1.Influence of central aortic and pulse pressure in coronary Artery Disease

Objective To investigate the influence of central aortic pressure and pulse pressure in coronary Artery. Methods The research involed 173 patients who were suspected coronary heart disease (CHD) in the Fifth People′s Hospital of Chengdu city. The patients were divided into non-coronary disease and coronary disease groups. Patients were divided into one- vessel, two- vessels and three-vessels in coronary disease groups. The data were analyzed by using spass 17.0 software. Results SBP-C,DBP-C and PP-C had statistical significance compared with non-coronary disease (P < 0.05).SBP-C and PP-C had significant difference between single-branch and multiple-branch group(P < 0.05).Logistic multivariate analysis indicated that SBP-C [P = 0.019,OR = 1.207, 95%CI(1.032,1.412)]and PP-C [P = 0.023, OR = 1.166,95%CI (1.021,1.332)] had significant correlation. Conclusion Compared with non-coronary, SBP-C and PP-C were higher, while DBP-C was lower. The higher SBP-C and PP-C are,the worse coronary artery is. SBP-C and PP-C are predispose risk factors for multiple-vessels coronary disease.
2.Clinical analysis of 326 cases of choledocholithiasis treated with endoscopic sphincterotomy

Objective To investigate the therapeutic efficacy of endoscopic sphincterotomy (EST) for treatment of choledocholithiasis. Methods The clinical data of 326 patients receiving EST from July 1990 to January 2009 were retrospectively analyzed. Long-term follow-up was performed in 195 of the 326 patients. Results The common bile duct stones were removed in 301 cases (92. 33%).The occurring rate of complications after EST was 6.13% (20/326). Of the 20 patients, 10 had pancreatitis (3.07%), 7 hemorrhage (2. 14%), 4 cholangitis (1. 220%) and 1 duodenal perforation (0.09%). Only 1 patient died. The follow-up showed that 20 patients had reflux cholangitis and 18 recurrence of common bile duct stones. Conclusion Therapeutic endoscopy is safe and effective for treatment of choledocholithiasis.
3.Male urethral duplication:a two-case report and literature review

Objective To summarize the clinical features and treatment of male urethral duplication.Methods The clinical data of 2 cases treated in June 2011 and April 2014 were analyzed retrospectively.The first case was a 5-year-old boy presented with passages of urine from two orifices in the penis.The second case was a 15-year-old boy presented with dorsal chordee and a sinus on the dorsum of the penis.The patient had a small amount of watery discharge occasionally dripping out of the opening for 10 years.The 2 patients underwent retrograde urethrography, which revealed a complete duplicated urethra with the channel arising from the proximal prostatic urethra ( class ⅡA2 according to the classification of Effman) .The 2 patients underwent excision of the accessory urethra under general anesthesia.Results The pathology reports of the 2 cases were hyperplasia of squamous epithelium and urothelial mucosa.Pathological diagnosis was urethral duplication.The first case was followed up for 1 year with a satisfactory functional and cosmetic outcome.The second case was followed up for 6 months and no watery discharge noticed from the residual dorsal chordee.Conclusions Urethral duplication is a rare congenital anomality affecting mainly boys.Clinical presentation varies depending on the different anatomical patterns of the urethral anatomy. Surgical management must be evaluated for each different anatomical variation.
4.Pretreatment with docosahexaenoic acid enhances the protective effect of angiopoietin-1 on rat brain microvascular endothelial cells during oxygen glucose deprivation

