1.Advances in clinical diagnosis and treatment of severe acute respiratory syndrome

It has been proved that severe acute respiratory syndrome (SARS) is caused by a novel coronavirus. Detecting disease in its early stage, understanding its pathways of transmission and implementing specific prevention measures for the disease are very important in the course of disease control. China has established the definitions of suspected and confirmed cases of SARS. But the laboratory tests and definitions for the diagnosis are limited.Now the primary measures for SARS include isolation, therapeutics application with ribavirin and corticosteroid, mechanical ventilation, etc. Convalescent plasma is being explored.There still are many problems to be solved in the course of conquering SARS.
2.Role of airway inflammation in the pathogenesis in patient with aspirin-induced asthma

The numbers of eosinophils and mast cells expressing 5-lipoxygenase of airway in aspirin-induced asthma (AIA) increased. LTC 4 synthase, the terminal enzyme for cys-LT production, is overexpressed markedly in bronchial biopsy specimens of most patients with AIA. These characterized the unique airway inflammation of AIA which may illuminat the pathogenesis of AIA to some extent.
3.Effect of early dirigation on the recovery of foot function after the operation of the fracture of calcaneus: the blind evaluation based on 6-month follow-up

BACKGROUND: Most of the displaced fractures of calcaneus reed operation treatment. Because early postoperative dirigation is sometimes ignored,most patients receive only good anatomic reduction but less pleased functional reduction.OBJECTIVE: To compare the effect of different starting time of the postoperative dirigation on the functional recovery of the joint of the foot after the operation of the fracture of calcaneus with joint surface displacement.DESIGN: Non-randomized and concurrently controlled retrospective analysis with blind evaluation.SETTING: Orthopedic Department of Renji Hospital of Shanghai Second Medical University.PARTICIPANTS: Cases with the fracture of the calcaneus with joint surface displacement, in the Orthopedic Department of Renji Hospital of Shanghai Second Medical University were chosen from October 2000 to June 2003. There were totally 64 cases(72 sides), male 40 sides and female 32sides, whose ages varied from 22 to 68 years old. According to the starting time of the postoperative dirigation, the cases were divided into two groups,the 1-week dirigation group with 34 cases(38 sides), male 20 sides and female 18 sides, and the 6-week dirigation group with 30 cases(34 sides), male 20 sides and female 14 sides.METHODS: Totally 64 cases were involved in the result analysis. In the two groups, the same operative therapeutic methods and postoperative dirigation were taken except the starting time of the dirigation. The dirigation was started respectively 1 or 6 weeks after the operation and lasted for 6weeks. Six months later, follow-ups were done according to the Maryland Grade(excellent 90-100, good 75 -89, moderate 50 -74, poor < 50) to evaluate the activity of the talocrural joint and the stability of the anterior half of the foot and the gait as well.MAIN OUTCOME MEASURES: Primary results: the postoperative function of the injured foot of the two groups. Secondary results: the adverse events and side effect.Maryland Grade: In the 34 cases(38 sides) of the 1-week dirigation group,the average mark was 87. 8, and in the 30 cases(34 sides) of the 6-week dirigation group, the average mark was 73.2, and the difference had signifigroup, 3 cases had poor healing of the incision, and achieved the healing after 2-week medication. And 2 cases had injury of the lateral dorsal cutaneous nerve of the foot. In the 6-week dirigation group 2 cases had poor healing of the incision and achieved the healing after 2-week medication, and 1 case had injury of the lateral dorsal cutaneous nerve of the foot. There was no significant difference between the two groups.CONCLUSION: After the operation of the fracture of calcaneus with joint surface displacement, early application of dirigation is beneficial to the functional recovery of the foot and talocrural joint without unsafety.

