4.Impact of implanted metal plates on radiation dose distribution in vivo by Monte Carlo code

ObjectiveTo investigate the impact of metal plate on radiation dose distribution by Monte Carlo (MC) code. MethodsThe metal plates with 0. 4 thick were placed in water at 5 cm, all the plate irradiated with 6 MV X-ray were simulated by MC code, SSD =100 cm. The percentage depth dose with or without metal implants were compared. ResultsThe surface absorbed doses on incident plane of stainless steel plate and titanium plate were increased by 19. 6% and 15.7% respectively as compared water,the dose influence was less than 1.5% more than 0. 3 cm outside the incidence plane. The doses on the exit surface of stainless steel plate and titanium plate were lowered by 8. 6% and 8. 2% when compared with water, the dose impacts of this places where were from the exit surface of stainless steel plate 、titanium plate more than 1.2 cm,0. 9 cm were less than 1.5%. The surface absorbed doses on incident plane of stainless steel plate were increased by 3.9% respectively as titanium plate, the dose of the exit surface of stainless steel plate and titanium plate was similar. Conclusions MC method is a fast and accurate calculation method. The influence of metal plate on the radiotherapy dose distribution is significant. Under the such condition, the impact of stainless steel plate is much more than that of titanium alloy plate.
5.STS Gene in a Pedigree with X-linked Ichthyosis

To investigate the gene mutation in a pedigree with X-linked ichthyosis (XLI) and to explore the relationship between the mutation and its clinical manifestations, genomic DNA of affected members, the normal member of the pedigree and 50 unrelated normal members was extracted with a whole blood genomic DNA extraction kit and the DNA was used as a template for the polymerase chain reaction (PCR)-mediated amplification of exon 1 and exon 10 of the STS gene. hHb6 (human hair basic keratin) gene was used as the internal control. Our results showed that the STS gene was deleted in affected members in the pedigree with X-linked ichthyosis. The normal member of the pedigree and 50 unrelated normal members had no such deletion. The proband and his mother had products in the internal control after PCR amplification. The blank control had no product. It is concluded that deletion of the STS gene existed in this pedigree with X-linked ichthyosis, and it is responsible for the unique skin lesions of X-linked ichthyosis.
6.The effect of 18β-sodium glycyrrhetinic acid on the nasal mucosa epithelial cilia in rat models of allergic rhinitis.

OBJECTIVE:
To investigate 18β-sodium glycyrrhetinic acid impact on nasal mucosa epithelial cilia in rat models of allergic rhinitis (AR).
METHOD:
AR models were established by ovalbumin-induction. Wister rats were randomly divided into groups as normal group, model group, budesonide (0.2 mg/kg) group and sodium glycyrrhetinic acid (20 mg/kg and 40 mg/kg) group after the success of AR models. At 2 weeks and 4 weeks after treatment, the behavioral changes of rats were observed and recorded, and nasal septum mucosae were collected after 2 week and 4 week intervention, and the morphological changes of nasal mucosae were observed by electron microscope.
RESULT:
Model group developed typical AR symptoms, the total score in all animals was > 5. With budesonide and sodium glycyrrhetinic acid treatment, the AR symptoms were relieved, and the total scores were reduced significantly (P < 0.01). Compared with the model group: after 2 weeks' intervention, thick mucous secretions on the top of columnar epithelium cilia in rat nasal mucosa was significantly reduced, and cilia adhesion, lodging, shedding were relieved in budesonide group and sodium glycyrrhetinic acid group, the relieve in budesonide group was slightly better than that in sodium glycyrrhetinic acid group; after 4 week intervention, Cilia adhesion, lodging, shedding were completely vanished, and the cilia were ranged in regular direction in budesonide group and sodium glycyrrhetinic acid group. Cilia in sodium glycyrrhetinic acid (20 mg/kg) group was more orderly, smooth than that in budesonide group and sodium glycyrrhetinic acid group (40 mg/kg), and the condition of cilia in sodium glycyrrhetinic acid group (20 mg/kg) was similar to the normal group.
CONCLUSION
18β-sodium glycyrrhetinic acid is effective to restrain the pathological changes of nasal mucosa cilia in rat models of AR.

