1.Effect of Oleanic Acid on Key Enzyme Activity in Insulin-Resistant HepG2 Cell Line

Objective To explore the effect of oleanic acid on key enzyme activity in insulin-resistant HepG2 cells. Methods The HepG2 cells were divided into normal control,model control,metformin,and oleanic acid groups.Glycogen content in insulin-resistant HepG2 cell model were detected by hepatic glycogen test kit upon treatment with oleanic acid.Activities of glucokinase ( GK) ,phosphoenolpyruvate carboxylase kinase (PEPCK),and glucose-6-phosphatase (G-6-Pase) were assayed by the glucose 6-phosphate dehydrogenase coupling colorimetric, lactate dehydrogenase coupling colorimetric and ammonium molybdate constant phosphorus methods. Results The oleanic acid enhanced glucose consumption,lowered the activity of G-6-Pase and PEPCK by 54.8% and 18.8%,respectively,and increased the activity of GK and glycogen content in also insulin-resistant HepG2 cells by 100.6% and 98.6%,respectively. Conclusion Aqueous extracts of shirako play a role in lowering PEPCK and G-6-Pase activities and inhibiting glucogenesis, resulting in the reduction of endogenous glucose in the cell. In addition,it can augment the activity of GK,accelerate the process of glucolysis,increase the glycogen content,and alleviate insulin resistance of HepG2.
2.Clinical significance of CRP、neutrophil granulocyte ratio in hospitalized patients with acute exacerbations of chronic obstructive pulmonary disease

Objective To study the clinical significance of C-reactive protein(CRP),neutrophil granulocyte ratio(N% ) in acute exacerbations of hospitalized patients with chronic obstructive pulmonary disease ( COPD ). Methods The serum CRP levels,the gross of N% were detected of the same 71 patients with acute exacerbations of COPD before treatment and after getting better. And the correlations among above-mentioned indexes,the days counter before getting better and the days counter during hospitalization were observed. Results (1) The levels of CRP and N% of patients after getting better were significantly lower than those before treatment(P <0. 01). (2) There was a significant correlation between N% and the days counter before getting better(r =0.240,P <0.05) and also a significant correlation between the days counter before getting better and the days counter during hospitalization (r = 0. 567, P <0.01). Conclusion The serum CRP level and N% were objective markers in estimating pulmonary infection in COPD patients with acute exacerbations. And N% was a reliable indicator of judging the days counter before getting better and better than CRP.
3.Left renal vein entrapment syndrome (report of 10 cases)

Objective To evaluate the diagnosis and treatment of left renal vein entrapment syndrome. Methods The clinical data of 10 cases of left renal vein entrapment syndrome were retrospectively analyzed.Their mean age was 24 years (range,14-37 years).Of them 7 cases presented with non-glomerular origin hematuria;2,with orthostatic proteinuria;1 with hematuria accompanied by proteinuria.Color Doppler sonography showed that the dilated segment of the left vein was three-fold than the strictured segment in diameter,and magnetic resonance angiography (MRA) showed that the left renal vein was compressed in 9 cases. Results Stent placement across the left renal vein was performed in 7 cases.Superior mesenteric arteriectomy and replantation were performed in 3 cases.The operations were all successful.Follow-up of 1-5 years showed that the clinical symptoms were improved and urine tests were normal. Conclusions Non-glomerular hematuria or orthostatic proteinuria is frequently found in left renal vein entrapment syndrome.Left renal vein entrapment syndrome can be diagnosed by Doppler sonography,MRA and cystoscopy in the presence of hematuria or proteinuria.Generally,conservative treatment and careful follow-up are conducted in these patients.Surgical and radiologic interventional procedures are effective alternatives for those with glomerular dysfunction and complications.

Objective To investigate the clinical significance of P27~ KIP1 and S-phase kinase-associated protein 2 (SKP2) expression in human gastric carcinoma. Methods The expression of P27~ KIP1 and SKP2 was determined by SP immunohistochemical method in 69 specimens of gastric carcinoma. Results The positive rates of P27~ KIP1 and SKP2 expression were 46.38% and 33.33%, respectively. The positive rate of P27~ KIP1 expression in gastric carcinoma decreased with the poor differentiation,deep invasion and progression of pathological grade (P0.05). 5.Exposure recurrent laryngeal nerves during the procedure of thyreoidectomy to prevent permanent nerve injuries

Objective To investigate whether routine exposure recurrent laryngeal nerves(RLN) during the procedure of thyreoidectomy would prevent permanent nerves injuries caused by thyroid surgery.Methods 1523 patients with thyroid disease underwent thyreoidectomy from January 1980 to December 2004.Routine exposure of RLN during the procedure of thyreoidectomy were in 274 cases(group 1) and not in 1249 cases(group 2),the incidence rates of temporary RLN injuries and permanent RLN injures were compared in these groups.Results 3 cases(3/274,1.1%) occurred permanent RLN injuries in group 1 and 47 cases(47/1249,3.8%) occurred RLN injuries in group 2.The incidence rate of permanent RLN injuries in group 1 was lower than that in group 2(P0.05) in these groups.Conclusion Exposure recurrent RLN during the procedure of thyreoidectomy can effectively prevent permanent nerves injuries caused by thyroid surgery.
6.The expression and significance of DNMT1 gene in oral squamous cell carcinoma

