1.A newly-designed temporary cardia stent for the treatment of achalasia:an experimental study in canine models

Objective To assess the efficacy and safety of a newly-designed temporary covered cardia stent for the treatment of achalasia in canine models and to investigate the histopathological changes at different points of follow-up time after the stent was removed. Methods The canine achalasia model was created by injecting benzyl-dimethyl-tetradecyl ammonium chloride (BAC) circumferentially into the lower esophageal sphincter (LES) of the dogs. Twenty-four dogs with achalasia were randomly and equally divided into two groups with 12 dogs in each group:control group (using routine esophageal stents) and study group (using newly-designed temporary covered cardia stents). Under fluoroscopic guidance stents were implanted in the esophagus and were taken away from the esophagus 4 days after stent insertion in experimental dogs of both groups. LES pressures and timed barium esophagography (TBE) were performed in all dogs before and immediately after the stenting procedure,as well as at one week,3 and 6 months after the stent was removed. Every three dogs were sacrificed each time at one week,3 and 6 months after the stent was removed. The esophageal cardia was excised and sent for pathological examination. Results All animals well tolerated the stent insertion / removal and the follow-up procedures. No severe complications such as esophageal perforation occurred. Comparison between two groups showed that stent migration occurrence was much lower in study group (n = 1) than that in control group (n = 5). The reduction of LES pressures in study group was more significant in comparison with control group (at 6-month follow-up,P = 0.027). The difference in barium column product (height ? width) between 0-min and 5-min TBE was statistically significant in study group (at 3-month follow-up,P = 0.009). Integrated analysis of multi-comparison for LES pressures among subgroups of each group revealed that the dogs in study group exhibited better outcomes than the dogs in control group. Both proliferating cell nuclear antigen (PCNA) and ?-smooth muscle actin (?-SMA) immuno-staining revealed that the inflammatory reaction reached its peak at 1-month follow-up. Trichrome staining indicated that the collagenous fiber proliferating index inclined to be stable at 3-month follow-up. Conclusion In treating achalasia in canine models the newly-designed temporary covered cardia stent is superior to the routine esophageal stent in respect of safety and efficacy.
2.Relationship between acceptance of disability and posttraumatic stress response in patients with brachial plexus injury

Objective To determine the relationship between acceptance of disability and posttraumatic stress response in patients with brachial plexus injury. Methods Total 160 patients with brachial plexus injury were recruited and investigated with the Impact of Event Scale-Revised (IES-R) and the Acceptance of Disability Scale (ADS). Results The total score of ADS was (79.07±11.99) which showed medium level of acceptance of disability. The total score of IES-R was 4-66 (33.51±14.41), which showed that most of the patients suffered from posttranmatic stress response. Significantly negative correlation was found between acceptance of disability and posttraumatie stress response(r=-0.480, P<0.001). Conclusions Nurses should pay more attention to the acceptance of disability and posttranmatic stress response in patients with brachial plexus injury, and provide appropriate health education and effective psychological intervention to improve the patients' mental health and quality of llfe.
3.Analysis of pathogens resistant bacterial infection of the biliary 2011-2013

Objective To understand the main pathogenic bacteria distribution and drug susceptibility in the common bile duct calculi with infection ,so as to provide the basis for rational use of antimicrobial agents .Methods Data′s of 296 cases of bile specimens treated from January 2011 to December 2013 were retrospectively surveyed and drug resistance of bacteria culture results were analyzed .Results Among all the 296 cases ,there were 199 cases of pathogenic bacteria in bile specimens ,the positive rate was 67 .23% ;among them ,there were 21 cases of two kinds of bacteria ,a total of 220 strains of pathogenic bacteria were detected ,and among them there were 158 strains of gram negative bacteria ,accounting for 71 .82% ;there were 46 strains of gram positive bacte‐ria ,accounting for 20 .91% ;there were 16 strains of fungi ,accounting for 7 .27% ;the lowest resistance of gram negative bacilli lies in amikacin ,followed by tazobactam ,aztreonam ,three to four generations of cephalosporins and quinolone .Gram negative bacilli has the highest drug resistance to pseudomonas aeruginosa and e .coli ,and it has a drug resistant rate higher than 50 .00% to most anti‐microbial drugs .Gram positive cocci has 0 drug resistance rate to add ring element ,has a drug resistance rate lower than 30 .00% to vancomycin and rina thiazole amine ,and has highest drug resistance rate (70 .00% -100 .00% ) to erythromycin ,clindamycin and chlorine lincomycin .Conclusion Among all the 296 cases of common bile duct calculi bile with biliary tract infection ,the main path‐ogenic bacteria were e .coli ,klebsiella pneumoniae ,eosinophilic malt narrow and excrement enterococcus bacterium ;and the drug re‐sistance to penicillin is highest .
4.63 cases of liver surgical site infection related factors analysis

