1.Experience of Professor HE Tian-you in treating chronic prostatitis with combined acupuncture and herbs

To summarize clinical experience of Professor HE Tian-you in treating chronic prostatitis with combined acupuncture and herbs.His originally created acupoint ‘san yin xue’as the main points in the acupuncture treatment,and other acupoints were selected according to pattern and symptoms.He emphasized Deqi (acupuncture esthesia) and was good at performing needling manipulation. Based on the main pathogenesis of obstruction of liver liver channel,Prof HE chose Bupleurum chinense,Turmeric Root Tuber,Rhizoma,Szechwan Chinaberry Fruit,litchi,Oldenlandia and spina gleditsiae to compose the main formula.Furthermore,some other herbs were added or subtracted in main formula according to pattern and symptoms.
2.Meta-analysis on Randomized Controlled Clinical Trials of Acupuncture and Moxibustion for Rheumatoid Arthritis

Objective To systematically assess the effectiveness and safety of acupuncture and moxibustion for treatment of rheumatoid arthritis. Methods Articles were searched from databases, i.e., CNKI, VIP, WanFang Data, CBM, Embase, and PubMed. Articles about RCT on acupuncture and moxibustion for rheumatoid arthritis and traditional medicine therapy were under meta-analysis. Results Totally 24 articles involving 2006 patients were included. The results of meta-analysis indicated that the overall effective rate of treatment in the group receiving acupuncture and moxibustion was higher than that in the drug group, with statistical significance [OR=2.73, 95%CI (2.18, 3.41), Z=8.78, P<0.000 01]. Sub-group analysis showed that:acupuncture and moxibustion showed statistical significance in decreasing serum RF levels compared with drug therapy [MD=-2.92, 95%CI (-5.60, -0.25), P<0.05]. Acupuncture and moxibustion showed statistical significance in decreasing serum ESR levels compared with drug therapy [MD=-2.99, 95%CI (-4.78, -1.21), P<0.05]. Acupuncture and moxibustion showed statistical significance in decreasing serum CPR levels compared with drug therapy [MD=-1.44, 95%CI (-2.44, -0.44), P <0.05]. Conclusion The overall effective rates of acupuncture and moxibustion for rheumatoid arthritis and the decreasing levels of RF, ESR, CRP are superior to drug therapy, but it still needs larger samples, multi-center, and high quality RCT to verify.
3.Effect of Heat Reinforcing Acupuncture Manipulation on Articular Synovium LDH, SDH and CCO in Rheumatoid Arthritis ;Rabbits

Objective To investigate the regulating effect of heat reinforcing acupuncture manipulation on body energy metabolism enzymes in rheumatoid arthritis (RA) and preliminarily explain the mechanism of heat-producing action of heat reinforcing acupuncture manipulation. Method Forty chinchilla rabbits were randomized into normal, model, equal reinforcement and reduction, twirling reinforcement, and heat reinforcing acupuncture manipulation groups. A model of cold syndrome-type RA was made by ovalbumin induction and exposure to low temperature in the other four groups not including the normal group. From two days after successful model making, the normal and model groups were grabbed and fastened (bound) by the same way as for the acupuncture groups, 30 min once daily. The equal reinforcement and reduction group received even reinforcing-reducing method;the twirling reinforcement group, twirling reinforcement method;the heat reinforcing group, heat reinforcing acupuncture manipulation. The needle was manipulated for 1 min and retained for 30 min once daily, for a total of seven days. The RA rabbit knee joint circumference was measured and the inflammation score was recorded according to synovial histopathological sections before and after treatment. After the completion of intervention, the rabbits were sacrificed and the articular synovium was rapidly separated for frozen sections. Articular synovium lactate dehydrogenase (LDH), succinate dehydrogenase (SDH) and cytochrome oxidase (CCO) activities were measured by histochemical staining. Result After acupuncture intervention, the RA rabbit knee joint circumference was shortened in all the equal reinforcement and reduction, twirling reinforcement, and heat reinforcing acupuncture manipulation groups, but the shortening effect on the RA rabbit knee joint circumference was better in the heat reinforcing acupuncture manipulation group than in the equal reinforcement and reduction and twirling reinforcement method groups (P<0.05);the inflammation score recorded according to the RA rabbit synovial histopathological sections was also decreased more in the heat reinforcing acupuncture manipulation group than in the equal reinforcement and reduction and twirling reinforcement method groups (P<0.05). Synovial LDH integral optical density, total positive area and area percentage were significantly higher in the model group than in the normal group (P<0.05), and significantly lower in the equal reinforcement and reduction, twirling reinforcement, and heat reinforcing acupuncture manipulation groups than in the model group(P<0.05) and also in the heat reinforcing acupuncture manipulation group than in the equal reinforcement and reduction and twirling reinforcement method groups (P<0.05). Rabbit synovial SDH and CCO activities were significantly higher in the model group than in the normal group (P<0.05). Rabbit synovial SDH and CCO integral optical density, total positive area and area percentage were significantly higher in the equal reinforcement and reduction, twirling reinforcement, and heat reinforcing acupuncture manipulation groups than in the model group (P<0.05) and also in the heat reinforcing acupuncture manipulation group than in the equal reinforcement and reduction and twirling reinforcement method groups (P<0.05). Conclusion Heat reinforcing acupuncture manipulation has a definite therapeutic effect on RA and can increase SDH and CCO activities to enhance aerobic metabolism and produce more local energy in a rabbit model of RA. That may be the mechanism by which heat reinforcing acupuncture manipulation produces heat.
4.Evaluation on effectiveness of salmeterol/fluticasone propionate combined with N-acetylcysteine in treatment of chronic obstructive pulmonary disease

