OBJECTIVE:To observe the effecacy and safety of using cefotaxime using different doses in perioprative period on the prevention of postoperative infection of lung cancer resection. METHODS:61 patients with non-small cell lung cancer who re-ceived lung cancer resection were retrospectively analyzed divided into cefotaxime 2 g group(26 cases) and cefotaxime 4 g group (35 cases)according to dosage. Cefotaxime 2 g group was treated with Cefotaxime for injetion 2 g 30 min before operation,add-ing into 0.9% sodium chloride injection 100 ml,by intravenous infusion,if the operation time was more than 3 h,cefotaxime 2 g was intravenously infused during operation and cefotaxime 2 g was intravenously infused after operation, interval of 12 h was re-quired for preoperative and postoperative medication time and the total medication time was no more than 48 h. Cefotaxime 4 g group was treated with Cefotaxime for injection 4 g(the usage was the same as cefotaxime 2 g group). Leukocyte,hemoglobin,al-bumin,infection rate and incidence of adverse reactions in 2 groups before and after operation were observed. RESULTS:There was no significant difference in the infection rates between 2 groups(P>0.05). Leukocyte, hemoglobin and albumin in 2 groups were significantly lower than before, the differences were statistically significant(P<0.05),however,there was no significant dif-ference between 2 groups(P>0.05). There were no obvious adverse reactions during treatment. CONCLUSIONS:Small dose of ce-fotaxime has good prevention effect on the postoperative infection of non-small cell lung cancer, with good safety, which meets the principles of rational use of antibiotics.

Objective To summarize the main nursing points of toxic epidermal necrolysis. Methods On the foundation of conventional therapy, an overall assessment was carried out among 10 patients with toxic epidermal necrolysis. On loose skin with erythema, a combination of zinc oxide and talcum powder was externally applied to skin lesions where blisters were not broken in order to promote dry-style exfoliation of the skin lesion. After infrared irradiation, gauze containing MEBO was applied externally to skin lesions with eroded secretions to moisturize them, thus facilitating healing of the skin lesion. Meanwhile, mucosa of special part of patient's body was well nursed. Protective isolation was enhanced in order to reduce secondary infection. The patient's conditions were observed closely. Diet guidance was also done. Results All the patients were dry-style exfoliated with treatment ranges reaching up to 30%to 60%of the affected area. Dry-style exfoliation time was between 5 to 10 days, with an average of 7.20 ±1.69 days. The area of skin lesion erosion ranged from 10% to 60%. Following the external application of MEBO gauze to moisturize and heal, skin lesion healing time ranged from 7 to 18 days with an average of 13.70 ±3.40 days. Conclusion According to the specific situation of toxic epidermal necrolysis, targeted nursing and treatment can promote the dry-style exfoliation of skin lesions, reduce the area of skin erosions, alleviate the suffering of patients and promote healing of the skin lesion.

A 16-year-old woman presented plaques on the left auricle and face over a period of 3 years. Fungal culture grew black-grey or dust velvety colony on Sabouraud's dextrose agar plate. A slide culture on potato dextrose agar plate showed conidiophores which were unbranched or occasionally loosely branched. The conidia were sympodial, zero- to two- septate, with rounded apices and truncated bases. The optimum growth temperature was 26℃ - 30℃. The fungus had the ability to liquefy glutin and hydrolyze starch. Anti-fungal susceptibility test showed the fungus was susceptible to itraconazole, terbinafine and amphoterecin B, but resistant to fluconazole. Cutaneous biopsy specimens revealed brown hyphae and budding yeast cells. The sequence of internal transcribed spacer (ITS) 1-ITS4 region of the isolate rDNA was assessed and compared against the Genebank databases. A 99% consistence was observed in the ITS sequence between clinical isolate and reference strain of Veronaea botryose Ciferri et Momtemartini. Based on the above findings, the mold was identified as Veronaea botryose Ciferri et Momtemartini. The lesions gradually subsided after 8-month treatment with oral itraconazole of 100 mg twice daily.
