1.Comply with the new reform to promote public hospital financial information management

This paper analyzes the concept of hospital financial information management and the current situation.From the perspective of public hospital reform, we proposed the hospital should fulfill the information-based budget management and salary management, construct hospital information-based cost control flowsheet, build message conducting and transferring program, that budget should be achieved as soon as possible, information technology should be applied in hospital management, and build a complex financial information management team as soon as possible.
2.Oxidative stress in diabetic peripheral neuropathy

Oxidative stress play a key role in the pathogenesis of diabetic peripheral neuropathy.Reactive oxidative species(ROS), derived from hyperglycemia through abnormal mitochondrial electron chain and NADPH oxidase et al, may damage lipids, DNA, and proteins by oxidation.ROS can also promote other classical mechanisms of diabetic peripheral neuropathy, and further cause apoptosis of neurons and Schwann cells, or inflammation of nerve tissue, and finally lead to nerve injury.The relationship between oxidative stress and mechanisms of diabetic peripheral neuropathy was reviewed in this article, in order to get a clue for more effective treatment strategies.
3.Hypoglycemic Effect of n ̄butyl Alcohol Extraction from Anoectochilus roxburghii

Objective To study the hypoglycemic effect and mechanism of n ̄butyl alcohol extraction from Anoectochilus roxburghii. Methods The total of 12 normal rats were used as normal control group, and 48 diabetic model rats were divided into model control group, and high dose, middle dose and low dose treatment groups with 12 rats in each group,three treatment groups received Anoectochilus roxburghii at dose 600,400,200 mg.kg-1 , respectively, normal control group and model control group were administered with equal volume of 0.9% sodium chlorade solution once a day.After 21 days of drug administration, the weights of rats, the levels of blood glucose and insulin were examined. Results Compared with normal control group, the body weight in model control group decreased significantly(P<0.01), while blood glucose significantly increased (P<0.01).Compared with model control group, the body weight in Anoectochilus roxburghii groups exhibited no obvious difference,while blood glucose decreased significantly (P<0.01 or P<0.05).The level of insulin in high dose group increased remarkably (P<0.05). Conclusion The n ̄butyl alcohol extraction from Anoectochilus roxburghii shows markedly hypoglycemic effect,specifically for which at high dose increases the level of insulin significantly,its mechanism may involve with promoting β cells regeneration.
4.An analysis of the cause of laparoscopic adrenalectomy failure and its complications

Objective To study and to analyse the cause of laparoscopic adrenalectomy failure and its complications. Methods 8 of 19 laparoscopic adrenalectomies were converted to open surgery .The cause of failure and its complications were analysed. Results The causes to convert to open surgery were:bleeding of right middle suprarenal vein in 1,adrenal tumor was sheltered of cauda pancreatis in 1,left adrenal cyst was sheltered by spleen in 1,CO 2 pneumoperitoneal pressure can not be maintained during the procedure in 2,tumers could not be found in 2,the telescopic port was inserted into musculus psoas major in the retroperitoneal approach and the bleeding influenced the operation field in 1.3 patients had high fever after operations and 1 had an abscess in the adrenal bed.Reactive pleuritis occurred in 1. Conclusions Fine technique and experiences were needed for laparoscopic procedures and good team work between the operators was very important.Complications were often associated with the initial cause that led to the laparacospic adrenalectomy failure.Laparoscopic adrenalectomy via the retroperitoneal approach has been easier than that via the transperitoneal approach.
5.Implantation metastasis to abdominal incision after open operation for bladder carcinoma

Objective To study the diagnosis, treatment, and prognosis of implantation metastasis to abdominal incision after open operation for bladder carcinoma. Methods 10 cases of implantation metastasis to abdominal incision after open operation for bladder carcinoma encountered from 1985 to 1999 were retrospectively studied,nine being male and 1 female.The average age was 52.7 years. Results These were 6 transitional cell carcinoma (all were grade G 3),2 transitional cell carcinoma with squamous carcinoma or adenocarcinoma,1 squamous carcinoma,and 1 adenocarcinoma.The diameter of the metastasis lesion was 1.4~11.5 cm?1.0~6.9 cm.The treatments included total cystectomy with resection of metastasis,TURBt with resection of metastasis,urinary diversion with chemotherapy and/or radiotherapy, resection of metastasis with chemotherapy and/or radiotherapy, and chemotherapy and/or radiotherapy alone.7 of the 10 died of the carcinoma,the survival times being 4~41 months (average 13 months).Only 1 patient survived for 41 months.Whereas the others died within 14 months.3 patients are still alive for 5~9 months with no recurrence. Conclusions The differentiation of the cancer cells of implantation metastasis is usually bad and often accompanied by squamous followed by carcinoma or adenocarcinoma.Total cystectomy with resection of the metastasis followed by chemotherapy and/or radiotherapy might prolong the survival time, but the prognosis is usually poor.
6.Treatment of calculus in ureter with Urocalun(report of 60 cases)

Objective To examine the efficacy and safety of Urocalun in treatment of calculus in ureter. Methods Sixty patients with calculus in the ureter were treated.The diameters of the stones were less than 1 cm.The patients were assigned to receive Urocalun orally,450 mg each time,3 times a day. Results Five weeks after Urocalun administration,the calculi were eliminated in 45 cases (75%).The calculi descended in 10 cases (17%).The positions of calculi remained unchanged in the rest 5 cases (8%).Four patients (7%) had mild uncomfortableness in stomach,nausea or thirsty after Urocalun administration. Conclusions Urocalun is effective for eliminating the calculus in ureters.
7.Plasma level of neutrophil extracellular traps in septic patients and its clinical significance: a prospective observational study