Objective To explore the effects of salmeterol/fluticasone propionate complicated or combined with N-acetylcysteine on the pulmonary function and arterial blood gas analysis of the patients with chronic obstructive pulmonary disease (COPD), and to evaluate its curative effect.Methods 84 cases of COPD patients were randomly divided into combination treatment group (n=44)and simple treatment group (n=40).The patients in combination treatment group were treated with salmeterol/fluticasone propionate combinated with N-acetylcysteine while the patients in simple treatment group were treated only with salmeterol/fluticasone propionate, both of which were followed up for 6 months.The changes of pulmonary function (FEV1%FVC,FEV1%Pred,PEF daily variation rate:ΔPEF%)and the arterial blood gas analysis indexes (PaO2 and PaCO2 )of the patients in two groups were recorded before treatment, 3 months after treatment, and 6 months after treatment. Results The FEV1%FVC,FEV1%Pred and PaO2 of the patients in combination treatment group and simple treatment group were obviously increased 3 and 6 months after treatment compared with before treatment (P<0.01 ), and the PaCO2 were obviously decreased (P<0.01).And there were significant differences of the indexes mentioned above of the patients between two groups (P<0.05 );but the indexes of each group had no significant differences between 3 months and 6 months after treatment (P>0.05).TheΔPEF% of the patients in two groups had no significant differences between inter-group and intra-group before and after treatment (P > 0.05 ). Conclusion Combination of salmeterol/fluticasone propionate and N-acetylcysteine can obviously improve the pulmonary function and arterial blood gas analysis indexes of the COPD patients, which is superior to the simple application of salmeterol/fluticasone propionate and has definite and lasting curative effect on the treatment of COPD.

A? plays a crucial role in Alzheimer's Disease and the soluble A? oligomers have been recognized as the emerging neurotoxins,which ultimately cause memory impairment and neuronal loss through different mechanisms.The development of novel drugs targeting A? oligomers indicates new and promising therapy approaches for AD.The pathological roles as the proximal toxins in AD and the compounds,targeting soluble A? oligomers,which are currently in preclinical and early clinical development,are reviewed.

7.Analysis of corticomuscular coherence during rehabilitation exercises after stroke.

To better evaluate neuromuscular function of patients with stroke related motor dysfunction, we proposed an effective corticomuscular coherence analysis and coherent significant judgment method. Firstly, the related functional frequency bands in the electroencephalogram (EEG) were extracted via wavelet decomposition. Secondly, coherence were analysed between surface electromyography (sEMG) and sub-bands extracted from EEG. Further more, a coherent significant indicator was defined to quantitatively describe the similarity in certain frequency domain and phase lock activity between EEG and sEMG. Through the analysis of corticomuscular coherence during knee flexion-extension of stroke patients and healthy controls, we found that the stroke patients exhibited significantly lower gamma-band corticomuscular coherence in performing the task with their affected leg, and there was no statistically significant difference between their unaffected lag and the healthy controls, but with the rehabilitation training, the bilateral difference of corticomuscular coherence in patients decreased gradually.

