1.Effects of soybean protein and animal protein on renal function of patients with diabetic nephropathy: a Meta-analysis

Objective To evaluate the effects of soybean protein and animal protein on renal function of patients with diabetic nephropathy.Methods In this Meta-analysis,literatures on the effects of soybean protein and animal protein on renal function of patients with diabetic nephropathy published during January 1980 and November 2011 were reviewed and analyzed quantitatively and synthetically by using Metaanalysis RevMan 5.0 software.Results Five case-control studies met the inclusive criteria and entered into random effect model.In comparison with animal protein,soybean protein significantly improved endogenous creatinine clearance rate (weight mean difference (WMD) =-11.51,95% confidence interval (CI)-16.37 to 6.65),proteinuria (WMD =-0.96,95% CI-1.36 to-0.58),urine creatinine (WMD =-1.11,95% CI-1.91 to-0.30),glomerular filtration rate (WMD =0.28,95% CI-0.12 to 0.68),serum urea nitrogen (WMD =-0.20,95% CI-0.60 to 0.19) and serum creatinine (WMD =-0.22,95% CI-0.54 to 0.10).Conclusions Soy protein seems to have potential to improve urine creatinine clearance,urine creatinine,proteinuria and serum urea nitrogen.However,animal protein may contribute to increased glomerular filtration rate.
2.The significance of detection of high sensitive troponin T and creatine kinase isoenzyme in diagnosis of pediatric myocarditis

Objective To learn the significance of detection of high sensitive troponin T and creatine kinase isoenzyme in diagnosis of pediatric myocarditis.To provide reliable laboratory diagnosis method for the disease.Methods 23 cases of pediatric myocarditis,28 cases of viral myocarditis with capillary bronchitis and 61.cases of myocarditis with neonatal pneumonia were selected as the research objects;and 48 cases of healthy control group,55 cases of capillary bronchitis and 49 cases of neonatal pneumonia were also selected.Blood samples were collected from all the patients and healthy controls,and the levels of high sensitive serum troponin T and creatine kinase isoenzyme were also measured.Results There was no significant difference in the detection results of high sensitive troponin T and creatine kinase isoenzyme between the healthy control group,children with capillary bronchitis and neonatal pneumonia (all P > 0.05);high sensitive troponin T and creatine kinase isoenzyme detection results of myocarditis,myocarditis complicated with bronchiolitis,myocarditis complicated with neonatal pneumonia were higher than those in healthy control group,the differences were statistically significant (t =13.723,6.628,10.079,9.475,17.650,15.364,all P < 0.05).The abnormal rates of combined detection of children with myocarditis,myocarditis combined with capillary bronchitis,myocarditis combined with neonatal pneumonia were higher than those of single detection of high sensitive troponin T and single detection of creatine kinase isoenzyme (x2 =7.426,6.310,6.720,4.308,4.381,6.900,all P <0.05).The high sensitive troponin T and creatine kinase isoenzyme in the children with the age of 1-12 months and 1-3 were lower than those with the age of < 1 month,the differences were statistically significant (t =3.498,4.043,4.202,4.132,all P < 0.05).Conclusion The simultaneous detection of high sensitive troponin T and creatine kinase isoenzyme can be used in the diagnosis of pediatric myocarditis,with good clinical application value.
3.Analysis on the relationship between the human EV71 associated HFMD children's clinical manifestation and the genetic polymorphism and serum concentration of mannan binding lectin

Objective To explore the relationship between the genetic polymorphism and serum concentration of mannan binding lectin (MBL)and the clinical manifestation of the hand-foot-mouth disease (HFMD) children infection by human enterovirus 71 (HEV71).Methods One hundred and thirty-eight children diagnosed as HFMD infected by HEV71 (including 80 mild cases and 58 severe cases) and 40 healthy,symptom-free children were investigated.The concentrations of serum MBL were measured in 40 healthy controls,80 mild HFMD cases and 56 severe HFMD cases at both acute and convalescent phases by a sandwich enzyme immunoassay with a human MBL ELISA kit.And the genomic DNA of all cases were extracted from blood according to standard phenol-chloroform procedure.Six SNPs in the MBL gene(-550G/C,-221G/C and +4C/T of the promoter,CGT52TGT,GGC54GAC,and GGA57GAA of the exon 1) were analyzed by a sequencing-based typing method.Results The MBL serum level of the severe HFMD circulatory respiratory failure group in acute phase was significantly increased compared with severe HFMD encephalitis group,the mild cases and the control,but in the convalescence phase it significantly decreased compared with them.The frequencis of type B/B mutation (+230 of the exon 1),type P/P mutation (+4C/T of the promoter),and type H/H mutation (-550G/C of the promoter) were a significant difference among mild group,severe group and the control(P=0.006,0.043,0.028,respectively).The frequencies of LYPB/LYPB genotype and HYPA/HYPA genotype were a significant difference among mild group,severe groupand the control (P=0.028,0.014,respectively).Conclusion Low MBL protein level as a result genetic polymorphism seems to be correlative with clinical manifestation of HFMD disease.The MBL gene mutation and low MBI.protein level may be used as one of the evaluation method of HFMD severeity.
4.Application of multi-adaptive filter based on radial basis function neural network for real-time somatosensory evoked potential monitoring

