1.Effect observation of conical stone needle on foot combined with bian anvil for insomnia.

OBJECTIVETo observe the effect of conical atone needle on foot combined with Nan anvil at Yintang (CV 29) for insomnia.
METHODSSeventy patients were randomly divided into a ibian technique group and an acupuncture group,35 cases in each one. In the Sian technique group, conical stone needle on foot combined with bias anvil at Yintang(GV 29) were used,and the therapy was applied at Yongquan(KI 1), Taichong(LR 3) and Gongsun(SP 4), etc. In the acupuncture group, common acupuncture was implemented at Sanyinjiao(SP 6), Shenmen (HT 7) and Sishencong(EX-HN 1), etc. In the two groups, treatment was given for 2 weeks,once a day. There were two days to have a rest between 5-time treatment. The change of Pittsburgh sleep quality index(PSQI) before and after treatment was recorded and the clinical effect was observed after 10-time treatment and in two weeks aftet treatment.
RESULTSAfter treatment,the total scores of PSQI were declined significantly compared with those before treatment in the two groups(both P<0. 01). The total PSQI scores after treatment,the curative and effective rates and total effective rates when followed up bad no statistical significance(all P>0. 05).
CONCLUSIONConical stone needle on foot combined with Sian anvil at Yintang(GV 29) treating insomnia is safe and effective,and this treatment acquires similar efficacy with less suffering.

2.Curative effects of hepatocyte growth factor in treatment of infant hepatitis syndrome caused by cytomega lovirus with ganciclovir

Objective To compare the curative effect of hepatocyte growth factor(HGF) in treatment of infant hepatitis syndrome (IHS) caused by cytomegalovirus with ganciclovir.Methods Sixty-eight cases with infant hepatitis syndrome caused by cytomegalovirus from neonatology department of our hospital from September 2010 to September 2014 were randomly divided into two groups, the observation group (32 cases)took HGF and ganciclovir therapy and the control group (36 cases) only received ganciclovir treatment Both groups were given conventional guard liver therapy.After one month of treatment, liver function, cytomegalovirus DNA content,extinction time of jaundice, treatment efficiency and the adverse reaction rate were evaluated for two groups.Results Liver function of children in observation group improved significantly after treatment compared with the control group, CMV-DNA quantitatively lower than the control group, the extinction time of jaundice was earlier than the latter,and the overall treatment efficiency was higher in observation group (P <0.05);The adverse reaction has no significant difference between two groups (P > 0.05).Conclusion HGF combined with ganciclovir treatment can significantly improve liver function in patients of IHS caused by cytomegalovirus infection, and can collaboratively reduce viral load.Symptoms subside quickly with fewer adverse reactions, which is worthy of clinical promotion.
3.Construction of prokaryotic expression vector of Id3 and expression of fusion protein in E. coli

Objective: To express and purify the inhibitor of differentiation(Id3) in E.coli.Methods: The DNA fragment in the coding region of the human Id3 gene was amplified by PCR and cloned into the pGEM-T Easy vector for sequencing.The Id3 cDNA fragment was then subcloned into the prokaryotic expression vector(pET-32a)(+) and transformed into E.coli BL 21(DE3).The expression of the histidine-tagged(His-Tag) fusion protein was induced with isopropy-?-D-thiogalactoside(IPTG),confirmed by Western blot and purified by the immobilized Ni2+ absorption chromatographic column.Results: The prokaryotic expression vector of Id3 was successfully constructed.Western blot confirmed the expression of the His-Tag fusion protein in E.coli BL 21(DE3) and that of the Id3 fusion protein with the relative molecular mass size of 33 000 after purified by the affinity chromatographic column. Conclusion: Recombinant Id3 can be expressed in E.coli BL 21(DE3) and the obtained fusion protein can be purified by the Ni-affinity chromatography column.
4.Measurement and clinical significance of serum DKK-1 and sRANKL levels in multiple myeloma

