1.Comparison of outcomes between phacoemulsification-trabeculectomy and ECCE-trabeculectomy for cataract combined with glaucoma

Background Phacoemulsification (Phaco) and extracapsular cataract extraction (ECCE) with trabeculectomy are primary approachs for cataract combined with glaucoma.To compare the efficiency and safety between the two methods is important for the choice of surgeries.Objective This study was to compare the clinical outcomes between ECCE combined with trabeculectomy and Phaco combined with trabeculectomy.Methods A randomized controlled study was designed.Sixty-three eyes of 63 patients with cataract and glaucoma were included from January 2013 to February 2014 in Xinjiang Autonomous District Traditional Chinese Medicine Hospital under the informed consent.The patients were assigned to parallel two groups according to random number table.ECCE+intraocular lens (IOL) implantation with trabeculectomy was performed in 33 eyes of 33 patients in the ECCEteabelectomy group,and Phaco+IOL implantation with trabeculetomy was carried out in 30 eyes of 30 patients in the Phaco-trabeculectomy group.The patients were followed-up for six months.Visual acuity,intraocular pressure (IOP),astigmatic degree and complication after surgery were compared between the two groups.Results The visual acuity was obviously improved after surgery in both groups.The distribution of eyes in different vision acuities was significantly different after operation between the two groups (H =0.125,P =0.032).The IOP was (14.13 ±5.19) mmHg in the Phaco-trabeculectomy group and (15.18 ± 6.04) mmHg in the ECCE-trabeculectomy group,without significantly interclass difference among the two groups (F=3.762,P > 0.05),however,the IOP were lower after surgery (Ftime =14.991,P < 0.05).The postoperative astigmatic power was (1.02-± 0.44) D in the Phacotrabeculectomy group and (3.76 ± 1.53) D in the ECCE-trabeculectomy group,showing a significant reduce in the Phaco-trabeculectomy group (t =3.089,P =0.034).The incidence rates of postoperative complications were 6.67% and 18.18% in the Phaco-trabeculectomy group and ECCE-trabeculectomy group,respectively,with a significant difference between them (x2 =6.112,P<0.05).Conclusions Compared to the ECCE+IOL implantation with trabeculectomy,Phaco + IOL implantation with trabeculectomy for cataract combined with glaucoma shows better clinical effectiveness,less complications and lower astigmatic power.
2.Effect of saikogenin d on prostaglandin E_2 (PGE_2) production in vitro in C_6 rat glioma cells

Aim To study the effect of saikogenin d (SGD) on prostaglandin E 2(PGE 2) production in C 6 rat glioma cells. Methods Radioimmunoassay was applied to determine PGE 2 production in the cells. Scintillation counting was used to measure liberation of -arachidonic acid (AA) from the cells labeled with -AA. Results In vitro, SGD alone at 1~20 ?mol?L -1 did not affect PGE 2 release from cells, but inhibited its release induced by A23187, a Ca 2+ ionophore. The inhibition was concentration-dependent, with the IC 50 value of about 3 ?mol?L -1. SGD (2~10 ?mol?L -1 ) had no inhibitory effect on A23187-induced AA release or on the conversion of AA to PGE 2 in microsomal preparations. Conclusion SGD inhibites A23187-induced PGE 2 production in C 6 rat glioma cells in vitro, without either inhibition of free AA liberation or a direct inhibition of cyclooxygenase (COX) activity.
3.Therapeutic analysis of severely contracture of the first web space with mocrosurgical operation

Objective To investigate the methods and the therapeutic effects of groin flap and stylus recurrent branch of radial artery flap for the repair of severely contracture of the first web space. Methods From March 2007 to six 2009,45 patients with severely contracture of the first web space received treatment of groin flaps and stylus recurrent branch of radial artery flaps, with 6 to 12 months clinical observation. Among then, 33 patients received treatment of groin flaps, 12 patients received treatment of stylus recurrent branch of radial artery flaps, and 28 patients received the first web space widening as well as functional reconstruction of thumb abduction. Results Most of the flaps healed by first intention but also with distal necrosis for 3 stylus recurrent branch of radial artery flaps and 1 groin flap. And the weth of the first web space of all patients were more than 90% of comparison. From clinical observation,function of opposition recovered well. While some groinflaps got a litte fat and clumsy .The color of most flaps was as similar as normal besides, some groin flaps occurrenced pigmentation. Conclusion Children and adults below 50 yesrs should choose the groin flap, and aged people and adults more than 50yesrs shuould choose the stylus recurrent branch of radial artery flap firstly.
4.Role of micro-neurosurgery training in the cultivation of specialty degree neurosurgery post-graduates

Taking microneurosurgery approach and applied surgical anatomy training as the core and combining theoretical teaching and perioperative training as the main contents , training program achieved significant effect among specialty degree neurosurgery postgraduates. In order to further improve the quality of training, it is proposed to set up micro-neurosurgery training center and more complete train-ing system based on micro-neurosurgery contents thus to improve clinical ability of specialty degree neuro-surgery postgraduates.
