1.Determination of doping in human urine by gas chromatography-high resolution mass spectrometry.

A method was evaluated for determination of twenty-one doping (including nandrolone, boldenone and methandienone) in human urine by gas chromatography-high resolution mass spectrometry. Samples were prepared by liquid-liquid extraction, concentrated, TMS derivatization and limit of detection at ng x mL(-1) by MID/GC/HRMS. According to the code of the World Anti-Doping Agency (WADA), precision and recoveries of the procedure were evaluated by replicate analysis (n = 6), the recoveries in the range of 66%-103%, with the RSD below 10.0%. The precision within the day of the method with three different concentrations was also determined RSD were less than 9.5%, 10.0% and 9.7%.
2.Right Atrial Myxoma Diagnosed by Echocardiography

Objective To investigate the echocardiographic features in patients with right atrial myxoma and thus evaluate the value of echocardiog?raphy in the diagnosis of right atrial myxoma. Methods The echocardiographic findings of 20 patients with right atrial myxomas were retrospectively analyzed,and the echocardiographic features of these patients were summarized. Results The main clinical symptoms of right atrial myxoma includ?ed chest tightness,shortness of breath,lower limb edema,and syncope. Of these 20 patients,the echocardiography showed that the average size of the right atrial myxomas was 3.11 cm × 2.90 cm to 7.44 cm × 4.52 cm. The myxomas were round or oval shape in 15 patients(75%)and lobulated shape in 5(25%). The myxomas were attached to the atrial septum in 15 patients(75%)and to right atrial free wall in 25%of the patients. The aver?age width of the basement was 1.62±0.30 cm,and the width was greater than 1 cm in 80%of the patients had the width over 1 cm. Some myxomas had internal areas of calcification and anechoic cystic areas. The percentages of patients with the blocking of right atrium,tricuspid and pulmonary hy?pertension were 70%,50%,and 10%,respectively. Conclusion The echocardiographic features of right atrial myxoma were specific. Echocardiog?raphy can accurately assess the tumor and identify the secondary changes in cardiac structure and hemodynamics and thus provide evidence for time?ly and accurate diagnosis of right atrial myxoma.
3.Construction of human naive phage antibody library and primary screening of the gab antibodies against gp96

Objective To construct a naive human Fab fragment phage display library,from which the anti-gp96 antibodies may be panned by the gp96 purified from the tissue of urothelial carcinoma in the urinary bladder and provide a basis to new therapy for the malignant tumors.Methods Peripheral blood lymphocytes were isolated from 800 ml of blood,which was obtained from four healthy blood donors.The heavy chain Fd and light chain cDNA synthesized from the total RNA of lympbocytes were amplified by PCR with variable regions 5' and 3' primers of heavy and light chain, and the amplification products were ligated into the phagemid vector pComb3, then the ligated sample was transformed into competent E.coli XL1-Blue by electroperation.The transformed cells were infected with VCSM13 helper phage to yield recombinant phage antibody Fabs.The phagemids abstracted from amplified E.coli were cut with endonucleases such as Sac Ⅰ,Xba Ⅰ,Xho Ⅰ and Spe Ⅰ, and both the phage antibody Fabs and phage-raids abstracted from amplified E.coil were amplified by PCR to monitor the insertion of the genes of light chain or heavy chain Fd fragment.The gp96 purified from the urothelial carcinoma tissue of the bladder by affinity chromatog-raphy on eoncanavalin-A sephnrose and DEAE-sephnrose ion exchange chromatography were utilized as antigens to process three rounds of panning to the original Fab antibody library.Results The quantity of total RNA and cDNA were qualified.By combination of light chain and heavy chain genes, an antibody library containing 6.6×106 clones was obtained, and both the cutting of enzymes and PCR showed that there were the genes of light chain or heavy chain Fd fragment in the phagemids.The gp96 protein was obtained from urothelial carcinoma tissue in the urinary bladder.After having been panned by gp96, the original antibody library gained enrichment by 68 times.Conclusion Utilizing the technology of phage surface display, specific antibody can be gained from the human naive Fab phage display library,which can be used for immunological therapy for tumors.
4.The value of color Doppler ultrasound in the diagnosis of cystic adventitial disease

