2.Value of immunohistochemistry for determining the primary site of adenocarcinoma cells in pleural effusion cell block

Objective To explore the clinical significance of immunohistochemistry (IHC) for determining the primary site of adenocarcinoma cells in pleural effusion cell block. Methods One hundred and eight cases of pleural effusion diagnosed as adenocarcinoma were collected by using liquid-based cytology from January 2013 to June 2016 in Yuncheng Central Hospital. Sediment cells were gathered through centrifuging and paraffin block was made, then 94 cases were selected for IHC detection. Results Among 94 cases of adenocarcinoma diagnosed by using liquid-based cytology, 92 cases were still diagnosed as adenocarcinoma by IHC, including 70 cases of lung source, 2 cases of breast, 6 cases of ovary, 8 cases of alimentary canal, 1 case of peritoneal serouspapillary carcinoma, and 5 cases of unknown source. The other 2 cases were diagnosed as mesothelial cells and mesothelial tumors respectively. Conclusion Pleural effusion cells block and multiple IHC can improve the tumor cell detection for the judgement of the benign and malignant tumor cells and differential diagnosis to find the primary site of tumors.
3.Relationship of adiponectin and brachial ankle pulse wave velocity in type 2 diabetes mellitus patients combined with hypertension

Objective To explore the relationship of adiponectin and brachial ankle pulse wave velocity (baPWV) in type 2 diabetes mellitus patients combined with hypertension.Methods Fifty-six patients with diabetes mellitus,69 diabetes mellitus patients combined with hypertension and 78 healthy people were recruited in our study who visited doctors in Fenyang Hospital from March 2010 to March 2012.The levels of adiponectin,brachial ankle pulse wave velocity and other biochemical indicator were determined.Analyzed the correlation between adiponectin and baPWV among the three groups.Results The levels of adiponectin in control group,diabetes mellitus group,and diabetes mellitus combined with hypertension group were (13.9 ±4.2) mg/L,(9.1 ± 3.8) mg/L and (7.6 ± 2.4) mg/L respectively,and the difference was statistically significant (F =62.53,P <0.01).The levels of baPWV in control group,diabetes mellitus group and diabetes mellitus combined with hypertension group were (1 617.4 ± 198.2) cm/s,(2 010.9 ±231.7) cm/s and (2 196.1 ±218.6) cm/s respectively,and the difference was also statistically significant (F =139.41,P < 0.01).The correlation between adiponectin level and baPWV among three groups were-0.629,-0.561,and-0.528,respectively (P =0.027,0.034,0.029).Conclusion The levels of adiponectin decreased in diabetes mellitus patients combined with hypertension,while the baPWV increased.There is a good correlation between adiponectin and baPWV.
4.Clinical significance of plasma α1-antichymotrypsin in children with asthma

Objective To study the role of α1-antichymotrypsin(α1-ACT)in the pathogenesis of childhood asthma for providing a theoretical base for prevention and treatment of childhood asthma. Methods Levels of plasma α1-ACT in 30 asthmatic children and 30 healthy children were determined by rocket immunoelectrophoresis. Results Plasma α1-ACT levels were higher in acute phase than in paroxysmal phase [(0.4178±0.0815)vs(0.2865±0.0799), t=6.301,P<0.001] and also higher in asthma group than in control group [(0.4178±0.0815)vs(0.2876±0.0496),t=7.474,P<0.001]. The plasma α1-ACT levels in asthma group and control of paroxysmal phase were similor [(0.2865±0.0799) vs (0.2876±0.0496),t=0.064, P>0.05]. The levels of plasma α1-ACT were higher in moderate and heavily attacked patients than that in mild attacked patients [(0.4553±0.0758) vs (0.3249±0.0926),t=4.187,P<0.001] and those in control group [(0.4553±0.0758) vs (0.2865±0.0799),t=9.38,P<0.001]. Conclusion α1-ACT is involved in pathophysiological course of childhood asthma. Measurement of plasma α1-ACT levels may be helpful in determining the state of the illness and to evaluate the severity and treatment of childhood asthma.
5.Protective effects of pomegranate glycosides on LPS-induced acute lung injury to mice

