Objective To explore the value of plasma brain natriuretic peptide test in the diagnosis and treatment of elderly heart failure patients with normal ejection fraction,and to provide reference for clinical diagnosis and treatment of the disease.Methods 104 patients with heart failure were selected,at the same time,30 elderly patients with normal heart function in our hospital were selected as the control group.The plasma brain natriuretic peptide and cardiac color Doppler ultrasound were examined.The plasma brain natriuretic peptide and left ventricular ejection fraction,left ventricular posterior wall thickness,left ventricular mass index,left ventricular diastolic diameter and ventricular septal thickness were compared between two groups ; at the same time,peptide and left ventricular plasma brain natriuretic ejection fraction were compared in heart failure patients with different levels of NYHA.Results The plasma level of brain natriuretic peptide,left ventricular posterior wall thickness,left ventricular mass index in the heart failure group were (401.37 ± 368.63) ng/L,(13.42 ± 1.33) mm,(130.62 ± 9.18) g/m2,which were significantly higher than those of the control group (all P ＜ 0.05).The left ventricular ejection fraction of the heart failure group was (51.36 ±4.41) ％,which was significantly lower than the control group(P ＜0.05).The left ventricular ejection fraction of patients with different NYHA grading in the heart failure group was significantly lower than that of the control group(all P ＜0.05),plasma brain natriuretic peptide levels in patients with different NYHA grading were significantly higher than those in the control group,and with the elevated levels of NYHA classification,the plasma brain natriuretic peptide level increased significantly(P ＜ 0.05).Conclusion The level of plasma brain natriuretic peptide in elderly heart failure patients with normal ejection fraction was significantly increased,and with the increase of NYHA grade,plasma brain natriuretic peptide level rised,it is important to assess diagnosis and severity of this disease.

Objective To study autonomic nerve regulator character and arrhythmias in the patients with essential hypertension(EH) and coronary heart disease(CHD). Methods The heart rate variability(HRV) and arrhythmias in 50 EH, 44 CHD, 52 EH + CHD patients and 35 healeh elderly subjects were investigated. Results There were significant differences of HRV in the patients with EH, CHD and healthy elderly subjects(P＜0.05).There was much significant difference of HRV between the patients with EH + CHD and healthy elderly subjects(P＜0.01), but no significant difference of HRV between the patients with EH and CHD(P＞0.05). The incidence rate of the premature atrial contraction, premature ventriculax contraction and atrial tachycardia in EH, CHD, EH+CHD and healthy elderly subjects, there were much significant differences( P＜0.01 ). Except the incidence rate of premature atrial contraction there was significant difference between the patients in CHD and EH + CHD (P＜0.05) ,and no significant differences of the incidence rate of other kinds of arrhythmias in the patients with EH,CHD and EH + CHD(P＞0.05 ). Conclusion The autonomic never function of EH and CHD patients was damaged, and the incidence rate of arrhythmias was increased(the premature atrial contraction often happened, then the premature ventricular contraction), and arrhymias happened in EH + CHD patients is most oftenly. The vagal hypoacfivity and sympathetic hyperactivity were major causes of reduction of HRV and probable causes in the elderly patients.

The existence of HBV-DNA in the mononuclear cells of peripheral blood and in the serum of 61 patients with HBV infection was determined with southern blot and dot blot hybridization,and that in the liver tissue of 31 patients out of the 61 with southern blot and in situ hybridization.The positive rate of HBV-DNA in the serum,mononuclear cells and hepatocytrs was 26.2% (16/61),24.6% (15/61) and 44.8% (13/31) respectively.There was no concordance of the existence of HBV-DNA in the serum,peripheral mono-nuclear cells and hepatocytes in an individual.For example,HBV-DNA was absent in the serum but present in mononuclear cells and hepatocytes in 11 cases.In fact,peripheral mononuclsar cells can serve as the reservoir for HBV to replicate.It cannot be denied that HBV can replicate in an individual even though HBV-DNA is negative in the serum.