Objective To evaluate the role of pretreatment with docosahexaenoic acid (DHA) in the protective effect of angiopoietin-1 (Ang-1) on rat brain microvascular endothelial cells (BMVECs) during oxygen glucose deprivation (OGD).Methods BMVECs were sub-cultured in vitro and divided with a random number table into 7 groups:normal control group,normal control + Ang-1 group,OGD group,OGD + Ang-1 group,OGD + DHA group,OGD + DHA + Ang-1 group,and OGD + DHA + GW9662 + Ang-1 group.The normal control and normal control + Ang-1 groups were cultured in DMEM containing serum,5 mmol/L glucose,and 1.25 mmol/L pyruvate;OGD groups were cultured in glucose-and serum-free DMEM.DHA (40 μmol/L) was added to OGD + DHA,OGD + DHA + Ang-1,and OGD + DHA + GW9662 + Ang-1 groups,and 5 μmol/LGW9662 [inhibitor of peroxisome proliferator-activated receptor gamma (PPAR-γ)] to OGD + DHA + GW9662 +Ang-1 group,before pretreatment for 1 hour in 5% CO2 and 95% air.After the pretreatment,Ang-1 (250 ng/ml)was added to normal control + Ang-1,OGD + Ang-1,OGD + DHA + Ang-1,and OGD + DHA + GW9662 +Ang-1 groups.The cells were cultured in 94% N2∶ 5% CO2∶ 1% O2 for 24 hours,except for normal control and normal control + Ang-1 groups,which were cultured in 5% CO2 and 95% air instead.The cell apoptosis rate was detected with flow cytometry,expressions of Bax,Bcl-2,caspase-3,Tie-1,Tie-2,pTie-2,pAkt,ZO-1proteins with Western blot.Results Compared with normal control group,the cell apoptosis rate in OGD,OGD + Ang-1,OGD + DHA,OGD + DHA + Ang-1,and OGD + DHA + GW9662 + Ang-1 groups were significantly increased (P =0.000,0.000,0.000,0.004,0.000);the expression levels of Bax,caspase-3,and Tie-1 were significantly enhanced (Bax:0.62 ± 0.03,0.38 ± 0.03,0.45 ± 0.03,0.26 ± 0.02,0.33 ± 0.02vs.0.16 ±0.01;caspase-3:0.76 ±0.05,0.42 ±0.04,0.52 ±0.02,0.32 ±0.02,0.40 ±0.02 vs.0.15 ±0.01;Tie-1:0.51 ±0.03,0.25 ±0.01,0.33 ±0.02,0.16±0.01,0.22±0.02 vs.0.12 ±0.01;all P=0.000);the expression levels of Bcl-2,Bcl-2/Bax,Tie-2,and pTie-2 were significantly decreased [Bcl-2:0.09±0.01,0.20±0.01,0.16±0.02,0.31±0.01,0.22±0.01 vs.0.34±0.01;Bcl-2/Bax:(14.93±1.86)%,(68.03±5.56)%,(36.93 ±2.22)%,(119.1 ±13.3)%,(64.23 ±6.07)% vs.(208.33 ±7.37)%;Tie-2:0.07±0.01,0.16±0.02,0.11 ±0.01,0.21±0.01,0.18±0.01 vs.0.26±0.01;pTie-2:0.05±0.01,0.15 ±0.01,0.07 ±0.01,0.22 ±0.02,0.16±0.01 vs.0.27 ±0.01;allP=0.000],in addition,pAkt and ZO-1 in OGD,OGD + Ang-1,OGD + DHA,and OGD + DHA + GW9662 +Ang-1 groups were also significantly reduced (pAkt:0.13 ±0.01,0.26 ±0.01,0.14 ±0.01,0.28 ±0.02vs.0.39±0.02;ZO-1:0.08±0.01,0.18±0.01,0.10±0.01,0.19±0.01vs.0.23±0.02;allP=0.000).Compared with the OGD group,OGD + Ang-1 and OGD + DHA + Ang-1 groups demonstrated significantly reduced cell apoptosis (both P =0.000).Compared with normal control + Ang-1 group,the expression levels of pTie-2,pAkt,and ZO-1 were significantly decreased in OGD + Ang-1 and OGD + DHA +Ang-1 groups (pTie-2:0.15 ±0.01,0.22 ±0.02 vs.0.52 ±0.02;pAkt:0.26 ±0.01,0.37 ±0.04 vs.0.67 ± 0.05;ZO-1:0.18 ± 0.01,0.24 ± 0.02 vs.0.39 ± 0.05;all P =0.000).Compared with OGD +Ang-1 group,OGD + DHA + Ang-1 group had significantly decreased cell apoptosis and expression levels of Bax,caspase-3 and Tie-1,and significantly increased expressions of Bcl-2,Bcl-2/Bax,Tie-2,pTie-2,pAkt,and ZO-1 (all P =0.000),while OGD + DHA + GW9662 + Ang-1 group showed no significant differences in these indexes (P =0.202,0.770,0.382,0.448,0.233,0.736,0.143,0.526,0.495,0.670).Conclusion Pretreatment with DHA may enhance the protective effect of Ang-1 on rat BMVECs under the condition of OGD,possibly via activating PPAR-γ,suppressing Tie-1 expression,and hence amplifying the activation of Tie-2.
5.Data analysis of mammography in spective for breast cancer in Shanxi cancer hospital