7.Process optimization of compound cotton rose hibiscus leaf ointment and its clinical efficacy and safety verification

BACKGROUND: Compound cotton rose hibiscus leaf ointment is developed from the continuous optimization of the preparation process of traditional famous prescription "Xiaozhong San". Xiaozhong San has been used in Ruijin Hospital Affiliated to School of Medicine of Shanghai Jiaotong University for over 40 years and has achieved good clinical outcomes. OBJECTIVE: To verify the clinical efficacy and safety of compound cotton rose hibiscus leaf ointment, laying a foundation for its popularization and application in the treatment of acute soft tissue injury. METHODS: 112 cases of acute soft tissue injury were divided into trial group (n=55) and control group (n=57) according to the random number table method. The experimental group selected the compound cotton rose hibiscus leaf ointment, and the control group selected the compound Zijing ointment. RESULTS AND CONCLUSION: (1) There were no statistical differences in the primary outcomes, such as pain, tenderness, swelling, dysfunction and in the secondary outcomes, such as ecchymosis, insomnia, dry mouth, constipation, tongue coating and pulse condition, between the two groups before treatment, at 3 and 7 days of treatment and after treatment termination (P> 0.05). (2) The two groups achieved obvious clinical outcomes in the treatment of acute soft tissue injury, but there was no difference in the clinical efficacy between the two groups (P> 0.05). (3) The two groups had no obvious abnormalities in the routine of hematuria and stool and the function of the liver and kidney. (4) There were only five cases of mild skin allergy, including two cases in the trial group and three cases in the control group. To conclude, the compound cotton rose hibiscus leaf ointment has good clinical efficacy and safety, which is worthy of clinical application.
8.The effect of 18β-glycyrrhetinic acid on tight junctions of the nasal mucosa epithelial cells in rat models with allergic rhinitis.

OBJECTIVE:
To observe 18β-glycyrrhetinic acid (GA) impact on ultrastructure of tight junctions (TJs) of nasal mucosa epithelial cells in rats models of allergic rhinitis (AR).
METHOD:
Ninety-six Wistar rats were randomly divided into control group, model group, loratadine group, and 18β-glycyrrhetinic acid group, and each group had 24 rats. Ovalbumin was used to establish a rat AR model. The behavioral changes and the tight junctions of nasal epithelial were observed and compared in different groups after 2,4,6 and 10 weeks intervention.
RESULT:
The length of TJs in allergic rhinitis model became shorter, electron-high-density plasma membrane became thicker, number of the integration loci reduced and gap of TJs widened or even ruptured. With the consistent effect of allergens,the changes of TJs in the model group aggravated gradually,and the changes of ultrastructure of TJs in 18β-glycyrrhetinic acid group was relieved apparently compared to model group and even were close to the control model with time.
CONCLUSION
18β-glycyrrhetinic acid can recover the ultrastructure of the tight junctions of AR rat nasal epithelial cells.

9.Analysis on single-center clinical effect of allogeneic hematopoietic stem cell transplantation for treatment of acute myeloid leukemia

