1.An Experimental Study of Niaoduling Granule for Chronic Renal Failure

[Objective] To observe the effects of Niaoduling Granule (NG) on renal function in rats with chronic renal failure (CRF) and its relationship with renal transforming growth factor (TGF) ?1. [Methods] CRF rat models were established by 5/6 subtotal nephrectomy and then the model rats were randomized into model group (Group B), NG group (Group C) and placebo control group (Group D) and other 8 rats served as mimic operation group (Group A) . Biochemical indexes, metabolic parameters and pathological features in kidney were examined and expression of renal transforming growth factor (TGF) ?1 was determined by immunohistochemical assay. [Results] Renal hypofunction, disorder of electrolyte, decrease of hemoglobin and CRF features were found in Group B. After treatment, renal function was improved and glomerulus-sclerosis index (GSI) and expression of renal TGF ?1 were decreased in Groups C and D (P
2.The key point of cooperation between operation room nurses and anesthesiologists

Objective To probe into the nursing experience of reasonable cooperation between op-eration room nurses and anesthesiologists. Methods Strengthening preoperative propaganda and psy-chological education,paying attention to the temperature and humidity in operation room,assisting the anesthesiologists to arrange patients'body position,giving cooperation of infusion and transfusion,actively involving in rescue work.Results Surgery patients got good care,anesthesia and surgery went along smoothly. Conclusions Smooth coordination between operation room nurses and anesthesiologists has great significance on the smooth going and satisfactory results of operation.
3.The expression profile of Wnt4 in rat kidney during renal development

Objective To explore the expression profile of Wnt4 in rat kidney during renal development and its effect on renal development. Methods Rats with embryonic age of 18 days (E 18 d) , 20 days (E 20 d) as well as postnatal age of 0 day (P 0 d), 1 day (P 1 d), 3 days (P 3 d), 5 days (P 5 d) and 7 days (P 7 d) were selected. Expression levels of Wnt4 in rat kidney during renal development were quantified by immunohistochemistry and Western blot in all time points. Results Immuno?histochemistry analysis showed that during E 18 d to P 7 d, Wnt4 mainly expressed in proximal tubules, ureteric bud, comma shaped bodies and S shaped bodies of nephrogenic zone;the expression in the distal tubule was weak;the expression in renal corpuscle decreased with time;Western blot analysis showed that the expression of Wnt4 in rat kidney began to decrease from E 18 d and reached bottom at P 1 d then rise again until P 7 d when it dropped again. Conclusion During renal development, Wnt4 proteins were involved in the development of the nephrogenic zone through regulating canonical Wnt/β-catenin signaling pathway, and was involved in extension of proximal tubules by inducing the non canonical Wnt/PCP signaling pathway. Expression of Wnt4 protein in rat kidney was closely related to nephron formation and development of proximal tubules.
4.Effects of ropivacaine combined with fentanyl for epidural labor analgesia

Objective To evaluate effects of ropivacaine combined with different concentrations of fentanyl for epidural labor analgesia. Methods In this multicenter double-blinded randomized study 128 parturients at full term and 2-3 cm of cervical dilatation who requested epidural analgesia were randomly allocated to one of 4 groups: group F0 received epidural ropivacaine alone (n = 33); group F1 received epidural ropivacaine with fentanyl 1 ?g?ml-1 (n = 30) ; group F2 epidural ropivacaine + fentanyl 2?g?ml-1(n = 33) and group F3 epidural ropivacaine + fentanyl 3 ?g?ml-1(n = 32). Epidural catheter was placed at L2,3 and advanced 4 cm into the epidural space in cephalad direction. A bolus of 15 ml of ropivacaine alone or with fentanyl was given after correct epidural placement was confirmed. EC50 of epidural ropivacaine was determined by up-and-down sequential experiment. The initial concentration of epidural ropivacaine was 0. 12% . If effective the next parturient received ropivacaine of lower concentration; if ineffective the ropivacaine concentration was increased. Each time the concentration of epidural ropivacaine increased/decreased by 0.01% . The analgesia was assessed using VAS score (0-10 0 = no pain, 10 = worst pain) . If VAS score was less than 3 within 30 min of ropivacaine administration, analgesia was defined as effective. EC50 of ropivacaine was calculated according to Dixon and Massey. Results Four of the 128 parturients enrolled were excluded because of uncertain results of interrupted observation. The EC50 of epidural ropivacaine for labor analgesia and the 95% confidence interval (95% CI) of EC50 were 0.110% (95% CI 0.109 0%-0.111 6%) in group F0; 0.089% (95% CI 0.087 7%-0.091 1%) in group F1; 0.073% (95% CI 0. 071 7%-0.0744%) in group F2 and 0.060% (95% CI 0.056 0%-0.634%) in group F3 respectively. The EC50 was significantly higher in group F0 than in group F1, F2 and F3 (P0.05) . The incidence of side-effect was significantly higher in group F3 than in group F0(P

