Diabetic retinopathy (DR) is the most serious and common blinding ocular complication in all microangiopathy of diabetes mellitus.The occurrence of DR associated with a variety of factom,the pathogenesis has not yet been completely clear,and there are several theories.In recent years,the relationship between the occurrence of DR and sex hormone levels become a hot ophthalmic research,research shows that the prevalence of DR significantly increased in postmenopausal women,in which estrogen change plays the main role.Learning connection between estrogen and DR will contribute to the clinical diagnosis and treatment of DR.This article reviews recent research advances in the relationship between estrogen and DR.
2.Case Series Reports of 40 Pediatric Cold Children with Lung-stomach Heat Retention Syndrome Treated by Yinlai Decoction Addition and Subtraction

Objective]To observe the clinical effect of Yinlai Decoction Addition and Subtraction on treating Pediatric Cold about lung-stomach heat retention syndrome.[Method]This study adopted the research design method of case series reports, continuously included 40 Pediatric Cold children with lung-stomach heat retention syndrome treated by Yinlai Decoction Addition and Subtraction from November 2011 to December 2012. Through clinical visiting, we could assess the treatment outcome of this time on the basis of evaluation standard. [Results]33 patients were cured, 5 patients turned better and 5 patients had no effect, the total effective rate was 95.00%. [Conclusion]Yinlai Decoction had clearing and dispersing function, it could treat lung and stomach simultaneously, therefore, it had obvious efficacy in Pediatric Cold about lung-stomach heat retention syndrome.

Establishing medical service payment system reform organization of diagnosis related groups and prospective payment system(DRGs-PPS) based on the theory of matrix management and learning organization.Firstly joint committee mechanism hasbeen established to guide the implementation of DRGs-PPS with the strong support of Beijing local government; Secondly series of related technical standards and organizational systems including AP-DRGs,AR-DRGs Group,Data Dictionary,ICD Group,International Classification of Surgical Operations Group,Pricing Classification Group,Training Group,Supervision Group,Support Group,Data Validation Group,BJ-DRGs Group have been created to support standard maintenance,project inspection and data analysis; Furthermore,learning organization has been realized by setting up various project groups and the establishment of DRGs-PPS platform help ensure performance evaluation of healtheare institutions and payment system reform.
4.Rhythmic changes of ambulatory blood pressures in pre-eclampsia patients

Objective To study the rhythmic changes of 24 h ambulatory blood pressure in pre-eclampsia(PE)patients and come up with coping strategies. Method The levels and circadian variations of blood pressures in 240 pre-eclampsia patients were monitored and observed with 24 h ambulatory blood pressure monitoring. Results Among the 240 PE cases,121 were diagnosed as mild PE and 119 as severe PE.There were significant differences in the average of 24 h circadian variation,the average of date time variation,decreased blood pressure at night time between the two groups(all P<0.05). Conclusion Through ambulatory blood pressure monitoring,nurses can monitor the rhythmic changes of 24 h ambulatory blood pressure,make reasonable arrangements for the treatment and care operations,reduce the factors that induce eclampsia and ensure the safety of mother and baby .
5.Clinical efficacy on pediatric recurrent pneumonia treated with point application in summer for the prevention in winter.

OBJECTIVETo study retrospectively the clinical efficacy on pediatric recurrent pneumonia treated with point application in summer for the prevention in winter, as well as the relationship of age, sick duration, attack frequency and skin reaction with the clinical efficacy.
METHODSOne hundred and thirty-five cases of pediatric recurrent pneumonia were divided into a one-year group, a two-year group and a three-year group, 45 cases in each one according to the duration of treatment. The acupoints for the application were Dingchuan (EX-B1), Feishu (BL 13), Gaohuang (BL 43) and Danzhong (CV 17) with the same herbal plaster (prepared with rhizome corydalis, semen brassicae, euphorbia kansui and asarum sieboldii at the ratio of 2:2:1:1) on the first day of each of the three periods of the hot season, 2 to 4 h in each treatment. The attack frequency and change rate were observed before and after treatment in the three groups. The clinical efficacy was assessed in the three groups.
RESULTS(1) After treatment, the attack frequency of pediatric pneumonia was reduced apparently in the three groups (all P < 0.01). The result in the three-year group was less than that in the one-year group and the two-year group and the change rate was the highest (all P < 0.01). (2) After treatment, the sick duration was shortened apparently in the three groups (P < 0.05, P < 0.01). The result in the three-year group was the most remarkable, statistically and significantly different as compared with the other two groups (both P < 0.01). (3) The total effective rate in the three-year group was better than that in either of the other two groups [84.4% (35/45) vs 51.1% (23/45, P < 0.01), 84.4% (35/45) vs 71.1% (32/45, P < 0.05)]. (4) The total effective rate in the children aged from 4 to 7 years was better than that in the group aged from 8 to 10 years and the group aged from 11 to 14 years [79. 7% (47/59) vs 71.7% (33/46, P < 0.05); 79.7% (47/59) vs 43.3% (13/30, P < 0.01)]. (5) The total effective rate in the children with the sick duration ≥ 4 year was lower than that in the group with the sick duration <2 years and that 2 to 4 years (both P < 0. 01). (6) The total effective rate in the children with the annual attack frequency of 2 to 4 times was better than that with the frequency ≥ 4 times (P < 0.01). (7) For the cases with skin reaction after treatment, the total effective rate was better than that in the cases without reaction (P < 0.05).
CONCLUSIONThe point application in summer for the prevention in winter reduces the attack frequency of pediatric pneumonia, shortens the sick duration and has achieved the better significant efficacy in the cases of lower age, shorter sick duration, less attack frequency and moderate skin reaction.