9.Utility of single fiber electromyography in the differential diagnosis of amyotrophic lateral sclerosis and cervical spondylosis

Objective To assess the utility of single fiber electromyography (SFEMG) in the differential diagnosis of amyotrophic lateral sclerosis (ALS) and cervical spondylotic radiculopathy and myelopathy. Methods SFEMG was performed in extensor digitorum communis muscles (EDC) of 3 groups of patients, including 61 ALS patients with findings of cervical spendylosis on MRI, 59 ALS patients with normal MRI of cervical spine, and 55 patients with cervical spendylotic radiculopathy and myelopathy. The parameters of SFEMG between different groups were compared. Results The mean jitter was (81.2± 25.9), (91.6 ± 32. 4) and (40. 9 ± 11.8) μ in ALS patients with finding of cervical spondylosis on MRI, ALS patients with normal MRI of cervical spine, and patients with cervical spondylosis respectively. M50 of the percentage of jitter >55 μs was 73%, 80% and 5% in the 3 groups respectively. M50 of the percentage of block was 10%, 20% and 0 in the 3 groups respectively. Fiber density was 2.9±0.5, 2.9±0.6 and 2. 4 ± 0. 6 in the 3 groups respectively. There was no significant difference in those parameters of SFEMG between the ALS with findings of cervical spondysosis and those with normal MRI. There was significant difference in those parameters of SFEMG between the patients with ALS (the mean jitter: (86. 3± 29. 6) μs; M50 of the percentage of jitter > 55 μs: 80% ; M50 of the percentage of block: 14% ; fiber density: 2. 9±0. 5) and those with cervical spondylosis (t = 14.49, Z = 8.96, 7. 68,t = 5. 83, all P = 0. 000). In 18 patients with ALS, conventional EMG studies showed active and chronic denervation in only 1 region at the first visit. SFEMG was performed in EDC which had normal Medical Research Council Scale and EMG. SFEMG showed increased fiber density in 16 patients, increased jitter in 13 patients and impulse block in 6 patients. Conclusion SFEMG shows significantly increased jitter and block in ALS whether there is MRI findings of cervical spondysosis or not, which can help to differentiate ALS from cervical spondysotic radiculopathy and myelopathy.
10.Value of single fiber electromyography in diagnosis of amyotrophic lateral sclerosis

Objective To assess the diagnostic value of single fiber electromyography(SFEMG)in amyotrophic lateral sclerosis(ALS).Methotis SFEMG was performed in extensor digitorum communis muscles(EDC)of 165 patients with AIS and 145 patients with other neurogenetic disorders.Grade scale of the medical research council(MRC)in EDC was assessed simultaneously.The patients were divided into four groups according to the MRC scale(MRC>4 us MRC≤4)and different disease (AIS vs other neurogenetic disorders).The parameters of SFEMG between different groups were compared.Results When the strength of EDC was normal(MRC>4),the mean jitter was(66.1±20.1)μs in ALS and (38.0±9.2)μs in other neurogenic disorders(t=9.05,P<0.01),median of the percentage of jitter> 55μs was 55%in AIS and 0 in other neurogenic disorders(Z=-7.81.P<0.01).median of the percentage of block was 6.7%in AIS and 0 in other neurogenic disorders(Z=-6.93,P<0.01).There was significant difference in those parameters of SFEMG between the two groups.When the strength of EDC was abnormal(MRC≤4),the mean jitter was(93.5±31.2)μs in ALS and(52.8±25.9)μs in other neurogenic disorders(t=9.37,P<0.01),median of the percentage of jitter>55 μs was 86%in ALS and 20%in other neurogenic disorders(Z=-8.46,P<0.01),median of the percentage of block was 20%in ALS and 0 in other neurogenic disorders(Z=-7.25,P<0.01).There was significant difference in those parameters of SFEMG between the two groups.When SFEMG was performed in EDC with normal muscle strength,the sensitivity and specificity of mean jitter>55μs for diagnosis of ALS were 70.2%and 92.7% respectively.Conclusions When SFEMG is studied for diagnosis of ALS,it is recommended to perform in muscles with normal strength.The mean jitter,the percentage of jitter>55μ s and block are of great importance in differentiating ALS from other neurogenic disorders.
