1.Relationship of the level of serum C peptide with albumin-to-creatinine ratio and nerve conduction velocity in type 2 diabetes mellitus

Objective To investigate the relationship of the level of serum C-peptide(C-P)with albumin-to-creatinine ratio(ACR)and nerve conduction velocity(NCV)in patients with T2DM.Methods The study included 132 patients with T2DM and 30 healthy controls.The NCV,ACR,fasting C-P and C-P at 2hOGTT(FC-P and 2hC-P)and plasma glucose(PG)were measured.The patients were separated into three groups of Ⅰ(45 cases),Ⅱ(52 cases)and Ⅲ(35 cases).The patients were further classified into two groups of normal NCV(70 cases)and abnormal NCV(62 cases).Results ①The differences of FC-P and 2hC-P among the ACR groups were significant(all P<0.05).②The ACR was correlated with course,2hC-P,2hPG significantly(all P<0.01).The sequence of the impact degree on ACR was from course(Beta:0.592)to 2hC-P(Beta:-0.297)and to 2hPG(Beta:0.201).③The levels of serum 2hC-P were significantly and gradually decreased in the groups of NCV(P<0.01).④The serum 2hC-P concentration was associated with ACR and NCV in different stages(P<0.01).Conclusions ①The level of serum C-P is tightly correlated with the vascular and neural dysfunction in T2DM.②Course,2hPG and 2hC-P may be the risk factors for ACR
2.Development on Medical Rescue Vest of Armed Police Forces

Objective To develop a kind of medical rescue vest which is suitable to single pawn in emergency mission. Methods According to the rescue mission of single pawn and medical soldier in the early stage, the quantity and species of rescue equipment in medical rescue vest are studied to meet the medical needs of rescue mission; the determine ways of recovery effect are researched to ensure the adaptation of personnel and equipment; the vest structure is designed by people somatotyoe, the principle of human work efficiency and combination of disaster field characteristics. Results In the course of rescue drilling and rescue practice, the rescue vest is convenient and practical and meets the technology needs of war injury cure such as hemostasis, bandaging, fixing, handling, ventilation and basic life supporting etc. Conclusion The lightweight and practical medical rescue equipments are provided for single pawn and health soldiers to improve the ability of complete rescue mission.
3.BOLD-fMRI study on the basic activity of the brain in major disorder depression and their first-degree relatives

Objective To study the characteristics and differences of the amplitude of low-frequency fluctuation (ALFF) and the fractional ALFF (fALFF) of the major disorder depression (MDD) and their first-degree relatives in the resting state by functional magnetic resonance imaging (fMRI),to understand the early brain function in patients with MDD.Methods 3.0T fMRI in the resting state was performed in 10 patients with MDD (the depression group),10 first-degree relatives of MDD (the first-degree relatives group) and 10 healthy volunteers (the control group).A statistics analysis of ALFF and fALFF were performed subsequently.Results The ALFF and fALFF values measured in left cerebellum, left precuneus and left medial prefrontal cortex of MDD group were significantly lower than those of the first-degree relatives group (P<0.05).The ALFF values measured in right posterior cingulate cortex and right superior parietal lobule of MDD group were significantly lower than those of the first-degree relatives group (P<0.05).The ALFF and fALFF values measured in left cerebellum, left precuneus and left superior parietal lobule of MDD group were significantly lower than those of the control group (P<0.05).The ALFF values measured in right cingulate cortex and medial prefrontal cortex of MDD group were significantly lower than those of the control group (P<0.05).There was no significance difference found in ALFF values between the first-degree relatives group and the control group,the fALFF values measured in left superior parietallobule of the first-degree relatives group were significantly lower than those of the control group (P<0.05).Conclusion As the widespread ALFF, fALFF abnormalities of brain in MDD and first-degree relatives of MDD,a hypothesis can be get that these abnormal brain region may be associated with cognitive network disorders and emotional distress in MDD.
4.Establishment and evaluation of hamster model of Clostridium difficile-associat-ed diarrhea

