1.Progress in study of matrix metalloproteinases and tumour invasion and metastasis

Invasion and metastasis are the distinct biological characterstics of cancer. They are the key factors influencing tumour patients' theraputic effect and prognosis. Recent study shows that metalloproteinases can facilitate tumour cell invasion and metastasis. In this review we evaluate the novel aspects of the function of matrix metalloproteinases in tumor invasion and metastasis. The regulation of the expression and activity of MMPs are summarized. The potential clinical application of metalloproteinase and their tissue inhibitors were also presented.
2.Association of plasma cystatin C and intima-media thickness of carotid artery in patients with early type 2 diabetes mellitus

Ninty-eight patients with newly diagnosed type 2 diabetes mellitus were divided into atherosclerosis(AS) group and non-AS group.Fasting plasma cystatin C level( CysC ) was determined.The results showed that there existed a significant correlation between CysC level and the number of carotid arteries plaque ( r =0.432,P<0.01 ).CysC was an independent risk factor( OR =2.21,95% CI 1.88-3.02 ) of carotid artery intimamedia thickening in patients with type 2 diabetes.
3.The potential legal issues about mandatory nursing intervention and the strategies of nursing management in psychiatric wards

Objective To explore the potential legal issues about mandatory nursing intervention in psychiatric wards,and put forward the strategies of nursing management.Methods The causes and the potential legal issues of the action of psychiatric nurses to carry out mandatory nursing intervention were analyzed.Results There were legal issues on the right to life and health,the right of informed consent,the right of personal freedom and the right of personality respected about mandatory nursing intervention in psychiatric wards and there might be negligence,misconduct and responsibility of violating the laws and regulations.Conclusions While carrying out mandatory nursing intervention in psychiatric wards,nurses need to respect the patients,make related informed well,protect various related rights of patients and families,implement seriously the relevant system so as to make mandatory nursing interventions more humanized,scientific,standardized,programming,and avoid the occurrence of medical disputes.
4.Developing of internal mandible distracter and the animal experimental research

ObjectiveTo introduce the progressing history of DO technique,and report the characteristics of the home-made type MS-1 internal mandibular distracter, and the effects of application of it in animal experiment. Methods15 Mongrel dogs were subjected to be lengthened of the unilateral or bilateral mandibles using the home-made type MS-1 mandibular distracter, and the regular X-ray ex aminations were made. ResultsAbout (20±5)mm in length of the andible had been achieved by the DO (distraction of osteogenesis)technique using the device. The accuracy of the distracter on lengthening would be attained to 98 % of the expectancy. The mineralization of the new bone fulfilled the gap zone about 5 weeks after the distraction had been finished during the consolidation period. ConclusionThe MS-1 type internal mandibular lengthening device has its special advantages on ap plication of cranio-maxillofacial surgery with a great expectancy.
5.Small incision total hip arthroplasty

Objective To investigate the feasibility of total hip arthroplasty through a small anterolateral incision. Methods Total hip arthroplasty through a small anterolateral incision was performed in 53 cases (58 hip joints) from February 2003 to December 2003, including 5 cases of simultaneous replacement of bilateral joints, 6 cases of total hip revision, and 4 cases of hip joint rigidity. The surgical modification features were as follows: the patient was placed in recumbent position, with the involved buttock region slightly hanging over the side of the surgical bed and normally drooping; the involved leg was placed in 10? of adduction, which was helpful to the exposure of the greater trochanter; after the exposure of the neck of the femur and the greater trochanter, osteotomy was performed firstly, then the femoral head was dislocated and removed. Results The length of incision was 7~12 cm (mean, 8.5 cm). The operation time was 70~140 min (mean, 88 min). The blood loss was 250~660 ml (mean, 470 ml). No severe complications occurred. Postoperatively, weight-bearing and mobilization exercise were begun in 6~14 days. All the cases were followed for 4~40 weeks (mean, 28 weeks). The Harris hip scores showed excellent outcomes in 37 cases, good in 12, fair in 4, with a rate of excellent or good results of 92.5% (49/53). Conclusions Total hip arthroplasty through a small anterolateral incision is a feasible procedure that is characterized with minimal invasion, less blood loss and quick recovery. It can be applied in total hip replacement for artificial hip joint revision, hip joint rigidity, or congenital dislocation of hip joint.
6.Preliminary study on biological behavior and staging of transplanted hepatoma in rats

