1.Methylation status of DLEC1 promoter in colorectal cancer patients and its clinical relevance

Objective To detect the methylation status of DLEC1 promoter in the tissue and serum of colorectal cancer(CRC) patients and to evaluate its clinical relevance.Methods Genomic DNA was extracted from the tissues(cancer tissue and corresponding adjacent normal tissue) and sera of 71 CRC patients;serum genomic DNA was also obtained from 20 patients with benign gastrointestinal diseases and 20 healthy donors.Promoter methylation status of DLEC1 gene was detected by methylation-specific polymerase chain reaction(MSP).Results The incidence of aberrant methylation of DLEC1 promoter was 45.1%(32/71) in CRC tissues,which was significantly higher than that in the adjacent normal tissues(7.1%,4/56)(P

Objective To study the chemical constituents in the seeds of Zizyphus jujuba var.spinosa.Methods The compounds were isolated by silica gel column chromatography and their structures were established by spectroscopic methods.Results A new keto-dammarane type saponin,jujuboside H(Ⅰ),along with three known compounds protojujuboside A(Ⅱ),spinosin(Ⅲ),and betulic acid(Ⅳ)were isolated.Conclusion Compound Ⅰ is a new compound named jujuboside H.
3.Asiaticoside attenuates bleomycin-induced interstitial pulmonary fibrosis

OBJECTIVE To study the therapeutic effect and the underlying mechanism of asiatico?side on bleomycin-induced rat interstitial pulmonary fibrosis(IPF). METHODS Male Sprague-Dawley (SD)rats were divided into normal control group,bleomycin 5 mg·kg-1 model group and asiaticoside 50 mg · kg-1 group. The model and asiaticoside group were administrated with bleomycin 5 mg · kg-1 to induce IPF,while the asiaticoside group was administrated with asiaticoside 50 mg·kg-1 by gastric perfusion. Hematein eosin(HE)and Masson staining were carried out to analyze the histopathological changes in the lung. Lung homogenates were used to examine hydroxyproline(HYP) content,and serum samples were used to measure the concentration of interferon-γ(IFN-γ),interleukin-4(IL-4) and tumor necrosis factor-α(TNF-α). In addition,immunohistochemical methods were used to locate lung transforming growth factor-β1(TGF-β1)and adenosine 2A receptor(A2AR)expression,and Western blotting was used to examine the expression levels of TGF-β1 and A2AR. RESULTS On the 7th,14th and 28th days,the scores of pulmonary inflammation were higher in model group than in control group (P<0.01),and the asiaticoside group showed mitigated alveolitis(P<0.01,P<0.05) compared with model group. Compared with control group,the scores of pulmonary fibrosis in model group were elevated(P<0.01),and the asiaticoside group showed reduced pulmonary fibrosis(P<0.05). On the 14th and 28th days,HYP content in the model group〔1.85±0.10,(2.48±0.18)mg·g-1〕was higher than in the control group〔0.79 ± 0.07,(0.84 ± 0.08)mg · g-1〕(P<0.01),but HYP content in the asiaticoside group〔1.32±0.131,(1.71±0.13)mg·g-1〕was lower than in the model group(P<0.05). IL-4 and TNF-αin the asiaticoside group were lower than in model group(P<0.05),but were higher in the model group than in the control group(P<0.01,P<0.05). The expression level of TGF-β1 protein in the asiaticoside group was lower than in the model group(P<0.05),but was higher in the model group than in the control group(P<0.05). The expression level of A2AR protein in the asiaticoside group was higher than in the model group(P<0.05),but was lower in the model group than in the control group(P<0.05). CONCLUSION Asiaticoside can mitigate bleomycin-induced IPF by inhibiting the expression of IL-4, TNF-αand TGF-β1,and raising the level of A2AR.
4.Effect of asiaticoside on endothelial-to-mesenchymal transition in hypoxia pulmonary hypertension

OBJECTIVE To investigate the effect of asiaticoside (AS) on endothelial-to-mesenchymal transition (EndoMT) in hypoxia pulmonary hypertension (HPH). METHODS Male Sprague-Dawley (SD) rats were divided into normoxia control group, hypoxia model group, and AS 25 and 50 mg · kg-1 group. Hypoxia model group and AS group were subjected to intermittent hypoxia exposure. Control group and model group received 1-1.5 mL saline daily, and AS groups were ig administrated with AS 25 and 50 mg·kg-1 for 4 weeks. Human pulmonary artery endothelial cells (HPAECs) were divided into normoxia control group and hypoxia AS groups. Hypoxia groups were cultured with AS 0, 25, 50, 100 and 200 mg·L-1 for 72 h under hypoxic (5%O2, 5%CO2) conditions. Anti-proliferation effect of AS was investigated by CCK-8 assay. Then, HPAECs were divided into normoxia control group, normoxia AS 100 mg · L-1 group, hypoxia model group, and hypoxia AS 100 mg · L-1 group. After five days of culture, migration ability of cells was detected by Transwell test. Expression of CD31 andα-SMA was detected by immunofluorescence and Western blotting in both in vivo and in vitro experiments. RESULTS In both in vivo and in vitro experiments, compared with normoxia control group, expression of CD31 was reduced (P<0.01) andα-smooth muscle actin (α-SMA) was increased (P<0.01) in hypoxia model group in both immunofluorescent analysis and Western blotting. Compared with hypoxia model group, expression of CD31 was increased andα-SMA was decreased (P<0.05, P<0.01) in AS treatment groups. Compared with normoxia control group, proliferation and migration ability of HPAEC were elevated in hypoxia model group (P<0.05). Compared with hypoxia group, AS 100 mg · L-1 depressed proliferation and migration of HPAEC under hypoxia exposure up to 72 h (P<0.05). CONCLUSION EndoMT might be involved in HPH and could be partly inhibited by AS.
5.Effects of Combination of Baoxinkang and Conventional Medicine Intervention on Cardiac Function and Adenylate Metabolism of Heart Failure Rats

