1.Psychosocial crisis intervention for coronavirus disease 2019 patients and healthcare workers.

OBJECTIVES:
Shelter hospital was an alternative way to provide large-scale medical isolation and treatment for people with mild coronavirus disease 2019 (COVID-19). Due to various reasons, patients admitted to the large shelter hospital was reported high level of psychological distress, so did the healthcare workers. This study aims to introduce a comprehensive and multifaceted psychosocial crisis intervention model.
METHODS:
The psychosocial crisis intervention model was provided to 200 patients and 240 healthcare workers in Wuhan Wuchang shelter hospital. Patient volunteers and organized peer support, client-centered culturally sensitive supportive care, timely delivery of scientific information about COVID-19 and its complications, mental health knowledge acquisition of non-psychiatric healthcare workers, group activities, counseling and education, virtualization of psychological intervention, consultation and liaison were exhibited respectively in the model. Pre-service survey was done in 38 patients and 49 healthcare workers using the Generalized Anxiety Disorder 7-item (GAD-7) scale, the Patient Health Questionnaire 2-item (PHQ-2) scale, and the Primary Care PTSD screen for the Diagnostic and Statistical Manual of Mental Disorders, 5th edition (PC-PTSD-5). Forty-eight healthcare workers gave feedback after the intervention.
RESULTS:
The psychosocial crisis intervention model was successfully implemented by 10 mental health professionals and was well-accepted by both patients and healthcare workers in the shelter hospital. In pre-service survey, 15.8% of 38 patients were with anxiety, 55.3% were with stress, and 15.8% were with depression; 16.3% of 49 healthcare workers were with anxiety, 26.5% were with stress, and 22.4% were with depression. In post-service survey, 62.5% of 48 healthcare workers thought it was very practical, 37.5% thought more practical; 37.5% of them thought it was very helpful to relief anxiety and insomnia, and 27.1% thought much helpful; 37.5% of them thought it was very helpful to recognize patients with anxiety and insomnia, and 29.2% thought much helpful; 35.4% of them thought it was very helpful to deal with patients' anxiety and insomnia, and 37.5% thought much helpful.
CONCLUSIONS
Psychological crisis intervention is feasible, acceptable, and associated with positive outcomes. Future tastings of this model in larger population and different settings are warranted.

2.Application of on-site management model based on 5M1E in high-risk patients with chest pain coming to hospital by themselves

Objective:To explore the effects of on-site management model based on 5M1E in high-risk patients with chest pain coming to hospital by themselves.Methods:Totally 263 high-risk patients with chest pain who went to Beijing Luhe Hospital, Capital Medical University by themselves from July 2017 to October 2018 were selected. Patients who received routine management from July 2017 to February 2018 ( n=128) were included into the routine management group, while patients who received on-site management based on 5M1E from March to October 2018 ( n=135) were included into the on-site management group. The arrival time at the emergency department gate (D) -first ECG time, D-blood collection time, blood collection time-chest pain biomarker result time, D-time of leaving emergency department for patients entering the catheterization room, D-balloon expansion time (D2B) for patients undergoing interventional therapy, and the treatment time of emergency patients with chest patients of the two groups of patients were recorded and compared. Results:The difference in D-first ECG time, D-blood collection time, blood collection time-chest pain biomarker result time, and D-time of leaving emergency department between the two groups of patients was statistically significant ( P<0.05) , whereas there was no statistically significant difference in D2B between the two groups ( P>0.05) . Conclusions:The on-site management model based on 5M1E can effectively shorten the time for consultation and treatment of high-risk patients with chest pain who come to the hospital by themselves, which is worth promoting in clinical practice.
3.Application effect of fast track surgery-clinical nursing path diagnosis and treatment model in obstetrics perioperative period

Objective:To explore the effect of comprehensive mode combining the concept of fast track surgery-clinical nursing path (FTS-CNP) on the perioperative treatment of obstetrics.Methods:A total of 60 patients who underwent cesarean section in our hospital from April 2015 to March 2017 were selected and divided into the control group and the observation group according to the random number table method. The control group patients received conventional perioperative management, and the observation group underwent FTS-CNP management mode, recording postoperative feeding time, postoperative activity time, hospitalization time, complication rate, and physical and mental health score values.Results:The eating time, postoperative activity time, length of stay, and complication rate in the observation group were (0.52 ± 0.15)days, (1.17 ± 0.38)days, (5.43 ± 0.90)days, and 0, and the control group were (1.54 ± 0.24)days, (2.19 ± 0.58)days, (7.80 ± 0.80)days, and 40%(12/30). There was a statistical difference between the two groups ( t value was 11.40, 4.64, 6.23, χ2 value was 13.33, P <0.05). Physiological and mental health of the observation group after cesarean section The scores were 152.21 ± 5.15 and 127.23 ± 2.38, and the control group was 115.14 ± 2.24 and 101.92 ± 2.58. There was also a statistical difference between the two groups ( t value was 11.21, 14.04, P<0.05). Conclusions:FTS group management mode can provide obstetrics perioperative patients with quality and efficient services, which is hopeful to achieve good social and economic benefits.
4.Analysis of the pregnancy outcomes in pregnant women with COVID-19 in Hubei Province