Objective To investigate the effect of atorvastatin on cardiac fibrosis and on the expression of NF-?B and TNF-? in a rat model of type 2 diabetes mellitus.Methods Sixty SD rats were randomly divided into 3 groups.In control group(n=20) the rats were fed with normal food and gavaged with normal saline.In diabetes group(n=20) the rats with type 2 diabetes was induced by intraperitoneal injection of low doses of streptozotocin(STZ),and the rats were fed with high fat diet.In atorvastatin group(n=20) diabetes was reproduced in rats and atorvastatin 20mg/(kg?d) was gavaged for 12 weeks.The pathological changes in myocardial cells were observed under light microscope.Collagen content was observed by Masson staining.The contents of TNF? were determined in plasma and cardiac tissue with RIA.RT-PCR was used to study the gene expression of NF-?B and TNF-?.The proteins of NF-?B p65 and TNF-? were assayed by immunohistochemical method.Results It was showed under light microscope that cardiac myocytes were compact and orderly with clear structure in control group.The cardiac myocytes were disorderly,distorted,with infiltration of inflammatory cells in the interstitium in diabetes group.The cardiac myocytes were orderly arranged,and the structure was close to normality in atorvastatin group.TNF-? in diabetic rats increased significantly in plasma and myocardium(P

Objective To study the effect of drug combined with psychological intervention on mental status and quality of life in patients with multi drug resistant pulmonary tuberculosis.MethodsIn Jiashan Second People's Hospital from January 2011 to December 2016 were 100 cases of multi drug resistant tuberculosis patients as the research object in the course of the study, were randomly divided into control group and experimental group two were 50 cases each.Patients in the control group were given routine drug treatment, and the patients in the experimental group were treated with psychological intervention on the basis of drug treatment.Comparison of the experimental group and the control group after 12 months of treatment, depression score and quality of life score.ResultsBefore treatment, there was no significant difference between the experimental group and the control group in the scores of life quality and the scores of depression and anxiety, which was not statistically significant.After treatment in December, the anxiety score of the patients in the experimental group was (41.2±8.2), and the score of depression was (36.4±4.5).The total score of quality of life was (75.2±11.2).The anxiety score of the control group was (52.3±7.4), and the score of depression was (45.4±5.6).The total score of QOL was (64.2±9.3).Thus, after the corresponding treatment, the experimental group of patients with depression and anxiety scores were significantly lower than those in the control group, the quality of life scores of the control group was significantly lower than the experimental group, with statistical difference (P<0.05).ConclusionThe drug combined with psychological intervention in treatment of multi drug resistant pulmonary tuberculosis can improve the therapeutic effect to a great extent, help the patients to relieve anxiety and depression and other negative emotions, improve the quality of life, with further clinical promotion and application significance.

Objective To evaluate the relationship between ambulatory arterial stiffness index (AASI) and intracranial and extracranial arterial stenosis in 146 patients with ischemic cerebrovascular disease. Methods Degree of intracranial and extracranial arterial stenosis in 146 patients with ischemic cerebrovascular disease was assessed by CT angiography (CTA). Then all patients were divided into 4 groups according to the numbers of branches with stenosis: no stenosis group, 1 branch group, 2 branches group, and 3 and more than 3 branches group. Dynamic blood pressure was monitored to calculate the AASI. Results There was no significant difference of AASI among the extracranial arterial stenosis groups (P=0.614). AASI was positively correlated with the degree of intracranial artery stenosis (r=0.743, P<0.05), and with 3 and more intracranial artery branches stenosis (r=0.797, P<0.05). Conclusion AASI is closely correlated with the degree and numbers of branches of intracranial artery stenosis.