Objective To explore the ultrasonic characteristics of cystic adventitial disease (CAD) and to evaluate the value of color Doppler ultrasound in diagnosis of CAD. Methods We retrospectively analyzed the ultrasonograms and other imaging methods of eight patients with CAD conformed by surgery in The first Hospital of China Medical University. Results Five of the eight CAD cases were confirmed by color Doppler ultrasound before surgery, and three were misdiagnosed. All the cases were single, with five in popliteal artery, two in iliofemoral artery, and one in external iliac vein. We divided the cysts into concentric cysts and eccentric cysts according to the cystic shape and the lumen stenosis. Six cases were eccentric cysts, in which two had septations;two cases were concentric cysts, and both had septations. The affected vessel expanded, but its shape did not change and the lumen was compressed. There were three layers of wall between the cyst and the lumen. The peak systolic velocity of the affected lumen increased and the distal end systolic peak velocity decreased when lesion occurred in artery. Distal end lumen reflux were slow when lesion occurred in distal veins. Angiography showed the stenosis of the affected lumen, which was described asscimitar sign. CT showed uniorlocular or multilocular cyst, and the compressed lumen. All of the eight patients underwent the diseased adventitia resected along with excision of the cyst. Conclusions The color Doppler ultrasound can display the location, shape, and internal echo of CAD, and thus it can accurately evaluate luminal stenosis and flow pattern. This is of important clinical significance for the diagnosis, treatment, and prognosis of CAD.
5.Longitudinal Strain of Primary Mitral Regurgitation Predicts Postoperative Worsening of Left Ventricular Systolic Function

Objective To analyze subclinical changes of left ventricular deformation by using two-dimensional (2-D) speckle tracking imaging (STI) in primary mitral regurgitation (MR) and to identify sensitive parameters of left ventricular deformation that reflect reduction of left ventricu-lar function after mitral valve repair.Methods We evaluated patients with primary MR who underwent mitral valve repair between May 2013 and June 2015.According to the reduction in ejection fraction (EF),the MR patients were divided into MR1 (EF reduction of ≤10%) and MR2 groups (EF reduction of >10%).The parameters of left ventricular deformation were measured by using 2-D STI.Results Compared with the MR1 group,the MR2 group had increased global longitudinal strain (GLS;P < 0.05).A positive correlation was found between postoperative EF reduction and left ventricular intemal dimension at end-diastole,left ventricular internal dimension at end-systole,end-diastohc volume,left ventricular mass index,regurgitant volume,and GLS (P < 0.05).GLS showed a diagnostic value in predicting EF reductions of > 10%.Conclusion STI is useful for evaluating the postoperative reduction of left ventricular function in primary MR patients and thus is helpful for optimizing surgical timing.
6.Evaluation of left ventricular torsion in patients with apical hypertrophic cardiomyopathy with two-dimensional ultrasound speckle tracking imaging

Objective To explore the value of two-dimensional ultrasound speckle-tracking imaging (STI) in assessing the left ventricular torsion (LVtor) in patients with apical hypertrophic cardiomyopathy (AHCM). Methods Thirty-four patients with AHCM and 21 healthy volunteers underwent two-dimensional echocardiography. High frame rate two-dimensional images of left ventricular short-axis views at the levels of mitral annulus and apex were obtained. Peak rotation (Prot) and the time to Prot, peak rotation velocity on basal and apical plane in left ventricular short-axis views, the peak twist (Ptw) and time to peak twist, half time of untwisting (HTU) were measured using two-dimensional strain software while twisting rate (Untw R) was calculated. All the above indices were compared between patients and healthy volunteers. Correlation was analyzed between HTU and E, A, E/A ratio. Results Compare with healthy volunteers, Prot and time to Prot and peak rotation velocity on apical plane in AHCM significantly increased, so did Ptw and peak rotation velocity of left ventricular (all P<0.05). HTU delayed with the Untw R and peak untwisting velocity decreased in the AHCM group (all P<0.05). There was negative correlation between HTU and E/A ratio in patients with AHCM. Conclusion When the left ventricular systolic function maintained normal in patients with AHCM, the peak rotation on apical plane and the LVtor increased with Untw R decreased. STI can sensitively evaluate the function alteration of myocardium in patients with AHCM.
7.Analysis on the gene mutations of MYOC in primary open angle glaucoma pedigree