Objective To investigate the changes of neutrophil extracellular traps (NETs) level in plasma of sepsis patients and judge its clinical value for early diagnosing of sepsis.Methods A prospective observational study was conducted. The patients after surgery aged > 18 years and expected to stay in the ICU > 24 hours admitted to intensive care unit (ICU) of the First Affiliated Hospital of China Medical University from November 2014 to February 2015 were enrolled. According to the criteria of sepsis diagnosis in 1991, patients were divided into non-sepsis group and sepsis group. The healthy people who taken a physical examination were enrolled in the healthy control group. 3 mL peripheral venous blood was collected at 1 hour after admission to ICU. A fasting blood was collected in the healthy control group in the morning. The plasma free DNA (cf-DNA/NETs) was determined by using the fluorescence microplate reader, white blood cell (WBC), neutrophil ratio (NEU), procalcitonin (PCT), C-reactiveprotein (CRP) in peripheral blood of the patients were detected, and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) and sequential organ failure assessment (SOFA) scores were calculated. The correlation between plasma NETs and the risk factors in sepsis patients was analyzed by Spearman correlation analysis. The value of cf-DNA/NETs and WBC level in the diagnosis of sepsis was analyzed by using the receiver operating characteristic curve (ROC).Results Twenty-three sepsis patients, 20 non-sepsis patients, and 22 healthy persons were enrolled. There were no differences in baseline variables including gender and age among three groups, which indicated baseline data equalization. The plasma concentration of cf-DNA/NETs in sepsis group was significantly higher than that in non-sepsis group and healthy control group (μg/L: 453.44±185.37 vs. 188.35±29.66, 203.83±43.25, bothP < 0.05),and there was no significant difference between last two groups (P > 0.05). WBC, NEU, PCT, CRP, APACHE Ⅱ and SOFA score in sepsis group were significantly higher than those of non-sepsis group [WBC (×109/L): 9.52±5.51 vs. 5.97±2.28, NEU: 0.787±0.110 vs. 0.655±0.067, PCT (mg/L): 7.14 (3.60, 13.29) vs. 6.07 (3.57, 7.91), CRP (mg/L): 64.44±13.14 vs. 27.00±19.47, APACHE Ⅱ: 10.25±4.92 vs. 6.00±1.22, SOFA: 6.0±5.1 vs. 5.0±1.2, allP < 0.05]. Correlation analysis showed that the level of NETs had no obvious correlation with gender, age, WBC, NEU, PCT, CRP, APACHE Ⅱ and SOFA scores (r value was 0.322, 0.262, 0.194, 0.312, 0.227, 0.454, 0.433, 0.333, respectively, allP > 0.05). The area under the ROC curve (AUC) of plasma cf-DNA/NETs for the diagnosis of sepsis was 0.981. When the cut-off valueof plasma cf-DNA/NETs was > 257.96μg/L, the sensitivity was 91.3%, specialty was 95.2%, and Youden index was 0.865. AUC of WBC in the diagnosis of sepsis was 0.663. When the cut-off value of WBC was > 6.0×109/L, the sensitivity was 78.3% and specificity was 25.0%.Conclusion The plasma cf-DNA/NETs levels increased significantly in sepsis patients. In the diagnosis of sepsis, plasma NETs levels had better advantages over WBC. NETs can be used as a biomarker for early diagnosis of sepsis.
8.Screening of Proteins Binding to HBsAg from Human Pancreas cDNA Library by Yeast Two-hybrid System

Objective To screen proteins in pancreas cDNA library interacting with HBsAg protein.Methods After being amplified and identified,the human pancreas cDNA library plasmid was transformed into yeast Y187.Thereafter,the bait plasmid(PGBKT7-HBsAg) was transformed into yeast cells AH109.Then,two different transformed yeast cells were mated.The diploid yeast cells were plated on synthetic dropout nutrient medium(SD/-Trp/-Leu/-His/-Ade) and that medium contained X-?-gal for selection two times.After plasmids of true positive blue colonies were extracted and transformed to E.coli,the results were analyzed by DNA sequencing and blast searching in GenBank database.Results Six proteins binding to HBsAg were screened,including aconitase(ACO1),CDK5RAP3,carboxypeptidase,etc.Conclusion Most of these proteins play significant roles in the evolution of 2 type diabetes mellitus,which may supply some new clues for exploring the pathogenesis of abnormality of lipid metabolism and tglycometabolism in the patients with chronic hepatitis virus infection.
9.Effects of liver glycogen storage on hepatic functional reserve during warm ischemia and reperfusion injury in rats

The changes in group L were the most severe. Conclusions(1)Prophylactic increase of hepatic glycogen storage could counteract the injury of hepatic functional reserve that occurs during warm ischemia and reperfusion. (2)The changes of hepatic functional reserve that occurs during warm ischemia reperfusion can be earlier and more sensitively reflected by ICGR_(15) and GLT than by the (routine) enzyme parameters of hepatic function.
10.Neutrophil extracellular traps and coagulation dysfunction in sepsis

Neutrophil extracellular traps (NETs) are net-like structure composed of DNA and nuclear proteins, which are produced by activated neutrophils under the circumstances of a variety of pathogens or drugs. As part of defensive mechanism, NETs have been proved to restrict the spread of pathogens and release of antimicrobial molecules. NETs can not only strengthen the adhesion between neutrophils and platelets, promote platelet mediated procoagulant reaction, but also lead to endothelial cell damage and coagulopathy in sepsis. In addition, NETs also plays an important role in pathophysiological processes of venous thrombosis. Therefore, NETs may become the biomarkers of evaluating coagulation dysfunction and potential therapy target in sepsis.