Objective:To study the effect of COVID-19 on pregnancy outcomes and neonatal prognosis in Hubei Province.Method:s A retrospective comparison of the pregnancy outcomes was done between 16 women with COVID-19 and 45 women without COVID-19. Also, the results of laboratory tests, imaging examinations, and the 2019 novel coronavirus (2019-nCoV) nucleic acid test were performed in 10 cases of neonatal delivered from women with COVID-19.Result:s (1) Of the 16 pregnant women with COVID-19, 15 cases were ordinary type and 1 case was severe type. No one has progressed to critical pneumonia.The delivery method of the two groups was cesarean section, and the gestational age were (38.7±1.4) and (37.9±1.6) weeks,there was no significant difference between the two groups ( P>0.05). Also, there wee no significant differences in the intraoperative blood loss and birth weight of the newborn between the two groups (all P>0.05). (2) Ten cases of neonates delivered from pregnant women with COVID-19 were collected. The 2019-nCoV nucleic acid test were all negative.There were no significant differences in fetal distress, meconium-stained amniotic fluid, preterm birth, and neonatal asphyxia between the two groups (all P>0.05).(3) In the treatment of uterine contraction fatigue, carbetocin or carboprost tromethamine was used more in cesarean section for pregnant women with COVID-19 (1.3±0.6), compared with Non-COVID-19 group (0.5±0.7),the difference was statistically significant ( P=0.001). Conclusions:If there is an indication for obstetric surgery or critical illness of COVID-19 in pregnant women, timely termination of pregnancy will not increase the risk of premature birth and asphyxia of the newborn, but it is beneficial to the treatment and rehabilitation of maternal pneumonia. Preventive use of long-acting uterotonic agents could reduce the incidence of postpartum hemorrhage during surgery. 2019-nCoV infection has not been found in neonates delivered from pregnant women with COVID-19.
5.Inhibition of p53 and/or AKT as a new therapeutic approach specifically targeting ALT cancers.

While the majority of all human cancers counteract telomere shortening by expressing telomerase, ~15% of all cancers maintain telomere length by a telomerase-independent mechanism known as alternative lengthening of telomeres (ALT). Here, we show that high load of intrinsic DNA damage is present in ALT cancer cells, leading to apoptosis stress by activating p53-independent, but JNK/c-Myc-dependent apoptotic pathway. Notably, ALT cells expressing wild-type p53 show much lower apoptosis than p53-deficient ALT cells. Mechanistically, we find that intrinsic DNA damage in ALT cells induces low level of p53 that is insufficient to initiate the transcription of apoptosis-related genes, but is sufficient to stimulate the expression of key components of mTORC2 (mTOR and Rictor), which in turn leads to phosphorylation of AKT. Activated AKT (p-AKT) thereby stimulates downstream anti-apoptotic events. Therefore, p53 and AKT are the key factors that suppress spontaneous apoptosis in ALT cells. Indeed, inhibition of p53 or AKT selectively induces rapid death of ALT cells in vitro, and p53 inhibitor severely suppresses the growth of ALT-cell xenograft tumors in mice. These findings reveal a previously unrecognized function of p53 in anti-apoptosis and identify that the inhibition of p53 or AKT has a potential as therapeutics for specifically targeting ALT cancers.
6.Reliability and validity of the Chinese version of Rheumatoid Arthritis Self-Efficacy Scale

Objective To translate Rheumatoid Arthritis Self-efficacy Scale (RASE) into Chinese and to evaluate its reliability and validity. Methods A total of 188 hospitalized patients with rheumatoid arthritis (RA) were selected as research subjects through convenience sampling method. According to the translation mode of the scale, Chinesization and cultural adjustment were conducted to the English version of RASEto test reliability and validity. Results Item analysis showed that the Chinese version of RASE could discriminate the high-score group from the low-score group (P<0.01). Pearson correlation analysis showed that the correlation coefficient between the score of each item and the total score of the Chinese version of RASE was positively correlated (P<0.01).Exploratory factor analysis extracted a total of 8 common factors, which explained 68.55%of the total variance. The Cronbachαof the Chinese version of RASE was 0.901, and Cronbachαof each dimension ranged from 0.660 to 0.867;the retest reliability was 0.955 after 1 week, and ranged from 0.819 to 0.984 for each dimension;the split-half reliability coefficient was 0.848. Conclusion The Chinese version of RASE has good reliability and validity, which can be applied to the research of self-efficacy of patients with RA in China.
7.Study on Quality Standard of Compound Lavandula angustifolia Ointment

