1.Clinical study on the clinical effect of Sophora powder combined with Xianfang-Huoming decoction for ulcerative colitis

Objective To explore the clinical effect of Sophora powder combined with Xianfang-Huoming decoction for ulcerative colitis.Methods A total of 80 UC patients with TCM syndrome of excessive noxious heat were randomly divided into 2 groups (40 patients each group). The Western medicine group(control group) were treated with Mesalazin Enteric-coated tablets, and the Traditional Chinese medicine(observation group) group were treated with Sophora powder combined with Xianfang-Huoming decoction. The tumor necrosis factor-α(TNF-α)and Interleukin-8(IL-8) were detected. The Mayo index scores were conducted, and the curative effect and the recurrence rate were observed.ResultsAfter treatment, the level of TNF-α (66.11 ± 3.72 ng/Lvs. 92.61 ± 4.32 ng/L,t=29.398,P﹤0.01), IL-8 (53.11 ± 2.72 ng/L vs. 69.31 ± 1.32 ng/L,t=33.888,P﹤0.01)in the observation groupwere significantly lower than those in the control group. The Mayo index score (2.07 ± 0.57 vs. 3.30 ± 0.93, t=7.132,P﹤0.01) in the observation group was significantly lower than that in the control group. The total effective rate (92.5% vs. 85.0%,χ2=1.127, P=0.288) in the observation group was not significantly different from that in the control group. The recurrence rate (5.4% vs. 26.5%,χ2=6.005,P=0.013) in the observation group was significantly lower than that in the control group.Conclusions The Sophora powder combined withXianfang-Huoming decoction can treat the UC patients with TCM syndrome of excessive noxious hest, and the its recurrence rate is low.
2.Estimation of the postmortem interval by determining nuclear DNA degradation from bone marrows and brains in rats

Objective To detect nuclear DNA degradation of bone marrows and brains in rat cadavers at different temperatures,and develop a new parameter for estimating early postmortem interval(PMI).Methods The brain and bone marrow were taken out for every 4h,during 0~40h after death at 10℃ and 20℃,respectively.And the single cell gel electrophoresis(SCGE) was carried out to detect the nuclear DNA degradation.Linear regression analysis was used to assay the relationship of the comet parameter HeadDNA%,Tail Length(TL) and Olive TailMoment(TM) with PMI.Results Different decline degrees of comet HeadDNA% were found in both brain cells and bone marrow cells after death,the decline of HeadDNA% in brain cells at 20℃ was faster.Compared with degradation in marrow cells,the linear relation between degradation of brain cells and PMI was better.Conclusion with that of comet parameters TL and TM,the perfect linear relationship between HeadDNA% and PMI was also observed.Conclusion Brain tissues are more suitable for PMI estimation by detecting degradation of DNA with SCGE.The HeadDNA% is more valuable for PMI estimation than TL and TM.
3.Study on the quality standard for Yinchai Granules

AIM: To establish the quality standard for Yinchai Granules (Caulis Lon icerae, Radix Bupleuri, Folium Eriobotryae, etc). METHODS: TLC was used to identify Radix Bupleuri, Folium Eriobotrya e and HPLC to determinate the chlorogenic acid content was determined by HPLC. RESULTS: The linear range of chlorogenic acid was 23.64~378.24ng, r=0.9999, respectively. The average recovery was 99.14%. CONCLUSION: The method is simple. The result is accurate and the reproducibility is good and can be used as the quality control of Yin chai Granules.
4.Analysis of 151 cases of different types of laparoscopic hysterectomy

12 weeks of gestation. Results No conversions to open surgery were needed, and no complications were observed. The operation time, intraoperative hemorrhage volume, time to first passing flatus, and length of hospitalization were respectively 109.2?14.4 min, 80.5?14.3 ml, 21.1?3.2 h, and 4.2? 0.4 d for CISH, 108.7?14.8 min, 79.8?10.0 ml, 20.6?2.7 h, and 4.2?0.4 d for LAVH, 107.6?24.2 min, 80.8?17.2 ml, 20.4?2.0 h, and 4.1?0.5 d for LSH, and 110.0?12.3 min, 80.0?18.7 ml, 20.8?1.9 h, and 4.2?0.5 d for LTH. The modified surgical procedures were successfully accomplished in all the 40 patients with extended uterus. Conclusions Different types of laparoscopic hysterectomy present different indications, advantages and disadvantages. The treatment should be individualized.

