1.The clinical value of 3-dimensional DSA in the diagnosis of intracranial aneurysms

Objective To explore the clinical value of 3-dimensional digital subtraction angiography(3D-DSA)in the diagnosis and therapy of intracranial aneurysms.Methods 2-dimensional digital subtraction angiography (2D-DSA)and 3D-DSA was performed respectively on 1 53 patients,including 1 5 1 cases with spontaneous subarachnoid hemorrhage (SAH),1 case with headache and 1 case with cerebral infarction;Meanwhile,to compare the difference in diagnosis of intracranial aneurysms between 2D-DSA and 3D-DSA.Results 1 12 patients(125 pieces)with intracranial aneurysms were detected on 2D-DSA,its detecting rate was 73.2%(1 12/1 53 );136 patients(1 52 pieces)with intracranial aneurysms were detected on 3D-DSA,its detecting rate was 88.9%(136/1 53)(χ2 =12.254,P <0.05).The displaying rate of 3D-DSA for the relationship between carrying tumor artery and aneurysms and the necks of aneurysms was 100%(1 52/1 52),however,the displaying rate of 2D-DSA for that was 61.6%(77/125)(χ2 =63.993,P <0.05). Conclusion 3D-DSA can improved the diagnostic accurate rate of intracranial aneurysms,it is important in chosing the therapeutic regimen.
2.Surgical diagnosis and treatment of splenic space occupying disease

Objective [WT5”BZ]To improve recognition and differential diagnosis of splenic space occupying disease.[WT5”HZ]Method [WT5”BZ]40 cases admitted from 1983 to 1999 were analyzed retrospectively。[WT5”HZ]Result [WT5”BZ]Twenty six out of 40 cases were of benign splenic disease including one case of cyst, 3 cases of splenic abscesses, 7 cases of splenic tuberculosis, 5 cases of splenic “true” cyst, 4 cases of splenic angiolymphoma, 3 cases of splenic aneurysm, and one each of splenic lymphoangioma, epidermoid cyst and hamartoma. There were 14 cases of malignant tumor, amongwhich,there were two cases of non Hodgkin′s lymphoma and two of leiosarcoma,one angiosarcoma and metastatic splenic malignant tumor in 9 cases.[WT5”HZ]Conclusion [WT5”BZ]Therapeutics should be adopted according the nature of splenic diseases, based on clinical and image characteristics.
3.Clinical study of volume resuscitation in children with septic shock

Objective To compare the effect of crystalloid and crystalloid plus albumin in the treatment of pediatric septic shock.Methods Data of 63 pediatric patients with septic shock admitted to Department of Critical Care Medicine of Hebei Provincial Children's Hospital were collected and retrospectively analyzed.The patients were divided into two groups according to whether they received albumin for volume resuscitation within 1 day after admission or not.The patients in observation group (crystalloid + albumin group,n =33) received normal saline (20 mL/kg) followed by 1 g/kg albumin 30 minutes after admission,and those in control group (crystalloid group,n =30) received only normal saline (20 mL/kg) 30 minutes after admission,and normal saline resuscitation was continued according to the effect of fluid therapy.Anti-infection and vasoactive drugs strategies were the same in both groups.The first-hour infusion volume,time showing stable hemodynamics,the incidence of pulmonary edema,and blood lactate levels at 0,6,12 hours after achieving the goals were compared,and blood lactate clearance rates were calculated.Results The first-hour infusion volume time in the observation group was lower than that in control group (mL:41.56 ± 10.50 vs.57.24 ± 7.54,t=4.596,P=0.000),and time showing stable hemodynamics was shorter than that in control group but without statistically significant difference (minutes:219.87 ±70.23 vs.287.10 ± 67.00,t=2.047,P=0.360).The incidence of pulmonary edema in observation group was slightly lower than that in control group [6.1% (2/33) vs.10.0% (3/30),x2±2.272,P=0.259].The lactic acid levels were decreased gradually along with rehabilitation time,while lactate clearance rate was increased in both groups.At 0 hour and 6 hours after resuscitation,the lactate level (mmol/L) in the observation group was significantly lower than that in control group [0 hour:3.65 ± 2.84 vs.5.72 ± 2.11,t= 1.940,P=0.046; 6 hours:2.12 ± 1.21 vs.4.09 ± 1.45,t=2.892,P=0.005],while the lactate clearance rate was significantly increased compared with control group [0 hour:(0.38 ± 0.15)% vs.(0.18 ± 0.09)%,t=1.447,P=0.018; 6 hours:(0.62 ± 0.14)% vs.(0.51 ± 0.11)%,t=1.920,P=0.047].However,at 12 hours after resuscitation,there were no statistically significant differences in the lactic acid level (mmol/L:1.46 ± 0.39 vs.1.54 ± 1.90,t=0.450,P=0.072) and the lactate clearance rate [(0.78 ± 0.19) % vs.(0.77 ± 0.18) %,t =0.091,P=0.928] between observation group and control group.Conclusion Albumin resuscitation in children with septic shock can stabilize hemodynamics earlier,reduce the incidence of pulmonary edema,and improve the successful rescue rate of refractory septic shock.
4.LDN-193189 inhibits progression and induces apoptosis in human dedifferentiated chondrosarcoma cell line NDCS-1