ObjectiveTo design multi-adaptive filter based on radial basis function (MAF-RBF) for efficiently extracting somatosensory evoked potential (SEP) in real-time SEP monitoring.MethodsWith the optimization of important parameters that influence the performance of radial basis function neural network,the performance of extracting SEP was compared to that of a multi-adaptive filter (MAF),which developed from the combination of well-developed adaptive noise canceller and adaptive signal enhancer.ResultsIn this simulation study,the outputs of MAF-RBF showed a similar waveform with SEP template signals,and a smoother waveform than the.output of MAF.ConclusionWith appropriate parameter values,MAF-RBFNN is able to extract the latency and amplitude of SEP from the extremely noisy background rapidly and reliably without averaging.
5.Application of adaptive noise canceller based on fixed-point algorithm for real-time somatosensory evoked potential monitoring

ObjectiveTo efficiently detect somsatosensory evoked potential (SEP) using field programmable gate array (FPGA) real-time system, fixed-point algorithm based adaptive noise canceller (ANC) was designed to improve signal to noise ratio (SNR). MethodsWith the optimization of important parameters that influence the performance of fixed-point algorithm ANC, the performance was compared to that of floating-point algorithm ANC which was isolated from the effect of quantization error. Results In the simulation study, the outputs of fixed-point-based ANC showed a little higher distortion from real SEP signals than that of floating-point algorithm ANC. In the optimal selection of μ value, fixed-point algorithm ANC could get as good results as floating-point algorithm. Conclusion With appropriate parameter values, fixed-point algorithm ANC is able to improve SNR of SEP as well as that of fixed-point algorithm ANC.
6.Advances in diabetic animal models and its application in the traditional Chinese medicine research.

The high and continuing soaring incidence of diabetes may become a huge obstacle to China's development. The antidiabetic drug development is one way to solve the problem. Animal model is a powerful tool for drug development. This paper compares and analyzes the three kinds of animal models for antidiabetic drug development in replicating principle, methods and characteristic, then summarized the application in the research of traditional Chinese medicine. At the same time, the analysis of the market, application and clinical advantages of hypoglycemic medicine from traditional Chinese medicine, is given in this paper, based on the literature analysis. From the point of the clinic advantage embodiment and new drug development, this paper will provide advisory and assistance support for the anti-diabetic fighting with traditional Chinese medicine.
7.Study on the Effect and Mechanism of Xuezhiping Capsule on Blood Lipid Levels of High Blood Lipid Model Golden Hamsters

OBJECTIVE:To study the effect and its possible mechanism of Xuezhiping capsule on blood lipid levels of high blood lipid model golden hamsters. METHODS:Golden hamsters were randomly divided into normal control group,model group, atorvastatin group (3 mg/kg),combination group (Xuezhiping capsule 1.3 g/kg+atorvastatin 1.5 mg/kg),Xuezhiping capsule low-dose,medium-dose,high-dose groups(Xuezhiping capsule 1.3,2.6,5.2 g/kg),10 in each group. Hamsters in normal control group received normal diet,the other groups were given high-fat diet to establish high blood lipid model. Then relevant drugs were intragastrically given 2 weeks later. Normal control group and model group were intragastrically given equal volume of distilled wa-ter,once a day,for 4 weeks. After last administration,blood lipid indexes(TG,TC,HDL-C,LDL-C,FFA),mRNA and protein expressions of lipid metabolism related genes (PPAR-α,CYP7A1,ACOX,SREBP-1c,ACC1) were detected. RESULTS:Com-pared with normal control group,serum contents of TC,TG,LDL-C,FFA in model group increased,HDL-C content decreased;PPAR-α,CYP7A1,ACOX mRNA and protein expressions were weakened,SREBP-1c,ACC1 mRNA and protein expressions were enhanced(P<0.05). Compared with model group,above-mentioned indexes were obviously improved in Xuezhiping capsule high-dose group,atorvastatin group and combination group (except for HDL-C in Xuezhiping capsule high-dose group)(P<0.05);TC,TG,FFA contents,and PPAR-α,CYP7A1,ACOX,ACC1 mRNA,and CYP7A1,SREBP-1c,ACC1 protein were obviously improved in Xuezhiping capsule medium-dose group(P<0.05);TC,TG contents,and PPAR-α mRNA,and CYP7A1, SREBP-1c protein were obviously improved in Xuezhiping capsule low-dose group (P<0.05). CONCLUSIONS:Xuezhiping cap-sule has a promising effect of lowering blood lipid,the mechanism may be related to activating PPAR-α and inhibiting SREBP-lc signaling pathway.
8.Application of magnifying chromoendoscopy in endoscopic mucosal resection of colorectal neoplasms

