4.The influence of continuous renal replacement treatment on renal damage and inflammatory mediators of patients with severe acute pancreatitis

Objective To investigate the influence of continuous renal replacement treatment on the therapy of severe acute pancreatitis(SAP) associated with kidney injury(AKI) and inflammatory mediators.Methods Clinical data of 54 SAP patients complicated with AKI,who received conventional treatment (control group) or conventional therapy combined with CRRT(observation group) were retrospectively analyzed.The clinical efficacy between the two groups before and after treatment was compared.Results The mechanical ventilation proportion in control group was significantly higher than in the observation group (x2 =4.464,P < 0.05).The mortality rate between the two groups had no statistical di.fference(P > 0.05).In observation group,TNF-alpha and IL-6 levels were significantly lower than before treatment(P <0.01),APN was significantly increased (P < 0.01).APN also increased in the control group,but the extent was less than the observation group.The levels of TNF-αand IL-6 in the control group were significantly higher than before treatment(P < 0.01).The urine output of observation group increased significantly after 72h therapy (P < 0.01).And blood urea nitrogen (BUN),creatinine (SCr),APACHE Ⅱ score were significantly decreased(P < 0.01),in the control group,urine output also increased (P < 0.01),but the extent was less than the observation group,APACHE Ⅱ score had no significant difference compared with before treatment(P >0.05).In control group,BUN and SCr levels significantly increased than before treatment (P < 0.05).Conclusion CRRT for the treatment of SAP patients complicated with AKI can significantly improve renal function and reduce mortality,and its mechanism may be clear a large number of inflammatory mediators and metabolites in body,reduce the body inflammatory response.
5.Comparison of the Content of Anthraquinones in the Decoction and Maceration of Rheum officinale

OBJECTIVE: To compare content of anthraquinones in the decoction and maceration of Rheum officinale.METHODS: Aloe emodin and four other components in R.officinale were taken as analysis objects.HPLC was used to determine the content of nomadic anthraquinone,conjugated anthraquinone and total anthraquinone in R.officinale during decoction process and maceration process.RESULTS: The linear range of aloe emodin,rhein,emodin and chrysophanol were 0.03~0.64?g and the linear range of physcion were 0.01~0.34 ?g.The concentration of total anthraquinones in decoction process was much higher than in maceration process.The change of dissolution of nomadic anthraquinones in two processes kept stable with process time.The content of conjugated anthraquinones in two processes was all decreased to some extent.CONCLUSION: The concentrations of anthraquinones in R.officinale are influenced by decoction process and maceration process.
6.Biomechanical features of cervical internal fixator:Finite element analysis

Finite element analysis is a matrix method for structure analysis. It divides a continuum composed of infinite material points and finite degree of freedom into an aggregation with finite small units. Finite element analysis is used to analyze the biomechanical properties of cervical vertebra under pathologic and physiologic conditions and the influence of different surgeries and fixation device on cervical biomechanics. In addition,the application of finite element analysis in cervical biomechanics study is prospected. Due to differences between finite element model and actual human body,calculation results are always limited and fixed. With development of computer and software,finite element analysis will become a focus of spinal mechanics.
7.Progress of laboratory diagnosis of acute pancreatitis in children

The traditional laboratory markers of acute pancreatitis in children include serum amylase and urine amylase, serum lipase, urinary trypsinogen-2, C-reactive protein, cytokines, amylase and creatinine clearance ratio and so on. They can be simply detected and economic, but the sensitivity and specificity are not satisfied. Several new laboratory markers of acute pancreatitis in children, which include procalcitonin, phospholipase A2, PankrinTM, trypsinogen activation peptide, carboxypeptidase B activation peptide, human pancreas-specific protein/procarboxypeptidase B, polymorphonuclear elastase, serum amyloid protein A, and as well as intercellular adhesion molecule- 1, have been found and applied in recent years .They can provide reliable judgment methods for early diagnosis and prediction of the severity of acute pancreatitis in children, but most of the new laboratory markers are rarely used in clinic because they are difficult to be detected and expensive. More sensitive, specific, cheap and operable laboratory markers which can be used in clinic are to be explored.

Objective] The article summarizes the treatment based on syndrome differentiation experience of flower medicine in functional gastrointestinal diseases.[Methods] It was summarized about the academic point of view of TCM flowers in functional gastrointestinal diseases and experience in clinical practice through the studying for flower herbs in the Classic and teacher's clinical practices as well as my practices, and with cases. [Results]It involves in various factors such as TCM theories of qi and blood yin and yang of the spleen and stomach as well as abnormality of emotion, mind disorder, pathological phlegm, diet and environment Qi, based on the function disharmony of spleen and stomach. And natural plant-flowers, it is well-informed, high nutrient, sweet tone, promoting the circle of qi and blood, relieving pain and improving emotions and sleeping, containing a variety of pharmacological components, less toxicity and side effects, in different doses and compatibility rules, it generates multiple synergistic effect.[Conclusions] Traditional Chinese flower medicine has the unique feature, and is worthy of popularization and application.
9.Analysis and Countermeasures of Quality of Warehouse Entry Inspection for Chinese Decoction Pieces in Our Hospital

Objective To establish relevant management measures and guarantee the safety of clinical medication by analyzing the quality of warehouse entry inspection for Chinese decoction pieces.Methods According to the records of warehouse entry inspection for Chinese decoction pieces during January 2014 to December 2014, reasons for unqualified Chinese decoction pieces were collected and analyzed.Results Our hospital checked and accepted 2376 batches of Chinese decoction pieces, among which 98 batched were unqualified (4.12%); the reasons for unqualified Chinese decoction pieces mainly included improper storage, adulteration, and nonstandard preparation, which accounted for 48.97%, 19.39% and 18.37% respectively.Conclusion There are some problems in the Chinese decoction pieces in our hospital. Dispensary of TCM should enhance the management of check and acceptance of Chinese decoction pieces, and improve quality for check and acceptance and quality of Chinese decoction pieces.
10.The climical value of serum progesterone and estradiol to the diagnosis of embryo arrest in early pregnancy patients

Objective To analyze the clinical relationship of progesterone(P) and estradiol(E2) for embryo arrest in early pregnant patients.Methods 118 patients with embryo arrest in early pregnancy were selected and divided into two groups,research group (58 cases,embryo arrest in early pregnancy) and control group (60 cases,threatened abortion but embryo keeping growth by tocolysis).Serum P and E2 were measured and compared between two groups.Receiver operating characteristic curves(ROC) of all indexes were drawn and the areas under the ROC curves were calculated to evaluate diagnostic value.Results Compared to control group,the serum P and E2 were lower in research group(P < 0.05).To diagnosis embryo arrest,the threshold for P was less than 118.4nmol/L,with 93.9% sensitivity and 87.3% specificity; while the threshold value for E2 was less than 1110pmol/L,with 95.6% sensitivity and 90.7% specificity.Conclusion Detection of serum P and E2 may be helpful for the diagnosis of embryo arrest in early pregnancy,and the serum E2 had the best diagnostic value.