Objective:To clarify the effects of the BMP receptor inhibitor LDN-193189 in the dedifferentiated chondrosarcoma (DDCS) cell line NDCS-1 and to explore the anti-tumor mechanism of LDN-193189 in DDCS. Methods:NDCS-1 was treated with 5 nmol/L of LDN-193189. MTT assay and clone formation experiments were used to verify that LDN-193189 suppressed cel proliferation. Transwel and wound healing tests were performed to demonstrate that LDN-193189 inhibited cell invasion. Western blot detection was used to show that LDN-193189 inhibited the suppression of BMPR2, p-Smad1/5, and RUNX2 protein expression. Results:The BMPR2 signaling pathway was inhibited by LDN-193189;thus, cell viability and invasion were significantly suppressed. Conclusion:LDN-193189 induces the inhibition of progression in vitro via the BMPR2-p-Smad1/5-RUNX2 signaling pathway in the human DDCS cell line NDCS-1.

7.Therapeutic effect of bone marrow transplantation on mustard-poisoned mice

AIM To study the therapeutic effect of bone marrow transplantation on mustard gas-poisoned mice. METHODS Mustard-poisoned mice model was constructed by injected mustard gas 20 mg·kg~(-1) subcutaneously. At 4 h, 1 d and 2 d after poisoned by mustard gas, the suspended bone marrow cells (106 cells per time) were injected into mice respectively. After poisoned 3, 5, 7 and 10 d, the numbers of leukocyte and marrow nucleated cells were recorded periodically. At the same time, after poisoned 4 h and 1 d by mustard gas, mice in another group were injected granulocyte colony stimulating factor (G-CSF) 74 μg·kg~(-1) combined with transplantation and changes of the cell number were observed. RESULTS Compared with poisoned group without transplantion, leukocyte number in bone marrow transplanted group presented ascending trend, but there was no statistical significance until the 10 d after poisoned. However, the number of marrow nucleated cell increased significantly from the 5th day. After mice injected bone marrow cells combined with G-CSF, leukocyte increased markedly in earlier stage compared with transplantation only. CONCLUTION The bone marrow transplantation has significant effects on improvement of marrow hematogenic system inhibited by mustard gas.
8.Effects of ethyl gallate on sepsis-induced acute lung injury in rats

