1.Clinical Experience in Treating Pregnancy Uterine Cavity Effusion by Tonifying Kidney to Remove Blood Stasis

Objective]To explore clinical experience in treating pregnancy uterine cavity effusion by tonifying kidney to remove blood stasis. [Method] With the study of TCM literature on pregnancy uterine cavity effusion, by combining with clinical practice, it expounds pathogenesis and treatment of syndrome differentiation of pregnancy uterine cavity effusion. [Result]The main pathogenesis of this disease focuses on kidney deficiency and blood stasis which can impair Chong and Ren channels,inducing insecurity of the fetal origin,so the clinical treatment principle centers on tonifying kidney to remove blood stasis,and the self-decoction that is the basic recipe may change with symptom, making the fetal origin secure. [Conclusion] Method of tonifying kidney to remove blood stasis is quite effective for pregnancy uterine cavity effusion in clinic,and it is worth further study.
2.Application of the severity score of hand foot and mouth disease in children

Objective To develop the severity score of pediatric hand foot and mouth disease (HFMD),guide health care providers in the early detection and intervention of critical ill cases,in order to improve the survival rate and reduce the incidence of complications and mortality.Methods Based on PCIS,the severity score of HFMD in children was designed by adding fever and nervous system damage symptoms,which was the common symptoms of severe cases.The data of the cases who admitted in April 2008 to November 2010 (group A,88 cases) was retrospectively analyzed to evaluate the validity and credibility of the score.The score were prospectively applied to all of the HFMD cases who admitted from May 2011 to October 2013(group B,118 cases),delivering corresponding treatment and care measures based on scoring result as well.At last the outcome of the two groups was compared.Results It exhibited quite good consistency between the scores and the clinical condition of patients with HFMD.The general information of two groups was not significantly different,but the survival rate,the incidence of neurological sequel and the mortality between two groups were significantly different by x2 test.Conclusions The score can objectively and accurately assess the severity of HFMD in children.Clinical application of this scoring method to assess the children with HFMD can identify critical ill cases as early as possible.Corresponding intervention delivering in time may improve the outcome.It is proved to have positive clinical value to increase the survival rate,decrease the incidence of neurological sequel and the mortality of critical ill HFMD cases,and is worthy of clinical application in pediatric department.
3.Intervention and prognosis of central venous catheter related thrombosis in children

Objective To explore how the different intervention measures affect the outcome of central venous catheter-related thrombosis (CRT) in children. Methods A retrospective analysis was carried out to collect the clinical data of patients with CRT from the nursing management system of the Children′s Hospital Affiliated to Zhejiang University School of Medicine which reported by each nursing unit from January 1,2015 to December 31,2015. Results Totally 108 cases were included (72 boys and 36 girls), median age of 24.5 months (ranged from 1 month to 14 years old). Nearly 42.59% (46/108) patients suffered from neurological diseases. Nearly 55.56%(60/108) CRT was detected in the first week after catheterization. Once CRT conformed, there were four kinds clinical intervention options applied. Intervention 1: thrombolytic therapy with urokinase combined anticoagulation with nadroparin calcium. Intervention 2: anticoagulant therapy only. Intervention 3: thrombolytic therapy alone. Intervention 4: no medications. The differences of effective between the four kinds of intervention were statistically significant (χ2=13.380, P=0.004). The single-factor regression analysis was done to each relevant factor. Finally the multivariate regression analysis showed four factors had impacts upon the results. The factors were as follows:gender (OR=10.400, 95%CI 1.879-57.563, P=0.007);interval (OR=1.107, 95%CI 1.035-1.184, P = 0.003), size of thrombus(OR = 1.562,95%CI 1.033-2.362,P=0.035; Intervention 2 (OR =11.757, 95% CI 2.254- 61.327, P = 0.003), intervention 4 (OR = 35.397, 95% CI 3.493-358.760, P =0.003). Conclusions The earlier and small size thrombus is more soluble. Thrombolytic therapy or combined anticoagulation is more effective. It is recommended that if no contraindications presents, thrombolytic combined with anticoagulant therapy should start early standardized treatment.
4.Study of change of Th17 cell caused by bacteria biofilm after infection

