Objective To investigate the indications,approaches,and essentials of cranio-orbital surgeries for orbital apextumors.Methods The clinical data of 48 patients with orbital apextumors treated by refined transcranial orbitotomy through superolateral approach were analyzed retrospectively,34 tumors were excised with orbitoptenonal approach,while 14 tumors were excised with orbltotrontal approach,orbitozygomatic approach,or zygomatic-ptefional approach.Results Of the 48 orbital apextumors,33 cases were totally resected,7 cases were subtotally resected,and 6 cases were partly resected.The other 2 cases were taken biopsies.The optic functions improved or retained in 40 cases,7 cases suffered from eye-moving disfunction or visual-acuity declines,and 1 case suffered from hemiparalysis.Conclusions Tumors in the posterior and apex part of the orbit,or tumors extended from the orbit to the cranial,can be well treated through transcranial approach.Appropriate methods may help to bring maximum convenience to exposure and excision of the lesion,and to conserve the functions.Unnecessary injuries could be avoided as well.

Objective To study the expression of GPC3 mRNA and GPC3 protein in ovarian carcinoma, and to investigate the clinical significance of their expression. Methods 35 specimens of ovarian carcinomas and 23 specimens of normal ovarian tissues were obtained from the patients undergoing operation during Jan. 2004 to Oct. 2005. The expressions of GPC3 mRNA in ovarian carcinomas and normal ovarian tissues were detected by real-time fluorescence quantitative PCR (RT-PCR). The clinical and pathological characteristics of ovarian carcinomas were analyzed. The expression of GPC3 protein was detected in 4 specimens of ovarian carcinomas and 3 specimens of normal ovarian tissues by Western bloting. Results The expression level of GPC3 mRNA in ovarian carcinomatous was down-regulated compared with that in normal ovarian tissues (P0.05). The expression of GPC3 protein was consistent with the expression of GPC3 mRNA. GPC3 protein was doubly or trebly expressed in normal ovarian tissues compared with ovarian carcinomas. Conclusion The present study suggests that GPC3 may be related to the occurrence and development of the ovarian carcinoma. The simultaneous determination of GPC3 and CA125 may increase the sensitivity of diagnosis for ovarian carcinoma.

Pemphigus is a severe chronic autoimmune mucocutaneous bullous disease. Glucocorticoids are considered as the first line of treatment for this disease. Dysfunctional uterine bleeding is also observed as a result of hypothalamic-pituitary-ovary axis dysfunction. This study reported one female patient with pemphigus vulgaris complicated with dysfunctional uterine bleeding upon systemic glucocorticoid usage. Before this disease was diagnosed, the patient experienced normal menstruation. The mechanism of dysfunctional uterine bleeding triggered by glucocorticoids is elucidated on the basis of case studies and literature review.

The digital X-ray imaging device was disinfected with trichloro-isocyanuric acid solution.The samples were taken with cotton swab pouring method before and 1,2,3 and 6 h after disinfection and the bacteriological examination was performed.Before disinfection the colony count of samples was all above the standard (＞ 10 cfu/cm2) with bacilli and gram-positive cocci predominantly.The highest count was detected in the X-ray photographic bed (58 cfu/cm2) and the lowest was in the exposure button(19 cfu/cm2).After disinfection the colony count in radiation suits still exceeded the standard,the colony count in diagnosis bed 2 h after disinfection,that in all parts of device 3 h after disinfection was off levels.The results indicate that the chlorine disinfectants can only achieve short-term effect in disinfection of digital X-ray imaging device.

Objective To evaluate the diagnostic value of hysterosalpingography in infertility caused by salpingian obstraction.Methods 892 cases of femiles infertility were reported in the group,ranged in age from 20 to 36 years and the mean age was 26 years.The hysterosalpingography was taken under fluoroscopic guidance during third to 7th day after menstruction,the radiographes were taken and analysed.The different segments and shapes of salpingian obstruction were correspondently divided into 3 levels and 3 degrees respectively according to the different therapeatic methods for infertility.Results There were total 776 cases with salpingian obstructional infertility,of them,both side involved in 582 cases,and one side in 194 cases,the total of abnormal fallopian were 1358 tubes including 1 st level 423,2nd level 561;3rd level 374,1 st degree 263,2nd degree 553 and 3rd degree 542.Conclusion Hysterosalpingography plays an important role in the therapy and diagnosis of the inferility caused by salpingian obstraction.

Objective To study the correlation between risk factors of CVD and cognitive deficit associated with cerebral infarction. Methods Cognitive function was tested in 101 cases of cerebral infarction using HDS-R at the end of 3 weeks and 6 months after onset of the stroke. Meanwhile, the data of risk factors and relative information of cerebral vascular disease were collected by using the scale of complications and inquiring the past history. The risk factors of CVD, such as neurological deficit, diabetes, hypertension, obesity, fever, heart diseases, smoking, overdrinking were compared between the cognitive deficit group and non-cognitive deficit group. Logistic regression analysis was adopted for data analysis. Results Of the 101 cases , 65 had cognitive deficit at the end of 3 weeks and 53 at the end of 6 months after onset of stroke. Objective To evaluate the therapeutic effects and prognostic factors of transanal local excision of rectal carcinoma. Methods We retrospectively analyzed 116 cases who underwent transanal lical excision for rectal carcinoma from 1995 to 2008 with the mented of Cox Regression analysis.Result The survival time of all the patients were from 14 to 160.5 months.median time was 58.5 months.Five-year overall survival rate was 72%.ten-year overall survival rate was 53%.There were 16 cases of local recurrence with a rate of 13.8%.In the univariate survival analysis:histopathology,depth of tumor invasion,radiotherapy,recurrence and metastases were the predictors of survival.In the Cox regression analysis:depth of invasion,recurrence and metastases were the independent prognostic taetors for surwval.For T_1 stage,its overall local recurrence rate was 6.3%,five-year overall survival rate was 93%,ten-year overall survival rate Was 85%:For T_2 stage,its overall local recurrence rate was 14.8%,five-year overallsurvival rate Was 63%.ten.year overall survival rate was 45%.For T_1 stage,there Was no local recurrence with radiotherapy:For T_2 stage,local recurrence rate Was 14.6%.From the surivival curve,there Was no difference betwecn the patient accepted radiotherapy or not(T_1:P=0.260,T_2,P=0.262).But for local recurrence,the differences Was significant(P=0.002). Conclusion The result of transanal local excision of rectal carcinoma is satisfactory with T_1 stage,but it is not suitable for T_2 stage tumors.

Objective To explore the feasibility,efficacy and safety of endoscopic submucosal dissection (ESD) for tumors originating from gastric muscularis propria.Methods A total of 20 patients with tumors originating from gastric muscularis propria confirmed by EUS and CT scan underwent ESD.Laparoscopic intervention was applied in 3 cases when difficulties in en bloc removal or perforation occurred.Results The mean ESD procedure time was 74.8 min (40-120 min),and the mean resected tumor size was 2.6 cm (1.5-3.5 cm).No severe complication occurred.Pathological findings were 17 cases of gastrointestinal stromal tumors and 3 cases of leiomyoma.Conclusion ESD is an effective method for treating the tumors originating from gastric muscularis propria,and laparoscopic intervention is necessary for en bloc resection in some cases.
