1.Impact of different surveillance models on the quality of injury surveillance

Objective To find out the impact of different surveillance models on the quality of injury surveillance and to provide a scientific basis for improving the quality of injury surveillance based in emergency departments and based on the basic data for evaluating the quality and advancing its method.Methods On the premise of the same surveillance,process and method,the injury reports the two trained groups(part-time and full-time)filled out within one month from July 28th to August 27th in 2007 were collected.They were processed in the computer,examined,and finally compared.Results The omitted rate of PTSIS was 31.44%,higher than that of FTSIS,especially during the latter part of night when it was easier for the two models to be omitted.There was a difference in error in the report-filling:Part-time group was often found to have omitted "telephone number","the cause of injury" and "intent",while the full-time group more readily left out "injury severity","patient disposition",and "responder".The part-time group did a better job on "the mechanism of injury" and "disposition",and the integrity of their report-filling was greater than that of the full-time group by 12.84%.Conclusions The omitted rate of PTSIS is higher than that of FTSIS,while the quality of the report from PTSIS is better than that of FTSIS.Therefore in the condition of the limited resources,part-time groups' active participation in the injury surveillance should be encouraged.The full-time groups are expected to be trained for it if resources permit.

Objective To investigate dynamic changes of vascular endothelial growth factor(VEGF)and endostatin(ES)in guinea.Pig of asthma and its effect on airway remodeling.Methods 40 guinea-pigs were randomly divided into normal control group(C),NS group(N),3d asthma group(A1),7d asthma group(A2)and 21d asthma group(A3),8 pigs ofeach group·Nothing was done on C Group guinea.Pigs.N group were intraperitoneal injected 2ml NS on the Ist day and the 8th day.NS inhalation started on the 15th day,1 time every day,3-5rain for each times,lasted for 3 day.Al,A2 and A3 Group were injected with 10rag ovalbumin(OVA),which made of aluminum hydroxide 20mg suspension in place of2ml NS,in place ofNS inhalation for3d,7d and 21d.At the end of24h-shot line after anesthesia BAL.Enzynle-linked immunosorbent(ELISA)was used to determine the concentration of VEGF and ES in bronchoalveolar lavnge fluid collection(BALF).Image analysis was used to determine airway circumference(Pi),outer circumference(Pe),and calculate the wall area(WA),airway smooth muscle area(SMC-A),airway smooth muscle cells(N).WA,SMC-A and N were standardized by Pi.Results With the extension of the duration of asthmatic guinea.Pigs,WA/Pi,SMC-A/Pi,N/Pi and concentration of VEGF in the airway were increased.Compared with group C and N,SMC-A/Pi,N/Pi and concentration of VEGF in asthma group were siguitlcanfly different.WA/Pi were siguificanfly different in A2 group and A3 group.The relation of the concentration of VEGF in Asthma guinea-pigs airway and WA/Pi was positive correlation.VEGF/ES and WA/Pi was highly positive correlated.Conclusions VEGF/ES inlbalance may play all important role in the airway remodeling of asthmatic guinea-pigs.
4.Clinical observation of valium and barbitone in the treatment of convulsion in neonate

Objective To explore the effect of valium and barbitone in the treatment of convulsion in neonate. Methods 156 children with convul sion were randomly divided into two groups, 77 cases in control group and 78 cases in treatment group. Results The instant and total effective rates of treatment group and control group were companed, respectively. There were significant difference between these two groups.Conclusion Valium and Barbitone is effective in treating neonate with convusion.

