2.Analysis of the relationship between electrocardiographic ST segment diversify and infarction of related artery and the site of occlusion in acute inferior myocardial infarction
Chinese Journal of Postgraduates of Medicine 2011;34(7):7-10
Objective To predict the infarction of related artery(IRA)and the site of occlusion by analyzing the diversify of electrocardiographic ST segment and coronary angiography findings in acute inferior myocardial infarction. Methods Sixty-four patients with acute inferior myocardial infarction were divided into two groups by coronary angiography: right coronary artery(RCA)occlusion group(51 patients)and left circumflex coronary artery(LCX)occlusion group(13 patients). RCA occlusion group included proximal, middle and distal components occlusion, and LCX occlusion group included proximal and distal components occlusion. The cases of STⅢ elevation > ST Ⅱ ,STaVL depression > STⅠ ,STV1 depression,STV7-V9 and STV3R-V5R elevation were recorded and compared. Results The percentage of STⅢ elevation > STⅡ, STaVL depression >STⅠ, STV1 no depression, STV7-V9 no elevation and STV3R-V5R elevation was significantly higher in RCA occlusion group than those in LCX occlusion group(P < 0.01 or < 0.05). The percentage of ST Ⅲ elevation ≤ ST Ⅱ, ST,VL depression ≤STⅠ ,STV1 depression,STV7-V9 elevation,STV3R-V5R no elevation was significantly higher in LCX occlusion group than those in RCA occlusion group(P <0.01 or <0.05). In RCA occlusion group:the sensitivity of STⅢ elevation > STⅡ, STaVL depression > STⅠ and STV1 no depression was 90.2%, 80.4% and 80.4%;the specificity of STⅢ elevation > STⅢ and STaVL depression > STⅠwas 84.6% and 84.6% ;the sensitivity of STV3R-V5R elevation was 51.0%, but its specificity was 100.0%. In LCX occlusion group: the sensitivity of STⅢ elevation ≤ ST Ⅲ and STaVL depression ≤ ST Ⅰ was 84.6% and 84.6%, and their specificity was 90.2% and 80.4%;the specificity of STV1 depression and STV7-V9 elevation was 80.4% and 78.4%; the sensitivity of STV3R-V5R no elevation was 100.0% ,and its specificity was 51.0%. In RCA occlusion group, the percentage of STV3R-V5Relevation was higher in proximal components occlusion than that in distal components occlusion[76.9%(10/13)vs. 27.3%(3/11),P=0.015]. Conclusions The IRA can be initially judged by analysis of the characteristics in the ST segment in Ⅰ,Ⅱ ,Ⅲ ,aVL,V1,V7-V9,V3R-V5R lead in acute inferior myocardial infarction. And the ST segment elevation in V3R-V5R is meaningful in the judgment of proximal and distal components occlusion in RCA.

Objective To investigate indications, technique, prognosis and complications of anterior flap hemipelvectomy (hemipelvectomy using a quadriceps femoris muscle flap with superficial femoral artery)in the treatment of pelvic tumors. Methods From April 2009 to October 2010,10 patients with pelvic tumors underwent anterior flap hemipelvectomy in our department. There were 9 males and 1 female, aged from 30 to 62 years (average,46 years). There were 7 cases of chondrosarcoma,1 case of epithelioid sarcoma,1 case of pleomorphic liposarcoma and 1 case of spindle cell sarcoma. Results Nine patients were followed up for 12 to 30 months (average,21.5 months).Superficial skin necrosis occurred in 1 patient, while there was no wound infection.Local recurrence (33.3%) occurred in 3 patients, including 2 cases of chondrosarcoma and 1 case of epithelioid sarcoma.Three patients died at last, including 2 patients with local recurrence and 1 with pulmonary metastasis from liposarcoma. Two patients survived with pulmonary metastasis, and remaining 3 patients survived without recurrence and metastasis. Conclusion Anterior flap hemipelvectomy is indicated for:(1) patients with extensive soft tissue sarcoma in the buttock, and the tumor or reaction zone involve the subcutaneous tissue; (2) patients with recurrent pelvic tumors, and the conventional posterior flap can't be used due to contamination by the original incision; (3) patients whose posterior flap has poor blood supply due to radiotherapy or other factors. The prerequisite of the surgery is that the blood stream between external iliac artery and femoral artery is unobstructed; moreover the blood vessels are not in the tumor reaction zone.Compared with conventional posterior flap, the anterior flap hemipelvectomy is an easier procedure, which not only can easily cover surgical defect and obtain better surgical margin, but also has less complications.