OBJECTIVE: To establish a staging standard for a transplanted hepatoma model in rats, which may be equivalent to the human primary liver cancer. METHODS: A transplanted hepatoma model was made in SD rats by implantation of histologically intact Walker-256 tumor fragment into the left lateral lobe of the liver. The body weight, tumor volume and serum albumin level of the rats were measured and assayed in different periods. Pathological sections were also fabricated to detect the invasion and metastasis of the neighboring tissues. RESULTS: The body weight loss and ascites occurred at the 12th and 18th day separately. The serum albumin level was (35.32+/-2.45) g/L at the 11th day, which showed no statistical difference as compared with the healthy rats (P>0.05), and the albumin level at the 18th day was significantly lower than that at the 11th day (P<0.05). At the 23rd day, 56.25% of the rats developed bloody ascites. CONCLUSION: The period between 5-11 days in the model rats may be correspondent to the early stage of human primary liver cancer, and the biological behavior of the model rats at the18th day may be correspondent to the late stage of human primary liver cancer. This staging standard might be the guideline for model selection in experimental study of primary liver cancer.
7.Clinical outcome efficacy analysis of three sequential incision and selective lymphadenectomy in treatment of 1162 cases with esophageal cancer

Objective To investigate the best models of esophageal cancer surgical approaches,lymphadenectomy ranges and postoperative comprehensive treatment.Methods From January 1998 to December 2007 in 10 years,1 162 cases of different parts with esophageal cancer were analysized which were treated with surgically adopting sequential abdomen,chest,neck three-incision approaches.Taking three-step screening way of preoperative imaging,preoperative ultrasound localigation fineneedle aspiration biopsy cytology and intraoperative rapid fieezing pathological examination were applied to make the selectivity of the first-times cervico-thoraco-abdominal three-field lymph node dissection (3FLND),first-times standard thoracoabdominal two-field lymph node dissection (2FLND) and second-times third-field cervical lymphadenectomy.Meanwhile,we implemented actively postoperative comprehensive treatment and observed the overall clinical efficacy and long-term survival.Results In the group of 1 162 cases with esophageal cancer surgery.The operating resection rate was 100 %,Radical resection of 97.6 % (1 134/1 162).Perioperative overall complication rate was 16.4 % (191/1 162).Anastigmatic fistula was 0.6 % (7/1 162).Five cases dead in 30 days after surgery.The whole group of lymph node metastasis rate was 52.6 % (611/1 162),lymph node metastasis degree in 12.1% (3 092/25 564).Full group of the first-times 3FLND was 348 cases.The positive rate was 94.8 % (330/348),Implementing the first-times 2FLND was in 814 cases and the second-times thirdfield cervical lymph node dissection was performed in 89 cases.The postoperative complications of the 3FLND was significantly higher than that of 2FLND (23.6 % vs 13.4 %).The difference was statistically significant (x2 =18.37,P < 0.001).However,The difference was not statistically significant between 3FLND and 2FLND (P > 0.05).There is no significant difference between the first-times 3FLND with the second-times 3FLND accumulating survival (P > 0.05).But different degree of lymph node metastasis determined prognosis.The difference was significant (x2 =35.57,P < 0.001).In our group the postoperative comprehensive treatment application rate was 87.2 %.Surgery combined with comprehensive treatment overall 1,3,5 years survival rates were 92.1%,69.2 %,49.6 % respectively.Conclusions The sequential three-incision approaches and selectivity of lymph node dissection have the radical nature thoroughly,fewer complications,high quality of life and the long-term survival advantages.It should be a widely used method in the clinical surgery of the esophageal cancer.Implementing the three-step screening way to selecting the first-times 3FLND,the firsttimes 2FLND and the second-times third-field cervical lymphadenectomy that have targeted strongly and high accuracy,a definite object in view and avoiding patient excessive trauma.Applying the comprehensive treatment after surgery and the re-treated of patients with recurrence that can further more improve the longterm survival of patients.The comprehensive treatment should be the target direction of the future development of esophageal cancer.
8.Progress in study of matrix metalloproteinases and congestive heart failure

