1.CT Manifestations of Progressive Muscular Dystrophy

Objective To observe CT findings of progressive muscular dystrophy(PMD)and to evaluate the diagnostic value of CT.Methods 100 healthy person were selected to measure the muscular density with CT including musculi of neck,back,waist,buttocks and legs,the density of musculi of back,waist,buttocks,and legs in 15 cases with PMD evaluated by comparative study.Results The arerage attenuation value of muscle in 100 healthy adults was 55 HU,while the musculi in the patients with PMD appeared atrophy,fatty degeneration and observable low CT attenuation value.Conclusion CT has important value in diagnosing PMD and help to guide the accurate localizations for biopsies.
2.Primary benign paroxysmal positional vertigo vestibular detection of evoked myogenic potential significance.

OBJECTIVE:
To analyze the clinical significance of ocular vestibular evoked myogenic potentials (oVEMP) and cervical vestibular evoked myogenic potentials (cVEMP) in primary unilateral benign paroxysmal positional vertigo (BPPV).
METHOD:
Fifty-two patients with unilateral primary BPPV (BPPV group) and 38 normal subjects (control group) received ocular vestibular evoked myogenic potential (oVEMP) and cervical vestibular evoked myogenic potential (cVEMP) test using tone burst stimuli. The response rate, latency and amplitude were analyzed.
RESULT:
In BPPV group, the response rate of oVEMP was 46.15% in lesioned side and 48.08% in healthy side, respectively. The response rate of cVEMP was 67.31% in lesioned side and 65.38% in healthy side, respectively. In control group, the response rate on the left ear was 84.21% for oVEMP and 92.11% for cVEMP. On the right ear, was 81.58% for oVEMP and 94.74% for cVEMP in control group, there was no significant difference in cVEMP and oVEMP P1, N1 N1-P1 latency and amplitude between left and right ear. The interaural amplitude ratio and asymmetry of cVEMP and oVEMP was significantly different between BPPV group and control group (P<0.05).
CONCLUSION
Unilateral primary BPPV with bilateral impaired vestibular otolith pathways function can be objectively evaluated by oVEMP and cVEMP detection. Abnormal oVEMP responses were more frequently detected than cVEMP.

3.The Application of Metabolomics in Pharmacokinetics to Multicomponent Herbal Medicine

The traditional approaches of pharmacokinetics (PK) focused on the dynamic changing process of single or several effective components of drugs in vivo,which was noted as limitations for the complexity studies on PK of multicomponent herbal medicine featuring multi-component,multi-target and multi-effect.It was turned into a bottleneck in the modernization process of traditional Chinese medicine,which could have made misunderstanding of pharmacological and toxicological knowledge of Chinese herbal medicine and its combination drugs.Owing to the advanced high-throughput platforms and various big data mining technology,metabolomics was capable for simultaneously detecting and depicting the variations of hundreds or even thousands of small molecules offering new opportunities for the PK studies on some complicated components.This review summarized recent PK studies over multicomponent drugs and chiefly introduced two remarkable applications to metabolomics in pharmacokinetics research,Chinmedomics and Poly-PK,integrating the theories of both metabolomics and traditional PK.The challenges and strengths of the two new strategies were also expounded.
4.A preliminary study of the training program of doctor-patient communication skills in redia-tric residency standardized training

To strengthen the pediatric residents' communication ability in rediatric residency standardized training is very important for improving quality of medical provider-patient relationship and prevent medical disputes. At present, in China's pediatric medical units at all levels of medical care for the hospitalized children groups there exist such characteristics as only child, parents doting over their children, excessive stress and fear in diagnosis and treatment and so on. From the analysis of current status, it can be seen that in clinical practice, pediatric physicians' doctor-patient communi-cation is affected by three aspects: children, parents, physicians , and there exist such problems as children's lack of coordination, parents' lack of medical care knowledge, residents' lack of clinical and communication experience. Therefore, it is necessary to strengthen the special training of pediatric residents' doctors and patients in the standardized training period, establish the feedback mechanism of communication between doctors and patients and. make targeted improvements in view of the weak link in the communication between doctors and patients, so as to improve the training program of the pediatric residents' doctor-patient communication ability.
5.Determination of Seven Residual Solvents in Liuwei Dihuang Glycoside by Gas Chromatography Method

