Reperfusion therapies(thrombolysis,mechanical thrombectomy,or stenting,etc.) for acute ischemic stroke are the most effective therapy.Reperfusion therapy may limit ischemic tissue enlargement,leading to a reduced infarct size and favorable clinical outcome by restoring the blood flow before the salvageable penumbra became the progress of ischemic brain infarction.intravenous (IV) recombinant tissue plasminogen activator (rt-PA) therapy is the only proven effective treatment of acute ischemic stroke.Although thrombolytic therapy has matured,but the standard intravenous thrombolytic therapy (non-enhanced computed tomography (CT)-guided,3 h time window,the intravenous injection of tPA) have many restrictions,including a short therapeutic time window,recanalization rate was only 50 %,and the major dangers of symptomatic hemorrhagic transformation.As a result,currently in clinical practice,only a minority of patients (usually 1% to 3%) received thrombolytic therapy.So there are some issues still need to be further explored,such as the expansion of thrombolytic time window,a new thrombolytic drugs are used for more than 3h incidence of acute ischemic stroke patients,the evaluation of new reperfusion methods (in particular,multi-modal imaging technology),intravenous,and intra-arterial thrombolysis combined application of thrombolytic therapy,and new anti-platelet drug combination,and the application of mechanical devices or by transcranial Doppler ultrasound to promote the role of thrombolytic drugs.

Objective To investigate the clinical effect of intratympanic injection of dexamethasone in treatment of sudden deafness in different time windows.Methods 60 patients (60 ears) with sudden deafness were selected.Sixty patients were divided into control group and observation group according to the random number table method,30 cases (30 ears) in the control group and 30 cases (30 ears) in the observation group.The two groups were given systemic basic therapy, intravenous vinpocetine, alprostadil improve microcirculation, while hyperbaric oxygen therapy,12 days for a course of treatment, treatment for 2 courses.In the control group, dexamethasone was intravenously administered for 12 days.From the 7th day, dexamethasone was injected into the tympanic cavity and injected once every other day.The observation group was treated with dexamethasone at the same time.Injection of dexamethasone, treatment method and time were the same as the control group.At the end of the treatment, the clinical curative effect of the two groups was compared.The average hearing thresholds of the two groups were observed before and after treatment.The dizziness, tinnitus and hearing recovery time of the two groups were observed.Results The total effective rate was 96.67% in the observation group and 93.33% in the control group, the difference was not statistically significant (x2 =0.97,P> 0.05).After treatment, the average hearing threshold of the two groups was significantly improved (t=7.69,8.23,all P<0.05),the average hearing threshold of the observation group was higher than that of the control group, the average hearing threshold before and after treatment had statistically significant differences (t=2.34,2.67,all P<0.05).In the observation group, dizziness, tinnitus and hearing recovery time were shorter than those in the control group (t=3.32,3.14,3.67,all P<0.05).Conclusion The effect of dexamethasone combined with systemic treatment of sudden deafness in patients compared with intratympanic injection of dexamethasone after intravenous administration of dexamethasone is not significantly different, but the improvement of hearing threshold after treatment compared with the latter is obvious, and the average time to disappear symptoms is shorter than the latter, and therefore worthy of clinical promotion.

Objective To compare one point and two points brachial plexus block anesthesia effect below the elbow surgery anesthesia.Methods 70 patients with elective elbow following orthopedic surgery were randomly divid-ed into two points group (group A)and one point group (group B)by random number table,each group had 35 cases.Analgesic effect of anesthesia was compared.The toxic reactions,neurovascular damage and other vascular complications of anesthesia were observed.Results The effective rate and incidence rate of complication of group A were 97.1% and 8.6%,those of group B were 77.1% and 11.4%(χ2 =6.248,0.159).The analgesic effect of group A was significantly better than group B (P <0.05),the incidence of anesthetic complications between the two groups showed no significant difference (P >0.05).Conclusion Axillary brachial plexus block of two points simul-taneous injection has better analgesic effect,block more perfect,it is the first selection of orthopaedics below the elbow surgery anesthesia method.

