1.Optimization of Extraction Process of Yazhoukang Capsule by Orthogonal Experiment

Objective To optimize the preparing procedures of water extraction and alcohol precipitation of Yazhoukang Capsule. Methods The ratio of dry extraction and the content of paeoniflorin were used as indexes, orthogonal test was used for studying the influence on the process of four factors including decocting times, amount of water, decocting times, precipitating concentration of alcohol. Results The optimized preparation conditions were added 16 times water, decocting for 3 times, 1 h for each time and alcohol concentration of 40%. Conclusion The established method is suitable for extracting Yazhoukang Capsules preparation.
2.Perioperative nursing of acute intracranial arterial occlusion treated with mechanical thrombectomy with Solitaire AB stent

Objective To summarize the perioperative nursing of acute intracranial arterial occlusion treated with mechanical thrombectomy with Solitaire AB stent.Methods Fifty-three cases of acute intracranial arterial occlusion were treated with mechanical thrombectomy with Solitaire AB stent.Before the operation,emergency green passage was established and after the operation,the nursing measures included close observation of conditions,intensified basic nursing,control and management of blood pressure and medication,prevention from the complications by reperfusion injury and early rehabilitative training.Results The mechanical thrombectomy with Solitaire AB stent was successful for all the 53 patients.The occlusions in the blood vessels of 44 cases were fully removed and those in 9 cases were partially removed,3 treated with stent for stenosis.Conclusion The nursing points for acute intracranial arterial occlusion treated with mechanical thrombectomy with Solitaire AB stent include preoperative effective preparation, close observation of the disease conditions such as changes in their consciousness and blood pressure,prevention from reperfusion injury and early rehabilitative training.
3.Study on the anti-tumor effects of the sonicate sensitized DC vaccine in vitro

Objective To explore the cytotoxicity of the cytotoxic T lymphocyte (CTL) induced by SW480 sonicate sensitized dendritic cells (DC) on the colon cancer cell line SW480. Methods PBMC were separated from the HLA-A*0201 donor and DC were cultured with rhGM-CSF, rhIL-4 and rhTNF-α. The same donor's primary CTL were stimulated by DC loaded with SW480 sonicate. The cytotoxicity of CTL on SW480 (HLA-A*0201 positive) and K562 (HLA-A*0201 negative) was determined by the MTT method. Results The cytotoxicity of the CTL on SW480 was stronger than that on K562 (P <0.05). Conclusion The DC vaccine can stimulate specific CTL which can trigger cytotoxic activity on the target cells and this cytotoxicity is related to MHC restriction.
4.Extraintestinal Manifestations of Ulcerative Colitis

Background:Ulcerative colitis(UC)is often accompanied by a variety of extraintestinal manifestations. Recently, the role of extraintestinal manifestations in diagnosis and treatment of UC has aroused widespread concern in clinical practice. Aims:To study the extraintestinal manifestations of patients with UC in order to elevate diagnosis level of UC. Methods:A total of 208 inpatients from June 2008 to September 2011 at Beijing Military General Hospital were enrolled. The extraintestinal manifestations of UC were retrospectively analyzed. Results:The ratio of male to female was 1. 14: 1, mean age was(41. 17 ± 13. 57)years,mean disease duration was(62. 6 ± 79. 4)months. Thirty-three patients had proctitis,52 had left-sided colitis,and 123 had extensive colitis. Forty-nine patients were mild UC,80 were moderate UC, and 79 were severe UC. The incidence of extraintestinal manifestations was 28. 8%(60 / 208);13 patients(6. 3% )had more than one extraintestinal manifestation. The main extraintestinal manifestations were oral ulcer(13. 0% ),arthropathy (11. 1% ),hepatobiliary disease( 3. 8% ) and skin lesion( 1. 4% ). The trend of incidence of extraintestinal manifestations increased from patients with proctitis,left-sided colitis to extensive colitis(21. 2% ,28. 8% ,30. 9% , respectively),however,the difference was not statistically significant(P > 0. 05). No significant difference in incidence of extraintestinal manifestations in patients with mild UC,moderate UC and severe UC was found(22. 4% ,33. 8% ,27. 8% , respectively)(P > 0. 05). Conclusions:Patients with UC are often accompanied by extraintestinal manifestations,and the recognition of extraintestinal manifestations is helpful for improving diagnosis and treatment level of UC.
5.Effect of group management mode on compliance of pelvic floor function exercise for patients with post ;partum urinary incontinence