Objective To screen the mutations of MYOC gene in a Chinese primary open angle glaucoma (POAG) family from Cbengqing and investigate the relationship between the mutations in MYOC/TIGR gene and POAG.Methods In a large 4-generation glaucoma family, myocilin gene (MYOC) was screened in 39 family members, 8 of which were confirmed patients. Normal controls included 100 normal Chinese subjects.The known mutations of MYOC gene ( including G34C, C136T, G144T, G227A, C624G,G736A, C1009G, A1036G, C1081T, G1099A, G1138A, A1139C, T1430A, C1441A and C1442T) were detected by single strand conformation polymorphism(SSCP) , po]ymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) analysis and DNA sequencing.Results G227A mutation was detected in 2 POAG patients and 1 asymptomatic patient, but not in the controls.Cl009del mutation was identified in all patients of the pedigree and an offspring member but not in the controls. No other mutations were detected.Since the C1009del mutation was revealed for the first time, a new GenBank number FJ237047 correponding to ACI62293 was applied.Conclusions The G227A mutation is a known site and there is no relationship between G227A mutation and glaucoma. But C1009del may be related to glaucoma which suggests that morbidity could be higher in the relatives of POAG than the controls.
8.The profile of cognitive response to facial expressions in 6- to 12-month-old infants

Objective To explore the profile of cognitive response to five facial expressions in 6-to 12-month-old infants. Method One hundred and fifty-five 6-to 12-month-old healthy infants were recruited to view five kinds of normative facial emotional expression pictures which presented on computer screen. The visual attention, self emotional and behavioral responses of each infant toward the facial expressions were recorded by a web camera and then were coded and analyzed from the videotaped observations. Results The visual preference to expressions was different in different groups. 6-to 8-month-old infants looked back ( such as 1.50 ± 0.73,1.39 ±0.81,1.67 ±0.87 ) more to all expressions than 9-to 12-month-old infants( 1.10 ± 0.53,1.04 ± 0.51,1.12 ±0. 61 ). 6-to 8-month-old infants exhibited no obvious difference(P> 0.05 ) while 9-to 12-month-old infants exhibited significant difference in self emotional responses (include positive and negative) to different expressions (P <0. 05 ). Infants appeared some positive behaviors including looking back at mother,finger-pointing and phonating to expressions during the observation. Conclusions Infants over 6-month-age could discriminate different facial expressions,recognize and comprehend happy expression earlier. 9-to 12-month-old infants began to understand the negative emotional significance of negative facial expressions gradually, but their comprehension to negative emotional expressions was rudimentary.
9.Real-time three-dimensional transesophageal echocardiography in evaluation of Carpentier's classification of mitral regurgitation

Objective To evaluate the feasibility of real-time threedimensional transesophageal echocardiography (RT3D TEE) in Carpentier's classification of patients with mitral regurgitation (MR).Methods Totally 124 patients with MR were divided into type Ⅰ—Ⅲ type according to Carpentier's classification,and the cases and methods of operation were followed up.Results In all of the 124 patients,9 cases were Carpentier's type Ⅰ,including 8 cases of atrial fibrillation,1 case of mitral valve cleft;102 patients were type Ⅱ,including pure mitral valve prolapse in 16 cases,mitral valve prolapse with chordae rupture in 80 cases,infective endocarditis in 6 cases;13 cases were type Ⅲ,including Ⅲa type rheumatic heart disease in 8 cases,Ⅲ b type of left ventricular systolic dysfunction in 5 cases.Totally 101 cases underwent MR surgery,including mitral valve plasty in 83 cases and valve replacement in 18 cases.Conclusion RT3D TEE is feasible and accurate in defining different types of MR,and it is useful to make management decisions and select the appropriate surgical strategy.
10.Psychological rehabilitation on 57 schizophrenic patients

ObjectiveTo standardize the service for mental disability in community, and evaluate scientifically the effect of rehabilitation measures and social factors on the prognosis of disease.MethodsIn the communities selected by cluster sampling method, family-social rehabilitation service was established. The observe groups or specified observers were selected by the community committee, and the maintenance treatment and rehabilitation evaluation were made by psychiatrists.ResultsAfter community rehabilitation treatment, schizophrenia could be compliant with maintenance treatment, which led to satisfactory social function evaluation and rehabilitation result.Conclusion Community rehabilitation treatment could increase the compliance with medical treatment and decrease the occurrence of relapse.