Objective To further understand the etiology,definition,clinical manifestation,prognosis and treatment of cutaneous leukocytoclastic vasculitis.Method Retrospective study of23patients with leuko-cytoclastic vasculitis followed up at Peking Union Medical College Hospital from1990to2001.Results Twenty patients were classified as hypersensitive vasculitis,one of whom was diagnosed as Wegeners granulo-matosis2.5years later,two were diagnosed as urticarial vasculitis,and one was systemic lupus erythematosus.Conclusion Cutaneous leukocytoclastic vasculitis,which is usually a benign syndrome,may be caused by in-fection or drugs.Its main clinical manifestaions are skin symptoms.

Objective To investigate the Correlation between Plasma Total Homocysteine and Copper in Patients with coronary artery disease complicated with diabetes mellitus. Methods 31 pateints with coronary artery disease complicated with diabetes mellitus were studied. 36 age-matched and gender-matched patients with coronary artery disease not complicated with diabetes mellitus and 30 normal subjects were served as control groups. The concentrations of copper and total homocysteine was determined and compared. Objective To investigate the association of Lipoprotein(a) concentrations and patients with aortic dissection.Methods 27 pateints with aortic dissection were studied. Sixty age-matched and gender-matched patients with acute myocardial infarction and fifty normal subjects were served as control groups. The levels of Serum lipoprotein(a), plasm C-reactive protein, white blood cell count were compared between the three groups. Objective To evaluate the quality of Cortex Cinnamomi from genuine producing areas and GAP planting base,and to compare with those of the commercial medicinal materials.Methods A RP-HPLC method was carried out to determine the content of cinnamyl aldehyde in cortex cinnamomi.Combined with the comparison of volatile oil yield,total ash content and acid-insoluble ash content,the quality of the samples was evaluated with the index of cinnamyl aldehyde content.Results The total ash contents and acid-insoluble ash contents of all samples were in the normal range.Cinnamyl aldehyde contents and volatile oil yields for samples from genuine producing areas and GAP planting base were up to the pharmacopoeia.There was a great fluctuation in cinnamyl aldehyde contents and volatile oil yields in commercial materials.Conclusions The quality is good and stable for the samples from genuine producing areas and GAP planting base,but it is not stable for the commercial materials.

The left ventricular (LV) myocardial remodeling process that occurs in various settings of congestive heart failure (CHF) had historically been attributed to intrinsic changes in the cardiac myocyte. However, it is now recognized that important changes also occur within the extracellular matrix (ECM) of the myocardium, contributing to the remodeling process. Matrix metalloproteinases are a family of proteolytic enzymes responsible for myocardial extracellular protein degradation.Several MMP species identified within the human myocardium may be dysregulated in congestive heart failure (CHF). The purpose of the present review is to present a brief overview of the matrix metalloproteinases (MMPs) within the myocardium that likely contributes to myocardial remodeling,to examine the results from basic studies to explore relationship with respect to MMPs activation and congestive heart failure.-
10.Clinical significance of neutrophil and albumin pretreatments in nasopharyngeal carcinoma prognosis

Objective:To investigate the association of pretherapeutic neutrophil, lymphocyte, monocyte, and serum albumin levels with nasopharyngeal carcinoma (NPC) prognosis in 401 patients. Methods:This study involved 401 NPC patients recently diagnosed and admitted in Sun Yat-sen University Cancer Center, China. Laboratory variables, such as immunology-related parameters and albumin level prior to treatment, were analyzed. The overall survival and disease-free survival rates were determined using the Kaplan-Meier method and were then compared using the log-rank test. Univariate and multivariate analyses were performed using the Cox proportional hazards model to identify prognostic factors. Results:Patients with neutrophil counts>5.20×109/L (n=172) had significantly lower overall survival and disease-free survival rates compared with those with neutrophil counts≤5.20×109/L (n=229) (P=0.014, P=0.009). In the 401 NPC cases, patients with high albumin levels (>43.00 g/L;n=253) had significantly higher overall survival (P=0.000) and disease-free survival (P=0.006) rates compared with those with low albumin levels (≤43.00 g/L;n=148). Multivariate analysis showed that the neutrophil level is an in-dependent risk factor for disease-free survival, whereas the albumin level is a strong independent prognostic factor for overall survival. Conclusion:The pretherapeutic neutrophil and albumin levels are independent prognostic factors for survival in NPC patients.
