Objective To analyze the effect of different procedures of alimentary reconstruction for the digestive and absorptive functions of patients after total gastrectomy. Methods From January 2012 to January 2013,there were 114 patients with gastric cancer,of which 58 patients received Roux-en-Y anastomosis were the control group,56 patients received P-type jejunal loop esophageal jejunal Roux-en-Y anas-tomosis were the observation group, the nutritional status and postoperative complications of two groups were compared. Results The level of RBC,HGB,TP and ALB of the observation group were significantly higher than that of the control group(P<0. 05),the incidence rate of in-traoperative complications, diarrhea and reflux esophagitis of the two groups had no significant differences (P>0. 05),but the incidence rate of postoperative dumping syndrome and abdominal distension of the observation group were significantly lower than that of the control group (P<0. 05). Conclusion The efficacy of P-type jejunal loop esophageal jejunal Roux-en-Y anastomosis for gastric surgery was superior to Roux-en-Y anastomosis.

Objective To study a new procedure of surgical repairment of rectourethral fistula.Method By the perineum approach one stage urethroplasty using island skin tubes from perineum and scrotum has been adopted after the resection of fistular urethra and repairment of the defect of the anterior wall of rectum to substitute the defect of urethra in 9 cases during 1992-1998.Result The 9 cases were all cured and have been followed-up for 2 6 years without recurrence of rectourethral fistula. Conclusion This procedure was regarded as an ideal technique for the closure of rectourethral fistula especially for those complicated with urethra stricture.

Objective To comprehensively analyse the transcriptional changes of genes and their biologic significance that occurred in the process of development of human fetal skin by using high-density oligonucleotide DNA array. Methods Samples of human fetal skin were obtained from aborted fetuese of 10W, 15W, 24W, 32W EGA (estimated gestational age) respectively. Total RNAs were isolated from of skin specimens of fetuses of different EGA, and mRNAs were purified and labeled with incorporation of fluorescent dUTP to prepare the hybridization probes by using reverse transcription polymerase chain reaction (RT-PCR). Approximately 21 329 human genes were spotted on a chemical-material-coated-glass plate in array. Results According to the hybridization results from oligonucleotide DNA microarray, gene expresion patterns and functions were analysed. Gene-chip disclosed a large scale of information in developmental human fetal skin, rendering a convenient way to investigate the temporal and spatial expressions of gene profile among skin cells. Many specific genes transcription expressed differently at different stages of development of fetal skin, suggesting their key roles in development, differentiation and regulation. Conclusions Microarray or DNA chip technology has revolutioned biological research by empowering to broaden the scope of collecting genomic information. Therefore, microarray-based study is able to reveal a substantial number of genes which might participate in embryogenesis and development of human skin. The present study demonstrated a previously unrecongnized role of gene expression in the control of human fetal skin growth and structure during its developmental stage. A complicated network of skin development process was fairly well characterized.

Objective To explore the mode of psychological and behavior intervention which suitable for convalescent cancer patients, and evaluate its clinical effects in order to provide empirical basis for nursing care of cancer patients. Methods havior intervention condition. All the patients were evaluated by General Self-Efficacy Scale (GSES),ProfiIe of Mood States-Short Form(POMS-SF) and Quality of Life Questionnaire-Core 30(QLQ-C30)before and after the intervention. Result Compared with the control group, the scores of self-efficacy and quality of life were significantly improved in the experimental group (P< 0.01),while the total score and factor scores of POMS-SF such as tension-anxiety (TA),anxiety-hostility(AH),fatigne-inertia (FI),and confusion-babelism(CB)were significantly decreased(P<0.05). Conclusion Psychological and behavior intervention is proved to be feasible and effective to improve the mental health status,self-efficacy and quality of life in convalescent cancer patients.

This study developed a novel approach of targeting malignant glioma with pMAGE-A1(278-286)-specific cytotoxic T lymphocytes (CTLs) induced from the peripheral blood mononuclear cells of healthy donors by multiple stimulations with human leukocyte antigen (HLA)-A2-restricted pMAGE-A1(278-286) peptide-pulsed dentritic cells. Cytotoxic assays were performed by the colorimetric CytoTox 96 assay to analyze cytotoxic activity of the induced CTLs against various target cells. The induced CTLs showed approximately 45% specific lysis against T2pMAGE-A1(278-286) (pMAGE-A1(278-286) peptide pulsed T2 cells) and U251 (HLA-A2(+), MAGE-A1(+)) at an effector:target ratio of 40:1, and approximately 5% cytolysis against T2pHIV, A172 (HLA-A2(-), MAGE-A1(+)), K562 and T2 cells without being pulsed with peptide at any effector:target ratio. The specific killing activity of the induced CTLs against T2pMAGE-A1(278-286) and U251 was much more obvious than in any other control group (P<0.05). The cytotoxic activity against the T2pMAGE-A1(278-286) and U251 was significantly eliminated by anti-HLA class I mAb W6/32. These results suggest that pMAGE-A1(278-286) epitope may serve as a surrogate tumor antigen target of specific immunotherapy for treating HLA-A2 patients with malignant glioma.

Objective:To assess the effect of rosiglitazone on inflammation in peritoneal dialysis patients. Methods:Fifty patients undergoing continuous ambulatory peritoneal dialysis were collected in our hospital. The treatment group was assigned to receive regular peritoneal dialysis and rosiglitazone 4mg once daily while the control group was received regular peritoneal dialysis for 12 weeks. Such serum examinations as fasting blood glucose (FBG), glycosylated hemoglobin A1c, haemoglobin, serum total cholesterol, high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol, triglycerides and high sensitivity C reactive protein (hs-CRP) were measured, tumor necrosis factorα(TNF-α) and interleukin-6 (IL-6) levels were measured by ELISA, and homeostasis model as-sessment of insulin resistance( HOMA-IR) was also evaluated before and after the treatment. Results:After the 12-week treatment by rosiglitazone, the levels of FPG, HOMA-IR, hs-CRP, TNF-αand IL-6 were declined significantly(P<0. 05), and the level of HDL-C was increased significantly(P<0. 05). Conclusion: Rosiglitazone shows significant anti-inflammatory, insulin resistance improve-ment and anti-lipidemic effects in peritoneal dialysis patients.

Acta Anatomica Sinica 1954;0(02):-
Objective To determine the anatomical basis of an algorithm to safely elevate the deep circumflex iliac artery osteocutaneous perforator(DCIAP) flap. Methods 1.Six unfixed corpses underwent whole body gelatine/lead oxide injection.Specimens were dissected by layers.Angiography and photography were used to document the precise course, size, location, and type of individual perforators in the lateral lumbar region. The surface areas of cutaneous territories and perforator zones were measured and calculated with Photoshop and Scion Image.2.One specimen also underwent whole body carboxymethylcellulose/lead oxide injection,CT scan and 3D-Reconstrution. Results An average of 1.6 DCIA perforators with a diameter of 0.7mm was present in 92% of specimens.Perforators were located 5~10 cm posterior to the anterior superior iliac spine,12~35mm above the crest, with a perforator zone of 31 cm~2.The DCIA reliably perfused the medial aspect of the iliac crest. Conclusion The DCIA reliably perfused the medial aspect of the iliac crest and lateral lumbar region. It offers a large quantity of bone on a pedicle of large diameter. The mobility of the skin component allows better tissue positioning during complex reconstructions.
