Objective: To observe the effect of home-made blenderized diet on nutritional support in old comatose patients. Methods: Sixty-three old comatose patients were fed with home-made blenderized diet by nosal gastric feeding.Triceps skinfolds were measured every month. The serum levels of albumin,pre-albumin,total number of lymph cells and hemoglobin were analysed every month. Results: No side effects of abdominal pain, dearrhea, nausea and vomiting were observed in the fed patients. The states of nutrition(such as triceps skinfolds,albumin,pre-albumin,total number of lymph cells and hemoglobin) were improved significantly after home-made blenderized diet. Conclusion: The home-made blenderized diet can support the old comatose patients effectively.

Objective To investigate the effect of different anesthesia methods on perioperative body temperature. Methods Eighty patients undergoing esophagus cancer resection were randomized into two groups of combined general and epidural anesthesia(GEA, group A,42 cases) and general anesthesia(GA, group B, 38 cases). Epipharynx and peripheral temperatures were recorded at the same time every 30 minutes for 3 hours after induction. Epipharynx temperature was recorded than after when circulating water mattresses and fluid warming were used to rewarming. The incidence of postoperative shivering was recorded. Results The peripheral temperature was gradually decreased after induction in both groups. The core temperature was decreased quickly within the first 1 hour in both groups, which was reduced slowly within the next two hours with a greater magnitude of temperature reduction in group A than that in group B(P<0.05). The incidence of shivering was higher in group A than that in group B during warming up. Conclusion The patients undergoing long procedures such as esophagus cancer recection under GEA is at more risk for hypothermia than those under GA.

Objective To develop an ELISA for detection of anti-dsDNA antibody by using plasmid DNA as antigen. Methods DNA in plasmid pBV220 for prokaryotic expression vector was purified by base-cleavage. The microtiterplates were pretreated by poly-L-lysine and coated by the plasmid DNA in a dilution of 1∶50 as antigen. An ELISA method for detection of serum anti-dsDNA antibodies was developed with HRP-SPA as enzyme-labeled marker. The IIF using crithidia lucilia as substrate was performed simultaneously for comparison. The serum samples from 64 patients with SLE, 8 with MCTD and 17 with RA were detected. Results The concentration of DNA was 1 54 g/L by UV spectrophotometer at wavelength of 260 nm. The positive percentage of ELISA for anti-dsDNA was higher than that of IIF. By comparison with IIF the positive percentages in SLE, MCTD and RA groups were 23 4% vs 17 2%, 12 5% vs 12 5% and 11 8% vs 5 9%, respectively, and the coincident rates between the 2 methods were 93 8%, 100% and 94 1% respectively. The sensitivity and specificity of the developed ELISA for detection of anti-dsDNA were 100% and 93 4% when IIF was as gold standard. Conclusion The ELISA by using plasmid DNA as antigen to detect anti-dsDNA has fine precision, sensitivity and specificity. Its positive rate is higher than that of IIF thus it will contribute to monitor the activities for SLE patients′ condition.

Objective To investigate the surgical indication, approach, resection methods and complications of total sacrectomy via posterior approach for primary malignant sacral tumors involving high level (S1,S2).Methods 5 cases of primary malignant sacral tumors treated by total sacrectomy via posterior approach and iliolumbar reconstruction from March 2010 to March 2011 were analyzed retrospectively. There were 3 males and 2 females. The mean age was 41,ranging from 32 to 55.The imageology examination showed osteolytic destruction, among which there were 4 cases of obvious soft tissue mass,1 case of obvious sacral foramina expansion with bone damage.MRI showed 1 case of the tumors in S1-S5,1 case of the tumors in S1 、S2,1 case of the tumors in S1-S3.The smallest tumor was 9.2 cm×7.6 cm×4.1 cm,while the largest was 22.0 cm× 19.0 cm× 16.0 cm.Preoperative TTNB were done on the 5 patients.Pathological diagnosis:2 cases of chordoma,1 case of malignant neurilemmoma,1 case of chondrosarcoma and 1 case of malignant hemangioendothelioma.5 patients had sacrococcygeal pain or lumbocrural pain before the surgery.Visual analogue scale (VAS):2 cases of 2,2 cases of 6 and 1 case of 8.Ilium stability reconstructions were all performed on 5 patients with spine pedicle screw-rod system. Results The mean operation time was 6.5 hours (range,4.5-11 hours),with the mean intraoperative blood loss of 3 700 ml (range,2 000-7 200 ml). There was no perioperative death. The mean follow-up time was 17 months (range,9-23 months). There were 2 cases of wound complications 2 weeks after surgery and healed by second intention with washing and drainage after debridement. There was no deep infection.1 case of rectal injury, which was performed with colostomy during the operation, and stoma returned 12 weeks after surgery.4 cases of sciatic nerve symptom of lower limbs and plantar flexion dyskinesia after bilateral S1 nerve roots resection. The patients walked with ankle brace fixed after the surgery. There was 1 case of implant breakage and no obvious spine down.5 patients had functional disability in sphincter after surgery.1 case of hemangioendothelioma recurred locally 9 months later.Local radiotherapy was performed since there was no reoperation indication. The patient is currently on the 13th month follow-up and survives with tumors.Compared with combined approach, the selection of posterior approach alone has relatively strict surgery indications. Conclusion Total sacrectomy via posterior approach is an effective way to treat the primary high-level malignant sacral tumors. The good surgical resection boundary is important to achieve the good oncology prognosis. The occurrence rate of postoperative complication is high, which has great influence on patients' postoperative neurological function.

Chinese Journal of Medical Imaging Technology 2010;26(2):243-246
Objective To investigate the correlation between relative cerebral blood volume (rCBV) and tumor character of meningiomas. Methods Thirty-six (GradeⅠ: n=30, GradeⅡ+Ⅲ: n=6) patients with meningioma underwent conventional MR and perfusion weighted imaging (PWI) . The pulse sequence of PWI was single shot GRE-EPI-T2~*WI. The CBV maps were calculated from the original data of perfusion images and the maximum rCBV of meningiomas was acquired from CBV maps through measurement on the region of interest (ROI). The differences of rCBV in tumor and edema between benign and malignant tumors were analyzed. Results The mean rCBV in tumor of benign meningioma (9.78±4.69) was higher than that of malignant ones (3.59±0.28) (t=7.168, P<0.001). The mean rCBV in edema of benign and malignant meningiomas was 3.06±1.76 and 3.58±1.93, respectively (P>0.05). Conclusion CBV map and quantitative study of rCBV are feasible for differential diagnosis of meningiomas and have instructive function for clinical treatment and assessment of prognosis.

Objective To study a kind of video transformation algorithm based on FPGA in view of the present situation that the video output of X-ray equipment can not be connected to common video adapter. Methods Through the control of non-standard video signal AD sampling data read-write succession in FIFO,800 digital sampling data was output(output resolution:800?600,refresh rate:60Hz)in the period of every line of effective signal. The output module combined these data with line and field synchronized signal and transform them into standard VGA signal. Results Non-standard medical video signal could be transformed into standard medical video signal by using VHDL in normal FPGA.Conclusion The speed of this algorithm is high, and the transformation effect is clear and smooth.
