1.Research on the Equity of Secondary Allocation for Hospital Performance Based on Gini Coefficient

Objective:To evaluate the equity of the secondary performance allocation in target hospital.Methods:Through the comparison on the Gini coefficient calculation,the calculation method of Gini coefficient developed by Jianhua Zhang was applied to estimate the secondary allocation result of performance salary in the target hospital from 2014 to 2015.Results:From 2014 to 2015,the Gini coefficient remained at 0.22 or so,the overall allocation was average.Conclusion:Estimating the Gini coefficient was beneficial for the hospital managers control the structure and level of the second performance distribution and modulate the program of the sencond performance distribution in time,so that the distribution of performance salary would be accordant to the designed program and acts as the direction of motivation.
2.GC-MS Analysis of the Liposoluble Constituents Extracted with Supercritical CO_2 Fluid Extraction from Aractylodes Mcrocephala

OBJECTIVE:To analysis and identify the liposoluble constituents extracted with supercritical CO 2 fluid ex-traction(SFE)from atractylodes macrocephala by gas chromatography-mass spectrometry(GC/MS).METHODS:Volatile oil was extracted from atractylodes macrocephala,to analsis and identify the chemical components in liposoluble constituents with GC/MS,DB-5MS capillary column was the chromatographic column and helium was carrier gas,the flow rate was 2.0ml/min,and the column temperature was raised from 70℃ to 250℃ at an increasing rate of 10℃/min.EI ion sources were adopted in mass spectra.RESULTS:A total of 19 chromatographic peaks were separated and identified,most of which were unsatu-rated fatty acids and esters,accounting for 96.2% of the total peak area.CONCLUSION:Using GC/MS,the chemical com-position of the liposoluble constituents in atractylodes macrocephala can be analyzed,which provides references for the further exploitation and utilization of atractylodes macrocephala.
3.Therapeutic analysis of severely contracture of the first web space with mocrosurgical operation

Objective To investigate the methods and the therapeutic effects of groin flap and stylus recurrent branch of radial artery flap for the repair of severely contracture of the first web space. Methods From March 2007 to six 2009,45 patients with severely contracture of the first web space received treatment of groin flaps and stylus recurrent branch of radial artery flaps, with 6 to 12 months clinical observation. Among then, 33 patients received treatment of groin flaps, 12 patients received treatment of stylus recurrent branch of radial artery flaps, and 28 patients received the first web space widening as well as functional reconstruction of thumb abduction. Results Most of the flaps healed by first intention but also with distal necrosis for 3 stylus recurrent branch of radial artery flaps and 1 groin flap. And the weth of the first web space of all patients were more than 90% of comparison. From clinical observation,function of opposition recovered well. While some groinflaps got a litte fat and clumsy .The color of most flaps was as similar as normal besides, some groin flaps occurrenced pigmentation. Conclusion Children and adults below 50 yesrs should choose the groin flap, and aged people and adults more than 50yesrs shuould choose the stylus recurrent branch of radial artery flap firstly.
4.The accuracy of Computed Tomography Angiography (CTA) in diagnosis of multiple intracranial aneurysms

Objective The purpose of this study is to explore the clinical value of CT angiography( CTA) in the diagnosis of multiple intracranial aneurysms.Methods The data of CTA and DSA from 74 patients with multiple intracra-nial aneurysms from July 2011 to March 2015 were reviewed retrospectively.Results One hundred seventy-seven aneu-rysms were detected by DSA, and 175 aneurysms were detected by CTA.Among the aneurysms identified by CTA, 4 aneu-rysms were false positive and the correct detection rate of CTA was 96.6%.One hundred sixty-five aneurysms identified by CTA were confirmed by DSA and the correct diagnostic rate of CTA was 96.5%.CTA failed to detect 6 aneurysms and mis-diagnosed 10 aneurysms.Conclusions The correct detection rate and diagnostic rate of CT angiography ( CTA) in multiple intracranial aneurysms is relatively high.But previous surgery, spasm of the vessels, the size and number of aneurysms, radiologists'experience can influence the accuracy of ( CTA) in the diagnosis of multiple intracranial aneurysms, indicating that we should combine CTA with DSA to avoid the misdiagnosis and missed diagnosis.
5.Analysis of Correlation between Obstructive Sleep Apnea Hypopnea Syndrome and Hypertension and Risk Fac to rs

Objective : To investigate the correlation between obstructive sleep apnea hypopnea syndrome (OSAHS ) and hypertension and its risk factors ,so as to provide basis for prevention and treatment of OSAHS accompanied by hypertension . Methods : A total of 108 OSAHS patients were enrolled and the general conditions were recorded . The blood pressure at 22 ∶ 00 before sleep and that at 8 ∶ 00 . next morning was determined routinely ,and the blood lipid level was detected . According to apnea hypoapnea index (AHI) ,the patients were divided into three groups ,the mild group (5/h ≤ AHI < 15/h) with 31 cases ,the moderate group (15/h ≤ AHI < 40/h) with 36 cases ,and the severe group (AHI ≥ 40 /h) with 41 cases . The levels of blood pressure were compared among the groups . According to whether OSAHS was accompanied by hypertension ,the patients were divided into simple OSAHS group and OSAHS accompanied by hypertension group . The relevant factors associated with OSAHS accompanied by hypertension were screened out by single factor analysis ,and then the multiple Logistic regression analysis was conducted .Results : (1)Among 108 patients with OSAHS ,60 cases(55 .6% ) were accompanied by hypertension ,including 15 cases of mild group(48 .4% ,15/31) ,19 cases of moderate group (52 .8% ,19/36) ,and 26 cases of severe group (63 .4% ,26/41 ) . There was no significant difference regarding the prevalence rate of hypertension among the three groups (P > 0 .05) .There were significant differences regarding systolic blood pressure(SBP) and diastolic blood pressure (DBP) at 22 ∶ 00 and 8 ∶ 00 among the three groups (P < 0 .05) ;SBP and DBP at 22 ∶ 00 and 8 ∶ 00 in severe group were significantly higher than those in mild group (P< 0 .05) .SBP at 22 ∶ 00 and 8 ∶ 00 in severe group was significantly higher than that in moderate group (P< 0 .05) .(2) Single factor analysis showed that age ,body mass index (BMI) ,smoking rate ,abdominal circumference ,AHI ,total cholesterol(TC) ,triglyceride(TG ) and low density lipoprotein cholesterol (LDL-C) in OSAHS accompanied by hypertension group were significantly higher than those in simple OSAHS group ,while HDL-C was significantly lower than that in simple OSAHS group ( P < 0 .05 ) . (3 ) Multivariate Logistic regression analysis showed that BMI ,AHI ,TC were the risk factors for OSAHS accompanied by hypertension .Conclusions :The level of blood pressure in patients with OSAHS was accordance with the disease severity ,while BMI ,AHI ,TC were the risk factors for OSAHS accompanied by hypertension .
6.An experimental study on TRIM21 promoting M1 polarization of macrophages to aggravate chronic apical periodontitis

