1.Semen quality and urogenital Ureaplasma urealyticum infection in 5 306 preconception males in one district

Objective To investigate the semen quality and the influence of urogenital Ureaplasma urealyticum infection in pre‐conception males in Shanghai .Methods From Jan ,2014 to Nov ,2015 ,5 306 preconception males in our andrology department were received semen analysis .The difference of semen index between ages and urogenital Ureaplasma urealyticum infected or not were analyzed .Results The average pH value was(7 .27 ± 0 .14) ,the average volume was(3 .15 ± 1 .42)mL ,the average sperm concen‐trationwas(57.51±40.22)×106/mL,theaverageoftotalspermcountwas(172.83±134.90)×106 ,theaveragevitalityratioof grade(A+B) sperm was(38 .50 ± 17 .54)% ,the average motile sperm ratio was(46 .36 ± 20 .08)% ,the average of total motile sperm count was(89 .86 ± 92 .82) × 106 .Motile sperm ratio of preconception males from twenty to twenty nine was(49 .60 ± 20 .70)% ,total motile sperm count was(93 .40 ± 95 .83) × 106 ,which was greater than other ages .Ureaplasma urealyticum turned to positive in 2 311 cases was (43 .55% ) .There were statistically significant differences in total sperm count ,total motile sperm count ,sperm concentration ,vitality ratio of grade A sperm ,vitality ratio of grade(A+B) sperm ,motile sperm ratio ,seminal acid phosphatase ,seminalα‐glucosidase and seminal plasma fructose between positive and negative samples(P<0 .01) .There was differ‐ence in pH and seminal plasma zinc between positive and negative samples(P<0 .05) .Conclusion The sperm quality of preconcep‐tion males in Shanghai is not encouraging ,urogenital Ureaplasma urealyticum infection in preconception men is very important .
2.CK assay with gene testing for screening of Duchenne muscular dystrophy in the newborn infants

Objective To establish a system using CK activity assessing with follow-up Duchenne muscular dystrophy (DMD) gene testing to newborn screening for DMD.This study provided a pathway to improve the health outcome for individuals with DMD.Methods Tests for CK were performed with Beckman original reagent on a Beckman Coulter AU 5800.Preliminary studies established a population-based range of CK in newborns using 5 892 deidentified anonymous blood samples,which were collected from Shanghai Changning Maternity and Infant Health Hospital between November 2013 and July 2014.Mutation analysis used multiplex PCR-denature high-performance liquid chromatography (PCR-DHPLC) method for screening large duplications and deletions and Sanger DNA sequencing for screening point DMD gene mutation.Results DMD gene mutations (point mutation,exon60,c.9072G > A) were found in 1 of 5 892 newborn subjects,which had CK level > 2 000 U/L large duplications and deletions in DMD gene were not found.Conclusions A system of analysis for newborn screening for DMD has been established.This path for newborn screening fits our health care system and minimizes the false-positive results for predicting DMD gene mutations by use of CK levels in blood
3.Shear wave elastography in the evaluation of fibrosis degree in renal allograft

Objective:To explore the value of shear wave elastography imaging(SWE) in the diagnosis of renal allograft fibrosis and analyze its advantages and limitations.Methods:The renal allograft of 61 patients who underwent renal allograft biopsy from June 2019 to April 2020 in the First Affiliated Hospital of Sun Yat-sen University were included in this study. According to the Banff classification, there were 51 patients with mild-degree fibration(interstitial fibrosis/tubular atrophy, IFTA 0-Ⅰ), and 29 patients with moderate or severe-degree fibration(IFTA Ⅱ-Ⅲ). Two-dimensional ultrasound, color Doppler flow imaging, SWE and kidney function test performed. All the results were compared between the two groups. Diagnostic performance was evaluated by receiver operating characteristic (ROC) curve using pathology as gold standard. The sensitivity, specificity, positive predictive value and negative predictive value were calculated for the diagnosis of moderate or severe-degree fibration.Results:Compared to mild-degree fibration group, creatinine( P<0.001), glomerular filtration rate( P<0.001), RI of arcuate arteries( P=0.022) and SWE value( P<0.001) significantly increased in the moderate or severe-degree fibration group. There were significant correlations between IFTA and creatinine ( r s=0.488, P<0.001), glomerular filtration rate ( r s=-0.452, P<0.001), RI of arcuate arteries( r s=0.228, P=0.042), SWE value( r s=0.584, P<0.001). Taking the cutoff value of SWE value deduced by ROC curve as 21.7 kPa, the area under ROC curve was 0.827. The sensitivity, specificity, positive predictive value and negative predictive value were 86.2%, 74.5%, 61.0% and 89.7%, respectively. Conclusions:There is a good correlation between the SWE value and the degree of fibrosis in the transplanted kidney. SWE can be used to distinguish mild from moderate or severe fibrosis of renal allograft, providing a potential noninvasive method for the assessment of kidney allograft fibration.
