2.Establishment and evaluation of the review criteria of automatic urine analysis workstations

ObjectiveTo establish the proper review rules for the microscopic screening of urine samples tested by automatic urinalysis work station which is composed of LabUMat urine dry chemical analyzer and Urised urine sedimental analyzer.Methods The paired comparison was used to analyze the results tested by microscopy and Urised.A total of 2015 random urine samples were enrolled to establish and validate review rules.All the samples were collected from the inpatients and ontpatients of General Hospital of the People's Liberation Army from May to November 2011 and tested by urinalysis work station.2015 urine samples were firstly tested by urinalysis work station,including both urine dry chemical analyzer and urine sediments analyzer.Then each urine sample was examined microscopically by two technicians-in-charge using double-blind method.The average results from the two technicians were used as review results.Compared with review results,we set up the review rules and evaluated the Irue positive rate,false positive rate,true negative rate,false negative rate (omission diagnostic rate) and review rate.According to different test methods by automatic urinalysis work station,four microscopic review protocols were defined:( 1 ) Protocol 1:based on chemistry results only,microscopy review was performed when any of WBC,RBC,PRO and NIT was positive; (2) Protocol 2:based on urine sedimental analysis only,microscopy review was performed when any of WBC,RBC and CAST count was over upper limit of the reference range ; (3) Protocol 3:if any of BLD vs.RBC,LEU vs.WBC was different between two systems,or quantitative results had two or more than two gradient differences,microscopy review was performed; (4) Protocol 4:if any of BLD vs.RBC,LEU vs.WBC was different between two systems,or CAST was over upper limit of the reference range,or alam appeared,microscopic review was performed.300 randomly selected urine samples were tested to validate the review rules.Omission diagnostic rate and review rate were used to evaluate the rules.Results According to our review rules,the positive samples rate was 41.14% (829/2015) and the negative rate was 58.86% ( 1186/2015 ) ; Positive samples were composed of RBC (50.30%),WBC (53.32%) and CAST (3.74%).The review rates of four protocols were 42.93% (865/2015),39.70% (810/2015),29.58%(596/2015),18.91% (381/2015 ),respectively.The false negative rates (omission diagnostic rates) were 6.36% (128/2015),4.42% (89/2015),1.34% (27/2015)and 1.04% (21/2015)respectively.Protocol 4 was selected as an ideal plan.Additional 300 urine samples were tested using protocol 4 in order to confirm the review rule.The review rate,consistency rate,true positive rate,false positive rate,true negative rate,omission diagnostic rate were 19.67% (59/300),91.67% (275/300),35.67% (107/300),7.67%(23/300),56.00% (168/300),0.67% (2/300),respectively.After image review revised,the review rate was 8.67% (26/300).ConclusionThe review rules established by our research for Urinalysis Work Station can find the abnormal urine samples effectively and exactly and can reduce the workload significantly.(Chin J Lab Med,2012,35:810-814)
3.Evaluation of an exercise and dietary intervention for obesity and overweight migrant children

Objective To describe body mass index (BMI) changes in migrant children with overweight and obesity in Beijing treated by the exercise and dietary intervention programme.Methods BMI was assessed at before and after the intervention in 30 migrant obesity and overweight children aged 9 ~ 12 in two schools from March,2010.Results BMI reduced significantly one week after the intervention ( P < 0.05 ),but from two months to six months after the intervention BMI increased again to the pretreatment level( P < 0.05 ).Conclusion The intervention was successful in decreasing BMI,however maintenance of BMI after treatment was not easy.The controlling of obesity and overweight is a long term process which needs the cooperation and supervision of their parents.
4.Glucocorticoid-induced rat primary amygdaloid neuron apoptosis

Aim To investigate the role of GC in indu-cing apoptosis of amygdaloid neurons.Methods Cul-turing primary neurons of amygdala,the neurons were identified by immunefluorescence techniques with anti-body against microtubule associated protein-2 (MAP2 ) and antibody against GC receptor.Using flow cytome-try to detect the effects of different concentrations of dexamethasone on the amygdala neuron apoptosis. Then the experiment was divided into four groups:CON ,DEX ,DEX +MIF and MIF .The rate of apopto-sis of the four groups was detected by TUNEL tech-nique and the expressions of BAX mRNA of four groups by Real-time PCR technique.Results (1 )Compared with the control group, the percentage of apoptotic cells increased significantly with DEX(10 -8 mol·L-1~10 -6 mol · L-1 )treatment in a concentration-de-pendent manner.(2)the TUNEL test showed that the percentage of apoptotic cells of DEX group increased significantly,compared with control group.While it decreased significantly in DEX+MIF group,compared with DEX group.There was no difference between MIF group and control group.(3 )Compared with control group,the expressions of BAX mRNA of DEX group increased significantly.While the expressions of BAX mRNA of DEX +MIF group decreased significantly, compared with the DEX group.There was no difference between MIF group and control group.Conclusion GC can independently induce the apoptosis of primary cultured neurons in the amygdala by combining with GC receptor.
5.Steep pulse changes the expression of tissue factor in ovarian tumor.

