1.The effect of medium chain triglyceride on blood lipid and expression of leptin in myocardium of rats fed with high fat diet

Objective To investigate the effects of medium-chain triglyceride (MCT) on levels of blood lipids and leptin in myocardium tissue.Methods Thirty-two male Sprague-Dawley (SD) rats aged 4 weeks were divided into control group [n = 8, given normal diet prepared by American Nutrition Society (AIN93)], common high fat diet group [HFD group (n = 16), given 23% lard+ 5% corn oil], MCT group (n = 8, given 23% MCT+ 5% corn oil) by random number table, after 8 weeks of feeding, the HFD group was further randomly subdivided into HFD group fed with diet as before and (HFD+ MCT) group whose diet was replaced bythe diet in MCT group, each subgroup 8 rats, and all the rats continued to be fed for further 8 weeks. The rats' diet intake and the changes of their body weight were observed weekly. After total 16 weeks, the rat tail venous blood was collected, the levels of triglycerides (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C) in serum were measured; The expression of leptin mRNA in rat myocardium was measured by semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR), and the contents of leptin in myocardium tissue and serum were measured by enzyme-linked immunosorbent assay (ELISA).Results Compared with the control group, the difference in daily food intake quantity of any one of the high fat diet groups was not statistically significant (allP > 0.05); the final body weight, blood lipid level , the level of leptin mRNA expression in myocardium, contents of leptin in myocardium and serum in the HFD, MCT, and HFD+ MCT groups were all significantly higher, the increase was the most pronounced in the HFD group [body weight (g): 395±13 vs. 326±10, TG (mmol/L): 2.01±0.06 vs. 1.46±0.07, TC (mmol/L): 2.67±0.12 vs. 1.62±0.06, LDL-C (mmol/L): 1.72±0.12 vs. 1.10±0.07, HDL-C (mmol/L): 2.07±0.03 vs. 1.56±0.05, mRNA espression (gray value): 0.357±0.023 vs. 0.202±0.004, leptin in myocardium (pg/mg): 19.30±2.81 vs. 11.50±1.40, leptin in serum (μg/L): 14.63±4.50 vs. 8.21±2.98, allP < 0.05], and the least increase was in the MCT group [respectively was (363±9) g, (1.65±0.07) mmol/L, (2.30±0.06) mmol/L, (1.38±0.05) mmol/L, (1.85±0.04) mmol/L, 0.255±0.005 (gray value), (14.44±1.87) pg/mg, (10.56±2.20)μg/L].Conclusion The high fat diet can significantly increase the body weight, blood lipid level and the contents of leptin in rat myocardium and serum, and MCT can alleviate the above effects of high fat diet to a certain degree, thus it has the potential of cardiovascular protection.
2.Analysis on clinical curative effect of compound polymyxin B ointment combined with basic fibroblast growth factor to treat the wound surface of deepⅡdegree burn of children

Objective To observe the clinical curative effect of compound polymyxin B ointment combined with basic fibroblast growth factor (bFGF) on wound surface of deep second-degree burn in children, and provide reference for the clinical treatment. Methods Eighty cases of children patients with deep second-degree burn were included in this study, who were admitted in Tianjin 4th Center Hospital from March 2015 to March 2016. The cases were divided into control group and combined treatment group, 40 cases for each group. Wherein, the control group was treated with bFGF, the combined treatment group was received compound polymyxin B ointment and bFGF. The occurrence rate of infection of burn wound surface, wound healing time and occurrence rate of scar after healing were compared between two groups. Results There were lower infection rates on day 7, 10, 15 and 20 after burn in combined treatment group than those in control group (P<0.05). The healing time was significantly shorter in combined treatment group than that in control group [(21.53±1.33) d vs. (25.76±1.50) d, t=13.345, P<0.01]. The scar occurrence was significantly lessin combined group than that of control group (U=5.077,P<0.05). Conclusion The compound polymyxin B ointment combined with bFGF show a certain effect of anti-infection, acceleration of wound surface healing and preventing the scar from generating for wound surface on deep second-degree burn in children, which having a significant clinical curative effect.
3.Closed-loop Titration of Propofol for General Anaesthesia in 20 Cases of the Laparoscopic Cholecystectomy