OBJECTIVE:To establish a method for the determination of 7 residual solvents(ethanol,n-hexane,benzene,tolu-ene,xylene,styrene,divinylbenzene)in Liuwei dihuang glycoside. METHODS:The column was DB-624 capillary column,carri-er gas was nitrogen,flow rate was 5.0 ml/min;detector was a hydrogen flame ionization detector with temperature of 250 ℃(pro-grammed temperature);equilibrium temperature was 80 ℃,sample loop temperature was 90 ℃,and transfer line temperature was 100 ℃;the equilibrium time of vial heating was 30 min,sample loop filling time was 0.05 min,injection time was 1.0 min;the carrier gas pressure was 95 kpa,and the vial pressure was 60 kpa. RESULTS:The linear range was 25-500 μg/ml for ethanol(r=0.998 7),0.025-10μg/ml for n-hexane(r=0.998 8),0.025-10μg/ml for benzene(r=0.999 9),0.1-40μg/ml for toluene(r=1.000 0),0.25-100 μg/ml for xylene(r=0.999 9),0.5-500 μg/ml for styrene(r=1.000 0) and 0.5-500 μg/ml for divinylbenzene (r=1.000 0);RSDs of precision,stability and reproducibility tests were lower than 4%;recoveries were 99.60%-102.70%(RSD=1.08%,n=9),90.70%-100.30%(RSD=4.51%,n=9),100.10%-109.80%(RSD=3.82%,n=9),99.50%-110.00%(RSD=4.40%,n=9),100.00%-109.10%(RSD=3.50%,n=9),93.40%-102.30%(RSD=3.73%,n=9) and 99.70%-101.70%(RSD=0.79%,n=9),respectively;the low limits of detection were 1.000,0.025,0.025,0.025,0.100,0.025,0.250 μg/ml respectively. CONCLUSIONS:The method is simple,stable and reproducible,and can be used for the determination of residual solvents(etha-nol,n-hexane,benzene,toluene,xylene,styrene,divinylbenzene)in Liuwei dihuang glycoside.
6.Clinical Observation on 24 Cases of Chronic Obstructive Pulmonary Disease of Stationary Phase Combined with Stable Angina Pectoris with Qi Deficiency-blood Stasis-phlegm Blockade Syndrome: Simultaneous Intervention for Heart and Lung

Objective To examine the clinical effect of simultaneous intervention for heart and lung on chronic obstructive pulmonary disease (COPD) of stationary phase combined with stable angina pectoris with Qi deficiencyblood stasis-phlegm blockade syndrome.Methods Ninety-six COPD stationary phase combined with stable angina pectoris patients with Qi deficiency-blood stasis-phlegm blockade syndrome were randomized into control group,Juhong [Exocarpium Citri Rubrum] tablet group,the Tongxinluo (通心络) group and the Ju&Tong group,24 cases in each group.The control group was given western medicine routine therapy.In addition to the treatment of the control group,Juhong tablet 3.6 g was given to the Juhong tablet group orally,twice each day;Tongxinluo capsule 1.04 g was given to the Tongxinluo group orally,three times each day;Juhong tablet and Tongxinluo capsule were given to the Ju&Tong group.Each group was treated for 8 weeks.The following intems were compared before and after treatment including the scores of cough,cough up phlegm,dyspnea and St.George's Respiratory Questionnaire (SGRQ),anginal attacks,durante dolors,nitroglycerin consumption,pulmonary function [including forced expiratory volume in one second (FEV1) and forced vital capacity (FVC)],as well as the levels of serum C reactive protein (CRP),interleukin 1β (IL-1β) and interleukin 10 (IL-10).Results After treatment,the scores of cough,cough up phlegm,dyspnea and SGRQ decreased in the Juhong tablet group,the Tongxinluo group and the Ju&Tong group.FEV1 and FVC increased.Anginal attacks,durante dolors,nitroglycerin consumption,as well as the levels of serum CRP,IL-1 βand IL-10 decreased.Moreover,the effect of certain indexes in the Ju&Tong group was superior to those in the Juhong tablet group and the Tongxinluo group (P < 0.05 or P < 0.01).Conclusion Simultaneous intervention for heart and lung might improve clinical symptoms and pulmonary function of COPD stationary phase combined with stable angina pectoris with Qi deficiency-blood stasis-phlegm blockade syndrome patients.Inhibiting chronic persistent inflammation might be one of the important mechanisms.
7.Salvia miltiorrhiza promotes vascular endothelial growth factor expression in frozen-thawed mouse ovarian in an early stage after transplantation

