1.Clinical application value of endometrial collector combined with liquid based cytology and histological examination for screening of endometrial cancer

Objective To analyze the clinical application value of endometrial collector combined with liquid based cytology and histological examination for screening of endometrial cancer.Methods One hundred and eighty-two cases high-risk people with endometrial cancer in Songgang People's Hospital of Bao'an district of Shenzhen from June 2014 to December 2015 were collected,among them 71 patients with endometrial cancer were found in the patients who were diagnosed by pathological diagnosis,and the remaining 111 were not diagnosed with endometrial cancer.The material satisfaction,sample collection time of sample collection and endometrial curettage device were comparatively analyzed and also the cervical dilatation production rate,the amount of bleeding,pain score(VAS) were analyzed.The application value of endometrial collector combined with liquid based cytology and histological examination for screening of endometrial cancer was judged according to the results obtained.Results There was no significant difference between the two methods based on satisfaction(χ2=1.953,P=0.089).A methods of cervical endometrial expansion rate(3.85% vs.93.96%) and acquisition time((16.35±2.19) min vs.(11.23±4.26) min),bleeding volume((1.64±0.63) ml vs.(15.73±5.41) ml),VAS score((0.82±0.24) points vs.(4.34±2.43) points) were significantly lower than that of curettage sample collection method(χ2=16.473,P=0.000;t=5.733,P=0.034;t=11.183,P=0.000;t=3.618,P=0.043).With curettage pathological diagnosis as the gold standard,the specificity,sensitivity and diagnostic coincidence rate of endometrial biopsy combined with liquid based cytology and histological examination for screening endometrial carcinoma were respectively 94.51%,95.05% and 94.96%.Conclusion Compared with curettage sampling method,endometrial curettage collector has features of high success rate,short time,low pain and high security and the cervical dilation rate is low.Endometrial curettage collector can be combined with liquid based cytology and histology to examine and screen endometrial cancer.This methods is worth promoting.

John Cunningham virus(JCV)is a type of human polyomavirus. It was first isolated from the brain of a patient with progressive multifocal leukoencephalopathy(PML)in 1971 and named after that patient. The seroprevalence of JCV in the general population is 40% to 60% . The mortality rate among patients with AIDS complicated by PML was shown to be 50% . For immunocompromised patients and pa-tients with long-term use of immunosuppressive drugs,JCV would cause fatal polyomavirus associated ne-phropathy(PVAN),viremia and some other related diseases. While the pathogenesis of JCV has well stud-ied,there are no specific prevention and treatment measures for infected individuals. Therefore,reliable, specific and sensitive JCV detection methods in clinical settings are needed. This review describes the pros and cons of different methods for JCV detection with potentials for clinical applications.
4.TP53 polymorphisms are involved in inverse colorectal cancer comorbidity in Chinese schizophrenia patients

The inverse cancer comorbidity in schizophrenia patients may be related to the genetic factors ,involving the regu‐lation of apoptosis .The tumour suppressor gene TP53 ,involved in neural apoptosis ,is one of the potential candidate genes associat‐ed with schizophrenia which might reduce colorectal cancer risk .We recruited 270 schizophrenia patients and 312 colorectal cancer patients without schizophrenia .To examine the genetic association between schizophrenia and colorectal cancer ,we analysed eight SNPs (rs12951053 ,rs1625895 ,rs2909430 ,rs9895829 ,rs1042522 ,rs8079544 ,rs8064946 ,rs17806770) covering 14 .35 kb in the re‐gion of TP53 .We observed that one of the eight genetic polymorphisms showed statistically significant differences between the colo‐rectal cancer subjects and the schizophrenia subjects (rs12951053 ,P=0 .000 1 ,OR 1 .70 ,95% CI 1 .30-2 .23) .In addition ,the hap‐lotype of A‐G (rs12951053‐rs8064946) ,giving a global P=0 .001 8 ,was the most significant .Our data indicate that the polymor‐phisms of rs12951053 in TP53 confer reduced susceptibility to colorectal cancer and suggest a potential protective mechanism a‐gainst colorectal cancer in the schizophrenia patients of Han Chinese origin .
5.Effect of sodium potassium magnesium calcium and glucose injection on internal environment and metabolism of glucose, fat and protein in patients undergoing craniotomy