The left ventricular (LV) myocardial remodeling process that occurs in various settings of congestive heart failure (CHF) had historically been attributed to intrinsic changes in the cardiac myocyte. However, it is now recognized that important changes also occur within the extracellular matrix (ECM) of the myocardium, contributing to the remodeling process. Matrix metalloproteinases are a family of proteolytic enzymes responsible for myocardial extracellular protein degradation.Several MMP species identified within the human myocardium may be dysregulated in congestive heart failure (CHF). The purpose of the present review is to present a brief overview of the matrix metalloproteinases (MMPs) within the myocardium that likely contributes to myocardial remodeling,to examine the results from basic studies to explore relationship with respect to MMPs activation and congestive heart failure.-
9.Experimental study of femoral head reshaping surgery for treatment of collapse stage of femoral head necrosis

[Objective]To assess the effect of femoral head reshaped surgery for treatment of collapse stage of femoral head necrosis in dogs.[Method]Experimental femoral head necrosis models were made in unilateral femoral head of 10 adult dogs.Under anaesthesia,the necrotic mass in the collapse area of femoral head necrosis was fully curreted through a passage from femoral neck to callapse head,a self-designed ball-tube apparatus enveloped with a cannula was put into under the curreted area,normal saline injected to fill the ball in the collapse cavity till the collapse area forced to reshape a spherical femoral head;After withdraw of normal saline,bone cement of calcium phosphate was injected into the reshaped cavity to maintain the reshaped femoral head.Finally the ball-tube apparatus was taken out.Preoperative and postoperative X-ray film of femoral head were taken to evaluate the reshaping.Femoral heads of experimental side and normal femoral head of contralateral side as control were take out after the dogs sacrifice for biomechanical examination of maximum compression strength.[Result]In experimental group,as compared to preoperative radiograms,the postoperative radiograms of 10 dogs showed the curreted necrotic cavity was fully filled with bone cement and reshape was maintained.The maximum comopression strength of femoral heads of control group was 49 Mpa to 60 Mpa,and that of experimental group was 49~54 Mpa with no statistical significant difference.[Conclusion]Femoral head reshaping surgery may be a method of choice for treatment of collapse stage of femoral head necrosis in experimental animal,but long-term effect should be evaluated thereafter.
10.In vitro study of Ganli Injection on reversing acquired multidrug resistance of human hepatocellular carcinoma

Objective To study the reversal effect on acquired multidrug resistance (MDR) of human hepatocellular caricinoma (HCC) by Ganli Injection. Methods Proliferative inhibition rate of HCC BEL-7402/5-Fu cell line and reversal index of 5-Fu acquired MDR were measured by MTT assay. Cell morphological changes were observed under inverted microscope and Wright-Giemsa stain method. Fluorescence intensity of Adriamycin (ADM) was detected by Flow cytometry. Results HCC BEL-7402/5-Fu cell line exhibited cross-resistance to general chemotherapeutics in clinic. Proliferative inhibition rates of HCC BEL-7402/5-Fu cell line treated for 72 h by Ganli Injection at the (matrine) concentration of 0.230, 0.115, 0.058 mg/mL were (9.25?2.38)%, (8.46?1.90)%, and (4.23?2.05)%, respectively. The corresponding reversal indexes to 5-Fu were 5.50, 3.83, and 2.25. Pre-treatment of Galli Injection at the concentration of 0.230 mg/mL for 2 h could increase the accumulation of ADM in cells to 38.61%.Conclusion Ganli Injection has the effect on reversing 5-Fu acquired MDR of human hepatocellular carcinoma in vitro.