Objective To evaluate the clinical efficacy and safety of endoscopic mucosal resection (EMR) assisted with magnifying chromoendoscopy in treatment of colorectal neoplasms. Methods Patients who met criteria for EMR including appropriate flat or depressed type and sessile lesions were enrolled. The association of morphology of colorectal lesions with histopathology was observed and the accuracy of estimation of invasive depth by magnifying chromoendoscopy was evaluated. Results Ninety lesions of 81 patients were reseeted by EMR (25 being sessile and 65 being flat or depressed). The histological results revealed low-grade dysplasia (LGD) in 58 lesions, high-grade dysplasia (HGD) in 20 lesions, and adenocarcinoma in 12 lesions. The average size of lesions was (1.4±0.5) cm in HGD, (1.6±0.5) cm in cancer and (1.0±0.4) cm in LGD with no significant difference (P> 0.05). It was shown that the flat and depressed lesions were more likely to be HGD or cancer as compared to sessile lesions, but with no statistical difference [41.5 % (27/65)vs. 20.0% (5/25), P= 0.084]. Moreover, the lesion with central depression was more likely to be HGD or cancer as compared to those without depressed surface [51.0% (25/49) vs. 17.1 % (7/41), P<0.01)]. The accuracy of estimating invasive depth by magnifying chromoendoscopy was 97.8% (86/90). Complete resection was confirmed histologically in 95.8% (88/90) of all lesions. Conclusions Colorectal lesions of depressed and flat types with central depression are more likely to be malignant. Estimation of invasive depth of colorectal neoplasia by magnifying chromoendoscopy in EMR treatment makes it more effective and safer.
9.Preoperative evaluation of tumor invasion depth in rectal caner with three-dimensional endoscopic ultrasonography

Objective To evaluate the accuracy of preoperative three-dimensional endoscopic ultra-sonography (3D-EUS) in tumor invasion depth (T-staging) of rectal cancer. Methods From May 2007 to November 2007, 21 patients with rectal cancer diagnosed by eolonoscopy and biopsy, underwent 3D-EUS, 2D-EUS and MRI before surgery to predict T-staging. The results were assessed according to post-surgical pathological findings. Results The accuracy of 3D-EUS in T-staging was 85.71%, which was significantly higher than that of MRI (57.14%, P<0.05), and similar with that of 2D-EUS (76.19%, P>0.05). Conclusion More accurate T-staging could be achieved by 3D-EUS, which could be combined with MRI to detect lymph node metastasis, and provide more information for therapy decision in rectal cancer.
10.Efficacy of implantation of a modified temporary metallic stent for cardia achalasia

Objective To study the efficacy and safety of implantation of a modified temporary self-expanding metallic stent for cardia achalasia.Methods A total of 30 patients diagnosed as having cardia achalasia were randomly divided into 2 groups,group A (traditional stents) and group B (modified stents)(n =15 in each group).Two days after stent implantation at the cardia by endoscopy,stents were withdrawn with endoscope guided by X-ray.LES Pressure,X-ray images ( including the diameter of the most dilated part of esophageal and the most narrow part of cardia) and the symptoms of dysphagia were compared before and half year after the treatment.The width changes of the most narrow stenting part on the point of stenting and 2 days after removal were compared.The side effects and complications during the treatment were recorded.Results All thirty stents were successfully implanted and removed.Stent dislocation occured in 2 cases in group A,but none in group B.Dysphagia had significant improvement after the treatment in both groups ( P < 0.05 ),but the recurrence rate of group A ( 26.7% ) was significantly higher than group B (6.67% ) in 6 months ( P < 0.05 ).LES pressure and X-ray images of both groups significantly improved after treatment ( P < 0.001 ),and those of group B were superior to group A ( P < 0.05 ).There was no difference in adverse reaction between the two groups.No perforation occured in any group.Conclusion Self-expanding metallic stents is safe for patients with cardia achalasia,with implantation convenience,symptomatic improvement,low recurrence,and few complications or dislocation.