The expression of DNA methyltranferase 1 (DNMT1 )mRNA and protein in 20 controls of normal oral mucosa tissue and 43 cases of oral squamous cell carcinoma(OSCC)was detected by Real-Time PCR and immunohistochemical staining and Western blot respectively. DNMTl mRNA CT values in OSCC and the controls were 0.958 6 ±0.986 6 and 0.459 5 ±0.525 8 respectively(Z =-2.028,P <0.05), The positive expression of DNMT1 protein in OSCC and the controls was 87% and 25% respectively(P <0.05).DNMT1 may play a role in the development of OSCC.
7.The clinical efficacy of Citric acid tandospirone in treating generalized anxiety neurosis

Objective To investigate clinical efficacy and adverse reactions of Citric acid tandospirone in treating generalized anxiety neurosis.By doing this study,we provide clinical evidence for citric acid tandospirone in treating generalized anxiety neurosis.Methods 92 patients with generalized anxiety were randomly divided into the control group and observation group according to the random number table.There were 46 cases in each group.In the observation group,the patients were given acid citrate edge tandospirone.At the starting the patients were taken 15mg/d. After 3 days the dose increased to 35mg/d by 5mg.The average dose was (31 ±3.4)mg/d.In control group,accord-ing to different patients sleep condition,oxazepam tablets were taken.The daily dosage does not exceed 30mg/d.After two weeks,the dose was gradually reduced to the end.The clinical efficacy,HAMA score and adverse reactions in the two groups were compared.Results Compared to prior treatment,after 2 weeks and 4 weeks the HAMA score in the two groups were significantly different (P<0.05).Compared with the control group,the score in the two group had significantly different after two weeks treatment (t=2.16,P<0.05).The total effective in the observation group was 86.9 %.In the control group the total effective rate was 82.6%.There was no significant difference between the two groups (χ2 =0.33,P>0.05).The rate of adverse reaction in the observation group was 32.6%.The rate of adverse reaction in the control group was 63%.The rate of adverse reaction in the control group and observation group had sig-nificantly different (χ2 =8.58,P<0.05) .Conclusion Acid citrate edge tandospirone on the treatment of general-ized anxiety neurosis showed obviously effect and had fewer adverse reactions.The clinical program is worth of further study and clinical application.
8.Investigation of the Impact Factors that Contribute to Improve the Academic Reputation of Medical Research Institutions in China

Objective To analyze the impact factors and their importance on academic reputation of medical research institutions in China.Methods Seventy medical researchers and 70 medical graduate students were selected by convenience sampling method.Brainstorming and expert consultation were applied to determine the 6 factors affecting the academic reputation of medical research institutions in China.A Face-to-face or e-mail interview was conducted using a designed questionnaire.Six affecting factors were nominated analyzed.Chi-square test was used to cross exam nominated rate and affecting factors;Bonferroni method was also applied to make paired comparisons.Results There is significant difference in nomination rates among 6 impact factors (P < 0.001).Pairwise comparison showed that information obtained by academic exchange (70.49 %) and the scientific understanding generated from research practice (67.21%) are top 2 impact factors to the medical research institutions;followed by all kinds of ranking list (44.26%),and the celebrity effect (42.62%).Conclusions Scientific communication and research practice are two most important factors affecting the academic reputation of medical research institutions in China.
9.The Clinical Effect Observation in Balloon Kyphoplasty for Aged Osteoporotic Thoracolumbar Burst Fractures

Objective To evaluate the possibility and safety of balloon kyphoplasty for the aged osteoporotic thoracolumbar burst fractures. Methods From October 2007 to December 2012, 78 patients with aged osteoporotic thoracolumbar burst fractures were treated by balloon kyphoplasty. The inflatable balloon was inserted through pedicle of vertebal arch to make fracture reduction, then the centrum was stuffed with bone cement. The whole procedure was pinpointed and detected by C-arm x-ray machine. Results All operations were completed successfully. The lumbar and back pain of the patients relieved obviously. The quality of patients' life was significantly improved. Imaging examinations revealed that the vertebrae altitude was recovered and the kyphosis was corrected obviously. Conclusion Balloon kyphoplasty is effective to treat the aged osteoporotic thoracolumbar burst fractures. But the operation had certain risk, we must do a good job in preoperative preparation, strictly handle surgical indication and accurately operate.
10.Vascular endothelial growth factor and nano-hydroxyapatite/collagen composite in the repair of femoral defect in rats

BACKGROUND: Previous studies have confirmed that nano-hydroxyapatite/collagen (nHAC) and mesenchymal stem cells for repair of bone defect have the ability of bone formation in vivo. OBJECTIVE: To observe the effects of vascular endothelial growth factor (VEGF) and bone marrow mesenchymal stem cells (BMSCs), nHAC composite in the repair of femoral defect in rats. METHODS: Sprague-Dawley rat models of middle part of the femur defect were established and randomly assigned to two groups. Control group was implanted with BMSCs/nHAC composite. Experimental group was implanted with VEGF/BMSCs/nHAC composite. At 2, 4 and 8 weeks postoperation, imaging and histology observation of femoral samples were performed. At 8 weeks postoperation, scanning electron microscopy was performed in new bony callus environment. RESULTS AND CONCLUSION: nHAC composite implantation in the rats did not show rejection or inflammatory reaction. Moreover, bone formed rapidly using VEGF and BMSCs, nHAC composite, which exhibited better bone regeneration capacity compared with BMSCs/nHAC composite. The way of ossification mainly was endochondral ossification. It is presumed that VEGF promoted the formation of local microvessels, differentiation and proliferation of osteoblasts, speeded up the speed of endochondral ossification, shortened bone repair time, and elevated the quality and velocity of osteanagenesis.