Objective To investigate the change of vascular endothelial growth factor (VEGF) expression in HepG2 cells under hypoxia. Methods HepG2 cells were cultured under hypoxia(hypoxia group) and normal condition (control group). VEGF expression of HepG2 cells was examined by immunohistochemical staining. The growth of HepG2 cells was examined by MTT colorimetry and cell count. Objective To explore the effect of lysolecithin choline (LPC) on blood-brain barrier(BBB) permeability in rats with acute pancreatitis. Methods Acute pancreatitis rat model was produced and rats were randomly divided into:(1)Test group-AP, rats received LPC by tail vein injection; (2)control group I, AP rats were given normal saline by tail vein injection; (3)control group II, sham operation without AP, but LPC was given by tail vein injection.Horseradish peroxidase(HRP) was used as a tracer to determine BBB permeability 7~10 days later.Results The test group showed local extravascular effusion of HRP, indicating that BBB permeability was markedly increased,while both control groups showed no apparent increase of BBB permeability,which were statistically significant(P
6.Clinical analysis of autoimmune hemolytic anemia in systemic lupus erythematosus

Objective To determine the clinical and serological characteristics of autoimmune hemolytic anemia (AIHA) in systemic lupus erythematosus (SLE) patients.Methods Seventeen patients were studied who had clinical features of AIHA among 222 hospitalized SLE patients in the Department of Rheumatology of Beijing Chaoyang Hospital were studied.Among the SLE patients without AIHA,34 patients were chosen by random sampling as controls.The patients' demographic data,clinical manifestations,laboratory test results and other examinations were analyzed retrospectively.Student's t test was used for the comparison of continuous variables,and chi-square test was used for the comparison of proportions.Results The prevalence of AIHA was 7.6% in cohort of lupus patients.The positive rate of IgG anticardiolipin antibody in the AIHA group was significantly higher than that in the control group (71% vs 15%,x2=15.366,P<0.01 ).The occurrence of antiphospholipid antibody syndrome between the two groups had no statistical significant difference (x2=0.000,P=1.000).The frequencies of arthritis (12%,x2=4.554,P=0.033),malar rash (6%,x2=4.443,P=0.033),leukocytopenia (12%,x2=8.061,P=0.005) and lymphocytopenia (41%,x2=5.075,P=0.024) were lower in the AIHA group than those in the control group.Three patients in the AIHA group had alveolar hemorrhage in the course of SLE,while there was none in the control group (x2=3.586,P=0.058).Conclusion When lupus patients with AIHA are compared with those without AIHA,the positive rate of anticardiolipin antibody is higher,but the occurrence of antiphospholipid antibody syndrome does not increase in SLE patients with AIHA.And the frequencies of arthritis,malar rash,leukocytopenia and lymphocytopenia are lower in those patients with AIHA,however,patients with AIHA are prone to develop alveolar hemorrhage.
7.Construction and screening of cDNA library of alveolar macrophages

Objective To construct cDNA library of alveolar macrophages after lipopolysaccharide (LPS) and dexamethasone (Dex) treatment for exploring the protein molecule interactive with glucocorticoid receptor (GR) in condition of inflammation. Methods After the cultured AMs were treated with LPS (10mg/ml) and Dex (10 -5 mol/L) for 4h, total RNA was extracted from AMs, then the cDNA was synthesized from total RNA of AMs and amplified using primers SMARTⅢ TM and CDSⅢoligo (dT) as the base of recombination. The purified PCR products as well as the linearized plasmid pGADT7-Rec were co-transformed into the competent yeast AH109. They were recombined by yeast homologous recombinase in the yeast cells and became the active cyclic plasmid. The transformed yeasts grew in the SD/-Leu plates. All the growing clones were harvested and then constituted the cDNA library. Furthermore, the bait pGBKT7-rGR transformed the yeast AH109 of library was constructed, and the protein molecule interactive with GR was screened. Results cDNA library of AMs was constructed with high multiplication and good capacity. 1.07?106 recombinants were obtained from the cDNA library. The amplified PCR fragments were between 0.3-1.5kb in size. One true positive clone, obtained by screening the cDNA library, was confirmed to be BAG-1 by sequencing and BLAST. Conclusion The yeast two-hybrid cDNA library of AMs was successfully constructed by Clontech SMART method; GR can interact with BAG-1 in yeast cells.
8.Biomechanical comparison and analysis of three methods of treatment to middle-part radius and ulna fracture