Objective To evaluate the synergistic effect of Baoxinkang on chronic heart failure ( CHF) rats by observing the effect of the combination of Baoxinkang and conventional medicine intervention on cardiac function and adenylate metabolism. Methods Sixty Sprague-Dawley rats were randomly divided into 6 groups, namely sham-operation group, model group, Baoxinkang group ( Baoxinkang 1 020 mg/kg) , conventional medicine intervention group (metoprolol 10 mg/kg, captopril 5 mg/kg, and digoxin 0.022 5 mg/kg), combination group 1 ( conventional medicine intervention + Baoxinkang 1 020 mg/kg) , and combination group 2 ( conventional medicine intervention + trimetazidine 10 mg/kg) . Abdominal aora was constricted to establish CHF rat model. The rats except for the sham-operation group and model group were given the corresponding medicine according to the experimental design for 6 weeks. Echocardiography ( ECHO) was performed to evaluate the cardiac function of rats. High performance liquid chromatography was utilized to investigate the contents of myocardiac adenylate of adenosine triphosphate ( ATP) , adenosine diphosphate ( ADP) , adenosine monophosphate (AMP) . Total adenylate nucleotide pool (TAN) was equal to ATP+ADP+AMP, energy charge (EC) was equal to ( ATP+0.5 ×ADP) /TAN. Results The results of ECHO showed that the heart size was reduced, left ejection fraction and cardiac output were increased in the combination group 1 compared with the model group and conventional medicine intervention group. The levels of ATP, TAN, and EC were significantly increased, whereas the levels of ADP and AMP were decreased in the combination group 1 (P<0.05 or P<0.01). However, there were no significant differences of heart size, left ejection fraction, cardiac output, ATP, ADP, AMP, TAN or EC between combination group 1 and combination group 2 ( P>0.05) . Conclusion Baoxinkang may have some synergistic effect on the improvement of CHF rat cardiac energy metabolism disorder treated by conventional medicine.
6.Total knee arthroplasty in the treatment of knees with bony ankylosis in extension

Objective To explore the clinical results of total knee arthroplasty (TKA) in the treatment of knees with bony ankylosis in extension.Methods From June 2000 to June 2007,10 patients had knees with bony ankylosis in extension were treated with TKA,including 4 males and 6 females,with an average age of 49 years (range,29 to 63 years).The primary diseases were as follows:ankylosing spondylitis in 3 cases,rheumatoid arthritis in 2 cases,hemophilic arthritis in 1 care,traumatic arthritis in 3 cases and pyogenic arthritis in 1 case.The range of motion (ROM) was 0° in all knees,and the average HSS (the Hospital for Special Surgery) score before operation was 32.5±10.26.Two patients underwent bilateral TKA,and 8 underwent unilateral TKA.Results All patients were followed up for 3 to 10 years (average,5.3 years).The average HSS score was improved to 87.75±6.45 at the last follow-up,and there was significant difference compared with that before operation (t=18.668,P=0.000).The average ROM was improved to 97.08°±11.57°at the last follow-up,and there was remarkable difference compared with that before operation (t=29.063,P=0.000).The postoperative complications included cutaneous necrosis in 2 cases,deep venous thrombosis of lower limb in 1 case,periprosthetic fracture in 1 case,and deep prosthetic infection in 1 case who underwent revised TKA later.The postopertative X-ray showed no looseness of the prostheses.Conclusion TKA through medial parapatellar approach,with dissection of the rectus femoris,secondary osteotomy and soft tissue balance intraoperatively is effective in the treatment of knees with bony ankylosis in extension,which can correct the ankylosed knee deformitis successfully.Combining with the correct rehabilitation exercise,it is possible to improve significantly the function of knee and life quality in these patients.
7.Utilization and analysis of health services for children at a community health service center in Shenzhen

