2.Clinical study on acupoint massage in improving cognitive function and sleep quality of elderly patients with hypertension

Objective:To investigate the effects of acupoint massage on cognitive function and sleep quality in elderly patients with hypertension. Methods:A total of 68 elderly patients with hypertension were enrolled, and their ages were all over 60 years old. They were divided into a control group and a treatment group by simple random method, with 34 cases in each group. Patients in the control group were treated by routine psychological health guidance and sleep knowledge education, while patients in the treatment group were treated by acupoint massage on the base of the interventions given to the control group. Both of their treatment courses were 3 months. Pittsburgh sleep quality index (PSQI) scores and mini-mental state examination (MMSE) scores before and after treatment were recorded and compared. Results:Two groups’ PSQI scores before treatment had no significant difference (P>0.05). After 3 months of treatment, their PSQI scores were statistically different (P<0.05). There was also no between-group statistical difference in MMSE score before treatment (P>0.05), but there was a significant difference after treatment (P<0.05). Conclusion:Acupoint massage for elderly patients with primary hypertension can improve their sleep quality and enhance their cognitive function, thus it is worthy of being applied in clinical nursing work.
3.Complete genome sequence of a genogroup I geno type 8 norovirus identified in Huzhou, China

We identified and characterized the full-length genome of a GI.8 norovirus strain CHN/Huzhou/N10 isolated in an outbreak in Huzhou,China.The full-length genome of CHN/Huzhou/N10 was amplified using five pairs of primers which were designed according to the full-length GI norovirus genome sequences published in GenBank database.Multiple alignments were performed using DNAStar,the phylogenetic relationship of CHN/Huzhou/N10 and the representative NoV (Norovirus) strains from each genogroup were assessed using the software MEGA version 6.0.The viral genome of CHN/Huzhou/N10 was 7 740 nucleotides in length,which was consist of three ORFs spanning 5-5 404 nt (ORF1),5 388-7 019 nt(ORF2),and 7 019-7 660 nt (ORF3),respectively.Phylogenetic analysis based on polymerase and capsid sequences VP1 and VP2 region indicated that CHN/Huzhou/N10 belonged to GI.8 genotype.The amino acid sequence analysis of the VP1 region showed that CHN/Huzhou/N10 had 16 mutations compared with the representative strain Boxer/2001/US,12 of these variations were located in the P2 subregion.Moreover,a single amino acid change (T347S) occurred at histo-blood group antigen (HBGA) binding site Ⅱ and another single amino acid change (T397E) occurred at HBGA binding site Ⅲ.In this study,the first full genome of norovirus GI.8 isolated in Huzhou,China was extensively characterized.The data would be helpful not only for the epidemiology study,but also for the diagnostic tool development and effective vaccine design in the future.
4.Preliminary results of locally recurrent nasopharyngeal carcinoma treated with intensity modulated radiation therapy

Purpose:To evaluate the physical dose distributions and tolerance in locally recurrent nasopharyngeal carcinoma treated with intensity modulated radiation therapy (IMRT).Methods:From June 1999 to August 2000, 10 patients with locally recurrent nasopharyngeal carcinoma proven by histology, without neck lymphoid node recurrence and distant metastasis, were treated by IMRT. The interval from the time of initial radiotherapy to recurrence ranged from 14—50(median 25) months. The initial radiotherapy in all patients were treated with external radiotherapy alone to a median dose of 69Gy/35fractions/52days. IMRT method was performed by means of the MIMiC collimator and inverse treatment planning system (NOMOS company, USA). The prescribed dose of re radiotherapy to target volume was 57Gy/19fractions/4weeks.Results:All patients, except for some patients with mild weight loss and reaction in the oral cavity during radiotherapy, tolerated this IMRT well. IMRT achieved the better immediate responses in 10 patients with complete response in 6 cases and partial response in 4 cases. Recurrence was found in two patients, lung distant metastasis in one patient. The median survival in the 10 patients was 14 months. The median planning target volume (PTV) was 95.8(60—134) cm 3. The dose distributions in the PTV were as follows: median average dose 59.65?2.47Gy, median homogeneity index (HI): 1.18(?0.06); median dose to 95% PTV ≥ 53.2 (?1.36)Gy. The median dose to the organs at risk (OAR) were as follows: spinal cord 9.46?5.23Gy; brain stem 20.24?3.55Gy; left parotid gland 18.53?5.30Gy, right parotid gland 19.68?6.21Gy; left lens 2.11?0.65Gy, right lens 2.94?0.57Gy;Chiasm12.34Gy; left optical nerve13.14 Gy, right optical nerve17.65 Gy.Conclusions:Better dose distributions treated with IMRT was achieved in the patients with locally recurrent nasopharyngeal carcinoma and the patients tolerated this radiotherapy well. 5.A comparative imageology study of TOF-MRA and SWI in the diagnosis of intracranial vascularmalformations