Objective To evaluate the effect of sodium potassium magnesium calcium and glucose (SPMCG) injection on the internal environment and metabolism of glucose,fat and protein in the patients undergoing craniotomy.Methods Forty-four patients of both sexes,aged 18-64 yr,of American Society of Anesthesiology Anesthesiologists physical status Ⅰ-Ⅲ,with body mass index of 18-30 kg/m2,were divided into either lactated Ringer's solution group (group R) or SPMCG injection group (group S) using a random number table.Before anesthesia induction,6% hydroxyethyl starch 130/0.4 was infused intravenously at 15 ml · kg-1 · h-1 for 30 min,followed by iv infusion of 6% hydroxyethyl starch 130/0.4 and corresponding volume of crystalloid solution in a ratio of 1 ∶ 2 at a rate of 5-10 ml · kg-1 · h-1 until the end of surgery.Before fluid infusion (baseline),immediately after the beginning of surgery,at 2 and 4 h after the beginning of surgery,at the end of surgery and at 1 h after surgery,arterial blood samples were collected for blood gas analysis,the pH value and concentrations of base excess,Na +,K+,Ca2+,Mg 2+and lactic acid were recorded,and the blood glucose concentration was determined.Before fluid infusion (baseline) and at the end of surgery,venous blood samples were taken for determination of the concentrations of plasma insulin,β-hydroxybutyric acid and 3-methylhistidine (3-MH).Results No hyperglycemia was found in the two groups.Compared with group R,the Mg2+ concentration was significantly increased,the concentrations of lactic acid and plasma β-hydroxybutyric acid and 3-MH were decreased (P<0.05),and no significant change was found in the other parameters in group S (P>0.05).Compared with the baseline,the blood glucose concentration was significantly increased after fluid infusion in the two groups,the Mg2+concentration was significantly decreased,and the concentrations of lactic acid and plasma insulin,β-hydroxybutyric acid and 3-MH were increased after fluid infusion in group R (P<0.05),and no significant change was found in the parameters mentioned above in group S (P>0.05).Conclusion For the patients undergoing craniotomy,SPMCG injection can maintain homeostasis,and the component 1% glucose can inhibit the decomposition of fat and protein without inducing hyperglycemia and glucose metabolic disorders.
6.Evaluation of diffusion weighted imaging after contrast in characterizing breast tumors

Objective To evaluate the effect of intravenous administration of gadolinium-DTPA on diffusion weighted imaging (DWI)of breast lesions and apparent diffusion coefficient(ADC)values,and to evaluate the diagnostic performance of post-contrast ADC for differentiating benign from malignant lesions in breast.Methods Thirty-eight histopathologically-confirmed breast lesions were analyzed.We examined the signal to noise ratio(SNR)of normal breast tissue and lesions and evaluated the contrast to noise ratio(CNR)of each lesion.We also assessed the relation between pre-and post-contrast ADC values.Results SNRs and CNRs of DWI before and after contrast were not significantly different.The mean post-contrast ADC values for malignant lesions significantly decreased after contrast injected (mean of -1 1.6%),while ADC values in the benign lesions were not significantly different before and after contrast.Conclusion Post-contrast ADC values may be a better indicator of potential breast tumors than pre-contrast ADC values.
7.Determination of indicators weight of qualification and evaluation system for clinical teachers of master of nursing specialist postgraduates

