1.Influence of psychological nursing on heart rate variability of patients with acute myocardium infarction

Objective To study the effect of psychological nursing on heart rate variability (HRV)in patients with acute myocardium infarction. Methods 80 patients with AMI were randomly divided into group A(routine nursing group)and group B (psychological nursing group) with 40 patients in each group,HRV was evaluated in both groups.The time domain and frequency domain of HRV of 24 h sinus rhythm in two groups of patients were analyzed. Results In group B,24 h average standard deviation of RR interval (SDNN),5 min segments of continuous average standard deviation of RR intervals (SDANN),the difference between adjacent cardiac RR interval of the root mean square (rMSSD),the percentage of adjacent canrdiac RR interval greater than 50 ms (PNN50),low frequency (LF),high frequency (HF) and other HRV parameters were significantly higher than group A. Conclusions Psychological nursing can significantly increase HRV and improve the electric stability of the ischemic myocardium.Psychological nursing can stabilize emotion of patients,decrease sympathetic tone,and may have a role in the prevention of sudden death.
2.A cohort study on the outcome of multidrug-resistant tuberculosis among newly diagnosed cases

ObjectiveTo investigate the anti-tuberculosis treatment response and outcomes in treatment naive patients infected with multidrug-resistant tuberculosis (MDR-TB).MethodsA total of 408 patients who were diagnosed with MDR-TB in Shanghai Pulmonary Hospital from January 2006 to January 2009 were recruited in this study.These patients were divided into two groups based on their previous treatment history:treatment naive group and re treatment group. The treatment response,outcomes andadverse eventswere observed. The outcomes of thesetwo groupswere compared by cohort analysis and x2 test.ResultsThe sputum conversion rates,the lesions absorption rate and the cavity closing or shrinking rate of the treatment naive MDR TB group were significantly higher than those of the re treatment group,while the adverse events rate was not significantly different between two groups (x2 =0.434,P>0.05).Among 89 treatment naive cases,66 cases (74.16%) were cured,8(8.99%) completed the full treatment course,7(7.87%) were treatment failure,3(3.37%) died,and 5(5.62%) were lost to follow-up.Among the 319 cases of re-treatment MDR TB group,134 (42.01%) were cured,31(9.72%) completed the full treatment course,116 (36.36%) were treatment failure,12(3.76%) died,26(8.15%) were lost to follow-up.The cure rate of the treatment naive MDR-TB group was significantly higher than that of re-treatment group (x2=28.783,P<0.01).The factors influencing the treatment outcomes included the stage of the disease,the range of lesions and cavity, the patients'generalnutritional status, underlying complications,and the drug-resistant strains. Conclusions The anti-tuberculosis treatment outcomes are better in treatment naive patients with MDR-TB infection compared to the treatment experienced and retreated patients.Therefore,the status of the drug resistance should be closely monitored in order to detect MDR-TB as early as possible.With the early diagnosis,the treatmcnt regimen may be modified timely and as a result the treatment outcomes can be improved.
3.Study on positive rate of blocking antibody in women with recurrent spontaneous abortion administered by route and frequency of paternal lymphocyte immunotherapy

Objective To investigates factors affecting the positive rate of blocking antibody treated by paternal lymphocyte immunotherapy in patients with recurrent spontaneous abortion (RSA).Methods From January 2008 to August 2012,326 RSA cases undergoing treatment in Infertility Center of Qilu Hospital were studied retrospectively.Those patients were divided into 2 groups randomly:260 cases in intradermal injection group were administered via bilateral forearm intradermal injections for immunotherapy once 21 days,then the blocking antibody was determined after 2 (23 cases),3 (73 cases),4 (74 cases),5(90 cases) times respectively,while in subcutaneous injection group,the 66 cases were administered via subcutaneous injection once 21 days,the blocking antibody measured after 3 times; In both cases,the blocking antibody was all determined 2 weeks later.The positive rate of blocking antibodies and the rate of successful pregnancy was recorded,and then followed up after the blocking antibody turning positive.Results (1)Positive rate of blocking antibodies:the positive rate of blocking antibodies were 17% (4/23),58% (42/73),72% (53/74) and 84% (76/90) in the 2,3,4,and 5 times of intradermal injection group,respectively (P < 0.05).In subcutaneous injection group,the positive rate of blocking antibodies was 38 % (25/66),which was significantly lower than that in group intradermal injection receiving 3 times immunotherapy (P <0.05).(2) The rate of pregnancy:the 176 patients out of 200 patients were pregnant when antibody was positive after immunotherapy,with 71.6% (126/176)of patients gained successful pregnancy(the length of pregnancy more than 5 months).Conclusions The route and frequency of administration of immunotherapy could influence the positive rate of blocking antibody.The rate of successful pregnancy will be increased after blocking antibody turning positive.
4.Response of cryopreserved ovarian tissue after autologous implantation in mouse stimulated with gonadotrophin

