1.History and Research Progress of Asari Radix et Rhizoma Processing

Setting Li Dai Zhong Yao Pao Zhi Zi Liao Ji Yao as clue, this article searched each edition of China Pharmacopoeia, National Processing Standard of Traditional Chinese Medicine issued in 1988, and relevant materials of local processing standards and modern processing study, and concluded and combed the contents about Asari Radix et Rhizoma processing in ancient and modern literature. There are records about Asari Radix et Rhizoma processing methods in history, including net processing, cutting processing, heating processing and processing excipient. However, Asari Radix et Rhizoma processing methods in modern time are relatively simple. In addition to the version of the Processing Standard of Traditional Chinese Medicines in Shanghai issued in 2008, containing the honey Asari Radix et Rhizoma, other editions of China Pharmacopoeia and processing standards only recorded net processing and cutting processing. Recent research showed that processing methods of modern stir frying processing, alkali processing, wine processing, low temperature ultrafine comminution processing and so on, can effectively reduce the toxicity of Asari Radix et Rhizoma and enhance the safety of clinical application. This article summarized the history and research progress of Asari Radix et Rhizoma processing, and provided a reference for the study on modern processing.

The effects of targeted silencing of heparanase gene by small interfering RNA (siRNA) on invasiveness and metastasis of osteosarcoma cells (MG63 cells) were investigated in the present study. Two complementary oligonucleotide strands were synthesized and inserted into pGenesil-1 vector based on the mRNA sequence of heparanase gene. The expression vector containing short hairpin RNA (pGenesil-shRNA) was constructed successfully. MG63 cells were randomly allocated into 3 groups: blank group, empty vector (pGenesil) transfected group and expression vector (pGenesil-shRNA) transfected group. Under the induction of Lipofectamine 2000, the recombinants were transfected into MG63 cells. Heparanase gene expression level was detected by RT-PCR and Western blotting. Cell proliferation was measured by MTT assay. Cell invasiveness and metastasis were examined by cell adhesion and Transwell-ECM assays. HUVECs migration assay was applied for the detection of angiogenesis. As compared with negative controls, the mRNA and protein expression levels of heparanase were down-regulated by 76.1% (P<0.01) and 75.3% (P<0.01) respectively in the pGenesil-shRNA transfected group. Meanwhile, the proliferation, adhesiveness, invasiveness and angiogenesis properties of MG63 cells were all significantly inhibited. It was suggested that targeted silencing of heparanase gene by siRNA could dramatically inhibit the invasiveness and metastasis of osteosarcoma cells.
4.Influence of mental intervention on self-image of breast cancer patients

Objective To investigate the influence of mental intervention on the self-image of breast cancer patients.Methods Fifty patients with breast cancer who were hospitalized in our hospital from January to September,2015 were enrolled as the control group in the study,where routine nursing intervention,health education on disease related knowledge and psychological nursing were carried out.During October 2015 to June 2016,50 patients with breast cancer were enrolled as the observation group,where psychological awareness intervention was carried out apart from the routine nursing intervention.The self-image level of breast cancer patients from the two groups was evaluated by self-image questionnaire for breast cancer patients (BIBCQ).Result After intervention,the scores on self-image in the observation group were significantly higher than those in the control group (P<0.05).Conclusion Mental intervention can improve the self-image of breast cancer patients and help improve their quality of life.
5.Classification decision tree in CT imaging:application to the differential diagnosis of solitary pulmonary nodules

Objective To establish classification and regression tree (CART) for differentiating benign from malignant solitary pulmonary nudules (SPN).Methods One hundred and sixteen consecutive cases with 116 solitary pulmonary nodules,which finally were pathologically proven 54 malignant nodules and 62 benign nodules,were prospectively registered in this research.Twelve clinical presentations and 22 CT findings were collected as predictors.A classification tree was established to distinguish benign SPNs from malignant ones.In the observer test,two groups (one made of junior radiologists and one of senior radiologists) were independently presented with clinical information and CT images without knowing the pathologic and machine-learning results.Performance of observers and CART were compared by receiver operating characteristic analysis.Results Receiver operating characteristic analysis showed areas under the curve of CART,senior radiologists and junior radiologists respectively were 0.910±0.029,0.827±0.038,0.612±0.052. Difference between areas (DBF) between CART and junior radiologists was 0.297 (P<0.01).DBF between CART and senior radiologists was 0.083(P<0.05).DBF between senior and junior radiologists was 0.214(P<0.01).CART showed a best diagnostic efficiency,followed by junior radiologists,and then senior radiologists.Conclusion Our data mining techniques using CART prove a high accuracy in differentiating benign from malignant pulmonary nodules based on clinical variables and CT findings.It will be a potentially useful tool in further application of artificial intelligence in the imaging diagnosis.
6.Computer-aided detection of nodule in low-dose CT screening for lung cancer