As a micro-wound and target-aimed technology without special limitation, Electric Pulses have been widely researched in tumor treatment and the effects have been demonstrated by a series of experiments, yet the mechanism has not been explained clearly. In this experiment, energy controllable steep pulse (ECSP) was used to treat nude mice bearing human ovarian tumor, and the result was compared with that of the control group. The expression of an important coagulant factor-tissue factor (TF) was analyzed, as TF was also a tumor indicator of invasion and metastasis, the result may indicate the relationship among ECSP, thrombosis and tumor invasion. In this study, to shed light on the mechanism of tumor treatment in electrical fields, nude mice bearing ovarian tumors were randomly divided into the treated group and the untreated group. We treated the former group and took out the tumor instantly. The thrombosis and necrosis of ovarian tumor were observed under microscope. The expression of TF was analyzed by SP immunohistochemistry and RT-PCR. Lower level of TF expression was noticed in the tumor tissue treated by ECSP, and more apparent thrombosis was also seen in this group. The results make it clear that ECSP can accelerate thrombosis and consume coagulant factors such as TF, and that low expression of TF in tumor tissue can cut out the signal paths of tumor invasion. So it is suggested that ECSP may restrain tumor invasion and metastasis by modulating thrombosis.

6.Comparison of detection of SNP and STR in formalin-fixed tissue

Objective The purpose of this study was to detect the degradation degree of long-term formalin fixed tissue and to compare the detection rate of STR with SNP. Methods DNA was extracted from 24 formalin-fixed tissues stored at -20 ℃ for five years, and the concentration and degradation index of DNA was quantified with Quantifiler? Trio DNA Kit. A 55-SNP multiplex SNaPshot assay and PowerPlex? 21 system were used to amplify SNP and STR loci, respectively. Results The results showed that the degradation indexes of 24 specimens were ranged from 1~8. The SNP genotypes of the 24 specimens were completely consistent with the non-degraded DNA from the same individuals and the successful genotyping rate was 100%. However, 33 allele dropouts were observed with STR genotyping in 8 samples, of which the degradation index was higher than 2.6, and the fragment size of the 75.8% allele was longer than 300bp. The likelihood ratio based on 16 typable STR loci in the sample was close to that onthe basis of 54 SNPs. However, likelihood ratio based on more than 17 STR loci was over that accord to 54 SNPs. There was a negative correlation between the fragment size of STR and the allele detection rate, and a negative correlation also observed between the degradation index of samples and the allele detection rate except for two samples with mild degradation. Conclusion This study validated that the long-term formalin-fixed tissues were susceptible to degradation, and the SNP was more suitable for detecting these tissues than STR typing system. However more SNP loci are needed to test in order to increase the discrimination power.
7.Application progress of esketamine in gynecology and obstetrics during perioperative period

The volume of gynecological and obstetric surgeries is large,and the high rates of adverse events incidence such as perioperative cycle fluctuations,postoperative nausea and vomiting and adverse emotional experiences,which is a concern for clinicians.Esketamine is a right-handed isomer of ketamine with a strong affinity for the N-methyl-D-aspartate receptor(NMDAR),which has a rapid anesthetic,analgesic and antidepressant effect.It can be administered through multiple routes and has been widely used in clinical anesthesia,postoperative analgesia and depression treatment.The application of esketamine injection in clinical practice is still relatively short,and the benefits and mechanisms during perioperative application have not been fully elucidated,making it a research hotspot in recent years.Many studies have reported that the application of esketamine injection in obstetrics and gynecology surgery can stabilize circulation,and reduce respiratory depression,nausea and vomiting and other adverse reactions.It is also beneficial for the prevention and treatment of postpartum depression under certain conditions,but sometimes there are also temporary pseudo-psychiatric symptoms and other adverse reactions.In this article,we review the pharmacological effects,perioperative application and impact on postoperative recovery of esketamine based on the specialty characteristics of obstetrics and gynecology surgery,and clarify its current application status,as well as possible clinical applications and research directions in the future,to provide reference for the clinicians.
8. Mutational analysis of RNA splicing machinery genes SF3B1, U2AF1 and SRSF2 in 118 patients with myelodysplastic syndromes and related diseases

Objective:
To investigate the incidence, molecular features and clinical significance of RNA splicing machinery genes mutation in myelodysplastic syndromes (MDS) and related diseases.
Methods:
Mutational analysis of splicing factor 3B subunit 1 (SF3B1) (K700E) , U2 small nuclear RNA auxiliary factor 1 (U2AF1) (S34, Q157P) and serine/arginine-rich splicing factor 2 (SRSF2) (P95) in 118, de novo MDS and related diseases were separately performed by using polymerase chain reaction (PCR) followed by sequence analysis.
Results:
Of 118 MDS patients, 76 males and 42 females, the median age was 53.5 (13-84) years old. 19.49% (23/118) had SF3B1 (K700E) mutation. As compared with those with wild type SF3B1, patients with SF3B1 K700E were of older[58 (32-78) years
9.Relationship between inducible nitric oxide synthase and delayed encephalopathy in carbon monoxide poisoning

