Objective To evaluate the value of low-dose lung screening with 16-slice CT scanner in physical examination.Methods Lung screening was performed in 114 cases with a 16-slice spiral CT scanner.The scan parameters were 120KV,20~50mA.The reconstructive modes were standard mode and lung mode.Results 41 cases with lung diseases were detected with low-dose 16-slice spiral CT.Conclusion Low-dose 16-slice spiral CT is good for lung scanning in physical examination.
2.Effects of sinomenine on IL- 1?, LL-8 gene expression of human peripheral blood

Objective:To study the anti-inflammatory and anti-rheumatic mechanisms of sinomenine, a pure alkaloid extracted from the Chinese medical plant sinomeniuim acutum, on cytokines-related genes modulation. Methods: Cultured normal human peripheral blood mononuclear cytes(PBMIN) with sinomenine in vitro, isolated total RNA from different groups of treated and controlled cells to investigate the effects on mRNA expression of IL-1?, LL-8 with LPS stimulation by one-step and two-step RT-PCR techniques. Results: The results from the one-step RTPCR showed that the inhibition ratio on IL-l?mRNA expression in the group of mmol/L sinomenine was about( 15.7?5.52) % ; with two-step RT-PCR techneque demonstrated that the inhibition ratio of sinomenine in the group of sinomenine with the concentration of 1 mmol/L, 100 umol/L on IL-1?mRNA expression were ahout( 17.07 ? 7 .69)% ,(25 .99 ? .84)% respectively and the inhitition ratio in the group with sinomenine concentration of 1 mmol/L,100umol/L on IL-8mRNA expression were(2l.01 ?7.79)%,(16.04?7.55)% respectively .Conclusion: The therapeutic effects of sinomenine on rheumatoid arthritis(RA) may partially related to downregulating gene expression of IL-1?, IL-8 cytokines of the peripheral blood mononuclear cells.
3.Changes of hemodynamics in patients with severe hepatitis treated with molecular adsorbents recirculating system

Objective To evaluate the chang es of hemodynamics and the relationship with the outcome in patients with severe h epatitis treated with molecular adsorbents recirculating system (MARS). Methods A total of 16 patients with severe hepatitis were t reated with MARS for 36 times altogether as the treatment group. Twenty patients with severe hepatitis were treated by comprehensive methods as the control grou p. The age, sex and liver function between the two groups matched. Their prognos is was observed and the levels of serum total bilirubin, the non-conjugated bil irubin and mean arterial pressure (MAP), and heart rate were observed in the pat ients during a single 6 h to 8 h treatment with MARS. The relationship between the change and prognosis was observed. Results There were significant differences in prognosis and complications between the two groups. Objective:To investigate the role of reactive oxygen species (ROS) in the apoptosis of human hepatocellular carcinoma HepG_2 cells induced by PIG11 protein overexpression. Methods:The fluorescent probe 2', 7'-dichlorofluorescin diacetate (DCFH-DA) was used to label the intracellular ROS in pLXSN-PIG11-HepG_2 cells,pLXSN-HepG_2 cells and HepG_2 cells.The intracellular fluorescence intensity was detected by using the flow cytometry (FCM). The apoptotic ratio of HepG_2 was determined by FCM after elimination of intracellular ROS with 10 mmol/L N-acetylcystine (NAC). Results:The intracellular content of ROS in pLXSN-PIG11-HepG_2 cells was significantly higher than that in pLXSN-HepG_2 cells[(15.60±0.92) vs (4.90±0.70), P<0.01)]. The apoptotic ratio of pLXSN-PIG11-HepG_2 cells was significantly decreased by pretreatment with 10 mmol/L NAC (P<0.01). Conclusion:The apoptosis-inducing effect of PIG11 over-expression is related with elevation of intracellular ROS levels.
5.Studies on the Change of the Content of Polysaccharides in Crude and Processed Fructus Corni