Objective:To explore the role of tripartite motif-containing protein 21(TRIM21)in chronic apical periodontitis(CAP)and its potential mechanism.Methods:Human CAP tissue and normal periodontal tissue were collected.The expression of inflamma-tory factors(IL-1 β,IL-6,TNF-a,TGF-β1),osteoclast related genes(TRAP,RANKL,CTSK),macrophage polarization related genes(CD86,iNOS,CD206,Arg1)and TRIM21 were detected by RT-qPCR.TRIM21 protein was detected by immunohistochemical staining,and the osteoclasts was detected by tartrate resistant acid phosphatase(TRAP)staining.The inflammation cell model was es-tablished by stimulating Raw264.7 cells with lipoteichoic acid(LTA)or lipopolysaccharide(LPS),and the expression of the above factors was detected.The bone marrow-derived macrophages(BMDMs)extracted from wild-type and TRIM21-/-mice stimulated by LPS were used to verify the expression of the above factors by RT-qPCR,the osteoclasts were detected by TRAP staining,and the po-larization of macrophages was detected by immunofluorescence staining.Results:In CAP tissue the expression of inflammatory factors,osteoclast related genes,CD86,iNOS and TRIM21 increased,while CD206 and Arg1 decreased,and osteoclasts were more than that in normal tissue.The stimulation of LTA/LPS promoted the proliferation of Raw264.7 cells,and the expression of these factors in cells was consistent with that in tissues.After LPS stimulation,BMDMs of TRIM21-/-mice had lighter inflammation,lower expression of os-teoclast specific genes,fewer osteoclasts and lower M1 polarization than those of wild type mice.Conclusion:TRIM21 might promote the progress of CAP by promoting M1 polarization of macrophages.

AIM:To investigate the clinical fea-tures of acute exacerbation chronic obstructive pul-monary disease(AECOPD)of complicated with type 2 diabetes mellitus(T2DM),and analyze the relat-ed clinical features and risk factors.METHODS:This was a single-center cross-sectional study.From March 2020 to January 2023,479 hospitalized pa-tients with AECOPD in the department of respirato-ry and critical care medicine,Suining Central Hospi-tal were included.There were 215 patients in AE-COPD group and 60 patients in AECOPD with T2DM group.The collected variables included demograph-ic data,complications,blood routine,infection in-dex,random blood glucose,blood gas analysis and lung function.The adoption rate and constituent ra-tio of the basic description classification data were expressed as mean standard deviation for the nor-mal distribution measurement data and median in-terquartile range for the skew distribution measure-ment data.T-test was used for normal distribution and non-parameter test was used for non-normal distribution.The categorical variables were tested by chi-square test.Rank sum test was used for rank variable data.Binary logistic regression model was used to investigate the independent factors associ-ated with T2DM in patients with AECOPD.Finally,the results of logistic regression were verified and visualized by nomogram,validation curve,ROC curve and DCA curve.P<0.05 was a significant sta-tistical difference.RESULTS:Univariate analysis showed that there were significant differences in body mass index(BMI),essential hypertension,cor-onary heart disease,atrial fibrillation(AF),pulmo-nary function(GOLD stage),blood neutrophil(NS),blood lymphocyte(LYM),arterial blood gas PaCO2,Alanine transaminase(ALT)and random blood glu-cose(RBG)between the two groups(P<0.05).The binary logistic regression model(C-index=0.847)was constructed with the above 10 variables,the results showed that BMI(OR=1.309),Af(OR=8.188),LYM counts(OR=0.474),PaCO2(OR=1.082)and RBG(OR=1.434)were independently associat-ed with type 2 diabetes in patients with AECOPD(all P>0.05).The results of logistic regression were verified and visualized by Nomogram and its-associ-ated ccurves.The MAE and AUC curves were 0.021 and 0.847 respectively,indicating that the model had good prediction consistency and accuracy.The DCA curve showed that Nomogram's risk threshold ranged from 0.01 to 0.99,suggesting that nomo-gram's model had better clinical predictive value.CONCLUSION:Our results showed that increased BMI,PaCO2 and random glucose,decreased blood lymphocyte,and atrial fibrillation is an indepen-dent clinical feature of AECOPD with T2DM.These results suggest that the immune function of pa-tients with AECOPD and T2DM are more severely impaired and more likely to be accompanied by atrial fibrillation,which is a potential cause of poor prognosis in these patients.Meanwhile,this conclu-sion needs to be further verified in multicenter study with large sample size.
