Recent epidemiologic data indicate that metfomin has an anti-tumor effect. However, the underlying anti-tumor mechanisms remain unclear.MicroRNAs (miRNAs)can exhibit pro-oncogenic or anti-oncogenic effects by regulating the differentiation and proliferation of cells. In vitro studies show that metformin can regulate the expressions of multiple miRNAs which are closely associated with tumor development,a process possibly relating to the anti-cancer roles of metformin.
3. Diffusion-weighted imaging b value and apparent diffusion coefficient for predicting invasiveness of bladder cancer
Chinese Journal of Medical Imaging Technology 2019;35(12):1847-1851
Objective: To explore the value of diffusion-weighted imaging b value and apparent diffusion coefficient (ADC) in assessment of invasiveness of bladder cancer. Methods: Totally 58 bladder cancer patients with 64 lesions confirmed by pathology were retrospectively analyzed. All patients underwent conventional MRI and DWI. The b values were set as 0, 600, 1 000 and 1 500 s/mm2, respectively. The invasiveness of bladder cancer was classified as low-, intermediate-or high-invasiveness according to its pathological phenotype. ADC of bladder cancers with different b value were measured, and the correlation between ADC and invasiveness of bladder cancer was analyzed. The diagnostic efficacy of ADC values for invasive bladder cancer was analyzed by using ROC curve. Results: The mean diameter of bladder cancer in high-invasive group ([3.37±1.58]cm) was significantly higher than that in low-invasive group ([2.18±0.51]cm) and intermediate-invasive group ([2.32±0.53]cm, both P<0.01). The invasiveness of bladder cancer was negatively correlated with ADC value (r=-0.673, P<0.05). When b value was set as 1 500 s/mm2, DWI could clearly show the lesion, and image contrast was relative higher. The diagnostic specificity and accuracy of tumor ADC value was better in <2.5 cm tumors. ROC curve showed that ADC value 0.93×10-3 mm2/s was optimal cut-off for diagnosis of invasiveness of bladder cancer. Conclusion: DWI and ADC can be used to predict the invasiveness of bladder cancer, especially of small-diameter bladder cancer, therefore providing guidance for follow-up treatment.

Objective:To investigate the role of epidermal growth factor receptor (EGFR)in the regulation of B[a]P-induced silent information regulator 1 (SIRT1 ) activation in the human bronchial epithelial cells (BEAS-2B),and to clarify the relationship between EGFR/SIRT1 signal transduction pathway and the occurrence and development lung cancer. Methods:The prediction analysis of transcriptional factor binding sites of SIRT1 was performed by MOTIF SearchTM software. The primary cultivated BEAS-2B cells were divided into control group (without B[a]P exposure)and B[a]P groups (the cells were exposed to B[a]P for 6,12,24,48 h).RT-PCR and Western blotting method were carried out to detect the EGFR mRNA and protein expression levels. The BEAS-2B cells transfected with SIRT1 promotor luciferase reporter gene plasmid were treated with human epidermal growth factor (hEGF)and Genistein (tyrosine protein kinase inhibitor)for 24 h,the morphology of BEAS-2B cells was observed by inverted microscope, and luciferase reporter assay was used to test the SIRT1 transcriptional activity. The human lung tissue biopsies were acquired and the immunohistochemical analysis was used to determine the EGFR protein expression level. Results:The transcriptional factor binding sites of SIRT1 contained EGF, 2Fe-2S and vWF.Compared with control group, the expression levels of EGFR mRNA in B [a]P groups were increased in a time-dependent manner, and reached the peak at 12 h (P < 0.05);the EGFR protein expression levels were also increased (P <0.05).The SIRT1 luciferase activity in hEGF group was increased compared with control group (P <0.05);when hEGF and B[a]P worked together, the SIRT1 luciferase activity was increased even further (P <0.001). The cells showed arrangement and morphologic changes gradually when the B[a]P concentration was above 30 μmol · L-1. Genistein (30 μmol · L-1 )inhibited the increase of SIRT1 luciferase activity induced by B [a]P ,and there was significant difference compared with control group (P <0.05).The immunohistochemistry results showed that EGFR expression level in lung cancer tissue was higher than that in normal lung tissue (P < 0.001).Conclusion:EGFR can regulate the B [a]P-induced SIRT1 expression in BEAS-2B cells, and to cause lung chronic inflammation;EGFR/SIRT1 signal transduction pathway may play a role in the occurrence and development of lung cancer.

A 26?year?old male patient presented with facial depigmented patches for 10 years, some of which turned to blue?grey 7 years prior to the presentation. Before the white patches turned blue?gray, the patient developed contact dermatitis due to topical application of self?made drugs. Skin examination showed blue?gray hyperpigmentation on the left upper lip and in the temporal region, with white hairs in the hyperpigmented lesions on the left upper lip. Dermoscopy revealed irregularly shaped, light to dark blue?gray patches on the left upper lip, which were intermingled with white patches in some regions, and white hair stubs were observed in the white patches. Histopathological examination of temporal lesions showed decreased melanocytes in some regions in the epidermis, pigmentation in both basal and suprabasal layers, perivascular infiltration of a small number of chronic inflammatory cells and melanophages in the superficial dermis, and melanophage infiltration around sweat ducts. Finally, the patient was diagnosed with blue vitiligo. He refused to receive any treatments, and follow?up was under way.