Objective To investigate the change of the content of polysaccharides in crude and processed Fructus Corni,thus to approach the process mechanism of Fructus Corni.Methods The content of polysaccharides in the crude and processed Fructus Corni was determined by phenol-sulfuric acid method.Results After processing with wine,the content of polysaccharides of Fructus Corni decreased from 10.12 %(content of total polysaccharidesbeing 51.41 %) to 5.91 %(content of total polysaccharidesbeing 53.10 %),decreased by 41.60 %as compared with that in the crude Fructus Corni.Conclusion After processing with wine,the content of polysaccharides decreases markedly.The results provide certain evidence for approaching the process mechanism of Fructus Corni.
6.Visceral sensitivity, gut barrier function and autonomic nerve function in patients with diarrhea-predominant irritable bowel syndrome

Objective:To evaluate visceral sensitivity,gut barrier function and autonomic nerve function (ANF) in patients with diarrhea-predominant irritable bowel syndrome (IBS-D),and to explore their roles in IBS-D pathophysiology.Methods:A total of 46 IBS-D patients (IBS-D group) were selected from the Department of Gastroenterology of China-Japan Friendship Hospital from October 2015 to March 2016,and 20 healthy volunteer were served as a control group (HC group).Clinical and psychological symptoms were evaluated by questionnaire,and visceral sensitivity to rectal balloon distention,gut barrier function and autonomic nerve function (ANF) were examined.The difference in the abovementioned indexes were compared between the 2 groups,and the correlations in the parameters were analyzed in the IBS-D group.Results:The scores of IBS symptom severity scale (IBS-SSS),Hamilton anxiety scale (HAMA),Hamilton depression scale (HAMD) and visceral sensitivity index (VSI) were significantly higher in the IBS-D group than those in the HC group (P<0.01).In the visceral sensitivity test,maximum tolerable threshold in the IBS-D group was significantly decreased compared to that in the HC group (P<0.01);there was no significant difference in first sensation threshold and defecating sensation threshold between the two groups (P>0.05).As gut barrier function markers,the serum diamine oxidase (DAO) and d-lactate were significantly increased in the IBS-D group (P<0.05).In ANF test,the total score and parasympathetic score as well as the proportion of abnormal scores in the IBS-D group were significantly higher than those in the HC group (P<0.05).In IBS-D group,the HAMA,VSI and serum DAO were positively correlated with IBS-SSS (r=0.528,0.575,0.507;P<0.01),while the 3 visceral sensitivity thresholds were negatively correlated with IBS-SSS (r=-0.636,-0.476,-0.697;P<0.01);in addition to the IBS-SSS,the HAMA,HAMD,VSI and serum DAO were also significant negatively correlated with the visceral sensitivity thresholds (all P<0.05);no significant correlations were found between the ANF and the other parameters.Conclusion:IBS-D patients show psychological symptoms,visceral hypersensitivity,impaired gut barrier function and abnormal ANF characterized by parasympathetic dysfunction;the former 3 factors are all associated with disease severity,and thus may play vital roles in IBS-D pathophysiology.
7.Study on the Hypoglycemic Effect of Insulin Self-microemulsion for Parenteral Administration

OBJECTIVE:To study the hypoglycemic effects of Insulin self-microemulsion for parenteral administration on mod-el rats with type 1 diabetes in vivo. METHODS:Rats were treated with streptozotocin(50 mg/kg)to reproduce model with type 1 diabetes,ip. The model rats were randomly divided into model group (normal saline),positive control group (Insulin injection 2.25 u/kg) and self-microemulsion low,medium and high dose groups (Insulin self-microemulsion 4.5,9 and 18 u/kg);and 10 normal rats were involved in sham-operation group (normal saline). Anesthesia and operation were conducted for all rats. Positive control group was administrated,ip;other rats were parenterally administrated. The blood glucose levels in groups were detected be-fore and after 15-600 min administration. Glucose tolerance test was conducted for the rats in normal control group,model group without glucose,model group with glucose and microemulsion group (Insulin self-microemulsion 9 u/kg). All group were given glucose except model group without glucose. RESULTS:Compared with sham-operation group,the blood glucose levels in model group within 0-240 min were increased,with significant difference(P<0.05). Compared with model group,there was hypoglyce-mic trend in positive control group after 15 min,the blood glucose levels within 30-480 min were decreased,the hypoglycemic peak was 36%,and the peak time was 30 min;there was also hypoglycemic trend in microemulsion low,medium and high dose groups after 30 min,the blood glucose levels within 45-360 min were decreased,hypoglycemic peak was 18%-21%,and the peak time was 90-120 min,with significant differences (P<0.01 or P<0.05). All rats had glucose absorption peak except for model group without glucose in glucose tolerance test,and glucose of rats in microemulsion group reached its peak and then quickly de-creased. CONCLUSIONS:Insulin self-microemulsion can obviously reduce the blood glucose of model rats with type 1 diabetes.
8.Clinical study of congenital central hypoventilation syndrome in four cases