Objective:To study of change of Th17 cell and IL-17 caused by bacteria biofilm after infection.Methods: To detect biofilm formation ability of bacteria isolated from bronchus alveolus washing-water of patients;to detect expressing ratios of Th17 cells and IL-17 level in peripheral blood of patients and healthy person;to detect expressing ratios of Th17 cells and IL-17 level in bronchus alveolus washing-water of patients;to analyze the above data using SPSS17.0 software.Results: Expressing ratios of Th17 cells and IL-17 level in peripheral blood of patients infected by producing biofilm and non-producing biofilm bacteria and healthy person are respectively(0.59±0.18)%and(108.8±20.5)pg/ml,(0.58±0.18)%and(100.1±20.7)pg/ml,(0.55±0.17)% and(100.0± 21.4)pg/ml,they were not different among data from patients and healthy person,P>0.05,expressing ratios of Th17 cells and IL-17 level in bronchus alveolus washing-water of patients infected by producing biofilm and non-producing biofilm bacteria were respective (1.37±0.34)%,(157.4±30.8)pg/ml and(1.11±0.21)%,(136.2±24.3)mg/ml,the data between patients infected by producing biofilm and non-producing biofilm bacteria were distinctly different, P<0.05.Conclusion: Bacteria biofilm can cause Th17 cell expressing ratio and IL-17 level to become high in infected part,this may be mechanism that infection become chronic.
5.Effectiveness and safety analysis of the combination in treatment of pedi-atric digestive benign diarrhea

Objective To investigate the clinical effect and safety of Smecta, Clostridium butyricum powder particles combined with Xingpiyang'er particles on pediatric digestive benign diarrhea. Methods All 112 cases of children with diarrhea indigestion as research subjects were randomly divided into study group and the control group, 56 cases in the control group were given smecta treatment, research group were given on the basis of the control group Clostridium and Xingpiyang’er particles. Results The study group stool frequency recovery time, body temperature returned to normal, stool recovery time was significantly better than the control group, the differences were statistically significant (P<0.05);the total effective rate of research group was 96.43%,significantly better than 73.21% in the control group, the differ-ences were statistically significant (P<0.05);after treatment, the HAMA score of research group was significantly lower than the control group, with statistical significance (P<0.05);adverse reactions rate of research group was 5.36%, with no obvious difference compared to the control group (P>0.05). Conclusion Children with indigestion diarrhea in chil-dren using smecta, Clostridium butyricum powder particle therapy combined Xingpiyang'er particles particks, can ef-fectively and quickly improve the symptoms of children with diarrhea, the effect is significant, it is worth promoting.
6.Eukaryotic expression of P-selectin functional segment on the membrane of CHO.

Cell adhesive molecular P-selectin was cloned, expressed and anchored on CHO cell membrane through GPI for selection specific antibodies. Total human platelet RNA was extracted and the functional segment of P-selectin gene was cloned by RT-PCR. The P-selectin functional segment gene was cloned into a eukaryotic expression vector pMCEw2-GPI containing an attenuated neo gene together with a downstream GPI, which was synthesized by overlapping PCR. The recombinant plasmid pMCEw2-GPI-P-selectin was then transfected to CHO(dhfr-) cells and screened with G418. ELISA, western-blot and immunofluorescence were carried out to detect the stability of P-selection expression on cell membrane. These results provided a necessary basis for the following study of selection the antibodies targeting P-selectin.

7.Central venous access devices-related thrombosis in critical ill children: a literature review

This paper reviewed three aspects of risk factors of catheter-related thrombosis in critically ill children with central venous access devices, including the child's own factors, catheter-related factors and external factors. It summarized the main research advances of preventive measures such as the strengthen assessment of pre-catheterization, drug and mechanical prevention of thrombosis, control of catheter-related other complications, thrombus screening routinely, risk assessment model is used. It is to provide experience for early clinical identification and formulation of preventive measures, so as to reduce the occurrence of catheter-related thrombosis in critically ill children and promote the central venous access device used safely in PICU.
8.Investigation of current situation of radiotherapy in Henan province