Objective To evaluate the feasibility of propofol infusion by a closed-loop system for the titration of anaesthetic in laparoscopic cholecystectomy guided by Bispectral Index ( BIS). Methods Forty patients subjected to laparoscopic cholecystectomy randomly allocated into two groups: the control group with opened-loop titratioin of propofol TCI induced at a target of 4 μg?mL-1 and aintained from 2 to 5 μg?mL-1 and the treatment group with closed-loop titration was performed using a proportional differential algorithm.For both groups,the BIS was set at 45-55.Remifentanil TCI was infused at a target of 4 ng?mL-1 and was maintained according to the situation.The change in medial arterial pressure(MAP),heart rate (HR) and BIS were recorded before anesthesia(t0 ),target BIS of 50(t1 ),at tracheal intubation(t2 ),during incision of skin (t3 ),5 min after the operation(t4 ),at t5 of gallbladder removing and at t6 of skin suturing.The dose of propofol was calculated. Results Haemodynamic data were similar between groups during the induction.But MAP was decreased significantly at t1 ,t3 ,t4 , t5 in the control compared with the treatment group (P<0.05).HR changes a lot at t5 in the control compared with the treatment (P<0.05).Total dose of propofol in the treatment group was statistically lower than that in the controls [(110.10± 8.34) vs (120.55±6.26) mg; (603.20±116.55) vs (759.50±116.37) mg,P<0.05)]. Conclusion Automated titration guided by BIS for propofol infusion is feasible without increase in haemodynamic adverse effects and is of less propofol consumption in the laparoscopic cholecystectomy.
4.Myocardial perfusion abnormalities in patients with isolated left ventricular noncompaction

Objective To investigate the myocardial perfusion abnormalities in patients with isolated left ventricular noncompaction (ILVNC) and analyze the correlation between MPI and MRI.Methods Nineteen patients (14 males,5 females,age range:15-76 years) with ILVNC diagnosed by cardiac MRI were recruited.All patients underwent 99Tcm-MIBI MPI.Both MPI and cardiac MRI were analyzed using a 17-segment model.The thickness ratio of the non-compacted to compacted layers of myocardium (NC/C) was calculated,and segments with NC/C>2.3 were considered as noncompaction.The incidences of delayed enhancement (DE) and myocardial perfusion abnormalities in non-compacted segments and compacted segments were calculated.x2 test was used for categorical data.The Pearson and Spearman correlation coefficient were used to assess the relationship between the numbers of myocardial segments with myocardial perfusion abnormalities/noncompaction/DE and lgLVEF.Results Of 19 patients,myocardial perfusion abnormality was found in 16 (84.2%) patients.The incidences of perfusion abnormality were 33.6% (36/107) in non-compacted segments and 31.9% (69/216) in compacted segments,respectively (x2=0.09,P>0.05).There were 31 segments with DE.The incidences of DE were 5.6% (6/107) in non-compacted segments and 11.6% (25/216) in compacted segments,respectively (x2 =2.94,P>0.05).The incidence of reduced perfusion was higher in segments with DE than those in segments without DE (54.8% (17/31) vs 30.1% (88/292) ; x2 =7.80,P<0.01).The lgLVEF and the numbers of myocardial segments with noncompaction/DE/myocardial perfusion abnormalities were not correlated(r=-0.35,0.15,-0.34,all P>0.05).Conclusion Most patients with ILVNC have myocardial perfusion abnormality,which can be observed both in non-compacted and compacted myocardium.Further research is required to elucidate the role of myocardial perfusion abnormality in ILVNC.
5.Analysis on clinicopathologic characteristics of 216 primary extranodular non-Hodgkin's lymphoma in head and neck

Objective:To study the clinicopathologic characteristic of the primary extranodular non-Hodgkin's lymphoma in head and neck.Method:Clinical manifestation and the characteristic of clinicopathology of 216 extranodular non-Hodgkin's lymphoma patients in head and neck were analyzed retrospectively.Result:The age of thirty-one to sixty and seventy-one to eighty years old were more common age range in the group of patients. Nasal cavity was the most commonly primary involved site(95 cases, 44.0% ) , secondly was tonsil ( 47 cases, 21.8%). The most common histologic subtype was NK/T cell lymphoma, which accounted for 49.5%(107 cases)of cases, secondly was diffuse large B cell lymphoma (DLBCL, 58 cases, 26.7%). The most common histologic subtype in different swtach groups respectively is: NK/T cell lymphoma in nasal cavity(74 cases,77.9%), DLBCL in paranasal sinus(6 cases,50.0%), DLBCL in tonsil(27 cases,57.4%),NK/T cell lymphoma in nasopharynx(17 cases,44.7%), and DLBCL in lingual root(5 cases,45.4%).Conclusion:We conclude that primary extranodular non-Hodgkin's lymphoma is common in head and neck patients. There is characteristic in age, primary involved site and histologic subtype, which is helpful to understand these characteristic for pathologic diagnosis.
6.Clinical observation of the Aiyishu injection combined with chemotherapy for middle and advanced cancer patients