Objective To explore the effect of group management mode on compliance of pelvic floor function exercise for patients with post partum urinary incontinence (PPUI). Methods A total of 80 cases of patients with PPUI were randomly divided into the observation group and the control group. Patients in the control group received one-to-one routine health guidance. Patients in the observation group received group management mode including special subject teaching guidance, induction and communication between patients. Three months after the intervention, two groups of patients were evaluated at six months after postpartum by the pelvic floor muscle strength, one-hour urine pad test and pelvic floor muscle function exercise compliance. Results Six months after postpartum, the cure rate of the pelvic floor muscle strength was 100%(40/40) for typeⅠmuscle, 100%(40/40) for typeⅡmuscle in the observation group. In the control group, the cure rate for typeⅠmuscle was 70%(28/40) and 65%(26/40) for type Ⅱmuscle. The difference between these two groups was statistically significant (χ2=14.118, 16.970, P=0.000). The total effective rate of urinary incontinence was 100.0%(40/40) in the observation group, 67.5% (16/40) in the control group, and there was statistical significance (χ2=25.232, P=0.000). Evaluation of the compliance of pelvic floor function exercise showed that the rates were 72.5% (29/40) for complete compliance, 27.5%(11/40) for incomplete compliance and 0 for total non-compliance in the observation group. In the control group, these rates were 2.5%(1/40), 55.0%(22/40), 42.5%(17/40). And there was statistical significance as well (χ2=54.847, P=0.000). Conclusions Group management mode can improve the compliance of pelvic floor muscle function exercise and the strength of pelvic floor muscle, and improve the degree of urinary incontinence in postpartum patients with urinary incontinence.

By feeding rats with high-fat diet insulin resistant models were induced.Some of the models were treated with intragastric administration of curcumin,then the effects were evaluated by hyperinsulinaemic-euglycaemic clamp technique.Curcumin reduced the level of serum resistin and the expression of resistin mRNA in adipose tissue,and thus ameliorated insulin resistance in rats.
7.Prokaryotic expression of P gene from rabies virus and application of the indirect ELISA assay in the detection of its gene products

The complete length of P gene from rabies virus was amplified by RT-PCR using a pair of specific primers designed according to the relevant sequences from GenBank. The PCR product was cloned into cloning expression vestor pGM-T to obtain the cloning expressed plasmid pGM-T-P. After double-digestion by NotI and EcoRI, the product was transferred into prokaryotic expression vetor pET-32a(+)to obtain the prokaryotically expressed plasmid pET-32a-P. The target gene was then expressed in the E.coli BL21(DE3) cell with IPTG induction. The highest expression of target protein was analysed by SDS-PAGE, and the good immunoreactivity to rabies virus antibodies was proved by Western-blot analysis. By using purified protein, the indirect ELISA assay for the detection of rabies virus antibodies in canine serum was applied after management of the optional working condition.

The effects of targeted silencing of heparanase gene by small interfering RNA (siRNA) on invasiveness and metastasis of osteosarcoma cells (MG63 cells) were investigated in the present study. Two complementary oligonucleotide strands were synthesized and inserted into pGenesil-1 vector based on the mRNA sequence of heparanase gene. The expression vector containing short hairpin RNA (pGenesil-shRNA) was constructed successfully. MG63 cells were randomly allocated into 3 groups: blank group, empty vector (pGenesil) transfected group and expression vector (pGenesil-shRNA) transfected group. Under the induction of Lipofectamine 2000, the recombinants were transfected into MG63 cells. Heparanase gene expression level was detected by RT-PCR and Western blotting. Cell proliferation was measured by MTT assay. Cell invasiveness and metastasis were examined by cell adhesion and Transwell-ECM assays. HUVECs migration assay was applied for the detection of angiogenesis. As compared with negative controls, the mRNA and protein expression levels of heparanase were down-regulated by 76.1% (P<0.01) and 75.3% (P<0.01) respectively in the pGenesil-shRNA transfected group. Meanwhile, the proliferation, adhesiveness, invasiveness and angiogenesis properties of MG63 cells were all significantly inhibited. It was suggested that targeted silencing of heparanase gene by siRNA could dramatically inhibit the invasiveness and metastasis of osteosarcoma cells.

OBJECTIVE:
Through 3.0 T MRI study the ear and sinus lesions of patients with acute carbon monoxide poisoning.
METHOD:
From 2012 to 2015 collected the MRI images of the 45 patients with acute carbon monoxide poisoning, observe their changes of middle ear and mastoid and sinus imaging.
RESULT:
The middle ear injury of mastoid 41 cases (91.1%), 22 cases (48.9%) of maxillary sinus injury, ethmoid sinus injury in 20 cases (44.4%), sphenoid sinus 9 cases (20.0%), 5 cases (11.1%) of frontal sinus injury. Carbon monoxide poisoning patients according to clinical symptoms can be divided into light, medium and heavy 3 groups, observing the ear sinus damage degree for comparison between groups, found to have significant differences (P < 0.05).
CONCLUSION
The patients with acute carbon monoxide poisoning ear and sinus injury should cause the attention of the medical staff, MRI can reflect people's ears from the details and the damage degree of the sinuses.

10.Analysis of 278 breast biopsy cases guided by mammography

Objective To analyze 278 breast biopsy cases guided by mammography and find the characteristic of the most suitable cases.Methods The initial diagnosis of breast cancer of 278 cases were divided into mass type (197 cases) and non-mass type (81 cases) according to the imaging features,the biopsy was performed in 14G core needle guided by mammography and contrasted to paraffin pathology of post operation.Results The cases of biopsy conformed by paraffin pathology were 260,accuracy rate is 93.52 %(260/278).192 cases were diagnosed pathologically in 197 cases of mass type,and 68 cases in 81 cases of non-mass type.The accuracy rates were 97.46 % (192/197) and 83.95 % (68/81) respectively.Conclusion The mass type with clinical and imaging features is suited for biopsy.To avoid missed diagnosis,it is important to frozen during operation for the cases with the negative result in non-mass type.