6.Serum Dickkopf-1 levels in patients with ankylosing spondylitis not affected by using tumor necrosis factor-α inhibitor

Objective Dickkopf-1 (DKK-1) is an antagonist of the Wnt signaling pathway of bone formation and it is the target gene of TNF.When DKK-1 is inhibited,osteoprotegerin (OPG) would increase,OPG can antagonize bone erosion caused by receptor activator of NF-κB ligand (RANKL).Detection the levelsof serum DKK-1,OPG,RANKL and TNF-α of patients with AS before and after applying TNF-α antagonist,the effects of anti-TNF-α treatment on signaling pathway of bone formation was explored.Methods Medical records and serum samples of 43 AS patients receiving TNF-α receptor-Ⅱ IgG Fc fusion protein for 12 weeks were collected and the levels of serum DKK-1,OPG,RANKL and TNF-α were detected by enzyme-linked immunosorbent assay.Paired t-test and Person's correlation analysis were used for data analysis.Results Disease activity indexes in AS patients with anti-TNF-α treatment,including VAS score,time of morning stiffness,BASDAI,BASFI,ESR and CRP,which were [(2.7±2.0) cm,(7±4) min,1.1±1.1,0.8±1.2,(10±9) mm/1 h and (16±26) mg/L] respectively,were reduced when compared with those before treatment,[(7.6±1.9) cm,(46±33) min,6.0±1.3,4.4±2.0,(39±29) mm/1 h and (76±43) mg/L] respectively,and the differences were statistically significant (P<0.05).Serum levels of DKK-1,(3.3±1.5) μg/L vs (3.2±1.3) μg/L,OPG,(144±71) pg/ml vs (136±62) pg/ml,and RANKL,(71±37) pg/ml vs (68±30) pg/ml,were not changed (t=14.043,8.031,20.166,13.521,7.679,9.563,all P>0.05).The levels of TNF-α increased from (1.9±1.3) pg/ml to (55.0±50.6) pg/ml (t=6.951,P<0.05).There were no relations between DKK-1 and TNF-α,RANKUOPG,patient VAS,duration of morning stiffness,BASDAI,BASFI,ESR and CRP,allP>0.05.Conclusion Clinical indicators are improved significantly by anti-TNF-α therapy; however,it should not be withdrawal suddenly.There is no relationship between serum levels of DKK-1 and level of inflammation in AS patients.
7.Investigations and analysis of anxious and depressive symptoms for patients with functional dyspepsia

Objective To investigate the psychological characteristics of the patients with functional dyspep-sia(FD). Methods The Zung serf-rating anxiety scale(SAS) and the Zung serf-rating depression scale(SDS) were employed to evaluate the anxiety mood and depression mood respectively of 110 FD patients ( group A) and 100 healthy volunteers (group B). The incidences of anxiety and depression in patients < 60 years and patients ≥60 years in group A were determined respectively. Results The anxiety ( 50.2±10.5 ) and depression scores ( 51.8±10.8)of group A were higher than group B [ (33.6±5.7) , ( 42.9±9.6) ] with a significant difference ( P<0.01 ). In group A, the anxiety incidence(66.0% ) in patients < 60 years was higher than patients ≥ 60 years (38.3%) ,while the de-pression incidence in patients ≥60 years(61.7% ) was higher than patients <60 years(42.0% ) ,both with signifi-cant differences ( all P<0.05 ). Conclusion FD patients show the psychological characteristics of anxiety and de-pression, and the characteristics type may vary in different age groups.
8.An epidemic survey of gallbladder disease in Hefei district