Objective To evaluate this disease' s incident trend for Shanxi province through hospital-based statistics. Methods Breast cancer data was obtained from the department of radiology in Shanxi cancer hospital for the period 2005-2007. Results Between 2005 and 2007, the numbers of women who accepted mammography rapidly increased from 533 to 1975. 62 women had been diagnosed as breast cancer in 2005, while this number had reached to 400 in 2007. The proportion of breast malignancies during this three year periods had statistically increased with 11.6 %, 17.8 %, and 20.3 % respectively (P<0.001). The mean age of breast cancer patients was older than benign disease patients (50 vs 42.8, P<0.001). The highest breast cancer age group was those aged 45 to 54 (37.6 %). Left breast cancer was higher than right side (53.4 % vs 46 %). Most histological type was invasive ductal carcinoma (78.95 %). The proportion of invasive lobular carcinoma and duetal carcinoma in situ were 8.7 % and 5.4 % respectively. Conclusion Breast cancer incidence rate has been increased in our city. These results are consistent with previous similar studies. Thus, it is essential to establish population cancer incidence registration and develop clinical data system. This will result in better understanding of cancer incidence trends and benefit in the evaluation of the effective cancer control measures.
6.Early effect of tonsillotomy on children′s snoring with tonsil hypertrophy

Objective To explore short-term effect of tonsillotomy on children′s obstructive sleep apnea hypopnea syndrome (OSAHS) with tonsil hypertrophy by Ellman double-frequency radio-frequency machine .Methods Totally 30 cases children′s OS-AHS with tonsil hypertrophy were had tonsillotomy by Ellman double-frequency radio-frequency machine .They were compared with 30 cases children′s OSAHS with tonsil hypertrophy by traditional tonsillectomy .Short-term effect of tonsillotomy were ana-lyzed by wound bleeding ,operation time ,postoperative pain ,difficulty eating ,PSG testing ,immune function ,growth and develop-ment .Results Postoperative symptom in 1 weeks and 6 months ,compared with the traditional tonsillectomy ,30 cases of OSAHS patients were found that there were less bleeding ,shorter operative time ,mild postoperative pain ,no difficulty in feeding ,improving sleep-disordered breathing ,no decline in immune function ,good growth and development .Conclusion Short-term effect of tonsillot-omy on children′s OSAHS with tonsil hypertrophy is good by Ellman double-frequency radio-frequency machine ,it is a minimally invasive ,safe and effective surgical method children OSA HS .
7.Urethroplasty in the distal penile segment using flap for the staged correction of severe hypospadias.