Objective To study the imaging of arteriovenous malformation(AVM)and cerebral cavernous angiomas(CCA)by 3D-TOF MRA and SWI.Methods The clinical manifestations and MR imaging characteristics of 44 cases with intracranial vascular malformations were retrospectively analyzed.The MRA imaging and SWI imaging of 26 CCA cases and 18 AVM cases were compared.Results The location,size,input.artery and output vein of arteriovenous fistula were clearly showed by 3D-TOF MRA.3D-TOF-MRA did not show the artery of hemangioma.The imaging of SWI can clear display the number and increase of cavernous hemangioma,comparing with the conventional sequence.Moreover,SWI was effective to show the tortuous veins of arteriovenous malformation.However,it was not able to directly show the overview of abnormal blood vessels and big arteries such as input artery.Conclusion 3D-TOF MRA technologies is powerful to show the arteriovenous malformation with high flow velocity and low resistance(Such as AVM),while SWI has advantages in showing the cavernous hemangioma with low flow velocity and small diameters.Joining 3D-TOF-MRA and SWI techniques will improve the detection of cerebral vascular malformations and reduce the misdiagnosis and missed diagnosis.
6.Study of change of Th17 cell caused by bacteria biofilm after infection

Objective:To study of change of Th17 cell and IL-17 caused by bacteria biofilm after infection.Methods: To detect biofilm formation ability of bacteria isolated from bronchus alveolus washing-water of patients;to detect expressing ratios of Th17 cells and IL-17 level in peripheral blood of patients and healthy person;to detect expressing ratios of Th17 cells and IL-17 level in bronchus alveolus washing-water of patients;to analyze the above data using SPSS17.0 software.Results: Expressing ratios of Th17 cells and IL-17 level in peripheral blood of patients infected by producing biofilm and non-producing biofilm bacteria and healthy person are respectively(0.59±0.18)%and(108.8±20.5)pg/ml,(0.58±0.18)%and(100.1±20.7)pg/ml,(0.55±0.17)% and(100.0± 21.4)pg/ml,they were not different among data from patients and healthy person,P>0.05,expressing ratios of Th17 cells and IL-17 level in bronchus alveolus washing-water of patients infected by producing biofilm and non-producing biofilm bacteria were respective (1.37±0.34)%,(157.4±30.8)pg/ml and(1.11±0.21)%,(136.2±24.3)mg/ml,the data between patients infected by producing biofilm and non-producing biofilm bacteria were distinctly different, P<0.05.Conclusion: Bacteria biofilm can cause Th17 cell expressing ratio and IL-17 level to become high in infected part,this may be mechanism that infection become chronic.
7.Clinical Study on Compound Sophora Flavescentis Radix (Kushen) Injection Combined with Gamma Knife for Treatment of Locally Advanced Pancreatic Carcinoma

Objective To evaluate the clinical efficacy of Compound Sophorae Flavescentis Radix (Kushen) Injection combined with gamma knife for treatment of locally advanced pancreatic carcinoma. Methods Totally 65 patients of locally advanced pancreatic cancer were randomly divided into two groups:32 cases of simple gamma knife group (group A), 33 cases of Compound Kushen Injection combined with gamma knife treatment (group B). Patients received enhancement computed tomography scanning and CA199 review at 1, 3, 6, 9, 12 months after treatment, and once every six months, a total of 36 months of follow-up. Short-term and long-term efficacy was observed after treatment, and survival curve analysis was carried out. Results Clinical benefit response was 53.13% in group A and 84.85% in group B (P=0.006);median survival time of two groups was 11.00 months in group A and 23.00 months in group B;the average survival time in group A was 16.37 months, and 22.28 months in group B (P=0.017). In the two groups, treatment of the 65 patients was completed smoothly as planned, and no one had serious complications, such as pancreatic leakage, skin burns, viscera perforation and radioactive enteritis. Conclusion Compound Kushen Injection combined with gamma knife can treat locally advanced pancreatic carcinoma safely and efficiently, which can improve patients’ life quality, especially in cancer pain control.
8.Effect of Shensong Yangxin Capsule on Cardiac Remodelling of Myocardial Infarction Mouse Model