Objective To set up weight coefficient of qualification and evaluation system so as to implement a normative and standardized system for assessment of clinical teachers of master of nursing specialist (MNS) postgraduates. Methods The indicator system was established by Delphi method. The indicators weight was obtained through analytic hierarchy process, and the logic consistency test was calculated. Results The first five weight of qualification indicators were as follows: registered nurse (0.1686), clinical working experiences (0.2352), clinical teaching experiences (0.2016), education background and professional title (0.1410), research subjects they preside over or participate in (0.0817). The results of indicators order in single level and whole system had a satisfied consistency (CR<0.10). Conclusions The study on weighting values of the system could help it more scientific,reliable and practical.
8.Observation on the effect of serf-care education and nursing package usage on non-throat patients

Objective To discuss the effect of self- care education at the early time of postoperation and the use of nursing packages during the later period on non- throat patients. Methods 40 patients having un-dergone total laryngectomy were divided into the control group and the experimental group according to the surgery date with 20 cases in each group. The control group received conventional nursing, while the experi-mental group was given the system intervention by specialized nurses. The intervention content included self-care education at the early time of postoperation and the use of nursing packages during the later period. The questionnaire survey was carried out 15 days and 3 months after operation.The coping style,self-care ability and survival quality were compared between the two groups. Results The comparison of coping style showed re-markable difference in the two groups. The comparison of self- care ability and level of health knowledge showed significant difference 15 days after operation except two other items,but the four items demonstrated significant difference 3 months after operation. Difference also existed in the total scorn of survival quality. Conclusions Early system serf- care education after operation and the use of nursing package during the later period can impel the patients to cope positively, enhance self-care ability and survival quality.
9.Effect of butylphthalide injection on serum neuron specific enolase, C-reactive protein and fatty acid binding protein levels in patients with cerebral vasospasm following aneurysmal subarachnoid hemorrhage

To investigate influence of butylphthalide injection on serum neuron specific enolase, C-reactive protein and fatty acid binding protein levels in patients with cerebral vasospasm following aneurysmal subarachnoid hemorrhage.Methods Ninety patients with cerebral vasospasm were admitted to The First Affiliated Hospital of Fujian Medical University, then the patients were divided into two groups: The control group (45 patients) was treated with nimodipine and triple-H therapy after surgery;in addition to nimodipine and triple-H therapy, butylphthalide injection was administered to the experimental group(45 patients).Transcranial doppler(TCD)was used for the evaluating cerebral artery blood flow velocity, and the serum neuron specific enolase(NSE), C-reactive protein(CRP) and fatty acid binding protein(FABP) levels in patients with cerebral vasospasm were measured. Results The experimental group improved significantly more than the control group, a significant decrease in cerebral blood flow velocity of the middle cerebral artery in the experimental group as measured by TCD (P<0.05).The serum levels of NSE, CRP and FABP in the patients in the experimental group decreased more significantly (P<0.05).And the incidence of cerebral infarction in experimental group was lower than that in control group (P<0.05).Conclusion The serum levels of NSE, CRP and FABP in the patients with cerebral vasospasm following aneurysmal subarachnoid hemorrhage could be significantly reduced by administration of butylphthalide injection, which also could improve cerebral blood supply.Therefore, administration of butylphthalide injection is an effective treatment for cerebral vasospasm.
10.Construction of emergency nursing rescue process in nuclear accident

Objective To explore the nuclear emergency nursing rescue process, and ensure safety and effectiveness of implementation of nuclear emergency nursing rescue. Methods Based on the analysis of the existing national standards, and on the basis of literature review, the method of Delphi was used to collect 23 experts′suggestions, and to make a preliminary draft nuclear emergency nursing rescue process on site. Results The experts' enthusiasm was very high, and the degree of authority (Cr) coefficient was 0.91. A on-site rescue unit partition graph, and two rescue unit process that were preliminary sorting and decontamination, and an on-site rescue process had been drew up. Conclusions After two rounds of expert enquiry, the emergency nursing rescue process is reasonable. And the study could provide objective basis for the implementation of the nuclear emergency nursing rescue.
