patients with maintenance peritoneal dialysis and 80 healthy people.The frequency distribution of VDR genotype in two groups was detected and analyzed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP).Serum biochemistry and bone mineral density of the patients undergoing maintenance peritoneal dialysis were detected by dual energy X-ray absorption (DEXA) and automatic biochemical analyzer,respectively.All data was analyzed with SPSS 19.0 software.Results The frequency distribution of VDR genotype between case group and control group had no significant difference (P > 0.05).The serum biochemical index such as Ca,P,alkaline phosphatase (ALP) and parathyroid hormone (PTH)] of patients with maintenance peritoneal dialysis had no significant difference among three VDR genotypes(P >0.05).Compared to VDR bb genotype,the serum Ca was significantly higher in VDR BB genotype (P <0.05).The bone mineral density (BMD) and T-Score of patients with VDR BB genotype were the lowest,the BB genotype was the highest,and the difference was statistically significant among three genotypes (P <0.05).Conclusions The frequency distribution of VDR genotype between case group and control group had no significant difference.However,there is a correlation between VDR gene BsmⅠ sites and bone mineral density of maintenance peritoneal dialysis patients.
5.The Effect of N-acetylcysteine on ICAM-1 Expression and Apoptosis in Neonatal Rats Brain after Hypoxic and Ischemic Brain Damage

Objective To explore the effect of N-acetylcysteine (NAC) on intercellular adhesion molecule-1(ICAM-1) expression and apoptosis in neonatal rats brain after hypoxic and ischemic brain damage (HIBD). Methods Ninety 7-day old SD rats were divided into sham operative group, HIBD group and NAC treatment group, each group containing 30 animals. ICAM-1 expression and the apoptotic rate of brain cells were measureed at 6h, 12h, 24h, 48h and 7days after model establishment by immunohistochemistry and TUNEL, respectively. Results Compared with sham operative group and NAC treatment group, the number of ICAM-1 expression cells and apoptotic cells in the brain tissue significantly increased in HIBD group. Conclusion NAC could inhibit ICAM-1 expression and cell apoptosis in the brain with HIBD, and had the preventive effect on hypoxic and ischemic brain damage in neonatal rats.
6.Breast reconstruction by deep inferior epigastric perforator flap

Objective To present a method for breast reconstruction with the deep inferior epigastric perforator (DIEP) flap and to summarize the operative experiences. Methods DIEP flaps were elevated without carrying rectus abdominis muscle. The breast was shaped after the deep inferior epigastric artery and vein were anastomosed to the internal mammary artery and vein. Results Since 2000, we have used the DIEP flaps in 15 cases of breast reconstruction. There were 100 % survival in 10 flaps, distal skin necrosis in 2 flaps, and a full loss in 2 flaps. With the follow-up of 6~12 months, the reconstructed breasts were well-shaped and there were no complications such as abdominal heria, bulge and weakness in donor sites. Conclusion DIEP flap is the development and refinement of TRAM flap and possesses of advantages such as rich blood supply, abundant tissue volume, minimal donor site morbidity and easy shaping. It is a new but reliable and safe technique for autologous breast reconstruction.
7.Anatomic features of intercostal nerves with relation to TRAM and DIEP breast reconstruction

Objective To supply anatomic foundation for TRAM and DIEP flaps breast reconstruction with both sensory innervation and rectus abdominis muscle function preserved. Methods Dissections of T8~T12 nerves in anterior abdominal wall were performed on 9 embalmed female cadavers (18 sides). By reconstructing DIEP flaps in 15 cases, the natures of the intercostal nerves and the relations with deep inferior epigastric perforators were investigated. Results Most intercostal nerves pierced the rectus abdominis muscle in the lateral one third. There were interconnections between muscular branches of segmental nerves. Sensory branches traveling with vascular perforators were divided into medial branch and lateral branch. The mean length of pure sensory nerve was (27.6?12.2)mm. Conclusion It is possible to preserve sensory innervation by coapting lower intercostal nerves to the 4th thoracic nerve in TRAM and DIEP breast reconstruction. Sparing lateral one third of rectus abdominis muscle does not cause denervated atrophy in the rectus abdominis muscle after TRAM breast reconstruction.
8.Study on the Accuracy and Reliability of the ABC/2 Formula for Volume Assessment of Intracerebral He-matoma

Objective To investigate the accuracy and reliability of the ABC/2 formula for measurement of intrace?rebral hematoma (ICH) volume by comparing it with software 3D-Slicer. Methods Computed tomographic image data of 184 patients with spontaneous ICH were collected. Hematoma volumes were estimated by using the ABC/2 formula and 3D-Slicer. Based on the volume calculated by 3D-Slicer, patients were further divided into three groups. Group 1 con?tained 55 patients with volume less than 30 ml, group 2 contained 68 patients with volume of 30-60 ml, and group 3 con?tained 61 patients with volume larger than 60 mL. Shape of the ICH was divided into regular (59 cases), irregular with frayed margins (107 cases) and multilobular (18 cases). Results of these two methods were compared according to hemato?ma size and shape. Results The estimated hematoma volume was (58.85 ± 37.38) mL in the ABC/2 formula and (50.08 ± 31.10) mL in 3D Slicer. The ABC/2 formula produced a mean overestimation of 8.77 mL and the mean percent deviation was 17.23%. The mean estimation errors were 2.90 mL (16.22%), 5.89 mL (13.55%), and 17.27 mL (18.76%) in groups 1, 2, and 3 when patients were grouped by hematoma size. The estimation errors were 3.46mL (8.76%), 7.99 mL (17.26%), and 30.81 mL (38.26%) in regular, irregular and multilobular hematomas when patients were grouped by shape. Conclusions The ABC/2 formula is a rough method for volume assessment of intracerebral hematoma and produc?es significant estimation errors. The size of estimation errors largely depends on hematoma shape.

10.Reconstruction of the 1/4 defect on upper-lip vermilion with a lower-lip vermilion compound tissue flap.

OBJECTIVETo investigate the reconstruction of 1/4 defect on upper-lip vermilion with a lower-lip vermilion compound tissue flap pedicled at oral commissure.
METHORDSAt the first stage, the lower lip mucosal flap pedicled by inferior labial artery was transposed to reconstruct the defect on upper lip vermilion and tubercle. The defect at the donor site was closed directly. At the second stage, the flap pedicle was cut off and revised.
RESULTS6 patients were treated with satisfactory aesthetic results. All the flaps survived completely. The oral commissure kept normal with no obvious scar at the donor sites.
CONCLUSIONSThe modified crosslip vermilion flap pedicled at oral commissure has the advantages of avoiding inconvenience in feeding, speaking and cleaning. The procedure is simple with available blood supply. Both aesthetic and functional results are satisfactory.