6.Observation on the Effect of Lidocaine Treated in Three Different Ways on the Prevention of General Anesthesia in the Elderly Patients

Objective:To compare the three effects of lidocaine in the prevention of general anesthesia in elderly patients.Methods:A total of 120 elderly patients (65-85 years old) underwent anesthesia with general anesthesia (ASA) Ⅰ ~ Ⅱ were randomly divided into thyrocricocentesis group (group H),throat surface anesthesia group (Group Y),intravenous injection group (group J) and control group (group D).Group H was injected with lidocaine for surface anesthesia;group Y used laryngeal spray for laryngeal sprayed lidocaine for surface anesthesia;group J was anesthetized induction of intravenous lidocaine to prevent intubation reaction;group D the control group was not treated with lidocaine.(SBP) and heart rate (HR) were measured before and after induction (T0),tracheal intubation (T1) and 1 (T2),3 (T3) and 5 min (T4),the changes of hemodynamics related indexes were compared.Results:Compared with the same group of T0,the SBP and HR of four groups of T1 moments were significantly decreased (P <0.05);In the other three groups,SBP was significantly increased at T2 and T3 (P <0.05),HR ofT2 was significantly increased (P <0.05);Compared with group D,SBP and HR in group H,Y and J were significantly different at T2,T3 and T4,he difference was statistically significant (P <0.05);but there was no statistically significant difference between the three groups (P> 0.05).Conclusion:The three ways of lidocaine can be used to prevent the general anesthesia reaction in elderly patients,the effect of three ways is parallel.However,cricothyroid membrane puncture increased the patient's pain,throat spray method increased the cumbersome operation and enhanced the cost of the patient,and intravenous injection method is simple and worthy of popularization and application in clinic.
7.Effect of vasoactive intestinal peptide (VIP) on the content of IL-17A in the brain tissue of rats with experimental autoimmune encephalomyelitis (EAE)

Objective To explore the effect of vasoactive intestinal peptide (VIP) on the content of IL-17A in the brain tissue of rat models of experimental autoimmune encephalomyelitis ( EAE) .Methods Sixty healthy female Wistar rats were randomly divided into normal control group, EAE control group, low-dose VIP group and high-dose VIP group. Ten healthy guinea pigs were used to prepare anti-IL-17A antibody.Myelin basic protein ( MBP) +complete adjuvant ( CFA) were used to establish the EAE model.Since the first day of modelling, the low-dose and high-dose VIP groups received intraperitoneal injection of VIP 4 nmol/kg (0.2 mL) and 16 nmol/kg (0.8 mL), respectively, every other day for 10 consecutive days.The normal control group and EAE group were injected with 0.8 mL saline instead of VIP.The incubation period, progression and the peak of neurological dysfunction scores ( NDS) of the rats were recorded.The levels of IL-17A in the brain tissue was determined by ELISA assay, and the GFAP+astrocyte activation in brain at morbidity peak in the rats was examined using anti-GFAP ( glial fibrillary acidic protein) antibodies.Results The incubation period were extended, the progression period was shortened and the peak neuological dysfunction score ( NDS) was decreased in the VIP-treated groups, in a dose-response relationship.The cytokine levels of IL-17A and the astrocyte activation degree in brain tissue were reduced in each VIP dose group, in a dose-response relationship.Conclusions VIP exerts therapeutic effect on experimental autoimmune encephalomyelitis through lowering the IL-17A content and inhibition of astrocyte activation in the brain tissue.
8.The relationship between MS and ischemic cerebrovascular disease

