1.Comparative Study of Budesonide and Beclomethasone Assisting with Terbutaline by Atomization Inhala-tion in the Treatment of Acute Pediatric Asthma Attack

OBJECTIVE:To compare clinical efficacy and safety of budesonide and beclometasone assisting with terbutaline by atomization inhalation in the treatment of acute pediatric asthma attack. METHODS:150 pediatric patients with acute asthma attack were randomly divided into group A and group B,with 75 cases in each group. Both group received symptomatic support treatment as low-flow oxygen inhalation,eliminating phlegm,relieving a cough,dilating bronchus and anti-infection,and atomization inhala-tion of budesonide,1 ml each time,bid. Based on this,group A was given atomization inhalation of budesonide,2 ml each time, bid;group B was given atomization inhalation of beclometasone,2 ml,bid. Treatment course of 2 groups lasted for 7 d. Clini-cal efficacy,asthma control,PEF% and the occurrence of ADR were observed in 2 groups;the time of symptoms and signs dis-appearance and hospitalization time were recorded,and symptoms and signs of children were scored 1 day before treatment and 7 days after treatment. RESULTS:During treatment,2 cases and 3 cases were withdraw from the test in group A and group B, respectively. The total effective rates of group A and B were 91.78% and 94.44%,and the rate of asthma control 95.89% and 94.44%,without statistical significance(P>0.05). The time of breathing difficulties disappearance,cough disappearance time, wheezing disappearance time and hospitalization time of group A were separately (2.11 ± 0.54),(3.28 ± 0.93),(4.38 ± 1.05), (5.83±1.29)d;those of B group were separately(2.07±0.52),(3.30±0.96),(4.45±1.08),(5.90±1.33)d,there was no sig-nificant difference between 2 groups(P>0.05). The clinical symptoms and signs score of group A before and after treatment were separately (4.28 ± 0.94) and (0.15 ± 0.04);those of before and after treatment of group B were separately (4.23 ± 0.91) and(0.11±0.03). There was significant difference before and after treatment(P<0.05),but there was no significant difference between 2 groups (P>0.05). The PEF% of group A before and after treatment were separately (51.4 ± 9.8)% and (67.2 ± 11.2)%;those of before and after treatment of group B were separately(52.0±7.1)%and(68.3±8.7)%. There was significant difference before and after treatment (P<0.05),but there was no significant difference between 2 groups (P>0.05). No obvi-ous ADR was found in 2 groups. CONCLUSIONS:Budesonide and beclometasone assisting with terbutaline by atomization inha-lation in the treatment for acute pediatric asthma attack possess the same effects and can efficiently relieve symptoms and signs, increase the rate of asthma control and shorten the rehabilita-tion course with good safety.

Photodynamic therapy (PDT) has been wildly used in the treatment of tumors and dermatologic diseases,especially,when the traditional therapies are ineffective or bring side effects.Moreover,with the deepening of the studies,its effects in anti-microbial has been found.PDT in the treatment of infectious diseases has attracted more and more attention of scholars.In this review,we discussed the mechanism of the PDT,studies in the sterilization of a variety of oral pathogenic bacteria by the PDT and its potential applications in clinic.
3.Association between mild cognitive impairment and hypersensitive C reactive protein, interleukin-1β and interleukin-6 in Xinjiang region, China

Objective To explore the association between mild cognitive impairment (MCI) and hypersensitive C reactive protein (hs-CRP),interleukin-1β (IL-1β) and interleukin-6 (IL-6) among Uygurs and Hans in Xinjiang region,China.Methods From July 2008 to October 2010,the epidemiological investigation was performed in Southern,Eastern and Northern of Xinjiang.Based on the diagnostic standard of United States psychiatric society of spirit obstacles diagnosis and statistics manual Ⅳ amendment version in the mild cognitive function,483 MCI patients were diagnosed.Finally,314 MCI patients were selected from above according to the completion of data.Moreover,299 subjects were randomly selected as the control group from the investigation.General information and fasting plasma were collected,and blood glucose,blood lipid and biochemical indexes,serum hs-CRP,IL-1β,IL-6 of concentration were tested.The association between MCI and hs-CRP,IL-1β and IL-6 were analyzed with SPSS 17.0 software.Results (1) The concentrations of serum hs-CRP,IL-1β and IL-6 in MCI group (3.40 (6.53) mg/L,0.09 (0.09) ng/L,136.08(96.77) pg/L) were significantly higher than that in control group (2.99 (3.91) mg/L,0.07(0.06) ng/L,79.32(68.79) pg/L) respectively (Z =-2.525,-2.946,-9.361,all P <0.05).(2)The concentrations of serum hs-CRP,IL-1β,IL-6 in Han MCI patients were significantly higher than that in Han non-MCI subject; The concentrations of serum IL-1β,IL-6 in Uygur MCI patients were significantly higher than that in Uygur non-MCI subjects; However,the hs-CRP concentration between MCI and non-MCI group is not statistically different among Uygurs.(3)Non-conditional Logistic regression analysis showed that serum IL-1β(OR =1.008,95% CI0.897-1.071,P =0.006),hs-CRP (OR =1.096,95% CI1.056-1.137,P =0.000),IL-6 (OR =1.011,95% CI1.008-1.014,P =0.000) were associated with MCI.Conclusion The hs-CRP,IL-1β and IL-6 were independent risk factors for MCI.
4.Effects of stepwise and fine-grained dietary management on the diet control of liver cirrhosis patients complicated with gastrointestinal hemorrhage