Objective To explore a method for establishing the hamster model of Clostridium difficile-associated diarrhea (CDAD)and the indicators for its evaluation.Methods Clindamycin was administered to hamsters subcutaneously (day 1),and 24 h later infected with C.difficile clinical isolates KH1 (ribotype 027,106-108 CFU/mL)or SH9 (ribotype 001 ,108-1010 CFU/mL)by gavage.Animals were observed for CDAD symptoms such as diarrhea,weight loss and death.At the end of ob-servation period (day 7 or death),the cecum was collected from each animal for histological evaluation of inflammation.Results Following a single dose of 100 mg/kg clindamycin subcutaneously,all the animals challenged with KH1 (108 CFU/mL)devel-oped diarrhea and then died within 5 days.All the hamsters challenged with SH9 (1010 CFU/mL)developed diarrhea as well but only 66.7% died at the end of observation period.Among other groups,only one or none developed diarrhea and then died. The symptoms of hamsters with diarrhea included loose stool,wet tail and weight loss.On histological examination,conges-tion,hemorrhage and neutrophil infiltration of the mucosa were observed in the hamsters died of CDAD.Conclusions We have successfully established a hamster CDAD model that allows for future investigations.
5.A Reconstruction Algorithm for Fluorescence Molecular Tomography Based on the Total Least-Squares Rule and the Normalized Conjugate Gradient Method

Objective To find an efficient and practical reconstruction algorithm for the fluorescence molecular tomography basing on total leastsquares rule as well as the normalized conjugate gradient method.Methods The finite element method was used to get the weighting matrix from the forward diffusion model.For the inverse reconstruction problem,the total least-squares rule was employed.The normalized conjugate gradient method was applied to find the fluorochrome information in the objective area.Results The simulation experiments showed that images reconstructed by the total least-squares rule were more accurate than those reconstructed by the least-squares rule under the same circumstances when noise existed.The presented algorithm could also give consistent results when the initial guesses varied in a wide range.Conclusion The algorithm presented in this paper offers a practical and efficient reconstruction for the fluorescence molecular tomography.
6.3-D Image of Near-infrared Fluorescent Probe in Tissue-like Solution.

Objective To develop the fluorescence molecular imaging system and obtain the 3D images of near-infrared fluorescent probe in tissue-like solution.Methods Weak optical signals were acquired with hypothermal cooled CCD camera.The system captured the fluorescent images of different projections at rotating the tissue phantom.The photon propagation in tissue followed the diffusion approximation theory.The 3D image of probe was calculated with a fast reconstruction algorithm based on finite element approach.Results The imaging system could obtain the full-angle dense raw dataset.The reconstruction algorithm had capacity to recover the position and contour of the fluorescent target included in diffusive medium.Conclusion The results from phantom experiments are promising and show the potentials to act as a platform for in vivo fluorescence tomography.
7.Assessment of nutritional status in obese patients prior to bariatric surgery

Objective:To assess the nutritional status of obese patients prior to bariatric surgery, and to explore the related factors of nutrient deficiency.Methods:Clinical data of 43 patients with obesity who underwent bariatric surgery at Beijing Hospital from Jan. 2011 to Dec. 2017 were retrospectively analyzed. Gender, age, BMI, body composition analysis data, blood test data of nutrients were collected. The software of SPSS 20.0 was used to conduct data analysis.Results:Nutrients deficiencies were found for vitamin D (100.0%), iron (26.1%), prealbumin (15.4%) and hemoglobin (7.0%). Hemoglobin, prealbumin, and serum iron levels were significantly higher in male patients than in female patients ( P=0.001, 0.000 and 0.001, respectively). Body fat percentage was negatively correlated with 25 hydroxyvitamin D ( r=-0.983, P=0.017), and positively correlated with serum sodium ( r=0.568, P<0.001). Conclusions:Obesity patients were presented with a variety of nutritional deficiencies before bariatric surgery. A comprehensive nutrients test should be performed before bariatric surgery, to detect and correct nutrient deficiencies preoperatively.
8.Determination of Residual Solvents in Rupatadine Fumarate by Headspace Gas Chromatography

Objective:To determine the content of cyclohexane, ethyl acetate, methanol, methylene chloride and trichloromethane in rupatadine fumarate by headspace gaschromatography. Methods:A DB-WAXETRR capillary column(30 m × 0. 32 mm,0. 25 μm)was used and the carrier gas was nitrogen. The detector was an FID and the inlet temperature was 200℃ . The column temperature program was with the initial temperature of 35℃,maintained 10 min,and then risen to 220℃ with the rate of 20℃·min -1 ,and maintained 5 min. Results:Cyclohexane,ethyl acetate,methanol,methylene chloride and trichloromethane showed a good linear relationship within the range of 77. 590 1- 698. 310 9 μg·ml -1(r = 0. 999 7),102. 166 6- 919. 499 4 μg· ml -1(r = 0. 999 8),62. 744 7- 564. 703 2μg·ml -1(r = 0. 999 9),12. 011 2- 108. 101 1 μg·ml-1(r = 0. 999 6)and 1. 262 8-11. 365 6 μg·ml -1(r = 0. 999 6). The average recovery was 103. 9% ,103. 5% ,104. 9% ,107. 1% and 103. 4% and RSD was 2. 3% ,2. 6% ,3. 1% ,2. 8% and 4. 5%(n = 9),respectively. The five residual solvents were not detected out in rupatadine fumarate. Conclusion:The method is stable,simple,sensitive and accurate,and can be used for the determination of residual solvents in rupatadine fumarate.
9.Complications associated with extracorporeal membrane oxygenation