Objective:To investigate the status of personnel, facilities, and technology of radiation therapy in Henan province in 2023.Methods:A unified online survey questionnaire was designed and distributed from March to April 2023 by the Henan Cancer Diagnosis and Treatment Quality Control Center to various medical institutions throughout the province to investigate the personnel, radiotherapy equipment, quality control equipment, imaging equipment, and radiotherapy technology development of each radiotherapy unit. Descriptive statistical methods were mainly used.Results:As of April 30, 2023, there were a total of 168 units engaged in radiation therapy in Henan province. The number of physicians involved in radiation therapy was 956, along with 365 medical physicists and 680 technicians. The equipment inventory included 212 medical linear accelerators, 1 cobalt-60 therapy machine, 32 afterloading therapy apparatuses, 4 Cyber Knife, 173 CT simulators, 2 MRI simulators and 94 conventional simulators. Linear accelerators were the primary radiotherapy equipment, 2.15 units per 1 million population on average. Additionally, there were 11 units offering 2D radiotherapy, 24 units offering 3D conformal radiotherapy, 130 units offering intensity-modulated radiotherapy, 41 units offering rotational intensity-modulated radiotherapy, and 33 units offering stereotactic radiosurgery. Regarding physical quality control equipment, 16 units were equipped with three-dimensional water tanks, 162 units were equipped with radiation dose meters, 114 units were equipped with morning check meters, 60 units were equipped with film dose meters, and 108 units were equipped with intensity adjustment plan verification systems.Conclusions:In 2023, there is a shortage of radiation therapy professionals in Henan province. Disparities are observed in the distribution of radiation therapy equipment among regions. Intensity-modulated radiotherapy has become the mainstream technology for radiation therapy in Henan province. The configuration of radiation therapy quality control equipment and standardized quality control work should be further improved.
9.Comparative study of IL-4, IFN-gamma gene methylation for the epigenetics of allergic rhinitis in Xinjiang Uygur, Han people.

OBJECTIVE:
To investigate the differences of IL-4, IFN-gamma gene promoter methylation of allergic rhinitis patients between Uygur and Han people of Xinjiang.
METHOD:
Methylation-specific PCR (MSP) detected IL-4, IFN-gamma gene methylation of each of 50 patients with allergic rhinitis in Uygur and Han.
RESULT:
Complete IL-4 gene promoter methylation rate was 44% (22/50) and 48% (24/50) in Uygur and Han AR patients, un-methylation was 26% (13/50) and 22% (11/50), coexistence rate of methylation and un-methylation was 30% (15/50) and 30% (15/50). Complete IFN-gamma gene promoter methylation rate was 12% (6/50) and 16% (8/50) in Uygur and Han AR patients, un-methylation was 8% (4/50) and 10% (5/50), coexistence rate of methylation and unmethylated was 80% (40/50) and 74% (37/50). Distribution of IL-4 gene methylation between Han and Uygur AR patients had no statistically significant (P > 0.05). Distribution of IFN-gamma gene methylation between Han and Uygur AR patients had no statistically significant (P > 0.05).
CONCLUSION
There is no difference of IL-4, IFN-gamma gene methylation in patients between the Han and Uygur.

10. Expressions and Significance of Livin, Survivin, Pak-1 and Caspase-7 in Different Gastric Mucosal Lesions

Background: Livin, survivin and Pak-1 are all related to the occurrence and development of gastric cancer. Livin and survivin play their roles by inhibiting the activity of caspase-7. Aims: To investigate the expressions and significance of livin, survivin, Pak-1 and caspase-7 in different gastric mucosal lesions. Methods: A total of 45 cases of gastric cancer and paracancerous tissue, 45 chronic atrophic gastritis with intestinal metaplasia, 45 chronic non-atrophic gastritis from Jan. 2015 to Dec. 2019 at the Second Affiliated Hospital of Baotou Medical College were collected. Immunohistochemistry was used to detect the expressions of livin, survivin, Pak-1 and caspase-7, and their correlations with clinicopathological features of gastric cancer patients were analyzed. Results: Compared with chronic non-atrophic gastritis group and paracancerous group, the positivity expression rates of livin, survivin and Pak-1 in intestinal metaplasia group were significantly increased (P<0.05), while caspase-7 was significantly decreased (P<0.05). Compared with intestinal metaplasia group, the positivity expression rates of livin, survivin and Pak-1 in gastric cancer group were significantly increased (P<0.05), while caspase-7 was significantly decreased (P<0.05). Expressions of livin, survivin, Pak-1 and caspase-7 were correlated with the differentiation degree, TNM stage, depth of infiltration and lymph node metastasis in patients with gastric cancer (P<0.05). Conclusions: Livin, survivin, Pak-1 and caspase-7 play important roles in the occurrence and development of gastric cancer.