Objective This study aimed to explore the clinical characteristics and treatment of congenital central hypoventilation syndrome(CCHS),to raise awareness on the CCHS for early diagnosis and treatment.Methods The clinical data of 4 babies with CCHS were analyzed,all of the babies were repeated bruising,hypercapnia,and difficult-to-wean.After the relevant checks,we precluded the primary diseases of heart,lung and neuromuscular dysfunction that lead to low ventilation,and detected the major pathogenic genes,Phox2b of CCHS,refer to literature and diagnostic criteria of CCHS.Results Four babies all had typical clinical features of CCHS:enough ventilation when awake.But when they were asleep,their respiratory rate slowed down,hypoventilation,cyanosis and hypercapnia appeared,and they can not be awaked by hypercapnia and hypoxemia caused by hypoventilation.Genetic testing confirmed the presence of Phox2b mutations.Two infants were supported by non-invasive biphasic positive airway pressure(BiPAP),an infant was discharged home at 3 months of age with non-invasive BiPAP,and an infant was discharged home at 1 month of age.Both of them were monitored and treated at home,and showed normal development.Conclusion For babies who were repeated bruising,hypercapnia,and difficult-to-wean,but no primary diseases of heart,lung and neuromuscular dysfunction that lead to low ventilation,CCHS must be considered.Genetic testing of Phox2b can be used as an important diagnostic tool,and non-invasive BiPAP is one of the efficacious methods in the treatment of CCHS.
9.Standardized Management System and Process for Teaching Ward Rounds of Clinical Practice of Medical Students

Objective To establish a standardized management system and process for teaching ward rounds of clinical practice,so as to improve the quality of clinical practice. Methods (1) We established the system and process for teaching ward rounds. (2) The students of 2008 grade were divided into two groups. The new system and process for teaching ward rounds was used in 71 students in the experimental group,and the primary teaching ward rounds system was used in 72 students in control group. (3) A survey concerned about students' abilities of grasping theoretical knowledge, clinical thinking, self-study, clinical operation and doctor-patient communication were investigated in both students and their tutors. Result The result showed that the abilities of grasping theoretical knowledge,clinical thinking,self-study,clinical operation and doctor-patient communication in the experimental group were enhanced much more than the control group ( <0.01) . Conclusion The standardized management system and process for teaching ward rounds of clinical practice can improve the students, comprehensive abilities and the quality of clinical practice.
10.Comparative study on outcomes of 16-slice spiral CT urography and CR excretory urogaphy

Objective To discuss the value of 16-slice spiral CT urography in clinical diagnosis.Methods To compare the images between CR excretory urography,CT and MPR,which are the results of 26 cases who were examined by both CR and 16-slice spiral CT urography.Results From the comparison,it is found that the spiral CT urography images are better than that of the CR excertory urography,and at the same time,the relationship between the parenchyma of the renal,renal pelvis,ureter,bladder and the urinary organs and adjacent structures can be demonstrated clearly.Conclusion The scanning time of the spiral CT is shorter.The machine can not only reconstruct powerfully but also work out better images,and the result is better than CR excretory urography which can not detect some details in its images.Therefore CR excretory urogaphy can be replaced by 16-slice spiral CT urography regardless of its cost.