Objective To liver surgical site infection related influencing factors were discussed.Methods Retrospective analysis the clinical data of A total of 1 187 patients with liver and gallbladder surgery,using SPSS12.0 software for statistical analysis of data.Results A total of 1 187 cases of liver and gallbladder surgery patients and 63 cases of surgical site infection occurred,the in-fection rate of 5.31%,including incision shallow tissue 51 cases,8 incision deep tissue,organ lacuna in 4 cases,accounted for 80.95%,12.70% and 6.35% respectively.Isolated pathogenic bacteria distribution and composition ratios were as follow,15 strains of gram-negative bacteria(55.56%),12 gram positive bacteria strains(44.44%),including gram-negative bacteria in e.coli, gram-positive bacterium with staphylococcus aureus detection most(25.93% and 14.81% respectively),not isolated fungus;Age, presence of diabetes mellitus,drainage time,postoperative complications,operation time and open operation for independent risk fac-tors for surgical site infection (P <0.05).Conclusion Liver and gallbladder surgery in patients with surgical site infection is asso-ciated with a variety of factors,should strengthen the preoperative patients with postoperative condition evaluation,improve the lev-el of operation at the same time,thereby reducing surgical site infection.

Objective To explore the effects of ethyl pyruvate (EP) on high mobility group box-1 protein (HMGB1) expression in severe acute pancreatitis (SAP) rats.Methods Ninety male wistar rats were divided randomly into three groups: Group A (SAP group); group B (SAP rats received ethyl pyruvate therapy); group C (control group). Specimens from rats in the three groups were taken at 3, 6, 12, 24 and 48 h after operation respectively. Objective To compare the skills level before and after pediatric advanced life support course and analyze the effect of the training. Methods The pediatric advanced life support was used as the textbook. The skills were got through attending theory classes, watching demonstrations and taking part in the simulator training. The questionnaires were filled strictly and the data was analysed. Results The test scores were increased after the training (P<0. 01). There were only 8.7% of the trainees had used the rescue equipments and 61.3% had never seen the rescue equipments before training. More than 80% of the trainees were satisfied with the training about the utility and novelty. Conclusion pediatric advanced life support course can successfully deliver a large number of healthcare providers with international unique pediatric emergency treatment skills ,and raise the participants abilities of rescuing critical children.