WTBZ]Breast cancer resistance protein (BCRP) is a recently discovered half-transporter belonging to the ABC transporter superfamily as P-glycoprotein(P-gp) and multiple resistance protein (MRP). The latest research results concerned BCRP on construction features of gene, relationship to differentiation of hemopoietic stem cell and correlated transport substrates were reviewed in the article and it's clinical significance was mentioned at the same time.
6.Antenatal Steroid Treatment to Prevent Early Non-oliguric Hyperkalemia in Extremely Low Birth Weight (ELBW) Infants

Objective To explore the effect of prenatal steroid (PNS) treatment on preventing early severe non-oliguric hyperkalemia in Extremely-Low-Birth-Weight (ELBW) infants.Methods Retrospective comparative analysis was performed on 31 ELBW infants who had been admitted to the hospital’s NICU between July 2001 and Jun 2005. Infants whose mothers received a full course of steroids before delivery (PNS group; n=18) were compared with those infants whose mothers did not receive steroids (NSG group; n=13). Infants were included in the PNS group if their mother was given one full course of dexamethasone ( 10 mg q12h4 doses) before delivery. The last dose must have been given at least 24 hours before delivery. The arterial blood gas , serum concentrations of potassium, sodium and creatinine were measured every 24 hours, and fluid intakes, urine outputs were monitored every day. Results Hyperkalemia was significantly lower in the PNS than in the NSG infants at the first 72 hours of age [5/18 cases ( 27.78%) vs 9/13 cases (69.23%)],P=0.023). However, the serum concentrations of sodium, creatinine, fluid intakes, urine outputs were not significantly difference in both groups. Conclusions Prenatal steroids treatment can reduce early severe non-oliguric hyperkalemia in EVBL infants.
7.Effect of puerarin on the expression of NF-?B65 and TNF-? in kidney of diabetic rats

Objective To observe the effect of puerarin on the expression of NF-?B65 and TNF-? in kidney of diabetic rats. Methods SD rats were randomly assigned to 5 groups (10 each):normal control group (group A),diabetic group (group B),low-dose,middle-dose and high-dose treatment groups (group C,D and E). After diabetes model was reproduced,animals in group C,D and E were i.p. injected with puerarin in a dose of 40,80 and 160mg/(kg?d),respectively; animals in group A and B were treated with corresponding normal saline. Fasting blood glucose (FBG),urinary albumin excretory rate (UAER),serum creatinine (Scr) and blood urea nitrogen (BUN) were determined at the 8th weekend after treatment. The histological changes in renal cortex were observed with light microscope and electron microscope. The qualitative and semi-quantitative expressions of NF-?B65 and TNF-? in nephridial tissue were determined by immunohistochemical method. The malignant phenotype of classical Hodgkin lymphoma ( cHL) is partly maintained by epige-netic aberrant ,of which consisted mainly abnormal histone methylation and deacetylation .Progress has been made in clinical trials related to the histone deacetylases inhibitors ( HDACis) in cHL.There is a close association noted between the epigenetic aberrants and the immune escape in cHL .DNA methyltransferase ( DNMT) inhibitors, HDACis and immune checkpoint blockade have shown synergistic effects .Further study to improve the cure rate in cHL by combination strategy is of significant importance .
9.The impact of bacterial lysate on asthma prevention in mouse

Objective To establish mouse allergic asthma model and observe the effect of bacterial lysates (OM-85BV) on airway inlfammation. Methods Forty-eight 4 to 6 weeks healthy male BALB/c mice were used as research subjects and randomly divided into six groups, a:control group;b:OM-85BV contral group;c:allergic asthma model;d:dexamethasone group (Dex group);e:OM-85BV A group;f:OM-85BV B group (the intervention time was prolonged 10 days than group e). BALB/c mice were sensitized and challenged with ovalbumin (OVA). Mice in groups c, d, e and f were intraperitoneally administered with antigen (OVA)-Al(OH)3 on days 1, 8 and 15, others were administered by PBS. From the 17th day to the 26th day, Mice in group f were treated with OM-85BV and others were treated with normal saline. In the next days, mice in groups c, d, e and f were intranasal given OVA for 5 consecutive days. Additionally, mice in groups b, e and f were treated with OM-85BV before challenge, while mice in the group d were administered by Dex, others were treated with normal saline at the same dose. Twenty four hours after the last intranasal administration, mice were anesthetized and dissected. Lungs were lavaged with PBS and bronchoalveolar lavage lfuid (BALF) was obtained. The total inlfammatory cells and eosinophils in BALF were counted. The total IgE levels of blood serum and IFN-γ/IL-4 levels of lavage were detected. The removed parts of lung tissue were collected for histological examination. Results Compared with groups a and b, lung tissue biopsies by HE staining from the asthma group showed obvious airway inlfammation. The situation of groups d and f was signiifcantly improved than group c, while the differences between groups e and c were not evident. Total cells and the number of eosinophils in BALF of group c (90.3±13.94×104/ml) were signiifcantly higher than that in groups a and b. Compared with the control group, levels of IL-4 in BALF (119.03±19.92 pg/ml) and IgE in serum (15.86±1.97 ng/ml) increased and levels of IFN-γin BALF (90.50±13.51 pg/ml) reduced signiifcantly. The corresponding levels of groups d, e and f were signiifcantly improved than group c (P<0.01). Conclusions Administered by OM-85BV helps regulate the balance between Th1/Th2 in asthmatic mice, reduce airway inlfammation, and prevent the occurrence and development of airway inlfammation.
10.Comparison of the biological stability of liposome-encapsulated nerve growth factor at different depot stages