Objective To study the effect of the Aiyishu injection combined with chemotherapy for middle and advanced cancer patients in the near future curative effect and survival quality.Methods 127 middle and advanced cancer patients were randomly divided into two groups,71 cases in therapeutic group and 56 cases in control group.The therapeutic group and control group both choose the same sickness plants and combined chemotherapy plan of pathology,therapeutic group added the Aiyishu injection 40 ml to drip. The treatment course lasted 14 days,with 3~4 courses,Simple chemotherapy was used in the control group. Results In the therapeutic group,there were 3 cases of complete response(CR),23 cases,of partial response (PR),the rate of response is 36.62 %(26/71).In the control group,there was 1 case of complete response(CR), 10 cases of partial response(PR),the rate of response is 19.64 %(11/56).There was significant difference be- tween the two groups(P0.05),whereas it declined significantly in the control group after chemotherapy(P
7.Construction of prefab two-grille latrine and its healthy effect

Objective To develop two-grille latrines with the effect of non-hazardous treatment. Methods According to the result of the pretest, one type of two-grille latrines was selected and constructed and its effect of non-hazardous treatment was evaluated. Results The faces-treatment pool, with cement-prefab structure and obturation, had a total cubage of 1.32 m3. The average total amount of faces in the pool was 1.05 m3 per family per month, and the pool could store the faces for over 34 days. The “reaching criteria rate” of the parasite ova sediment and the value of the fecal coliform bacteria were 100% among the excrement in the outlet. Conclusions The prefab two-grille latrines can reach the criteria of non-hazardous treatment, and they can be used in well-off rural areas without the usage of manure.
8.Clinical Observation on Treatment of Advanced Menorrhea with QingJing Capsule

Objective To investigate the effect of Qingjing Capsule (QC) in treating advance menorrhea (AM) and to evaluate its safety. Methods Double- blind, double- imitating, parallel- controlled and randomized trial was performed in 63 cases of AM with deficiency- heat syndrome.Thirty- two cases was in QC group(Group A) and other 31 in Gujing pill group(Group B).The menstrual cycle, clinical symptoms and signs, basal body temperature (BBT), serum levels of estradiol (E2) and progesterone and endometrium biopsy were observed to evaluate the effects of QC. Results The short- term markedly- effective rate was 90.63% and 77.42% and the short- term effective rate was 96.88% and 93.55% in Group A and Group B respectively. The long- term markedly- effective rate was 83.87% and 67.74% and the long- term effective rate was 93.55% and 80.65% in Group A and Group B respectively. Designing of natural product-like compounds using natural products as template structures is an important strategy for the discovery of new drugs. Gambogic acid (GA), which is a Garcinia natural product with a unique caged xanthone scaffold, inhibits potent antitumor activity both in vitro and in vivo. This review summarized the researches on the identification of the antitumor pharmacophore of GA, and the design, structural optimization and structure-activity relationship (SAR) of natural product-like caged xanthones based on it.
10.Effect of L-carnitine on Patients with Microvascular Angina

ObjectiveTo observe the therapeutic efficacy of L-carnitine on patients with microvascular angina (MVA).Methods48 MVA patients were divided randomly into the trail group and control group with 24 cases in each group. The patients of the trail group were treated with L-carnitine vein dropping, those of the control group took isosorbide-mononitrate orally. All treatment lasted for 12 weeks.ResultsAfter the treatment, attack times of MVA and consumption of nitroglycerin of the patients in two groups decreased obviously ( P<0.01). The time for inducing angina, the time for 0.1 mV ST segment depression, the total exercise time and ∑ST depression by exercise were improved obviously ( P<0.05). The serum total cholesterol, triglyceride and low density lipoprotein cholesterol decreased by 20.3%, 32.7% and 16.6% respectively, and high density lipoprotein cholesterol levels increased by 34.5% in the trail group ( P<0.01).ConclusionL-carnitine has affirmative effect on MVA, same as isosorbide-mononitrate.