Objective To evaluate the effects of stepwise and fine-grained dietary management on the diet control of liver cirrhosis patients complicated with gastrointestinal hemorrhage. Methods Eighty liver cirrhosis patients complicated with gastrointestinal hemorrhage were assigned to experimental group (41 cases) and control group(39 cases) by random digits table method. The patients in experimental group were treated with stepwise and fine-grained dietary management and the patients in control group were given conventional dietary treatments. The compliance diet, mastery of nutrition knowledge, malnutrition risk at admission, intake food, discharge and 2 months after discharge between two groups were monitored. The incidence of rebleeding was tracked after 1 year of discharge. Results The incidence of malnutrition risk in Child-Pugh class C was higher than that in Child-Pugh class B( Z=-4.3, P<0.05 ) . The incidence of malnutrition risk in patients with high education level was lower than that in patients with lower education( r=-0.453, P<0.05 ). The experimental group significantly outperformed control group in mastery of nutrition knowledge at discharge:80(70, 84) points vs. 52(42, 64) points, in compliance diet and malnutrition risk after 2 months after discharge:86(76, 91)%vs. 53(46,57)%, 1(1, 2) points vs. 2(1,3) points, the rebleeding rate in experimental group was significantly less than that in control group within 1 year after discharge 14.6%(6/41) vs. 35.9%(14/39), and the differences were statistically significant between two groups ( χ2 = 4.226- 51.232, all P < 0.05 ). Conclusions Stepwise and fine-grained dietary management can improve the mastery of nutrition knowledge, compliance diet and nutritional status, and reduce the occurrence of rebleeding, worth popularizing further in clinical care of liver cirrhosis patients complicated with gastrointestinal hemorrhage.
5.The study on Ezscan screening for dysglycaemia in Chongqing area

Objective To assess the screening capacity of Ezscan for abnormal glucose metabolism .Methods A total of 1 732 permanent residents of Chongqing city underwent physical examination from August 2013 to August 2014 were selected as subjects ,questionnair survey ,physical examination and Ezscan measurement were performed .Additionally ,the levels of fasting blood‐glucose(FPG) ,OGTT 2h blood glucose ,HbA1c and serum lipid were detected .Results The prevalence rate of abnormal glucose metabolism for residents with high Ezscan score was higher than that in those with low Ezscan score .The AUC of Ezscan screening for abnormal glucose metabolism was 0 .616 ,the optimum diagnosis tangent point was 24 .0% ,the sensitivity was 67 .0% .The optimum diagnosis tangent point of FPG was 5 .5 mmol/L for abnormal glucose metabolism ,the sensitivity and speci‐ficity was 70 .5% and 61 .7% respectively ,and the AUC was 0 .824 .The sensitivity of FPG for screening abnormal glucose metabo‐lism was 47 .2% ,when the cut‐off value of FPG was 6 .1 mmol/L .Conclusion The sensitivity of Ezscan for screening abnormal glucose metabolism is higher than that of FPG (when the cut‐off value was 6 .1 mmol/L) ,Ezscan can be used as an efficiency meth‐od for early detection of abnormal glucose metabolism in the asymptomatic population .

8. Effects of nutritional interventions based on nutritional risk assessment strategies on nutritional status and liver function in patients with cirrhosis

Objective:
To investigate the effects of nutritional interventions based on nutritional risk assessment strategies on nutritional status and liver function in patients with cirrhosis.
Methods:
Totally126 patients with cirrhosis admitted from July 2016 to October 2017 were enrolled. The prospective study was used and the patients were randomly divided into the treatment group and the control group with 63 cases in each group. The control group received routine nursing intervention, while the treatment group was combined nutritional intervention based on the nutritional risk assessment strategy. After 3 months of follow-up, serum nutrition indicators, liver function, and complications were compared between the two groups.
Results:
The serum albumin, prealbumin and transferrin levels in the treatment group were (42.36±5.24) g/L, (0.37±0.08) g/L, and (3.01±0.42) g/L, which were significantly higher than those in the control group, which were (34.24±5.36) g/L, (0.30±0.07) g/L, (2.65±0.52) g/L (

Objective To understand the evolutionary characterization of hemagglutinin (HA)gene of pandemic H1N1 influenza virus in Guangdong during 2009-2011. MethodsWe selected 83 pandemic H1N1 strains isolated in Guangdong during 2009-2011. The HA1 genes were sequenced and analyzed comparatively by Bioedit 7.0 and MEGA 4.0. ResultsThe evolutionary rate of Hal gene of pandemic H1N1 and seasonal H1N1 viruses was 5.2×10-3 substitutions/site/year, higher than that of seasonal H1N1 viruses. Most amino acid changes in HA1 molecules accumulated on the surface of the molecule and were partly located in antigenic sites. Two fatal infections were detected with a mutation at HA residue 222, in one virus with a change D222G, and in one virus D222N. ConclusionThe phylogenetic analysis demonstrates that the influenza epidemic in Guangdong at the beginning of 2011 are due to occurrence of genetic changes of pandemic H1N1 virus. The amino acid change at residue 222 of the HA1 are likely to be associated with severe or even fatal illness.
10.Correlation between interleukin-10 and T regulatory cells in leprosy patients

To detect the correlation between Interleukin-10 and CD4+CD25+Foxp3+regulation T cells ( Treg) and to explore the significance and mechanism of them in leprosy patients.Methods:We used the cytometric bead arra (CBA) to detect the serum Interleukin-10 in 30 leprosy patients and we used flow cytometry to test the Treg cells in 51 leprosy persons.Results:The con-centration of serum IL-10 in leprosy patients(130.00±54.27)pg/ml was higher than normal people(15.64±7.11)pg/ml,(P<0.05);The percentage of Treg cells in leprosy patients(17.626±8.1977)%was higher than normal people(9.998±1.7062)%,(P<0.05).The relevance between IL-10 and Treg is positive and linear ,( P<0.05 ).Conclusion: The imbalance between IL-10 and Treg cells may lead to immune abnormal in leprosy patients.