Objective To detect serum concentrations of Dickkopf-1(DKK-1) and soluble receptor activator of nuclear factor-κB ligand (sRANKL) in patients with multiple myeloma (MM) and to investigate its clinical significance. Methods Serum DKK-1, sRANKL, osteoporotegerin(OPG) and tartrate-resistant acid phosphatase 5b (TRACP-5b) levels were quantified in 30 newly diagnosed MM patients and 20 healthy control subjects by using sandwich ELISA. Results The serum DKK-1, sRANKL,OPG and TRACP-5b levels were significantly higher than those in the healthy controls (42.96 μg/L vs 5.33 μg/L, 1.83 pmol/Lvs 0. 79 pmol/L, 1799. 30 pmol/L vs 822.40 pmol/L, 5.81 U/L vs 0. 28 U/L, respectively; all P<0. 05). Serum levels of DKK-1 were positively correlated with sRANKL and TRACP-5b, respectively.Serum concentrations of DKK-1 and sRANKL were significantly elevated in stage Ⅲvs stages Ⅰ and Ⅱaccording to International Staging System (ISS) (46. 33 μg/L vs 37.91 μg/L, 2.26 pmol/L vs 1.19pmol/L, respectively, all P <0.05). Serum concentrations of DKK-1 , sRANKL and TRACP-5b were significantly higher in patients with more than 3 lytic bone lesions than those with only 1-3 lytic bone lesions (46. 30 μg/L vs 31.98 μg/L, 2. 18 pmol/L vs 0. 69 pmol/L, 6.02 U/L vs 5. 13 U/L, all P < 0.05).Conclusions MM patients have increased serum DKK-1, sRANKL, OPG and TRACP-5b levels as compared with the healthy controls. Serum concentrations of DKK-1 and sRANKL have close relationship with MM stage and lytic bone disease.
5.Anatomical study and clinical application of free medialis pedis flap based on the top branch of plantaris arch

Objectives To observe anatomical location of the top branch of plantaris arch which extends from the superficial branch of the medial plantar artery,design and apply flap base on it. Methods On the basis of the anatomical data from 8 feet of 4 fresh cadavers and probed of the Doppler from 50 feet of normal person,defined the anatomic location and body surface projection of the top branch of plantaris arch,a type of new flap design were introduced in the paper. Results The flaps have been used in six cases clinically to treat skin defect of the hand meanwhile,sensation of the flap was regained by suturing nerve.All flaps survived,the appearance of the flaps were excellent,the function of the injured hands were recovered well. Conclusions The design of the flap is reasonable,it is suitable that the free medialis pedis flap of the top branch of plantaris arch is used to repair a small skin defect of the hand.
6.Prosthetic infection after tension-free hernioplasty in 12 cases

Objective To evaluate the prevention and treatment of prosthetic infections after tension free hernioplasty. Methods The clinical date of 12 prosthetic infections admitted from May 2005 to May 2008 were analyzed retrospectively. Patch was inserted under prefascial retromuscular site in 8 cases, between fascial edges in 2, Mesh plug and patch Lichentstein in one each. As for the material used there were flat knitted polypropylene in 7 cases, polytef in 1 case and composite prosthesis in 4 cases. Treatments were based on prothetic materials and levels infected. Infectious patchs were removed completely in 8 cases and partially removed in 2 cases, Open dress change with patchs left in situ in 2 cases. 3 cases in 10 cases of removed patchs were given immediate suturation with closed drainage. Results Infection was cured in all cases without mortality. There was no hemorrhage and injury of bladder during the procedures. No retention of urine, pain and other complications during postoperation. Change of dressing was made from 7 days to 6 months( median 2 weeks). Selected antibiotic was used from 3 to 7 days (average 4.5 days). All patients were followed up from 18 to 78 months (average 38.5 months) without hernia recurrence and infection. Conclusion It was most important to prevent infections after tension-free hernioplasty. Management should be individualized according to different type of patch used and the surgical procedure in hernia repair.
7.Epigenetic factors in rheumatoid arthritis

Epigenetics is defined as heritable changes in genc expression that are,unlike mutations,not attributable to alterations in the sequence of DNA.The predominant epigenetic mechanisms are DNA methylation,histone modification and microRNA.Exploring the precise roles of epigenetic factors on autoimmunity has become an active research area.Rheumatoid arthritis (RA) is a chronic autoimmune inflammatory disorder with a prevalence of approximately 0.5% to 1.0% worldwide.The mechanisms underlying the initiation and progression of RA are still not completely understood.In recent years,the epigenetics of RA have been widely investigated.Alterations in the modification of histones and DNA methylation are the two major epigenetic mechanisms that may potentially cause a breakdown of immune tolerance and the perpetuation of RA.These epigenetic changes may be ideal targets for new personalized treatments.

10.A Simple Method for Dispelling Motion Artifacts in MRI Examination of Brain

Objective To explore a method that restrain and dispel motion artifacts of MRI examination for better image quality and higher working efficiency.Methods The sponge package of CT tube was used to design and make sponge cushions in types of 2 cm?7.5 cm?22 cm,3 cm?7.5 cm?22 cm and 4 cm?7.5 cm?22 cm,which were used to MRI examination by quadrature coil of brain.Results The method of"adding cushion" was adopted to examine brain of MRI in 2 638 cases.Among them,2 633 cases were successfully scanned,which accounts for 99.81%;5 cases failed,accounts for 0.19%.For the failed cases,4 cases were successfully scanned in the second time by using medical bandage in cooperation,while 1 case failed.Conclusion The method of"adding cushion" in routine MRI examination of brain can restrain and dispel motion artifacts,improving image quality and work efficiency.It is an easy,reliable,ideal method for restraining and dispelling motion artifacts in brain MRI examination.