Objective This study is designed to elucidate the relationship between MS and ischemic cerebrovascular disease.Methods MS was defined using the criteria of the International Diabetes Federation (IDF),divided into the metabolic group (MS group) and without metabolic group (non-MS group).Results (1)The prevalence of MS in the ischemic cerebrovascular disease patients was 52.7%.(2)The logistic regression analysis about MS and ischemic cerebrovascular disease.(3)The rate of carotid artery IMT thicken,carotid artery plaques,cerebral stenosis in MS group is higher than non-MS group(P<0.05).(4)Patients in MS group mainly with cerebral infarction LAA type,patients in non-MS group with SAO type cerebral infarction primarily.(5)Levels of adiponectin in plasma from patients in MS groups is below the average compared to those in plasma from patients in non-MS group(P<0.01).Conclusion (1)MS is a very common and important risk factor of the ischemic cerebrovascular disease.(2)In the cerebral infarction concurrent with MS patients,the highest correlation between incidence of ischemic cerebrovascular disease is hypertension.In turn,high triglycerides for diminishing hyperglycemia,central obesity (waistline),various abnormal index interaction,makes MS increased the risk of ischemic cerebrovascular disease.(3)MS promote cerebral atherosclerosis,is closely with cerebral stenosis.(4)The levels of adiponectin are decreased in patients with metabolic syndrome and ischemic cerebrovascular disease.
9.Transfection of double gene co-expressing adenovirus vector into arthritis rats

BACKGROUND:Abnormal activation of lymphocytes and nuclear factorκB-dependent non-specific inflammation are two major manifestations of joint damage in rheumatoid arthritis. Co-stimulatory signal CD40/CD40L is the dominant co-stimulatory factor in the recognition and activation of T cel s. IκBαeffectively inhibits nuclear factorκB pathway, prevent the inflammation in the central link, and suppress the damage caused by inflammatory factor in the synovial tissue.
OBJECTIVE:To investigate the therapeutic effect of double gene co-expressing adenovirus vector on arthritis based on an arthritis model rat transfected by CD40LIg-IRES2-IκBαco-expressing adenovirus vector.
METHODS:The pAdCD40LIg-IRES2-IκBαco-expressing adenovirus vector was established. Arthritic model was established through multi-subcutaneous injections of complete Freund's adjuvant of type col agen II (1 g/L) into Wistar rats. Then 20 arthritic rats were divided into two groups:untreated group and transfection group, receiving an injection of saline and pAdCD40LIg-IRES2-IκBαadenovirus vector to distal joint cavity of limbs, respectively.
RESULTS AND CONCLUSION:At 14 days post-transfection, compared with the untreated group, the mean arthritis index score, the CD40L expression of lymphocytes in synovial fluid, the nuclear factor-κB p65 expression in synovial tissue, and levels of interleukin-2, interleukin-6, tumor necrosis factor-α, matrix metal oproteinase-3 and matrix metal oproteinase-9 in synovial fluid of rats in transfection group were significantly lower than those in untreated group. Focal transfection of the CD40LIg-IκBαco-expression adenovirus vector can effectively inhibit arthritic symptoms, and reduce the expressions of inflammatory cytokine in synovial fluid and inflammatory molecule in synovial tissue of arthritic rats, which shows good therapeutic effect.
10.Effect of asymptomatic hyperuricemia after kidney transplantation on kidney function of graft

Objective To investigate the effect of asymptomatic hyperuricemia after renal transplantation on renal function of the grafts.Methods The follow-up data were retrospectively collected and analyzed in 144 patients with renal transplantation from January 2010 to March 2015.The patients were classified into three groups according to the level of serum uric acid (SUA):group A (normal group),group B (asymptomatic hyperuricernia with average SUA less than or equal to 360 μmol/L after treatment),and group C (asymptomatic hyperuricemia with average SUA greater than 360μmol/L after treatment).The renal function indexes such as serum creatinine (SCr) and estimated glomerular filtration rate (eGFR) were compared among three groups from 12 to 48 months after transplantation.Results The SCr and eGFR showed no significant difference between group A and group B at 12th month (P>0.05),but ther are superior than Group Ⅲ (P<0.05).Conclusion After renal transplantation,asymptomatic hyperuricemia can lead to impaired renal function,and there are no significant differences in renal function between renal transplant recipients with normal SUA levels after treatment and those without hyperuricemia.