Objective Extracorporeal membrane oxygenation (ECMO) provides a treatment for patients with acute heart-lung failure. However, as an invasive procedure, it associated with high incidence of complications. It is important to a-vert and reduce the complications for improving the success rate in critically ill patients. We investigate the complications associated with ECMO after cardiac surgery and their management. Methods Clinical data from 117 postoperative patients[32 male, mean age (48.7 ± 16.5) years]supported with ECMO in the cardiovascular intensive care unit( ICU) from March 2005 to June 2008 were analyzed retrospectively. The cardiac operations they had undergone included coronary artery bypass grafting (n = 20), coronary artery bypass grafting and remodeling of left ventricle(n =9), coronary artery bypass grafting and valvular operation(n =5), repair of ventricular septal perforation following acute myocardial infarction(n =2), valvular operation( n = 46), heart/lung transplantation (n = 20/1), correction of congenital heart defects ( n = 10), and aortic operations ( n = 4). Venoarterial bypass was established in 110 patients by cannulation of the right atrium and femoral artery, and that of the right atrium and ascending aorta in 5 cases. Left atrial drainage to ECMO was added in 2 cases. Venovenous bypass was established in 2 patients with hypoxemia following cardiac surgery. Heparin was infused for maintaining the activated coagulation time (ACT) at 160 to 200 seconds for centrifugal pump(114 cases),and 200 to 250 seconds for roller pump(3 cases) to avoid thrombotic events until decannulation was achieved. Results The mean ECMO duration was 61 hours (range 3 to 225 hours). 48(41.0% ) patients died, 18 of them died of complications after weaning from circulatory assistant successfully. Complications occurred in 74 (63.2% ) patients included reoperation for hemostasis (n = 24), renal failure requiring renal replacement therapy (n =29), nosocomial infections ( n = 32) , ischemia in the extremities(n = 5), plasma leakage of oxygenators ( n = 29), gastroenteral hemorrhage ( n = 14), hemolysis ( n = 7 ), neurological complications ( n = 4) and centrifugal pump failure (n =1). Conclusion Bleeding is an early complication associated with ECMO support. The risk of nosocomial infection, renal failure and plasma leakage of oxygenators increases with the duration of ECMO support.
10.Impact of antihypertensive therapy compliance on risk of first-ever cerebral infarction among patients with essential hypertension

ObjectiveTo evaluate the impact of compliance with antihypertensive therapy in pailents with essential hypertension on risk for their first-ever cerebral infarction.MethotisQuestionnaire survey and auxiliary examinations were conducted in 114 patients with essential hypertension hospitalized for acute cerebral infaretion at the First Hospihal Affiliated to Harbin Medical University during December 2006 to December 2007,as well as in another 114 patients with essential hypertensive without history of cerebral infarction as controls during the sanle period.Univariate and multivariate 10gistic regression analyses were performed to study the relationship between first-ever cerebral infarction and compliance with antihypertensive agents and other relevant factors.ResultsAntihypertensive agents compliance,course of hypertension,and history of smoking and alcohol drinking could significantly affect their first occurrence of cerebral infarction in patients with essential hypertension(P<0.05),with odds ratios(OR)of 0.429(95%C10.186-0.993) and 2.142(95% CI 1.052-4.364)for good and poor compliance with antihypertensive agents,respectively,as compared to those without antihypertensive treatmenL Mild drinking was a protective factor for cerebral infarction with an OR of 0.494(95%CI 0.252-0.968).kngth of hypertension with 10-19-years and more than or equal to 20 years.as compared to those with le88 than five years of hypertension,was also a risk factor for it,with an OR of 2.118(95% CI 1.075-4.174).ConclusionsCompliance of essential hypertensive patients with antihypertensive therapy was an important factor that affect their contracting first-ever cerebral infarction.Good compliance could obviously refrain them from it.Patients with poor-compliance or without treatment prone to contract cerebral infarction more easily than those with good compliance.It is necessary to improve compliance with antihypertensive agents in patients with essential hypertension as soon as possible.as well as quitting smoking and limiting alcohol drinking for prevention and control for their first-ever cerebral infarction.