Objective To evaluate the effects of ethyl gallate (EG ) on sepsis-induced acute lung injury (ALI) in rats .Methods Forty-eight healthy male Wistar rats ,aged 10-12 weeks ,weighing 150-250 g ,were randomly divided into 3 groups (n=16 each) using a random number table :control group (group C) ,group ALI , and group EG .The animals were anesthetized with intraperitoneal pentobarbital sodium 50 mg/kg .Sepsis was induced by cecal ligation and puncture (CLP) .EG 1 ml/h (50 mg·kg-1 ·h-1 ) was infused intravenously via the femoral vein for 4 h starting from 6 h after CLP in group EG .The equal volume of normal saline was given instead in group ALI . The rats were sacrificed at 10 and 24 h after CLP (T1 ,2 ) , and the lungs were removed for determination of wet/dry lung weight ratio (W/D ratio ) , superoxide dismutase (SOD ) activity , contents of malondialdehyde (MDA ) and glutathione (GSH ) , and 3-NT expression (by immuno-histochemistry ) in lung tissues .The pathological changes of the lung were examined with light microscope .Results Compared with group C ,the W/D ratio ,MDA content and 3-NT expression were significantly increased and GSH content was decreased at T1 ,2 ,the SOD activity was significantly decreased at T2 in group ALI ,and the MDA content and 3-NT expression were increased at T1 ,and the W/D ratio ,MDA content and 3-NT expression were increased and GSH content was decreased at T2 in group EG ( P<0.05) .Compared with group ALI ,the MDA content was significantly decreased at T1 ,and the W/D ratio ,MDA content and 3-NT expression were decreased ,and GSH content and SOD activity were increased at T2 in group EG ( P<0.05) .The pathological changes of the lung were obvious in group ALI , and significantly attenuated in group EG .Conclusion EG can attenuate sepsis-induced ALI by antioxidative effects in rats .
9.Application of transesophageal echocardiography in extracorporeal circulation cardiac valve replacement

Objective To discuss the clinical value of transesophageal echocardiography used in extracorporeal circulationcardiac valve replacement. Methods 89 patients received extracorporeal circulation cardiac valve replacement in our hospital from January 2012 to December 2014 were included in the study. Transesophageal echocardiography were performed preoperatively, intraoperation and postoperatively and the findings were compared to the result of preoperation transthoracic echocardiography exam. The measurement of aortic annulus diameter and detection rates of calcification obtained by transthoracic and transesophaged echocardiography were compared. Changes in heart function parameters before and after operation were compared. The alternations in surgery plan made after pre-operative transesophageal echocardiography exam and any relevant treatment or procedure performed according to intra-operation transesophageal echo findings were recorded. All the patients were followed up for 6 months - 40 months. Results Before extracorporeal circulation cardiac valve replacement, the aortic annulus diameter measured and detection rates of calcification by transthoracic echocardiography were smaller and lower than actual surgical pathology results ( P < 0. 05 ) . No differences in the results from transesophageal echocardiography of two-dimensional and three-dimensional examination when compared to pathological findings ( P > 0. 05 ) . Conclusions For patients receiving extracorporeal circulation cardiac valve replacement, transesophagus echocardiography examination is helpful to guide the selection of valve stent and operation methods.
10.The impact of testosterone replacement therapy on muscle strength and function among the elderly: meta-analysis and systematic review

Objective:To the effects of testosterone replacement therapy on muscle strength and function among the elderly (≥65 years old).Methods:We searched English and Chinese databases of PubMed, SinoMed, etc, upto July 4, 2020. All retrieved literatures were reviewed according to inclusion and exclusion criteria in accordance with PICOS principles. The Cochrane risk bias assessment tool was used for quality evaluation. Two researchers extracted data and evaluated literature quality independently. The outcomes included muscle strength (assessed by handgrip strength, leg extension strength, etc) and function (assessed by walking speed according to 6-minute walking test, 6MWT). Review Manager 5.3 software was used for statistical analysis. The fixed or random effects model was used to merge data of upper-and lower-extremity strength or 6MWT to produce forest plot and funnel plot. The subgroup analyses were conducted based on the characteristics of included studies. The sensitivity analyses were conducted for excluding literatures with small sample size, etc.Results:A total of 15 relatively high quality researches (14 English literatures and 1 Chinese literature) were included. The results of this meta analyses showed TRT could improve upper-extremity (0.21[0.11, 0.32]) and lower-extremity (0.34[0.12, 0.55]) muscle strength while not physical function (17.62[-13.06, 48.31]) among the elderly men. Subgroup analyses showed that region, source of participants, administration route and intervention period, while not the baseline testosterone level had effect on the pooled effect size. Funnel plot suggested a certain degree of publication bias. Sensitivity analyses revealed that the meta analyses were robust.Conclusion:TRT can improve muscle strength while not physical function among elderly men.