Objective:To observe the relationship between inducible carbon monoxide synthase (iNOS) and delayed encephalopathy after acute carbon monoxide poisoning (DEACMP), and explore its mechanism of action in DEACMP.Methods:This study was designed as prospective cohort study. Patients with acute carbon monoxide poisoning who met the diagnostic criteria and were admitted to Emergency Intensive Care Unit(EICU) of our hospital from June 2019 to June 2021 were selected as subjects. Patients were divided into the DEACMP group and non-DEACMP group according to the occurrence of DEACMP. Serum samples were collected on the first 24 h after admission and on day 7 and 14 after admission, and the serum nitric oxide (NO), neuronal nitric oxide synthase (nNOS), inducible carbon monoxide synthase (iNOS), and endothelial nitric oxide synthase (eNOS) level were measured by enzyme-linked immunosorbent assay. The generalized estimating equation was used to estimate the difference of NO, nNOS, iNOS and eNOS between DEACMP and non-DEACMP patients.Results:A total of 78 patients with carbon monoxide poisoning were included in our study finally, including 49 (62.82%) males and 29 (37.18%) females, with an average age of (53.96±14.95) years, 20 (25.64%) patients with DEACMP, and 1 (1.28%) death. Univariate analysis showed that patients with DEACMP had an average increase of 3 h (95% CI: 1.00, 5.00) in carbon monoxide exposure time and a 5-point decrease in GCS score (95% CI: 1.00, 6.00) than the patients without DEACMP, and the proportion of patients with severe carbon monoxide poisoning in the DEACMP group was higher than that of the non-DEACMP group (90.00% vs. 32.76%). According to the analysis of generalized estimation equation, on day 7 and 14 after admission, Compared with non-DEACMP patients, neither by performing unadjusted nor adjusted analysis with the iNOS of DEACMP patients was significantly higher than that in non-DEACMP patients regardless of whether exposure time, GCS score, coma time or severity of carbon monoxide poisoning were adjusted or not ( P <0.01 or P <0.05). Except for the level of nNOS in the GEE model adjusted with carbon monoxide exposure time, the levels of NO, nNOS and eNOS showed no significant difference between DEACMP and non-DEACMP patients ( P >0.05). Conclusions:The expression of iNOS level is increased in DEACMP patients, and its continuous expression may be involved in the pathogenesis of DEACMP.
10.Evaluation of urine free and fractionated catecholamine metabolites in the diagnosis of pheochromocytoma

Objective:The diagnostic values of urine free and fractionated catecholamine metabolites (including metanephrine MN and normetanephrine NMN, usually known as MNs) were established and their clinical value was evaluated.Methods:Using high performance liquid chromatography tandem mass spectrometry(HPLC-MS/MS), urine free MNs (f-MN, f-NMN) and fractionated MNs(t-MN, t-NMN) from 180 cases of non-pheochromocytoma and 54 cases of pheochromocytoma (PCC)patients were detected respectively.Receiver operating characteristic curve (ROC) was used to establish clinical reference cut-off value for different forms of MNs, and diagnostic efficacy of free, fractionated and total MNs was evaluated.Results:(1) The cut-off values of f-MN, f-NMN, t-MN and t-NMN were 47.8 μg/24 h, 52.3 μg/24 h, 224.9 μg/24 h and 664 μg/24 h, respectively. The cut-off values of total f-MNs and total t-MNs were 126 μg/24 h and 1 070 μg/24 h, respectively. (2) The correlation between f-MN and t-MN ( r=0.976, P<0.001), f-NMN and t-NMN ( r=0.940, P<0.001) was good. The area under ROC curve(AUC)of f-MN was lower than that of t-MN(0.579 vs 0.730, P<0.001), the sensitivity was slightly lower than that of t-MN((37.01% vs 51.85%, P=0.036), and the specificity was similar (99.44% vs 96.67%, P>0.05). There was no significant difference in sensitivity (90.74% vs 92.59%, P>0.05), specificity (99.44% vs 96.67%, P>0.05) and AUC (0.944 vs 0.959, P>0.05) between f-NMN and t-NMN. The combined diagnostic value of MN and NMN (total MNs) was higher than MN (free type:0.932>0.579, fractionation type: 0.960>0.730), which was similar to NMN. Conclusions:The diagnostic performance of urine free NMN or total MNs for PCC is similar to that of fractionated typewhich can meet the clinical needs.With few influencing factors, free type MNs may be used as an alternative indicator for PCC screening in the future.