Objective To investigate the response of cryopreserved ovarian tissue after autologous implantation in mouse stimulated with gonadotrophin.Methods Thirty six female mice were randomly divided into three groups,with 12 mice in each group.In group of fresh ovarian tissue,fresh ovarian tissue was implanted into kidney capsule of mice:in group of cryopreserved ovarian tissue,ovarian tissue was implanted into kidney capsule of mice after cryopreserved by vitrification for two weeks.We investigated the response of ovarian tissue two weeks later after autologous implantation stimulated with gonadotrophin.Immunohistochemistry staining method was used to observe the expression of follicle stimulating hormone receptor.Results Before and after stimularian with gonadotrophin,the mature follicle rate of group of fresh ovarian tissue was 2.3%and 4.2%.that of group of cryopreserved ovarian tissue was 2.3%and 4.0%,and that of group of control was 2.6%and 5.8%.Regarding the percentages of mature follicle.there were significant differences after stimulation with gonadotrophin(P<0.05).After stimulating with gonadotrophin the percentages of mature follicle were the same in the fresh tissue group,cryopreserved tissue group and control group(P>0.05).The integrated optical density of follicle stimulating hormone receptor of fresh ovarian tissue in antrofollicle and pre-antrofollicle were 9408±2777 and 4531±1903.that of cryopreserved ovarian tissue were 9175±3093 and 4808±1386.and that of the control ovarian tissue were 8838±2064and 5516±1136 respectively.There was no significant difference between any two groups(P>0.05).Conclusion The follicle stimulating hormone receptor is preserved by cryopreservation and transplantation,small pieces of ovarian tissue response to gonadotropin stimulation is normal.
5.Study on the Dissolution of Xiaocaihu Pill from Different Manufacturers

OBJECTIVE:To establish a method for the determination of dissolution of Xiaocaihu pill,and compare the differ-ence of preparation from different manufacturers. METHODS:Using 0.1 mol/L HCl as dissolution medium,rotating basket method was used to determine the dissolution of preparations. HPLC was adopted to determine the content of baicalin:column was TSKgel ODS C18 with mobile phase of methanol-water-phosphoric acid (65∶35∶0.7,V/V/V) at a flow rate of 1 ml/min,detection wave-length was 280 nm,column temperature was 30 ℃,and injection volume was 5 μl. RESULTS:The linear range of baicalin was 0.488-124.8 mg/L (r=0.999 9);RSDs of precision,stability and reproducibility tests were lower than 2.0%;recovery was 100.14%-104.78%(RSD=1.58%,n=9). The average t50(50% dissolution time)of baicalin was 85.81 min. CONCLUSIONS:The method is simple with good precision,stability and reproducibility,and can be used for the dissolution determination of Xiaocaihu pill. Xiaocaihu pill from different manufacturers shows great differences,both preparation formulation and clinical use should attach importance to the dissolution of solid preparations.
6.Different effects of simvustatin on proliferation of rat smooth muscle progenitor cells versus endothelial progenitor cells

Objective To investigate the different influences of simvastatin on proliferation of rat smooth muscle progenitor cells(SPCs) versus endothelial progenitor cells (EPCs) and identify the compounds that differentially inhibit SPCs and EPCs proliferation for clinical usefulness. Methods Total mononuclear cells (MNCs) were isolated from bone marrow of rats by Fieoll density gradient centrifugation, and then the cells were plated on fibronectin-coated culture dishes. SPCs outgrew from the culture of MNCs in the presence of platelet-derived growth factor-BB and basic fibroblast growth factor, whereas EPCs were obtained in the presence of vascular endothelial growth factor. SPCs were identified as adherent cells positive for α-smooth muscle actin (α-SMA) by indirect immunofluoreseent staining. EPCs were characterized as adherent cells double positive for DiLDL-uptake and lectin binding by direct fluorescent staining. SPCs and EPCs were stimulated by simvastatin (0.01~10.00 μmol/L) or vehicle control for the respective time points (6 h, 12 h, 24 h and 48 h). SPCs and EPCs proliferation were assayed with 3H-TdR incorporation and manual counting respectively. Results Simvastatin obviously inhibited SPCs proliferation. At the concentration of 0. 01 μmol/L for 12 h,simvastatin significantly reduced the number of SPCs by (5.8±3.1)% compared with control group (P<0.05). Simvastatin significantly stimulated EPCs proliferation, which was dose- and time dependent and reached maximum at 1 μmol/L after 24 hours (2.0±0.1 fold increase, P<0.01).Conclusions Simvastatin displays different effects on SPCs (inhibited) and EPCs (promoted)proliferation. Local application of simvastatin may inhibit arterial restenosis and promote reendothelialization of injured vessels.
7.Investigation on Medical Students′Recognition Degree of Cross Transplantation between Non Relatives

