1.Using the pathology report in initial treatment decisions for diffuse large B-cell lymphoma in the era of precision medicine:reports from the 57th American Society of Hematology annual meeting

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma in the Western world,and is potentially curable with standard R-CHOP chemoimmunotherapy. We are now in an era that the heterogeneity of DLBCL is defined genetically and molecularly,and rational subset-specific therapeutic targets are guiding clinical trials.Primary mediastinal DLBCL is a unique clinicopathologic entity, and alternatives to R-CHOP may confer superior outcome. Rearrangement of the myc oncogene occurs in 10%of patients with DLBCL, and confers a very poor prognosis with standard R-CHOP, particularly when there is concomitant rearrangement of bcl-2, a condition referred to as double-hit DLBCL. A larger subset of DLBCL demonstrates overexpression of both myc and bcl-2 by immunohistochemistry. Analyze the source of cells by gene expression profile, immunohistochemistry algorithms,or a novel Lymph2Cx platform,provides prognostic information, and guides therapeutic decisions in both relapsed and de novo disease. This article reviews latest research presented at the 57th American Society of Hematology (ASH) annual meeting on the definition of specific subsets of DLBCL and selection of subtype-specific treatment,including novel approaches under investigation. Understanding these key features of the pathology report, and limitations of these assays defining subsets of DLBCL, allows for a precision medicine approach to this disease.

Chelation therapy could remove transfusional iron burden. Three chelators are currently available, including deferoxamine, deferiprone, and deferasirox, which can be used as monotherapy or in combination.Several factors must be considered in the design of optimal and individualized chelation regimens,which include chelator availability and its properties,degree of organ-specific iron loading,ongoing transfusional iron burden, and patient preference. Comparative effectiveness trials may help to determine the ideal strategy. This article reviews latest research presented at the 53rd annual meeting of the American Society of Hematology (ASH) on the use of iron chelators.
3.The hemophilias and their clinical management: reports in the 55th ASH annual meeting

Outcomes for patients with hemophilia have improved dramatically over the past 50 years.With the increased availability of safe clotting factor concentrates,the primary focus in clinical management is now the prevention of long-term complications,most notably the debilitating hemophilic arthropathy that is associated with severe disease.Definitive evidence of improved clinical results from primary prophylaxis started in young patients-with severe hemophilia A and a minimal bleeding history is presented.Furthermore,recent studies showing benefits for initiating prophylaxis in older adolescents and adults with established joint disease are examined.lnhibitors to factor Ⅷ are the most problematic complication of factor replacement therapy.Patient-specific and treatment-related factors that contribute to the risk of inhibitor formation are discussed and controversies and clinical evidence related to approaches for tolerance induction are reviewed.Immune tolerance induction is the proven method for eradication of inhibitors.This article reviews latest research presented at the 55th ASH annual meeting on the hemophilias and their clinical management.
4.Computing thinking-based teaching reform of computer network course

Computing thinking is one of the hotspots in computer education field,computer network is the core course of computer,and social information process is increasing the need of computer network professionals.The characteristics of computer network course,teaching of computer network course and problems related with teaching of computer network course were analyzed in aspects of its teaching contents,teaching methods and examination methods in order to train more excellent applied professionals of computer network and improve the computing thinking of undergraduates.The ways or methods for training the computing thinking of undergraduates were put forward,which can thus provide new ideas for the teaching reform of computer network course in colleges and universities.
5.Suppression of MDR1 gene by RNA interference in multidrug-resistant cancer cells

AIM: To explore the feasibility of using vector-based small interfereing RNA(siRNA) to inhibit the expression of MDR1 mRNA and P-glycoprotein and to reverse the multidrug resistance of drug-resistant ovarian cancer cell line.METHODS: An adriamycin-resistant human ovarian cancer cell subline OVCAR/AR was established by stepwise inducement.Another mutidrug-resistant human ovarian cancer cell subline OVCAR/MDR was established by transfecting multidrug resistant gene 1(MDR1) into ovarian carcinoma cell line OVCAR-3.Transfection of MDR1 mRNA specific siRNA expressing plasmids(pSN/mdr1a and pSN/mdr1b) into OVCAR/AR and OVCAR/MDR cells was performed using liposome transfection reagents.MDR1 mRNA expression level was quantified using real time reverse transcription polymerase chain reaction(real time RT-PCR).Flow cytometry(FCM) was performed to assess the expression of p-glycoprotein(P-gp).Multidrug resistant to anticancer agents was evaluated by 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide(MTT) assay.RESULTS: Basal MDR1 mRNA expression level in drug-resistant cell line OVCAR/MDR was higher than that in OVCAR/AR cell line,and was both higher than that in its parent cell line OVCAR-3.The expression of MDR1 mRNA and P-gp protein in both OVCAR/AR and OVCAR/MDR cells was inhibited dramatically by transfecting with pSN/mdr1a and pSN/mdr1b.The reversal rate of chemoresistance to adriamycin in OVCAR/AR and OVCAR/MDR transfected with pSN/mdr1a and pSN/mdr1b was 79.5% and 93.9%,compared with control group.CONCLUSION: MDR1 expression in the drug-resistant cell lines is partially inhibited by treatment with vector-based MDR1 specific small interference RNAs at the mRNA and protein level,which increases the chemotherapy sensitivity of these drug resistant ovarian carcinoma cell sublines.
6.Efficiency analysis of the first time platelet transfusion in patients with blood diseases