OBJECTIVE:To establish the quality standard for Compound Lavandula angustifolia ointment. METHODS:TLC was used for the qualitative identification of ethanol extract from Scutellaria baicalensis and volatile oil of L. angustifolia. GC method was used for qualitative identification of dementholized peppermint oil. GC method was used to determine the content of menthol. The determination was performed on Agilent DB-WAX capillary column,with temperature programming. The injector temperature was 250℃,and the temperature of detector was 250℃.The injection volume was 1 μL and the split ratio was 5:1 by split sampling. RESULTS:TLC spots of ethanol extract of S. baicalensis and volatile oil of L. angustifolia were clear and well-repeated without interference from negative control. The chromatographic peaks in TLC of test samples of dementholized peppermint oil had same retention time as that of substance control.The linear range of menthol injection amount was 0.113 4-1.133 5μg (r=0.999 4). RSDs of precision,intra-day precision,stability and reproducibility tests were not higher than 2.0%. The recoveries were 95.40%-99.82%(RSD=1.61%,n=6). CONCLUSIONS:Established quality standard can be used for the quality control of Compound L.angustifolia ointment.
8. Research advances in the v-raf murine sarcoma viral oncogene homolog gene mutation in ameloblastoma

V-raf murine sarcoma viral oncogene homolog B1 (BRAF) is a pro-oncogene, which is one member of the RAF family. Mutated BRAF is found in approximately 8% of human tumors. BRAF gene mutations lead to continuous activation of the mitogen-activatd protein kinase (MAPK) pathway, which resulting in abnormal cell proliferation and tumorigenesis. In recent years, recurrent MAPK signaling mutations were identified in ameloblastoma, among which BRAF-V600E is the most prominent type. This provides new strategies for the targeted treatment of ameloblastoma. This paper reviewed the latest advances in BRAF gene mutation associated with ameloblastoma and its potential clinical significance.
9.Component Analysis of 40% Ethanol Eluate of Polyamide Resin Column of Wei Medicine Hyssopus of-ficinalis and Its Improvement Effect on Inflammation in Asthmatic Mice

OBJECTIVE:To conduct component analysis for the 40% ethanol eluate of Hyssopus officinalis,and investigate its improvement effect on inflammation in asthmatic mice. METHODS:The 40% ethanol eluate of polyamide resin column was col-lected,and HPLC-high resolution mass spectrometry was used for the component analysis of 40% ethanol eluate of H. officinalis. Totally 72 mice were randomly divided into blank group (normal saline),model group (normal saline),dexamethasone group (positive control,1.6 mg/kg) and 40% ethanol eluate of H. officinalis high-dose,medium-dose,low-dose groups (200,100,50 mg/kg),12 in each group. Except for normal group,mice in other groups were intraperitoneally injected 0.2 mL ovalbumin (OVA)for sensitization in 0,14 d and intragastrically administrated in 25-31 d,once a day. After administration,2 mg/mL OVA was dropped in nose for 7 d. After 24 h of last dropping in nose,tumor necrosis factor α(TNF-α),interleukin-4(IL-4),interfer-on-γ(IFN-γ)levels in bronchoalveolar lavage fluid(BALF)were detected;pathological changes in lung tissue were observed. RE-SULTS:Totally 11 compounds were identified,the relative percentage content of 40.89%. The main components were rosmarinic acid,luteolin 7-O-β-D-rhamnoserhamnose (1→6)-α-D-pyran glucoside,hyperoside,etc. Compared with blank group,TNF-α, IL-4 levels in BALF in model group were increased,IFN-γ level was declined,and IL-4/IFN-γ ratio was enlarged(P<0.01);lung tissue was seriously damaged,there was infiltration of inflammatory cells around the blood vessels. Compared with model group, TNF-α,IL-4 levels in BALF in dexamethasone group,40% ethanol eluate of H. officinalis high-dose,medium-dose groups were declined,IFN-γ level was increased,and IL-4/IFN-γ ratio was reduced (P<0.01);pathological changes in lung tissue were improved. CONCLUSIONS:The established analysis method can effectively analysis the chemical components of 40% etha-nol eluate of H. officinalis,which has certain regulatory effect on releasing inflammatory factors and reducing inflammatory lesions in lung tissue of mice with bronchial asthma.
10.Study on the Spectrum-effect Relationship of Anti-inflammation Effect in vitro of Extracts from Uygur Medicine Hyssopus cuspidatus

OBJECTIVE:To provide reference for clarifying the pharmacodynamic material basis of anti-inflammation effect of Uygur medicine Hyssopus cuspidatus. METHODS:HPLC was conducted to establish the fingerprint of 8 extracts with different po-larities. Using macrophage RAW264.7 as object,the inhibitory effect of extracts with different polarities on inflammatory factor ni-tric oxide(NO),TNF-α and IL-6 in supernatant of cell culture medium was detected,the differences of anti-inflammatory activity in vitro were compared. Grey correlation analysis method was used to analyze the spectrum-effect relationship of peak area of common peaks and anti-inflammation activity. RESULTS:The fingerprint of 8 extracts with different polarities showed obvious differences. Anti-inflammation in vitro results suggested that 30% ethanol extract had the strongest inhibitory effect on inflammatory cytokines in vitro. There were 14 common peaks in the established HPLC fingerprints,the 5 common peaks that was closely related to the in-hibitory effect of inflammatory factors in vitro were peak 8,6,5,10,13,respectively;peak 8 was rosmarinic acid. CONCLUSIONS:The established fingerprint and its spectrum-effect relationship with anti-inflammation activity in vitro can provide certain reference for its pharmacodynanic material basis study.