Objectives To explore the process and regularity of graft after vascularized bone allograft. Methods An adult rabbit model of massive femoral defect was established and reconstmcted with vascularized bone allograft based by laterial rotating femoral vessel. The experiments were carried out in two groups, the experimental group(vascularized bone allograft)and the control group(nonvascularized bone allograft). Then observation on periosteum, cortex and marrow was performed after operation containing in ink-infused specimen. Results The revascularization in the experimental grouD was observed synchronicly on periosteum, cortex and marrow after operation, while the phenomenon of vascularization took place from periosteum to marrow in the control group. The density of micro-vessel in experimental group were 10.0±1.8,15.8±1.5 and 13.8±1.5,13.8±1.5 respectivly after 2,4,8 and 16 weeks.and those were 2.8±0.8,6.0±0.9,5.5±1.0,6.0±1.1 in control group. The ink-infused experiment demonstrated a better revascularization in the experimental group. Conclusion The vaseularization can promote revascularization of graft during bone allograft.

Objective To investigate the status of microchimerism in recipients and the relation between microchimerism and immunologic tolerance after vascularized allograft bone transplantation. Methods X-ray and histological examinations were performed on recipients after massive vascularized shaft of femur from female Japanese white rabbit donors was transplanted to male recipients. Microchimerism in different organs and tissues were checked postoperatively using a semi-quantitative polymerase chain reaction (PCR) with a Y-chromosome specific primer at different time. Results X-ray and histological examinations showed typical bone union in the experiment group but irregular new bone calluses surrounded the transplanted bones, with high density sequestrum in the control group.Semiquantitative PCR with a Y-chyomosome specific primer indicated that the incidence of microchimerism in organs and tissues in the experiment group was higher than that in the control group postoperatively and increased with time. Conclusions After vascularized allograft bone transplantation, organs and tissues show microchimerism that has a positive correlation with the histocompatibility of the transplanted bones.

Objective To explore the prevalence and influencing factors of obstructive sleep apnoea syndrome(OSAS)among hypertensive patients. Methods Two hundred and ninety-eight hypertensive patients who used a portable OSAS monitoring device were selected as our subjects. Of which,197 patients who completed their OSAS monitoring were divided into OSAS group( 165 cases) and non-OSAS group. Stepwise linear regression and Logistic regression analysis were used to analyze the relevant factors correlated to AHI and OSAS with hypertension. Results The average age of the 197 patients(122 were males)were 59. 93 ± 10. 18 years old. The prevalence of OSAS was 83. 8% in hypertension patients. Body mass index( BMI),systolic pressure (SBP),diastolic pressure(DBP),low density lipoprotein cholesterol(LDL-C)and waist circumference in OSAS groups were(26. 37 ± 2. 77)kg/ m2 ,(159. 59 ± 14. 42)mmHg,(98. 71 ± 9. 40)mmHg,(2. 85 ± 0. 82)mmol/L,(90. 56 ± 8. 62)cm,higher than those in non-OSAS group((22. 43 ± 2. 04)kg/ m2 ,(146. 83 ± 4. 60) mmHg,(85. 51 ± 7. 52)mmHg,(2. 22 ± 0. 60)mmol/ l. ,(81. 84 ± 8. 00)cm). There were statistical differences between the two groups(t = - 7. 654,- 4. 945,- 7. 490,- 4. 110 and - 5. 289;P < 0. 001). BMI,SBP and DBP were positively associated with AHI,and they were in depended risk factors of OSAS(OR(95% CI)=2. 554(1. 408 - 4. 632),1. 432(1. 154 - 1. 777),1. 286(1. 058 - 1. 562);P < 0. 05). Conclusion The prevalence of OSAS is high in hypertension patients. Higher BMI,SBP,DBP are associated with OSAS in hypertension pateints.