BACKGROUND: Nerve growth factor (NGF) is characterized by poor stability both in vitro and in vivo, and liable to lose its bioactivity.OBJECTIVE: To study the stability of liposome-encapsulated NGF injection preserved under various conditions.DESIGN: A controlled study of liposome-encapsulated NGF.SETTING: Department of Surgery of the First Affiliated Hospital of Nanjing Medical University.MATERIALS: This study was carried out at the Central Laboratory of the First Affiliated Hospital of Nanjing Medical University between July 2002and March 2004. NGF from rat submaxillary gland was purified, encapsulated by liposome, and prepared into lyophilized dosage form before preserved under different conditions (at 4 ℃, room temperature, 40 ℃ or 40 ℃ with saturated humidity, respectively).METHODS: Chicken embryo dorsal root ganglion cultured in serum-free medium was used to evaluation the bioactivity of NGF in vitro. The dorsal root ganglion from 8-day-old chicken embryo was inoculated in a polylysine-coated 24-well culture plate and cultured in Dulbecco modified Ea-gle medium (DMEM) containing different testing samples. Only DMEM was used for culture in the negative control group, while DMEM containing NGF at different concentrations used in the positive control group. The ganglion was cultured at 37 ℃ with 50 mL/L CO2 and saturated humidity for 24 hours, and the growth of the nerve fibers was observed under an inverted microscope. The bioactivity of NGF was also evaluated in simulated condition in vivo by adding lyophilized liposome-encapsulated NGF and positive control NGF specimen into 0.5 mL rat serum, which, along with the blank control serum, was added into 2.5 mL DMEM at 0.5, 1, 2, 3, 4,6 hours and thoroughly mixed. The bioactivity of NGF was assessed accord-ing to the length and density of the dorsal root ganglion and graded the prominences (recorded as "++" or "+++ "), and very long and dense growth of the prominences (++++).MAIN OUTCOME MEASURES: In vitro bioactivity of NGF preserved for 10, 30, 60, and 90 days by testing the growth of cultured chick embryo dorsal root ganglion in serum-free DMEM and in test of rats serum containing NGF added at 0, 0.5, 1, 2, 3, 4, and 6 hours into DMEM.RESULTS: Lyophilized liposome-encapsulated NGF exhibited stable bioactivity (+++) after preservation at 4℃ and room temperature for 10-90days; at 40 ℃ for 10-60 days, the it retained its the bioactivity (+++),which, however, slightly decreased by 90 days (++); its bioactivity was preserved (+++) at 40 ℃ with saturated humidity for 10 days (+++), slightly decreased at 30-60 days (++) and noticeably lowered (+) at 90 days. When preserved for 0, 0.5, 1, 2, and 3 hours in rat seum, the NGF preparation retained stable bioactivity (++++ or +++), which slightly decreased at 4 hours and 6 hours (++).CONCLUSION: Liposome-encapsulated NGF has stable bioactivity but its preservation at relatively high temperature with high humidity is difficult.Lyophilized liposome-encapsulated NGF exhibits better bioactivity than NGF-liposome suspension after preservation under various conditions.