Objective To study the efficiency of the first time platelet transfusion in patients with blood diseases.Methods Investigate the effect of platelet transfusion in 87 patients with blood diseases.All the patients were transfused 10U platelet (platelet≥2.5×1011 ).Observe CCI after platelet transfusion and analyse the influence factors to the efficiency of the first time platelet transfusion such as the kind of diseases,splenomegaly,fever,age and sex.Results After platelet transfusion,66 patients (75.9 %) were effective.The efficancy rates were 80.0 %,78.1%,65.2 %,82.3 % in AA,ITP,AL,MDS (x2=2.88,P>0.05).In 31 patients with a fever and infaction the efficancy rate was 58.1%,but it was 85.7% in 56 patients without a fever and infaction (x2=8.3308,P<0.01).The efficancy rate in 59 patients with non-splenomegaly was 86.4 %.It was higher than that in 28 patients with splenomegaly which was 53.6%(x2=11.2033,P<0.01).The efficancy rate was 76.0% in male patients and it was 75.7 % in female patients (x2=- 0.0012,P>0.05).The efficancy rate was 74.5 % in young group (<60 years old)and 77.8 % in old group (≥60 years old)(x2=0.1231,P>0.05).Conclusion Fever and splenomegaly are the influence factors to the efficiency of the first time platelet transfusion.
7.Clinicopathological study of cutaneous anaplastic large cell lymphoma complicated with acute myeloid leukemia

Objective To explore the morphological, immunohistochemical characters and prognosis in one case of patients with cutaneous anaplastic large cell lymphoma complicated with acute myeloid leukemia (C-ALCL-AML). Methods The histopathology, immunohistochemical markers and follow-up information of one case of ALCL-AML was analyzed and the correlated literature was reviewed. Results The patient, 69 year-old, female, was initially present with shin lesion on one finger and abnormal myelogram. The histopathology of shin lesion showed that tumor cells were composed of large cells with abundant cytoplasm,the nuclei were large and irregular, and were infiltrated by Neutrophil and eosinophil. The CD30,CD3 and CD43 of tumor cells were positive, but ALK negative by immunohistochemical method. The number of WBC in peripheral blood was 15.5×109/L and 51 archaeocytes were in every 100 karyotes. Bone marrow aspiration detection showed that bone marrow was hyperplasia and the ratio of myeloblast was 78 %. This patient was diagnosed as C-ALCL-AML, partly differentiation type(AML-M2a). Conclusion C-ALCL-AML is very rare. Its diagnosis is dependent on clinical data, histopathology and immunohistochemical markers. The first choice of treatment is chemotherapy, but its prognosis is poor.
8.The Measurement and Significance of DNA Content in Primary Lung Cancer

Objective: Our aims were to measure DNA content in primary lung cancer and to study the relationship between the DNA content and TNM stage, histological differentiation of tumor cell, cellular proliferation, and apoptosis. Methods: The DNA content and cellular proliferation were analyzed using flow cytometry. Tumor cell apoptosis was detected by using TUNEL method. Results: (1) The DNA index (DI) distribution ranged from 0.829 to 2.514. There were 41 cases (77.4%) of DNA aneuploid. The distribution of DI and DNA aneuploid was independent of histological subtypes(P>0.05).(2) With the increase of TNM stage, the DI and the rate of DNA aneuploid increased(P<0.05).(3) There was relationship between DI and histological differentiation of tumor cell. The DI was higher in tumors of poor differentiation than those in tumors of moderate and good differentiation(P<0.05 and P<0.01). (4) The cellular proliferation index of aneuploid tumors was significantly higher than that of diploid tumors(P<0.01), while apoptosis index of aneuploid tumors was significantly lower than that of diploid tumors (P<0.01). Conclusion: Correlations exist between DNA content and TNM stage, hiological differentiation, cellular proliferation, and apoptosis.
9.Design and application of cross contamination experiment for Beckman AU5800 automatic biochemical analyzer

Objective To design and verify an analytical method for the cross contamination of Beckman AU5800 automatic bio‐chemical analyzer′s reagent needle and stir bar .Methods The outer ring items (AST ,ALP ,TP ,LDH ,CK‐MB ,HBDH ,Urea ,UA , TG ,LDL‐C ,APOB ,Mg ,P ,Zn ,AMY ,CHE ,LPS ,ADA) were tested as the experiment objects .The cross contamination experiment was proceeded according to the test order .The impact rate range of 95% -105% was the criteria for cross contamination of items . Results Among all of the outer ring items ,TP ,HBDH ,APOB and TG were found to have cross contamination effect on CK‐MB , Mg ,Mg and LPS respectively .Conclusion The method established in this experiment can effectively analyze the cross contamina‐tion of all the items in Beckman AU5800 automatic biochemical analyzer .In addition ,it can help to adopt corresponding measures to reduce cross contamination and assure the accuracy of test results .

The dimension and factors for the assessment of the value of domestic and foreign academic li-braries were analyzed according to the assessment of the value of Shanxi Medical University Library, and the frame-work for the assessment of the value of academic libraries was proposed in terms of the impact of academic libraries on higher education.