Objective To evaluate the incidence of benign gallbladder disease and the relationship between the hyperlipidemia and gallbladder stone in Hefei district by a survey in 6?946 persons. Methods All benign gallbladder diseases were discovered by Colour Dopple ulterasound. The data of lipidemia were analyzed and a comparative study of 1?051 cases suffering from gallbladder stone with the control group was done. Results The morbidity of benign gallbladder diseases was 21 55%. The prevalence of gallbladder stone, gallbladder polypoid lesion, cholecystitis, intrahepatic cholelithiasis or choledocholithiasis were 15 13%, 3 39%, 1 35%, 0 96%, 0 70% respectively in all examined. The morbidity of gallbladder stone was 70 2% in the benign gallbladder diseases. The incidence of hyperlipidemia both in gallbladder stone group and control group were 56 9% and 32 3% ( P

Objective To investigate the regulatory effect of insulin-like growth factor binding protein 7 (IGFBP7) on the synthesis and secretion of fibronection and the inhibiting effect of anti-IGFBP7 antibody on apop-tosis of hepatic stellate cell-T6 (HSC-T6). Methods HSC-T6 cultured in vitro were divided into blank control group, IGFBP7 20 μg/L group, anti-IGFBP7 antibody 0.25 mg/L group, anti-IGFBP7 antibody 0.50 mg/L group and anti-IGFBP7 antibody 1.00 mg/L group. HSC-T6 were treated by IGFBP7 for 24 hours, then the expression of fibronectin was detected by Western blot and the content of fibronectin by ELISA. After incubating HSC-T6 with anti-IGFBP7 antibody for 14 hours, the inhibiting effect of anti-IGFBP7 antibody on HST-T6 was evaluated by MTT assay, and the effect of anti-IGFBP7 on the apoptosis of HST-T6 was detected by flow cytometry. All data were analyzed by one-way ANOVA, t test and SNK-q test. Results The content and expression of fibronection in IGFBP7 20 mg/L group were significantly higher than those in the blank control group (t = 22.06, 7.43, P < 0.05). Anti-IGFBP7 antibody had an obvious inhibiting effect on the proliferation of HSC-T6 (F=14. 70, P < 0.05), and it also enhanced the ratio of apoptosis of HSC-T6 (F = 63.79, P < 0.05). Conclusions IGFBP7 can increase the synthesis and secretion of fibronectin which is the principal component of extracellular matrix. Anti-IGFBP7 antibody can inhibit the proliferation and promote the apoptosis of HSC-T6.
10.Study on the correlation between subclinical hepatic encephalopathy and Helicobacter pylori infection

Objective To investigate the status of Helicobacter pylori(Hp)infection with subclinical hepatic encephalopathy (SHE) and the level of blood ammonia. Methods Sixty-five SHE patients and healthy volunteer were selected. In all subjects Hp was assessed by rapid urease test (RUT) and histological examination. It would be regarded as Hp infection if both the RUT and histological examination were positive. The level of blood ammonia was detected and number connection test (NCT), digit symbol test (DST)and electroencephalogram (EEG)was performed. The patients could be diagnosed with SHE whichever of the three tests was abnormal. The patients with Hp infection were randomized received one-week standard treatment. Bacterial eradication was assessed with <'14>C-urea breath test after the treatment of 6-8 weeks, meanwhile above mentioned examinations were reassessed. Results The ratio of Hp infection in SHE patients (46.2%, 30/65 )was higher than that in healthy controls ( 33.3 % ,20/60), but there was no significant difference(P> 0.05).The blood ammonia level before and after eradication had no significant difference(P> 0.05 ). No improvement was observed in above mentioned examinations after treatment. Conclusion There is no relationship between the level of blood ammonia and Hp infection in SHE.