Objective The aim of this study was to observe the outcome and evaluate the clinical efficacy of pulsed electromagnetic fields (PEMF) on pain management in patients with osteoporosis.MethodsA total of 58 patients with osteoporosis were treated with PEMF stimulation.Pains as main indicators of their symptom were evaluated before and after treatments.The clinical outcomes were assessed by observing the visual analogue scale (VAS).Statistical analysis was carried out to support the data.Results After PEMF treatment,the patients exhibited significant pain relief from mean VAS of 6.97±1.83 to 3.36±1.69(t-test,P＜0.05 ).Of the 58 patients,28 obtained significant symptom alleviation and 18 obtained moderate symptom alleviation,while 8 obtained slight alleviation,indicating the total clinical efficacy of 93%(54/58).Conclusion The current study provides the clinical outcomes indicating that PEMF may help to release the pain from primary osteoporosis and it might be an applicable supplementary treatment method for the osteoporosis related pain.

OBJECTIVE:To investigate the compatible stability of Voriconazole for injection after mixed with Fructose injec-tion or Invert sugar injection. METHODS:Referring to package inserts,Voriconazole for injection 200 mg was dissolved with Wa-ter for injection to 20 mL,and then combined with Fructose injection 250 mL and Invert sugar injection 250 mL,respectively. At room temperature,the appearance of mixtures were observed 0,1,2,3,4,5 h after mixing,and pH value and the number of in-soluble particles were determined;the content of voriconazole was determined by HPLC. RESULTS:Under above condition,the appearance and pH value of mixtures had no significant change within 5 h;the number of particles ≥10 μm and ≥25 μm were all in line with the standard of Chinese Pharmacopoeia (2015 edition);the relative content of voriconazole was decreasing (95.28%-100%),but it changed within ±5%(RSD<2%,n=6). CONCLUSIONS:Voriconazole for injection could keep stable within 5 h after mixed with Fructose injection or Invert sugar injection.

Objective To amplify natural killer (NK) cells in vitro and explore its killing effect on ovarian cancer cells.Methods (1) The separation of NK cells and identification.A total of 20 ml peripheral blood of one healthy volunteer was collected in Nov.2015,Peking University People's Hospital.The peripheral blood mononuclear cells of normal volunteers were isolated,cultured in vitro and amplificated cultivation for 14 days with K562 cells transfected and expressing interleukin 21 (IL-21-K562) as nourish cells.The number and dynamic state of the growth cells were monitored during the cultured process.Cells were harvested and counted after 14 days cultured.The NK cells phenotypes were detected by flow cytometry.(2) The killing effect of NK cells on ovarian cancer cells:the ratio of effector cells (NK cells) and target cells (ovarian cancer cells and its control) was 50∶ 1,20∶ 1,10∶ 1,5∶1 or 1 ∶ 1,NK cells killing effect on ovarian cancer cells was detected by the lactate dehydrogenase (LDH) release experiments.Results (1) The results of NK cells establishment and phenotypic characterization:the cells were induced in vitro for 14 days by amplification culture.With the extension of incubation time,the number of NK cells increased constantly,from 2.0× 107 on day 0 to 5.1 × 109 on day 14.Obvious amplification of the total number of cells were detected for 255 times.Living cells unstained by trypan blue eventually reached 95％ above.Before and after the induction and amplification in vitro,the percentage of NK cells (CD3-CD56+cells) in CD3-cells were 2.33％ and 85.32％,respectively (P＜0.01),which covered the whole lymphocytes 1.06％ and 69.42％,respectively (P＜0.01),which showed that NK was the main cell type in the amplificated lymphocytes.(2) The killing rate of NK cells on ovarian cancer cells in vitro:the results detected by LDH release experiments showed that NK cells could performed strong nonspecific killing effect on ovarian cancer cell lines SKOV3,HOC1A,3AO and CAOV3,as well the normal ovarian cell line T29 and NK sensitive cell line K562,and the killing effect increased significantly along with the increase of effector cells and target cells ratio (P＜0.01).When the ratio was 1 ∶ 1,the killing rate was 37％ for K562,while the rate of killing of other cells was around 10％ (P＜0.05).When the effect-target ratio was 20∶1 and 50∶ 1,in addition to CAOV3 cells (more than 70％),NK cells had a kill rate of more than 80％ for other ovarian cancer cells lines and their control cell K562 and T29 cells (P＞0.05).Conclusion NK cells could be established in vitro and have a good non-specific killing effect on ovarian cancer cells.