Objective To investigate the angiogenesis effects of Salvia miltiorrhiza in heterotopically grafted frozen-thawed mouse ovaries. Methods The ovaries thawed after cryopreservation were xenografted into the donated kidney capsules of 8- to 12-week adult male mice. The mice were divided into two groups, saline and Salvia miltiorrhiza groups, the mice either in the saline or in Salvia miltiorrhiza groups were administered I.p. Daily either saline(0.5ml) or Salvia miltiorrhiza(0.5g)respectively, from the day prior transplantation. The two groups were sacrificed 1 day,2 days and 7 days after transplantation respectively, the grafts from thawed,1 day,2 days,7 days were removed for follicle counting, immunohistochemical studying and detecting of the mRNA expression of vascular endothelial growth factor(VEGF). Results The number of follicles and survival rates in grafts after transplantation of Salvia miltiorrhiza group were more than that of saline group (P<0.05);the expression of VEGF increased after transplantation,peaked on day 7,there was no difference between the two groups (P>0.05);the apoptosis index of Salvia miltiorrhiza group was less than that of saline group (P<0.05), the mRNA expression of VEGF188 and VEGF164 of Salvia miltiorrhiza group was more than that of saline group on 48 hours after transplantation(P<0.05). Conclusion Salvia miltiorrhiza may provide benefits for folliculogensis and decreasing the apoptosis index of follicles. Nevertheless,a increase in the VEGF188 and VEGF164 isoform in the Salvia miltiorrhiza group may suggest the positive effect of exogenous Salvia miltiorrhiza therapy in the early stage of angiogenesis.
8.Effect of anti-tuberculosis treatment in tuberculosis patients co-infected with human immunodeficiency virus in seven counties, Yunnan

Objective To understand the potential risk factors influencing the effect of standard anti-tuberculosis (TB) treatment for TB patients co-infected with human immunodeficiency virus (HIV) and provide evidence for the improvement of anti TB therapy.Methods A retrospective study was conducted among 445 TB/HIV patients diagnosed and registered in 7 counties in Yunnan province from January 2010 to June 2012.A structured questionnaire was used to collect the patients' demographic characteristics,diagnosis and treatment information after informed consent.Chi-square test was conducted to compare successful rate of anti TB treatment among the patients with different demographic characteristics.Multivariate logistic regression analysis was conducted to identify risk factors influencing the effect of anti TB treatment.Adjusted OR>1 means the risk factor of treatment failure.P value less than 0.05 was set as significant level.Results After standard anti TB treatment,397 patients were cured.The five risk factors influencing treatment effect were the existing of 4 suspected TB symptoms when seeking medical care for the first time(adjusted OR=2.208),TB/HIV patients detected in HIV/AIDS screening (adjusted OR=5.856),severe case (adjusted OR=4.607),non-full-course supervision during treatment (in intensive phase adjusted OR=4.129,full-course management adjusted OR=8.090) and interruption of therapy (adjusted OR=21.517).Conclusion Early detection of TB/HIV patients and conducting full course supervision during treatment can improve the effect of anti TB treatment.It is necessary to strengthen the early detection of TB/HIV patients and standarded treatment in Yunnan province.
9.Prevalence of chromosome 9 abnormalities among pediatric specimens.

OBJECTIVETo perform cytogenetic analysis for children, especially newborns suspected for chromosome abnormalities.
METHODSPeripheral blood or born marrow specimens were respectively cultured in proper media. Karyograms were analyzed following G-banding.
RESULTSOf 154 blood specimens, numerical chromosomal abnormalities were identified in 20 patients, which included 19 with trisomy 21. Structural aberrations were identified in 13 patients, among which chromosome 9 aberrations were seen in 6 cases. These included 3 inversions, 1 deletion, 1 insertion and 1 duplication. All aberrations were located in pericentromere region of chromosome 9 with clinical manifestations including congenital heart disease, peculiar facial appearance, paralysis, dysplasia and/or movement disorder. Chromosome polymorphisms were found in 20 patients, most of which had absence of satellites or variation of heterochromatin on chromosome 9. Of 10 bone marrow specimens from children suspected for acute leukemia, chromosome abnormalities were identified in 5 patients.
CONCLUSIONCytogenetic analysis is useful for children featuring multiple congenital abnormalities. Chromosome 9 abnormalities and their clinical relevance should attract more attention.

10.The diagnosis and treatment of primary abdominal cocoon

Objective To investigate the clinical characteristics,diagnosis and treatment of primary abdominal cocoon.Methods The clinical data of 16 cases of primary abdominal cocoon were reviewed and analyzed.Results Only in 2 cases in which correct preoperative diagnosis was estabished by CT scan.The other 14 cases were diagnosed during operation.Parts or all small intestines were covered with a layer of milky white dense fibrous thin membrane,formed mass-likely adhesion,and fibrous film attached to the mesenteric roots.Extensive adhesion existed in between intestines.The operation included lysis of peritoneal adhesion plus fibrous membrane excision in 14 cases,lysis of peritoneal adhesion plus fibrous membrane excision and segmental enterectomy in 2 cases,and in 2 cases appendectomy was done.There was no anastomotic leakage or other major complications except for wound infection in 1 case.All the patients were followed up from 3 months to 7 years with a median follow-up time of 3.6 years,and no recurrent cases were found.Conclusions Surgery is the main method for the treatment of abdominal cocoon with evident clinical symptoms,and the prognosis is largely fair.