Objective To summarize and analyze the clinical effect of allogeneic hematopoietic stem cell transplantation(alloHSCT) for treating acute myeloid leukemia(AML) in this center.Methods The clinical data of 87 patients with AML receiving allo-HSCT in this hospital from January 2008 to June 2015 were retrospectively analyzed on the overall survival rate(OS) after transplantation,leukemia-free survival rate (LFS) and relapse rate (RR),transplant-related mortality (TRM),moreover the efficacies of HLA-matched (kinship and unrelated) and haploidentical transplantation were compared.Meanwhile the influence of different transplantation of pre-leukemia remission status on transplantation curative effect was analyzed.Results Among 87 cases,OS after 2 years was (73.6±4.7)% and 2-year LFS was (62.1±5.8)%.During the follow-up period,27 cases (31.3%) relapsed and 23 cases (26.4%) died.The 2-year OS in HLA-matched (kinship and unrelated) and haploidentical transplantation were (76.3± 5.3)% and (68.5±7.2)% respectively(P<0.05) and the 2-year LFS were (60.2±4.8)% and (56.3±5.7)% respectively (P< 0.05).The incidence rate of aGVHD and infection in haploidentical transplantation group was significantly higher than that in the HLA matched group (kinship and unrelated),but their RR were 26.9 % and 29.2 % respectively (P>0.05).The transplantation effect analysis in different leukemia remission state showed that RR in the unreaching CR before transplantation group,reaching CR but minimal residual(MRD) positive group and negative group were 68.7 %,34.7 % and 16.6 % respectively.The 2-year LFS were (18.7±5.2)%,(56.5± 6.3)% and (79.2% ±5.7)% respectively (P<0.05).The 2-year OS were (31.2± 5.3)%,(69.6± 7.2) % and (89.6±5.3)% respectively (P<0.05).Conclusion allo-HSCT is an effective method for treating AML.Recurrence is the main cause of failure after transplantation therapy.
10.Feasibility Evaluation of using PAX1/JAM3 methylation markers as cervical cancer screening for female self-collected samples

Objective:To explore the application value of PAX1/JAM3 methylation detection by cervical self-collected specimen in cervical cancer screening and the management of premenopausal and postmenopausal women.Method:This study is a single center cross-sectional study. From January 2023 to November 2023, cervical self-collected and physician-collected specimens at the colposcopy clinic were detected the PAX1/JAM3 methylation (PAX1 m/JAM3 m) testing. The consistency between self-collected and physician-collected specimens for PAX1 m/JAM3 m detection were compared based on histopathology. In addition, the clinical efficacy of methylation detection with high-risk human papillomavirus (hrHPV), liquid-based cytology (LBC), and their combination for cervical cancer screening were compared in the study. Results:A total of 301 women were recruited to undergo referral colposcopy examination, and statistical analysis was conducted on 272 women with pathological and diagnostic information. Among them, 102 cases (37.5%) were diagnosed as normal cervical tissue or chronic cervicitis, 72 cases (26.4%) were cervical intraepithelial neoplasia grade 1 (CIN1), 43 cases (15.8%) were CIN2, 29 cases (10.7%) were CIN3, and 26 cases (9.6%) were cervical cancer. According to the minimum quantity formula, they were divided into a consistency cohort of 81 participants and a validation cohort of 191 participants. The consistency between cervical self-collected and physician-collected specimens for detecting PAX1 m/JAM3 m. Results from spearman correlation analysis showed a positive correlation between the self-collected and physician-collected results of PAX1 m/JAM3 m detection, and the correlation coefficient R values are 0.858 ( P<0.001) and 0.828 ( P<0.001). The sensitivity and specificity of PAX1 m/JAM3 m detection for diagnosing CIN2 or more severe lesions (CIN2+) were 77.6% [95% confidence interval ( CI) 65.3%-86.4%] and 87.2% (95% CI 80.5%-91.9%), respectively. In clinical performance comparison, the sensitivity of PAX1 m/JAM3 m combined with HPV16/18 detection, 89.7% (95% CI 79.2%-95.2%), was the same as that of hrHPV detection in CIN2+and 96.0% (95% CI 80.4%-99.3%) in CIN3+, which is higher than 92.0% (95% CI 75.0%-97.8%) of hrHPV and 82.6% (95% CI 62.9%-93.0%) of LBC or the combination of sPAX1 m/JAM3 m and LBC low-grade and higher squamous intraepithelial lesion testing [87.0% (95% CI 67.9%-95.5%)]. Conclusions:Self-collected specimens by women for detection of PAX1 and JAM3 methylation as a promising screening tool for cervical cancer has operational and clinical feasibility. The methylation test can optimize the current cervical cancer screening plan, reduce the number of referral women with false positive diagnosis to colposcopy, and is of great significance for reducing fertility protection and preventing missed diagnosis in women of childbearing age.