Objective To analyze the topology of IFN-induced transmembrane(IFITM) protein in porcine peripheral blood lymphocytes(PBMCs) and detect the change of IFITM mRNA transcription in PBMCs after porcine reproductive and respiratory syndrome virus(PRRSV) infection in vitro.Methods PRRSV,porcine circovirus 2(PCV2) and Japanese encephalitis virus(JEV) negative anticoagulant blood of piglets were collected aseptically and isolated for PBMCs.Porcine IFITM CDS sequence was amplified by PCR,sequenced and analyzed for topology.PBMCs were infected with PRRSV in vitro.Cell samples were collected at 12,24,36 and 48 h after infection,detected for PRRSV infection by RT-PCR,and detected for mRNA transcription level changes of IFITM1,IFITM2 and IFITM3 by RT-PCR.Results The porcine PBMCs were successfully isolated and the full-length sequence of IFITM CDS derived from PBMCs was cloned.The porcine IFITM protein might have two topological structures.PBMCs inoculated with PRRSV for 24 h produced obvious cytopathic effect.PRRSV was replicated in PBMCs.The transcription levels of IFITM1,IFITM2 and IFITM3 mRNA in PBMCs were significantly up-regulated at the early stage of PRRSV infection,and reached the peak at 12h after infection,and then gradually decreased;The transcription level of IFITM1 mRNA increased at 36 h after virus infection and then declined rapidly.Conclusion PRRSV infection in vitro significantly up-regulated the transcription level of IFITM mRNA in PBMCs,indicating that IFITM was involved in the antiviral immune response of PBMCs.This study provided a reference for revealing the natural immune response against PRRSV in vivo.

Objective To screen monoclonal antibodies against different binding sites of connexin 43(Cx43) and establish a double antibody sandwich ELISA method for quantitative detection of Cx43 protein in order to further study the structure,function and subcellular localization of full-length and truncated Cx43 protein.Methods The sequence of Cx43 protein was analyzed by bioinformatics method. The peptide fragments were designed and synthesized for different domains, and coupled with keyhole limpet hemocyanin(KLH) protein, which were used as immunogen to immunize 15 female BALB/c mice.Hybridoma cells were prepared by cell fusion technology, and cell lines stably secreting monoclonal antibodies against Cx43 protein were screened. The antibodies were purified by Protein G affinity chromatography, and its subtype, affinity and specificity were identified. Two high affinity monoclonal antibodies were used as capture antibody and detection antibody respectively(labeled with HRP) to establish a double antibody sandwich ELISA method. The sealing solution(5% milk-PBS, 3%BSA-PBS), washing solution [0. 01 mol/L PBS(pH 7. 2), 0. 01 mol/L PBS-0. 5% Tween 20(pH 7. 2)(i. e., PBST)] and sample diluent [3% BSA-PBS and 0. 01 mol/L PBS(pH 7. 2)]were optimized, and the method was verified for the linear range, sensitivity, specificity, precision and accuracy. The content of Cx43 protein in mouse brain tissue, intestine tissue, femur tissue and MC3T3-E1 mouse embryonic osteoblast lysate was detected by the established method.Results Three hybridoma cell lines secreting monoclonal antibodies against Cx43 were screened and named as 1-1E2, 2-1G11 and 3-2G12 respectively. The antibody subtypes were IgG1, IgG1 and IgG2b respectively, and the antibody sensitivity reached 0. 002 μg/mL, among which 2-1G11 and 3-2G12 had higher affinity. A double antibody sandwich ELISA was developed with 2-1G11 as capture antibody and HRP-labeled 3-2G12 as detection antibody. The optimum sealing solution was 3% BSA-PBS, washing solution was PBST, and sample diluent was 0. 01 mol/L PBS(pH 7. 2). Cx43 protein standard showed a good linear relationship with the mean value of A_(450)in the concentration range of 3. 12-200 ng/mL, R~2> 0. 98. Cx43 protein could be specifically detected by this method, and there was no cross reaction with KLH protein, bone morphogenetic protein-2(BMP-2) and human recombinant TGM2 protein. The CV of precision verification was 7. 72%. The recovery rates of high, medium and low concentrations of test samples were all in the range of 85%-115%. The content of Cx43 protein in MC3T3-E1 mouse embryonic osteoblasts was higher(58. 03 ng/mL), while the content in osteogenic tissue was lower(38. 4 ng/mL).Conclusion Specific monoclonal antibodies against different epitopes of human Cx43 were successfully prepared, and a double antibody sandwich ELISA method with high sensitivity and strong specificity for quantitative detection of Cx43 protein was established, which provided a reliable detection method for the study of structure and function of Cx43 protein.

Laparo-endoscopic single site (LESS) surgery is in line with the development trend of minimally invasive surgery.Resolving the problems caused by the tiny single trocar is very important for the popularizing of LESS surgery.From February to September in 2012,34 patients with cholecystolithiasis or polypoid lesions of the gallbladder received gasless singleincision laparoscopic cholecystectomy at the Xuanwu Hospital.All the operations were successfully done,with no conversion to conventional laparoscopic surgery or open surgery.The mean operation time,intraoperative blood loss and duration of postoperative hospital stay were 70 minutes (range,45-135 minutes),18 ml (range,5-40 ml) and 3.4 days (range,3-4 days).There was no complication within the follow-up of 3 weeks,and all patients recovered well with satisfied cosmetic effect.Combination of gasless single-incision laparoscopic techniques could decrease the difficulty of LESS surgery and help to popularize LESS surgery.