Objective To discuss the technical points, safety and clinical effectiveness of DSA-guided percutaneous acetabuloplasty (PA) for the treatment of acetabular metastases. Methods Fifteen patients, including 6 males and 9 females, with acetabular malignant metastases were enrolled in this study.A total of 19 lesions were detected. The lesions were 13 - 25 mm in size (mean 19 ± 4 mm), the dose of PMMA used for per lesion was 5 - 13 ml (mean 8.2 ± 2.3 ml). All patients complained of greater or less degree of pain in their hips, 8 patients had to use walking stick, 4 patients showed limping although they could walk independently and three patients could not walk alone. DSA-guided percutaneous acetabuloplasty was performed in all patients and follow-up after the procedure was conducted for 1 - 12 months. Visual analogue score (VAS), walking state score and analgesic dosage taken by the patients were used for the evaluation of the clinical effectiveness. The complications were analyzed. All the data obtained were statistically analyzed with paired samples t test and analysis of variance by using SPSS12.0 statistical software.Results The procedure was technically successful in all patients. Pain rating evaluated by the VAS decreased from a mean of 7.8 before surgery to a mean of 4.2 in 24 hours after surgery (P < 0.01 ), which further decreased to 2.5 in one month (P < 0.01 ), while walking state score increased from a mean of 1.5before surgery to a mean of 2.5 in 24 hours after surgery (P < 0.01 ). The analgesic dosage taken by the patient was reduced in 14 patients and remained the same in one patient. The bone cement leakage into paraacetabular soft tissues occurred in three cases and peripheral vascular exudation of acetabulum was observed in three cases with no obvious clinical symptoms. Conclusion As a safe, reliable and minimally-invasive technique, DSA-guided percutaneous acetabuloplasty has excellent anti-pain effect in treating acetabular metastases. This therapy can markedlyimprove the patient's walking ability and the quality of life.

Objective To screen proteins from human pancreas cDNA library,which interact with hepatitis C virus(HCV)E1 protein.Methods The human pancreas cDNA library was amplified,purified and evaluated,and then the purified library plasmids were transformed into yeast strain Y187.The reconstructed plasmid pGBKT7-E1 was transformed into yeast strain AH109 and screened on the nutrient deficiency medium SD/-Trp.The transformed AH109 mated with Y187 that contained the library plasmids.The diploid yeast cells were plated on nutrient deficiency medium SD/-Trp/-Leu/-His/-Ade and SD/-Trp/-Leu/-His/-Ade containing X-α-gal for selecting.The plasmids in diploid yeast cells were extracted and electrotransformed into E.coli DH5α.The plasmids in DH5α were extracted,sequenced and blasted.Result Sixteen proteins interacting with HCV E1 were found.Conclusion Some of the sixteen pancreatic proteins may be related with metabolisms of glucose and lipid.
10.Research of the correlation between pelvis asymmetry and Crowe classification of unilateral developmental dysplasia of the hip in adults

Objective To investigate the relationship between pelvic asymmetry and Crowe classification of unilateral developmental dysplasia of the hip in adults. Methods According to the inclusion criteria, 100 cases of unilateral DDH in adults were collected, including 78 females and 22 males, with an average age of 56.3 years old (range, 21-79 years). In terms of Crowe classification, there were 30 cases of Crowe typeⅠ, 24 cases of Crowe typeⅡ, 23 cases of Crowe typeⅢ, and 23 cases of CroweⅣ. The normal?side and affected?side pelvic vertical height (the distance from the highest point of the ischial tuberosity to the iliac crest peak) were measured via the radiogram of anteroposterior pelvis. Furthermore, we compared the affected?side pelvic vertical height with the normal?side and analyzed the relationship between Crowe classification and the difference in pelvic verti?cal height. Results The average pelvic vertical height was 23.38±1.46 cm in the normal?side and 22.41±1.38 cm in the affected?side. The average D?value of pelvic vertical height between two sides is 0.98 ± 0.76 cm. From Crowe typeⅠto Ⅳ, the average heights of the normal?side were 22.89±1.09 cm, 23.23±2.07 cm, 23.75±1.16 cm, 23.79±1.22 cm, while those of the affected?side were 22.70 ± 0.98 cm, 22.41 ± 1.98 cm, 22.47 ± 1.12 cm and 21.92 ± 1.32 cm. The average D?values in each group were 0.19 ± 0.37 cm, 0.82±0.43 cm, 1.28±0.32 cm, 1.87±0.59 cm. The differences between the normal?side and the affected?side in each type were statistically significant. The average D?value increased significantly with Crowe type increased, and there was a positive correla?tion between the D?value of bilateral pelvic vertical height and the degree of hip dislocation. Conclusion The pelvic asymmetry does exist in adult patients with unilateral developmental dysplasia of the hip. Moreover, the asymmetry has positive correlation with the degree of hip dislocation.