Objective:Living organ transplantation prolongs the life of many dying patients, but there are many disputes, especially the definition ofliving organ transplantation between non relatives. The definition of this ter-minology is closely related to medical students′cognitive ofliving organ transplantation between non relatives. The medical students are the main reserve army of medical staff and the executor of living donor organ transplantation in the future, who directly determine the organ transplantation and affect the doctor-patient relationship. This paper is to investigate and analyze the status of the medical students′recognition of cross transplantation between non rela-tives. Methods:A total of 450 medical students of Qingdao university were selected with cluster method and were investigated using self-designed questionnaire. Data were analyzed by SPSS 22. 0. Results:The overall recogni-tion rate toliving organ cross transplantation between non relatives was relatively low. The difference was statisti-cally significant (P<0. 05), namely that students in higher grade had lower recognition. The difference was statis-tically significant (P < 0. 05) between different specialties, and the recognition degree of students majoring in clinical medicine and ecsomatics was higher. Conclusion:The medical students of Qingdao University understand the concept of living organ cross transplantation between non relativesfuzzily, and the recognition degree is low.
8.Effects of borneol on pharmacokinetics of protocatechuic acid in rabbits.

OBJECTIVETo study the effect of borneol on the pharmacokinetics of protocatechuic acid (PA) in rabbits.
METHODA method for determinating PA in rabbit plasma by high performance liquid chromatography (HPLC) was established. The 12 healthy New Zealand rabbits were randomly divided into 2 groups. PA concentration in plasma samples of rabbits was determined by HPLC after oral administration of PA (30 mg x kg(-1)) and PA combined with borneol (protocatechuic acid 30 mg x kg(-1) + borneol 15 mg x kg(-1)). The data were processed by DAS 2.0 software to calculate the pharmacokinetic parametes.
RESULTChromatographic peaks were separated well by the HPLC method. Regression analysis of the data of PA concentration against its peak area showed a good straight line in the range of 0.04-2.0 mg x L(-1) (r = 0.9983). In comparison with PA alone, the pharmacokinetics parameters of PA (t1/2Ka, T(max), V1/F, CL/F) in the PA + borneol combination group were decreased, while the AUC(0-infinity) was increased.
CONCLUSIONBorneol could significantly promote absorption, increase the dose absorption and slow elimination of PA in rabbits. Borneol could improve the bioavailability of PA.

Diabetic osteoporosis(DOP)is a serious complication of diabetes mellitus(DM),with bone loss and microstructure destruction.The onset of DOP is insidious,and early symptoms are not obvious.As the condition progresses,the disability and mortality rates increase in DM patients.The advanced glycation end products receptor(AGEs-RAGE)axis can mediate different signaling pathways involved in regulating osteoblasts.This article reviews the mechanism of AGEs-RAGE axis mediated p38 mitogen activated protein kinase signaling pathway in regulation of osteoblasts in DOP.
10.Curriculum design and exploration of clinical practice teaching in anesthesiology and critical care medicine

Clinical practice teaching of anesthesiology and critical care medicine course belongs to the module of clinical anesthetic application in the anesthesia teaching system. The basic idea for establishing this course is the clinical anesthesiology practice teaching which combines with clinical critical cases in accordance with the classification of the disease systems, and bases on pathophysiological theories of various organs. Real clinical cases were collected during the practice teaching, and discussed via students-teaching method. Then we comprehensively utilized task-oriented teaching, case-based teaching and problem-based learning, guided students to complete comprehensive analysis of various organ diseases on the basis of team cooperation, and improved students' practical problem-solving ability for clinical critical diseases by asking questions after reading books and guidelines.