[Objective]To discuss the difference of mechanical stability in using different fixation methods in treating radius and ulna fracture,in order to provide a theoretical basis for clinical applications.[Method]In the experiment,15 pieces of fresh radius and ulna were used to produce middle-part transverse fracture models(A3.2)which were fixed by three different methods(double plate fixation group,double-intramedullary nail fixation group,ulnar intramedullary nailing + radial plate group).Through biomechanical tests,axial stiffness,bending stiffness,torsional stiffness and the equivalent stiffness were compared.[Result]There were no difference between double-plate group and ulnar intramedullary nailing + radial plate group,the stability in these two groups were higher than that in double-intramedullary nail fixation group.[Conclusion]Ulnar intramedullary nailing + radial plate has a good biomechanical property,it can provide firm fixation and with less trauma.So it is a good fixation.
9.Effects of Dexmedetomidine on perioperative hemodynamics and postoperative sedation and analgesia in elderly patients treated with radical gastric cancer surgery

Objective:To investigate the effects of Dexmedetomidine for supplemental anesthesia on perioperative hemodynamics and postoperative sedation and analgesia in elderly patients receiving radical gastric cancer surgery under general anesthesia.Methods:From January 2019 to April 2020, 63 patients admitted to our hospital for radical gastric cancer surgery were included as observation objects.They all underwent general anesthesia and were divided into the Dexmedetomidine group(odd-numbered group)and the normal saline group(even-numbered group)according to the parity of serial numbers the subjects were assigned to.Changes in perioperative hemodynamic index values and the amounts of anesthetic drugs used in the two groups were monitored and recorded, and postoperative sedation and analgesia effects were evaluated by using the Ramsay sedation scale and the visual analog scale(VAS).Results:The mean artery pressure(MAP)and heart rate(HR)were lower in the Dexmedetomidine group than in the normal saline group before anesthesia induction(T1), immediately after intubation(T3)and immediately after extubation(T5)( P<0.05). The Ramsay scores were higher and the VAS scores were lower in the Dexmedetomidine group than in the normal saline group at 1 h and 4 h after surgery( P<0.05). The doses of intraoperative propofol and remifentanil were lower in the Dexmedetomidine group than in the normal saline group[(1121.5±198.5)mg vs.(1395.6±332.7)mg, (3.1±0.9)mg vs.(5.5±1.2)mg, P<0.05]. Conclusions:The use of Dexmedetomidine for supplemental anesthesia can effectively maintain the stability of the patient's intraoperative hemodynamics and enhance the sedative and analgesic effects while reducing the amount of anesthetic drugs in patients receiving radical gastric cancer surgery under general anesthesia.
10.Therapeutic effects of two modes of non-invasive mechanical ventilation on patients with acute cardiogenic pulmonary edema

Objective To compare the therapeutic effects in respect of ventilatory response and the change of hemodynamics of two modes of mechanical ventilation [ proportional assist ventilation (PAV) vs.Bi-level positive airway pressure ventilation (BiPAP) ] on patients with acute cardiogenic pulmonary edema (ACPE).Methods Thirty-two patients diagnosed as ACPE were recruited from May 2008 to April 2009.After conventional therapy ( cardiotonic,diuretic,vasodilators,oxygen) were ineffective for half an hour,32 patients were randomly divided into three groups:control group ( 12 cases kept conventional treatment without mechanical ventilation),BiPAP group ( 12 cases were treated with BiPAP mode of non-invasive mechanical ventilation plus conventional treatment) and PAV group (8 cases were treated with PAV mode of non-invasive mechanical ventilation along with conventional treatment ).Results PaO2,RR and oxygenation index were improved significantly in three groups after 1 hour treatment ( P < 0.05 ).While PaO2 and oxygenation index in noninvasive ventilation groups were higher than those in control group ( P <0.05 ).The time required for amelioration of dyspnea in noninvsaive ventilation groups was shorter than that in control group ( P < 0.05 ).The peak airway pressure and the index of degree of comfort ( VAS score,auxiliary respiratory muscles score) in PAV group were lower than those in BiPAP group (P < 0.05 ).Conclusions Both modes of noninvasive mechanical ventilations could improve the oxygenation and relief of dyspnea in patients with ACPE.PAV and BiPAP had the similar effect in patients with ACPE.The synchronization and comfort in PAV group were better than those in BiPAP group.The PAV mode of noninvasive mechanical ventilation was well accepted by patients with ACPE.