ObjectiveTo evaluate the performance of computer-aided detection (CAD) system for detection of pulmonary nodules in 64-slice low-dose CT screening and to investigate whether CAD can improve the performance of radiologists in detecting pulmonary nodules.MethodsOne hundred low-dose screening CT examinations were randomly selected from the database containing 578 consecutive cases between Jun 2007 and Jun 2008.All the examinations were performed on a 64-MSCT scanner with the exposure of 120 kVp,30 or 40 mA,or automatic exposure control.Before the study started,the screening reports had been made with double reading by two radiologists.All the selected images were analyzed with the lung VCAR software from GE Healthcare with a nodule diameter threshold 3.0 mm.All discrepancies between the screening reports and the CAD results were reviewed and the true non-calcified nodules were determined in consensus by two experienced chest radiologists.Detected nodules were classified by density,size and location.The performance of the double reading and the CAD system were compared and analyzed statistically.McNemar-Bowker test was used for the statistical analysis.ResultsA total of 257 true noncalcified nodules were determined in all 100 low-dose screening CT examinations.The detection rate of CAD system was 91.1% (234/257),with the missed rate of 8.9% (23/257).Twenty three nodules were missed by CAD,in which 10 were solid with the diameter ranged from 2.4 to 6.0 mm,and 13 were nonsolid with the diameter ranged from 2.1 to 8.6 mm.Of the 23 nodules,17 were located in the outer zones of lungs and 6 in the inner zones.The double reading showed a detection rate of 59.1% ( 152/257 )and a missed rate of 40.9% ( 105/257),which was significantly lower than CAD.The diameter of all the 105 missed nodules by radiologists were ranged from 2.4 to 11.8 mm,in which 94 nodules were solid,10 were partly solid and 1 was nonsolid,with 69 located in outer zones of lungs and 36 in the inner zones. Conclusions The capability of the CAD system for detecting non-calcified pulmonary nodules is superior to double reading in low-dose screening CT examination,especially for the nodules located in the inner zone of the lung.When lung VCAR is used,nonsolid pulmonary nodules are more easily missed so that it should be paid more attentions by radiologists.

ObjectiveTo establish a new method for preparing synaptosomes.MethodsDensity gradient centrifugation method was used to isolate synaptosomes of mouse, checking by transmission electron microscopy.ResultsSynaptosomes prepared by this method had intact morphological characteristics, surrounding with a continuous oval-shaped membrane structure, moreover, mitochondrion and lots of synaptic vesicle in them.ConclusionThis method is applicable to establish a rapid, convenient and useful method for preparing synaptosomes.
8.Analgesic effects of continuous fascia iliaca compartment block versus continuous epidural analgesia after total hip arthroplasty

BACKGROUND:Epidural analgesia has been considered a gold standard for postoperative analgesia in the lower limbs. Its outcomes are accurate and adverse reactions are few, so it can be used in the clinic. However, this method has adverse reactions such as hypotension and urine retention. Low molecular weight heparin should be used after operation, which can increase the possibility of epidural hematoma, and limits its application to epidural analgesia in the clinic. At present, few studies concerned ultrasound guided continuous fascia iliaca compartment block technology.
OBJECTIVE:To evaluate the efficacy of postoperative pain relief and the joint rehabilitation between a continuous fascia iliaca compartment block and a continuous epidural analgesia for patients undergoing total hip arthroplasty.
METHODS:A total of 60 patients undergoing a selective total hip arthroplasty were assigned to continuous fascia iliaca compartment block group and continuous epidural analgesia group (n=30). Al patients in both groups received a pre-fluence before general anesthesia. Continuous fascia iliaca compartment block group were injected with 0.25%ropivacaine 30 mL via iliac fascia gap. Continuous epidural analgesia group received 0.20%ropivacaine 10 mL via epidural catheter, indwel ing catheter. When the analgesic effect was identified, anesthesia intubation was carried out. After operation, medicine was given via iliac fascia and epidural analgesia pump in both groups respectively. Postoperative analgesia in single dose was not given. If pain could not be endured, analgesia would be rescued (parecoxib 20-40 mg/time) according to pain degree. Visual analogue scale scores, supplemental analgesia of parecoxib, complication of anesthesia, Harris hip joint scores, day of first walk, and duration of hospital stay were recorded.
RESULTS AND CONCLUSION:No significant difference in visual analogue scale scores, supplemental analgesia, Harris hip joint scores and duration of hospital stay was detected. Day of first walk was earlier in the continuous fascia iliaca compartment block group than in the continuous epidural analgesia group. The complications were apparently lower in the continuous fascia iliaca compartment block group than in the continuous epidural analgesia group. These data indicated that after total hip arthroplasty, two kinds of analgesia methods could provide satisfactory postoperative outcomes. Hip joint was perfectly recovered. However, the complications of continuous fascia iliaca compartment block were less, and helpful to patients’ early off-bed activities, and could be considered as a good choice for analgesia after total hip arthroplasty.
9.Simultaneous Determination of 11 Appetite Suppressants in Weight-loss Functional Foods by Accelerated Solvent Extraction- High Performance Liquid Chromatography Tandem Mass Spectrometry

An analytical method based on high performance liquid chromatography tandem mass spectrometry has been developed for the determination of 11 appetite suppressants (fenfluramine,phenylpropanolamine,sibutramine,sertraline,rimonabant,bupropion,citalopram,fluoxetine,benfluorex,topiramate,zoni-samide). Various weight-loss functional foods were extracted under accelerated solvent extraction and separated on Waters Atlantis T3( 150 mm×2.1 mm,3μm) column. The analysis was carried out on HPLC-MS/MS by electrospray ionization using multiple reaction monitoring. Identification was achieved by ihe retention time and the ion ratio,quantification was done by the external standard curves. The limits of detection for the appetite suppressants were 0.05-4.0 mg/kg. The mean recoveries at the three spiked levels(low,middle,high) were 78.3% - 103. 6% ,with the intra-day precision less than 12% and the inter-day precision less than 15%. The method is reliable,sensitive,reproducible,and adapts to the determination of the appetite suppressants in different weight-loss functional foods.