Objective:To investigate the protective effect of punicalagin in LPS induced ALI.Methods: ALI mice model was induced by intraperitoneal injection of LSP(20 mg/kg bodyweight) to induce acute lung injury in mice.A total of 60 mice were divided into 6 groups,10 rats in each group:normal control group,LPS group,LPS+punicalagin group (10,20,40 mg/kg bodyweight),LPS+dexamethasone group.2 h before LPS injection,punicalagin and dexamethasone were intraperitoneal injection in mice,and then by intra-peritoneal injection of a lethal dose of LPS, normal control group mice were given intraperitoneal injection of an equal volume of PBS.HE staining,observed the lung tissue of mice in the different treatment conditions of the pathological changes and the degree of lung injury,weighted the wet lung and dry lung of the mice respectively, observed degree of pulmonary edema in mice, inspect MPO activity of lung homogenate, analyzed the activation and infiltration of polymorphonuclear, counted the total number of cells and the number of inflammatory cells in alveolar lavage fluid, detected the concentration of inflammatory cytokines in bronchoalveolar lavage fluid with ELISA,and analyzed the impact of punicalagin on inflammation of lung tissue.Results: HE staining of lung tissue in mice and found that the LPS-induced lung inflammatory cell infiltration, interstitial edema and pulmonary parenchymal damage were significantly reduced.Different dose of punicalagin could significantly reduce the lung wet/dry lung weight ratio of ALI in mice ( P<0.05).Punicalagin ALI could reduce lung tissue caused exudation,edema.The different doses of punicalagin could significantly reduce the inflammatory cells in bronchoalveolar lavage fluid,the sum of the number of neutrophils and macrophages (P<0.05).Punicalagin could significantly reduce the concentration in the bronchoalveolar lavage cytokines IL-6,IL-12,TNF-αand IL-1β(P<0.05).The MPO activity was detected that punicalagin reduce neutrophil infiltration induced by LPS in the lung tissue.Conclusion: Punicalagin has protective effects on LPS induced acute lung injury.
6.Effects of Anisodamine Combined with Oxytocin on Labor Process of Lying-in Women with Prolonged Active Phase in the First Labor Process and Related Indicators of Newborn

OBJECTIVE:To investigate the effects of anisodamine combined with oxytocin on labor process of lying-in women with prolonged active phase in the first labor process and related indicators of newborns. METHODS:216 lying-in women with pro-longed active phase in the first labor process were randomly divided into control group(108 cases)and observation group(108 cas-es). All lying-in women were artificially ruptured,based on it,control group was given 2.5 u Oxytocin for injection,adding into 500 ml 5% Glucose injection by intravenous infusion,the drip rate was appropriately adjusted based on contractions;observation group was additionally given 10 mg raceanisodamine hydrochloride,adding into 10 ml 5% Glucose injection by intravenous infu-sion by 5 min slow intravenous injection. The fetal heart was warded to closely observe the labor process during medication. The cervical maturity,labor process time,delivery mode and postpartum situation of lying-in women,Apgar score,body mass and inci-dence of adverse reactions of newborn in 2 groups were observed. RESULTS:The lying-in women cases with no less than 9 cervi-cal maturity,natural delivery rate and 5 min newborn Apgar score in observation group were significantly higher than control group, lying-in women cases with 7-9 scores and less than 7 scores,cesarean section rate,perineal/ straight cut rate and 24 h postpartum hemorrhage were significantly lower than control group,the second,the third and total labor process were significantly shorter than control group,the differences were statistically significant(P<0.05);there were no significant differences in the time of incubation period and active period,forceps delivery rate,body mass of newborn and incidence of adverse reactions in 2 groups (P>0.05). CONCLUSIONS:Anisodamine combined with oxytocin can accelerate the cervical dilation of lying-in women with pro-longed active phase,shorten labor process,reduce cesarean section rate and improve prognosis,it did not affect the body mass of newborns,with good safety.
7.Effect of Joint Commission International Accreditation on Pharmaceutical Care in Pharmacy Intravenous Admixture Service