Objective To investigate the effect of Shensong Yangxin capsule on cardiac remodelling of myocardial infarction mouse model and the possible molecular mechanisms. Methods Adult male C57BL/6J mice were divided into sham operation group(n=10), model control group(n=20)and Shensong Yangxin group(n=20)according to random number table. Left anterior descending branch of coronary artery was ligated to establish myocardic infarction model in the model control group and Shensong Yangxin group. From the 2nd day after the surgery, Shensong Yangxin ( 400 mg . kg-1 ) was intragastrically administered, and the death rate of the mice was observed.Four weeks after the surgery, echocardiography was used to measure the cardiac function;myocardiac infarction area was detected by pathological staining;the expression levels of cardiac remodelling markers and extracellular matrix proteins were detected by RT-PCR. The possible molecular mechanisms were screened by Western blotting. Results As compared with the model control group, Shensong Yangxin significantly reduced the mortality after myocardial infarction in mice(P<0.05), as well as the myocardial infarct size(P<0.05).The mRNA expression levels of cardiac remodelling markers ANP, BNP, and β-MHC and the extracellular matrix proteins(collagenⅠ, collagen Ⅲ, CTGF, TGFβ) decreased significantly in the Shensong Yangxin group as compared with the model control group. Western blotting showed that Shensong Yangxin significantly decreased activation of smad3, and reduced expression level of smad4. Conclusion Shensong Yangxin attenuates cardiac remodelling after myocardial infarction and the mechanism may be related with blockage of smad signaling pathway.
9.Research on the clinical value of urinary NTx,serum BAP and BMD in patients with bone metastasis from malignant tumors

Objective To evaluate the variance and clinical significance of urinary pyridinoline cross-linked N-telopeptides of Type Ⅰ collagen(uNTx) ,serum bone specific alkaline phosphates(sBAP) and Bone mineral density(BMD) in patients with bone metastasis malignant tumors .Methods The levels of 53 case patients′,who with bone metastasis ,uNTx and sBAP were measured by ELISA and BMD was detected by dual energy x-ray absorptiometer .40 cases of non-bone metastasis people was assayed .Results The levels of all bone markers in patients with bone metastasis was significantly higher than in the patients without bone metas-tasis(P<0 .05) .The levels of the two biomarkers showed a positive correlation with the number of metastatic loci in bone (P<0 .05) .But there was not significantly different between the levels of uNTx or sBAP with the severity of the bone pain (P>0 .05) . BMD decreased in patients with bone metastasis group ,but it was no difference in the non-bone metastasis(P>0 .05) .Conclusion uNTx and sBAP are helpful for early diagnosis and prevention of patients with bone metastasis from malignant tumors .Patients with malignant gumor is often accompanied by a decrease in BMD .
10.Differentiation of helper T lymphocyte in type Ⅲ prostatitis

Objective To investigate the differentiation of CD4+T helper cell in expressed prostatic secretions(EPS)in type Ⅲ prostatitis. Methods Seventy-six patients were studied,aged from 18 to 47(average 31.8).All patients presented with typical clinical symptoms for over 3 months.Cases were classified as type Ⅲ A(47 cases),type Ⅲ B(29 cases)and control group(16 cases)according to NIH classification system.Type Ⅲ A was also divided into group Ⅲ A1 with 26 cases(mild inflammation)and group Ⅲ A2 with 21 cases(severe inflammation).Th1 cytokine IFN-γ,Th2 cytokines IL-4 in EPS were detected by double antibody sandwich ELISA,and IFN-γ/IL-4 was determined.Results Compared with group control,IFN-γ in group Ⅲ A and Ⅲ B were significantly up regulated (134.78±43.67 pg/ml,109.82±30.09 pg/ml,P<0.05),while IFN-γ level in group Ⅲ A was higher than in group Ⅲ B(P<0.05).IL-4 in groupⅢ A was not changed(51.99±20.59 pg/ml,P>0.05).IL-4 in group ⅢB was higher(76.40±17.99 pg/ml,P<0.05).IFN-γ/IL-4 in group ⅢA was elevated significantly(2.94±1.12,P<0.05),while IFN-γ/IL-4 in group ⅢB was not changed(1.49±0.48,P>0.05).IFN-γ/IL-4 in group Ⅲ A2 was higher than in groupⅢA1 significantly(3.67±0.82vs 2.34±0.97,P<0.05). Conclusions Th1 eell differentiation took the dominance in type Ⅲ A prostatitis.Th1/Th2 equilibrium was shifted to Th1.It is probably one of the pathogenies that Th1 dominant differentiation leads to local inflammation in prostate of type Ⅲ A prostatitis.