To explore the current status of children receiving primary health services at a community health service center in Shenzhen so as to provide references for the formulation of children health care policies.According to the patient electronic data in 2011,the clinical profiles were analyzed for children aged under 7 years seeking the advice of general practitioners (GPs) at Niuhu Community Health Service Center.Children aged under 7 years accounted for 12.4% of all subjects.The causes of seeing the GPs were as follows:acute upper respiratory.infection(58.2%),acute bronchitis(14.9%),gastroenteritis(5.6%),trauma(3.3%)and dermatitis(3.2%).And no difference existed in the workload between pediatrists and surgeons,internists or herbalists.The common and frequent community diseases may be diagnosed and managed at a community health service center.
8.COMMD7 inhibits migration and invasion in liver cancer stem cells via regulating mesenchymal-epithelial transition

Objective To determine the effect of COMMD7 inhibition on invasion and migration in liver cancer stem cells (LCSCs),and investigate the possible mechanism.Methods After LCSCs were infected by shRNA lentiviral vectors of COMMD7,adhesion assay and Transwell assay were used to detect the invasion and migration,and phalloidin staining was employed to observe the morphological changes.Western blotting was adopted to measure the expression of E-cadherin,N-cadherin and Vimentin.Results COMMD7 knockdown significantly inhibited the invasion and migration of LCSCs.The relative cell quantity of adhesion was 1.00 ± 0.12 and 2.35 ± 0.20 respectively in control cells and infected cells,suggesting there were significantly more adhesive cells in the infected group (P < 0.05).The relative cell quantity per visual field of migration was 1.00 ±0.04 and 0.24±0.03,and that of invasion was 1.00 ±0.05 and 0.24 ±0.04 respectively in the control cells and infected cells,and there were significantly less invasive and migrated cells in the infected group (P <0.05).What's more,COMMD7 knockdown also induced some morphological changes of cells corresponding to the weakened abilities of migration and invasion.All the changes above were associated with up-regulation of E-cadherin (P < 0.05) and down-regulation of N-cadherin and Vimentin (P <0.05),the molecules related to mesenchymal-epithelial transition (MET).Conclusion COMMD7 knockdown inhibits the invasion and migration in LCSCs,which may be through its regulation on the MET course.
9.Case reports and clinical analysis of 8 patients with primary Sj?gren's syndrome diagnosed as anti-synthase syndrome

Objective:Anti-synthase syndrome (ASS) is a rare autoimmune disease. To increase the understanding of the disease and reduce the rate of miss diagnosis.Methods:The clinical data of 8 patients with positive anti-synthase antibody afterprimary Sj?gren's syndrome (pSS) were retrospectively analyzed and descriptive statistical analysis was carried out.Results:The diagnosis of Sjogren's syndrome (SS) was in accordance with the revised European criteriaof SS issued by the US-Europe consensus Group in 2002 or the classification criteria of American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) SS in 2016, and the diagnostic ASS was in accordance with the diagnostic criteria of Conners in 2010 or Solomon in 2011. Eight(100%) patients had a history of interstitial lung disease, and 7 (88%) patients had fever (oral temperature >38.5 ℃). All patients were positive for anti-Ro-52 antibody, 4 patients were positive for anti-PL-7 antibody, 2 patients were positive for anti-EJ antibody, 1 patient was positive for both anti-PL-7 antibody and anti-EJ antibody, and 1 patient was positive for anti-PL-12.Conclusion:pSS patients with severe interstitial lung disease or high fever of unknown causes should be screened for anti-synthase antibodies and the possibility of ASS.
10.Reconstructive options for critical limb ischaemia in infrapopliteal arteries

Objective To assess reconstructive options for critical limb ischaemia in infrapopliteal arteries. Methods A retrospective review of all CLI patients who underwent infrapopliteal reconstruction was carried out. Patient history, demographics, procedure details, complications, and follow-up information were collected and analyzed. Patency, limb salvage rate was determined by Kaplan-Meier analysis. Results During the period (from December 2003 to January 2008 ), 123 CLI patients with arteriosclerosis occlusions were treated on an intention-to-treat basis with infrapopliteal percutaneous transluminal angioplasty (PTA).Thirty-three thromboangiitis obliterans and twenty-three arteriosclerosis occlusions suffering CLI were treated by infrapopliteal bypass procedures. Primary patency and limb salvage rate of infrapopliteal PTA at 6, 12 and 24 months was 67%, 54%, 49% and 91%, 85%, 78% respectively, Primary patency and limb salvage rate of infrapopliteal surgical bypass at 6, 12 and 24 months was 90%, 83%, 79% and 92%,87%, 80% respectively, the patency of infrapopliteal PTA was lower than infrapopliteal surgical bypass (P <0. 01 ), but the limb salvage rate of infrapopliteal PTA and open surgery was no significant difference (P > 0. 05 ). Conclusion Endovascular treatment (PTA) in patients with infrapopliteal arteriosclerosis occlusions and critical ischaemia is safe, effective. Infrapopliteal PTA can be used as the choice of therapy and surgical bypass reserved in those endovascular treatment failed. While in CLI patients with thromboangiitis obliterans infrapopliteal artery bypass remains the best treatment option.
