AIM:To explore the changes of excitatory amino acids and N-methyl-D-aspartate receptor (NMDAR) in brain undergoing ischemia-reperfusion injury following hepatic portal occlusion (HPO) in vivo.METHODS: The electrolytes and pH of portal vein blood, the content of Glu and Asp in brain, and the expression of NMDAR mRNA were studied with blood-gas analysis, HPLC and semi-quantitative RT-PCR method in two groups(HPO or sham), respectively. Three time points(6 h,12 h and 24h after reperfusion) were included.RESULTS: HPLC measure showed that the content of cortex Glu and Asp in group HPO elevated significantly after reperfusion compared with sham group, the peaks were at 6 h and 24 h Glu:(349?145) ?g?g-1wt,(456?203) ?g?g-1wt vs (238?24) ?g?g-1wt,(225?59) ?g?g-1wt;Asp:(134?50) ?g?g-1wt,(166?50) ?g?g-1wt vs (99?24) ?g?g-1wt,(71?20) ?g?g-1wt.Moreover, semi-quantitative RT-PCR analysis discovered that the expression of NMDAR mRNA increased significantly , the subunit NR1 mRNA was higher in reperfusion 6 h and last to 12 h(1.63?0.06 vs 1.18?0.05; 1.73?0.06 vs 1.17?0.03), the peak of NR2B mRNA was in 12 h (1.75?0.04 vs 1.18?0.05) ,but they did not further increase after reperfusion for 24 h.CONCLUSION: HPO increases the content of Glu and Asp and the expression of NMDAR in brain cortex. The Glu-NMDAR pathway plays a role in the mechanism for the brain injury after HPO. The products of IR injury might be the key factor to cause EAAs acumulation in synapse space.

Objective To evaluate the efficacy and safety of emergency percutaneous coronary intervention(PCI) in patients with non-ST-segment elevation acute coronary syndrome(ACS).Methods A total of 233 patients(emergency group) were treated with emergency PCI within 48 h of heart attack and another 152 patients(delayed group) were treated with PCI after 3-14 days of medical therapy.All culprit lesions were treated.Procedural success rate,the time from admission to angina relief,the length of hospital stay and cardiac events incidence in 30 days were observed.Results The procedural success rates for the emergency group and the delayed group were similar(98.1% vs 95.5%),but cardiac events incidence in 30 days was significantly lower in the emergency group than that of the delayed group(2.9% vs 14.1%,P

Patients with epidermoid cysts, formated probably in the process of embryonic development, the reasons of rapid growth may be considered for inflammatory stimulation.

[ ABSTRACT] AIM:To investigate the effects of maternal limb ischemic preconditioning ( LIP) on the mitochon-drial structures and functions of the hippocampal neurons induced by reoxygenation in the intrauterine distress fetal rats. METHODS:Pregnant rats (n=40) were randomly divided into 4 groups: sham (S) group, LIP group, fetal distress ( FD) group and LIP+FD group.Intrauterine ischemia model was established through the experimental design.The ultra-structure of the mitochondria in CA1 area of the hippocampus was observed .The mitochondrial membrane potential and re-active oxygen species ( ROS) were measured .The content of ATP and MDA in the hippocampus tissue was detected.The activity of Mn-SOD was observed.RESULTS:Compared with sham group, the ultrastructure of mitochondria in CA1 area of the hippocampus was damaged in FD group and LIP+FD group.The mitochondrial membrane potential, the content of ATP and the activity of Mn-SOD were decreased.However, the content of ROS and MDA was increased.Compared with FD group, the ultrastructure of mitochondria in CA1 area of the hippocampus was intact in LIP+FD group.Furthermore, the reduced mitochondrial membrane potential and ATP content were inhibited.The activity of Mn-SOD was increased, but the content of ROS and MDA was decreased in LIP+FD group.CONCLUSION:Limb ischemia preconditioning inhibits the damage the mitochondria of fetal hippocampal neurons induced by reoxygenation in the intrauterine distress fetal rats.

Objective To observe periventricular white matter changes and matrix metalloproteinase-3 (MMP-3) expression after two-vessel occlusion in rats.Methods A total of 40 Wistar rats were randomly divided into four groups:sham operation group,model group 1 (20 days after operation),model group 2 (40 days after operation),model group 3 (60 days after operation)(n =10 per group).Model of chronic cerebral hypoperfusion was induced by permanent ligation of the bilateral common carotid arteries in the rat.The change of periventricular white matter was observed by electron microscope.The myelin basic protein (MBP) levels was determined by Western blotting,and the expression of MMP-3 was analysed by immunohistochemical stain and Western blotting.Results Permanent ligation of the bilateral common carotid arteries resulted in demyelination in the corpus callosum and internal capsule in rats.The expressions of MMP-3 in model groups were markedly elevated compared with sham group (model group 1,39.17± 4.167; model group 2,43.33±3.502; model group 3,54.16±2.787; sham group,26.67±3.830; all P＜0.01).The content of MBP in model groups was gradually decreased compared with sham group (model group 1,54.50±4.087; model group 2,47.83±4.875; model group 3,36.50±4.231; sham group,65.01±7.688; all P＜0.01).Additionally,the upregulation of MMP-3 had significant correlation with the loss of MBP in the periventricular white matter (r =-0.883,P＜0.01).Conclusions Chronic cerebral hypoperfusion induces progressive periventricular white matter demyelination,and the upregulation of MMP-3 may involve in the pathological process.

Objective To search for the methods of inducing the high stability rat middle cerebral artery occlusion (MCAO) model and to noninvasively assess the method of model effect. Methods Six kinds of filaments with different diameters were used to induce rat MCAO models and their success rate, incidence of subarachnoid hemorrhage, and infarct volume were compared. The model scores were performed to assess the model effect after the operation and at 24 hours after reperfusion. Results Higher model success rate and lower incidence of subarachnoid hemorrhage were achieved when the 0. 28 mm in diameter and 0.32-0.34 mm in tip diameter filaments were used. 1he sensibility and specificity were higher when the model scores were performed at 24 hours after reperfusion. Conclusions Using the filaments of 0.28 mm in diameter and 0. 32-0. 34 mm in tip diameter for intraluminal thread could improved the stability of the models. The model scores at 24 hours after reperfusion could noninvasively assess the model effect.

Objective To investigate the feasibility and efficacy of vaginal sacrospinous ligament fixation (VSSLF) procedure which reserving uterus in the treatment of uterine prolapse. Methods 8 cases with uterine pro-lapse underwent VSSLF procedures which reserving uterus from July 2006 to December 2008. Those with incontinence d urine simultaneously underwent ventrofixation of the middle piece of the urethra and folding of the ligamentum pos-terius of the uretha and those with anterior or posterior vaginal wall prolapse underwent eolporrhaphia anterior-posteri-or. Results All patients successfully accomplished by VSSLF,without intraoperative or postoperative hemorrhage or organ injury. All patients' prolapse symptoms were relieved efficently according to the pelvic organs prolapse quantifi-cation(POP-Q). Patients were followed-up for 18 months after operation. 8 patients all recovered well without any complication related to the procedure after operation. Conclusion VSSLF is a simple,safe,effective and micro-inva-sire way for patients with uterine prolapse who want to reserve uterus.