OBJECTIVETo discuss the applicatioen of flap for the urethroplasty in the distal penile segment in the staged correction of severe hypospadias.
METHODSFrom Oct. 2004 to Aug. 2014, 25 cases with severe hypospadias were treated by staged urethroplasty for urethra reconstruction. The urethral meatus were located at peroscrotum in 4 patinets, at scrotum in 8 patients and at perineum in 13 cases. At the first stage, the urethral plate was divided and chordee was corrected. Then tubularized transverse island flap was used to prefabricate partial distal urethra. The defective urethra was repaired by using the Thiersch-Duplay principle at the second stage.
RESULTSAll patients completed both stages of the operation. The follow-up duration was 6-72 months (average, 24 months). In the first-stage, the modified tabularized transverse preputial island flap was performed on 10 patients, whereas the modified preputial double-faced island flap was performed on the other 15 patients. All of the prefabricated partial distal urethras had no evidence of stenosis or scarring. The result of the second-stage procedure was a complete penis with integrated urethral. All patients were satisfied with cosmetic and functional results. Neither stricture nor diverticula was observed. A good urinary stream during the urination was achieved in 19(19/ 25, 76%) patients. Five cases (5/25, 25%) developed urethrocutaneous fistula and one cases developed meatal stenosis (1/25, 4%) after the second stage repair.
CONCLUSIONSStaged urethroplasty using flap is a good choice for severe hypospadias. The successful rate is relatively high with good cosmetic and functional results.

8.Effects of androgen on autoimmune ovary failure of mouse

Objective To identify the effects of androgen on mouse model of autoimmune ovary failure.MethodsFifty mice were randomly divided into five groups as control group,early therapeutic group and its model group,late therapeutic group and its model group.Autoimmune POF was induced in forty mice by Pzp3.Then,the animals were randomly divided into four groups: two were treated with androgen from the 15th and 20th day after immune.The control group was treated with distilled water.The therapeutic efficacy of each treatment was evaluated by the changes in sexual cycles of mice,the serum level of AzpAb,infiltration of cells positively expressing CD45 in the ovary as well as the percentage of growing follicles of the ovary.Results The sexual cycles,serum level of AzpAb,infiltration of CD45 cells in the ovary of each model group were more significant than those in control group;early therapeutic group was less than model group;and serum level of AzpAb in early therapeutic group was lower than that of late therapeutic group.The percentage of growing follicles in the ovary of each model group was lower than that of control group;early therapeutic group was higher than model group and late therapeutic group.Conclusion The use of androgen in mice with autoimmune POF may notably ameliorate the pathogenetic conditions of the disorder,and the treatment during prophase was apparently more effective than that during the advanced stage.
9.Clinical characteristics of low rectal carcinoma with preoperative 30 Gy radiation

Objective To observe the clinical pathological changes lower rectal carcinoma after preoperative radiation.Methods Thirty cases of rectal carcinoma underwent preoperative radiotherapy at a dose of 30 Gy.Gross size of the tumors was measured and histological changes were analyzed before and after radiation.Results The rates of complete regression,partial regression and no response to radiation of the tumors were 23.3%.53.3%and 23.3%.Significant tumor regression RCRG Ⅲ wag seen in 6 cages (20.0%) after radiotherapy.while partially tumor regression RCRG Ⅱ was seen in 15 cases (50.0%). Conclusion Preoperative radiation at a dose of 30 Gy on lower rectal carcinoma induces degeneration and necrosis of the tumor cells with complete or partial regression in most cases.Necrosis,fibrosis and thickening of vascular intima in the rectal cancer tissue after radiotherapy is more frequent than those without radiotherapy.It may be the potential reason for increased resection rate and sphincter saving after radiotherapy. It hag a great significance for sphincter-preservation surgery in this group of rectal carcinomas.

Here we describe a Syrian couple having recurrent pregnancy loss in the first trimester, fetal malformations, and/or neonatal death. The father had a balanced chromosomal translocation t(5;15), an sY125 microdeletion of locus b in the azoospermia factor (AZF) gene, and an MTHFR C677T homozygous polymorphism with normal phenotype. Interestingly, his healthy wife had another MTHFR A1298C homozygous polymorphism. The couple experienced two pregnancy losses and had two stillborn children with severe malformations due to partial trisomy of the short arm of chromosome 5. The couple does not have any living offspring after 10 years of marriage.