Objective To explore the role of Joint Commission International(JCI) standards on the improvement of pharmaceutical care in pharmacy intravenous admixture service (PIVAS) during the JCI accreditation.Methods The changes of PIVAS in Huangshi Central Hospital were analyzed and compared during JCI accreditation in respect of facilities,software management,service processes,safety awareness of the PIVAS staff and pharmacy management,service evaluation,error rate,etc.Results After JCI accreditation,the process of PIVAS were optimized,the software were updated ,and the safety awareness of PIVAS staff was improved;the management of high alert drugs was implemented more strictly;more importance was attached to the details of pharmaceutical care;the satisfaction degree of the clinic units to PIVAS was increased from 87.0% in 2014 to 98.0% in 2015;error rate was decreased from 0.40‰ in 2014 to 0.15‰ in 2015.The incidence rate of defective product was decreased from 0.32‰ in 2014 to 0.07‰ in March 2015.The correct rate of medical waste classification treatment was increased from about 85.5% in 2014 to 100.0% in 2015.Hand hygiene compliance rate and correct rate were increased up to 100.0%.6S site management activity participation rate was increased up to 100.0%.Conclusion The application of JCI standards and the process of JCI accreditation improve the quality of pharmaceutical care in hospital PIVAS.It can improve the medical safety in hospital.
8.The clinical observation of percutaneous nephrolithotomy holmium laser lithotripsy on 397 cases of kidney stones

Objective To investigate the clinical efficacy and safety of kidney stones minimally invasive percutaneous nephrolithotomy holmium laser lithotripsy.Methods 397 cases patients with kidney stones were randomly divided into the control group and the experimental group,the control group were treated with the extracorporeal shock wave lithotripsy treatment,and the later group were treated with holmium laser lithotripsy for percutaneous nephrolithotomy.Results The experimental group of taking the net rate was 97.48%,and the time of taking the net rate was 87.15 %,higher than those of the control group(x2 =14.72,24.78,all P < 0.05).Compared with the control group,the shortened operation time,(t =2.36,P < 0.05),the reducing amount of bleeding (t =3.27,P < 0.05),the shortened length of stay hospital (t =1.28,P < 0.05),low complication rate (x2 =17.06,P < 0.05) took place in the experimental group.Conclusion Minimally invasive percutaneous nephrolithotomy holmium laser lithotripsy treatment of kidney stones is safe and effective,fewer complications,faster recovery,which is worthy of promotion.

Obesity is a severely public health problem the whole society faces, and it is correlated closely with many diseases, such as diabetesⅡ, hypertension, coronary heart disease,gallstone, and so on. Therefore it threatens people's survival quality severely. Obesity is a multiple-factor disease including genetic, metabolic and behavioral factor, and the gene is the main determining factor. With the development of molecular biology technique, people have founded several genes involved in obesity. Among these genes, the research on obese gene is the most profound. The protein leptin is the expression product of the obese gene. This review elucidates the structure, the main biological function, the mechanism of leptin and it's relationship with obesity.
10.Advances on intrauterine infection and pulmonary disease in premature infants

Intrauterine infection is one of the most important cause of adverse outcomes in pulmonary diseases among preterm infants.The lung tissues of such infants are still immature, whose contact with pathogens when exposed to intrauterine inflammatory environment may trigger local or even systemic inflammatory response, causing different degrees of harm to fetal or neonatal lung tissue.Such damage could lead to bronchopulmonary dysplasia in preterm infants, neonatal pneumonia, neonatal respiratory distress syndrome, etc., affecting their quality of life.This review is about the research progress and the pathogenesis of intrauterine infection in pulmonary diseases of premature infants.
